Descriptive study aiming to understand the family dynamics of elderly people aged 80 years or older, receiving family care at home. Caregivers and elderly, registered at the family health unit of Greater Porto, comprised a sample of 107 pairs. Data were collected between 09/2009 and 03/2010. The following instruments were applied: family APGAR, quality of life and lifestyle scale. The results that stand out are: increase in male partners serving as caregivers for dependent partners; substantial inclusion in the list of family caregivers: grandchildren, nieces, nephews, siblings. The caregiver lifestyle was considered regular, as well as the caregiver and elderly’s quality of life, although the family dynamics showed to work well. Limitations prevent further generalizations, but offer relevant support for the development of the family health and family nursing program.

The occurrence of health issues in the family affected by the ability of the family itself to carry out the role and task of family health care. Family nursing has been developed in various countries as a solution to solve individual, family, and community health issues in a region. The family health status in Indonesia is measured by the Family Health Index (FHI) value, comprising the category of unhealthy families (FHI < 0.50), pre-healthy families (FHI value of 0.50 to 0.80), and healthy families (FHI > 0.8). This study aims to generate a literature review on strategies that can be used to improve the Family Health Index. The search database includes CINAHL, MEDLINE, and Google Scholar with several keywords: 'family nursing', 'family health', and 'family health status'. 102 articles published between the year of 2000 until 2019 have been obtained. The results of the literature review indicate that family nursing is an innovative and efficient strategy to achieve a preferable health status as well as a better quality of human life. Family nursing has a positive correlation with the increased ability of families to carry out their task in maintaining, preventing and overcoming family health issues independently. Research on the families' ability to carry out health care tasks to improve the health status by measuring FHI has never been done previously. Future research is required to discuss the relationship of families' ability in carrying out the five health care tasks to improve FHI independently in Indonesia.

OBJECTIVE: To analyze the strengths and limitations of the Family Health Strategy from the perspective of health care professionals and the community. METHODS: Between June-August 2009, in the city of Vespasiano, Minas Gerais State, Southeastern Brazil, a questionnaire was used to evaluate the Family Health Strategy (ESF) with 77 healthcare professionals and 293 caregivers of children under five. Health care professional training, community access to health care, communication with patients and delivery of health education and pediatric care were the main points of interest in the evaluation. Logistic regression analysis was used to obtain odds ratios and 95% confidence intervals as well as to assess the statistical significance of the variables studied. RESULTS: The majority of health care professionals reported their program training was insufficient in quantity, content and method of delivery. Caregivers and professionals identified similar weaknesses (services not accessible to the community, lack of healthcare professionals, poor training for professionals) and strengths (community health worker-patient communications, provision of educational information, and pediatric care). Recommendations for improvement included: more doctors and specialists, more and better training, and scheduling improvements. Caregiver satisfaction with the ESF was found to be related to perceived benefits such as community health agent household visits (OR 5.8, 95%CI 2.8;12.1), good professional-patient relationships (OR 4.8, 95%CI 2.5;9.3), and family-focused health (OR 4.1, 95%CI 1.6;10.2); and perceived problems such as lack of personnel (OR 0.3, 95%CI 0.2;0.6), difficulty with access (OR 0.2, 95%CI 0.1;0.4), and poor quality of care (OR 0.3, 95%CI 0.1;0.6). Overall, 62% of caregivers reported being generally satisfied with the ESF services. CONCLUSIONS: Identifying the limitations and strengths of the Family Health Strategy from the healthcare professional and caregiver perspective may serve to advance primary community healthcare in Brazil.

The practice of community health nursing (CHN) may enhance the life experiences of families and communities, particularly amongst the poor and socially marginalised. CHN provides for a deeper understanding of the health status of families living within communities, for example, where and how they live, their cultural context and their ability to identify resources available to assist with their health care (Allender, Rector & Warner 2010:17).This qualitative phenomenological study reflects on the self-reported lived experiences of undergraduate CHN students at the University of the Western Cape in the City of Cape Town, South Africa. These students conducted a family health assessment (FHA) learning task at the homes of families within communities.Purposive and convenience sampling was used by students who had conducted an FHA. Fourteen students agreed to participate in the study, of whom nine were interviewed, two withdrew and the remaining three were not interviewed since no new data were emerging during interviews, indicating that saturation had been reached.During in-depth interviews with seven female and two male students, data for the exploration of the lived experiences was gathered through the following question: ‘How did you experience the FHA?’ Field notes were taken and used to capture non-verbal communication of participants. The focus of the study was to explore the lived experiences of students and not those of the family on whom the FHA was completed.Data collected were categorised into themes, guided by the systematic data analysis process of Tesch (1990) cited in Cresswell (2003:192). Four themes emerged: challenges of family selection, challenges of safety, socio-cultural challenges and academic challenges experienced by the participants. This study will inform future research and curriculum planning for CHN education in a multifaceted context.
