With the COVID-19 pandemic still raging and the vaccination program still rolling out, there continues to be an immediate need for public health officials to better understand the mechanisms behind the deep and perpetual divide over face masks in America. Using a random sample of Americans (N=615), following a pre-registered experimental design and analytic plan, we first demonstrated that mask wearers were not innately more cooperative as individuals than non-mask wearers in the Prisoners’ Dilemma (PD) game when information about their own and the other person’s mask usage was not salient. However, we found strong, revealed preference evidence of in-group favoritism among both mask and non-mask wearers when information about the other partner’s mask usage was known. Holding other things constant, non-mask wearers were 23 percentage points less likely to cooperate than mask wearers when facing a mask-wearing partner, and 26 percentage points more likely to cooperate than mask wearers when facing a non-mask wearing partner. Our analysis suggests social identity effects to be one of the main drivers of people’s decision whether to wear or shun face masks during the pandemic. Policy makers should therefore take social perception of face masks into account when designing not only what public messages to deliver, but also who to deliver these messages.

AbstractMask wearing has been advocated by public health officials as a way to reduce the spread of COVID-19. In the United States, policies on mask wearing have varied from state to state over the course of the pandemic. Even as more and more government leaders encourage or even mandate mask wearing, many citizens still resist the notion. Our research examines mask wearing policy and adherence in association with COVID-19 case rates. We used state-level data on mask wearing policy for the general public and on proportion of residents who stated they always wear masks in public. For all 50 states and the District of Columbia (DC), these data were abstracted by month for April ⍰ September 2020 to measure their impact on COVID-19 rates in the subsequent month (May ⍰ October 2020). Monthly COVID-19 case rates (number of cases per capita over two weeks) >200 per 100,000 residents were considered high. Fourteen of the 15 states with no mask wearing policy for the general public through September reported a high COVID-19 rate. Of the 8 states with at least 75% mask adherence, none reported a high COVID-19 rate. States with the lowest levels of mask adherence were most likely to have high COVID-19 rates in the subsequent month, independent of mask policy or demographic factors. Mean COVID-19 rates for states with at least 75% mask adherence in the preceding month was 109.26 per 100,000 compared to 249.99 per 100,000 for those with less adherence. Our analysis suggests high adherence to mask wearing could be a key factor in reducing the spread of COVID-19. This association between high mask adherence and reduced COVID-19 rates should influence policy makers and public health officials to focus on ways to improve mask adherence across the population in order to mitigate the spread of COVID-19.

It is now commonly accepted that the appearance and expansion of Internet-based communication has given rise to new possibilities for forging social networks and establishing “communities of interest.” One such ‘interest group’ which has found a community online comprises those who wish to end their lives. This has become a cause for concern to public health officials, governments, and suicide prevention professionals, while on the other hand possibilities for outreach and prevention in the new medium are being explored. The ways in which suicidal individuals have availed themselves of the information resources and connective possibilities of the Internet are outlined below, along with how governments and other parties interested in suicide prevention have responded to this new domain of risk.

When James Olcott spoke before Connecticut farmers for “anti-stream pollution,” he urged the public to mobilize to stop water pollution by “ignorant or reckless capitalists.” In identifying the “ignorant and reckless capitalists,” Olcott focused the attention of the farmers on industrial waste and the role of manufacturers in their search for profits in causing pollution. Although manufacturers and the courts argued that industrialization brought wealth and prosperity to New England and hence was a general good, Olcott challenged this idea. He saw the issue as a conflict between industrialization and its costs on the one hand and the public good on the other. Concern over industrial pollution and the potential conflict between it and public health had already arisen in Massachusetts. Although the Massachusetts State Board of Health realized that the interests of the “capitalists” and those of the public health officials might be in conflict, in 1872 it hoped that with improved knowledge, “a way will be eventually found to joining them into harmonious relations,” much as Lyman believed science and technology would resolve the conflict between fishers and mill owners. The board's interest in “harmonious relations” also reflected a realization that at least for the last several years, the courts had seen pollution as an inevitable consequence of civilization and had been favorable toward industrialists, especially if no obvious alternative to dumping pollution existed. In 1866, William Merrifield sued Nathan Lombard because Lombard had dumped “Vitriol and other noxious substances” into the stream above Merrifield's factory, “corrupting” the water so badly that it destroyed his boiler. Chief Justice Bigelow ruled that Lombard had invaded Merrifield's rights. “Each riparian owner,” the judge wrote, “has the right to use the water for any reasonable and proper purpose. . . . An injury to the purity or quality of the water to the detriment of the other riparian owners, constitutes in legal effect, a wrong.” In 1872, Merrifield again went to court, claiming the City of Worcester regularly dumped sewage into Mill Brook, by which the waters became greatly corrupted and unfit to use.”

ABSTRACTFalls among community-dwelling seniors constitute a major public health concern because of the potential morbidity and mortality associated with the fall. This study examined the informal care networks accessed by Canadian seniors who had visited the Emergency Department as a result of a fall, and considered the implications of the processes of asking for and receiving help on the senior's identity. Four themes were identified. The first was valuing independence. The remaining three themes concerned threats to the participants' identities linked to the need to ask for or receive help from family and friends. They were: becoming indebted, feeling devalued and becoming a burden to others. Seniors were noted to excuse family members from the expectation of helping because of work and family commitments, and illness. Participants described a mutually beneficial relationship with friends wherein both parties valued their independence and provided assistance to the other when needed. Their comments suggested that assistance was viewed as a good to be traded among peers. Our findings indicate that seniors value their independence and may not seek help even when it appears to be available, if asking threatens valued identities. Health and social care practitioners and policy makers responsible for planning and delivery of services should take this into account in order to ensure the best possible care for injured community-dwelling seniors.

  Background: In 2012, mechanically tenderized meat raised public health concern when an E.Coli 0157:H7 outbreak was linked to the tenderization process. It was discovered that the machinery pushed the E.Coli from the surface of contaminated meat products such as steaks and roasts, into the interior, where it was able to survive the cooking process. Concerns were raised by Lorraine McIntyre and the BCCDC about this issue, and their desire to improve their knowledge base in order to adequately assess the risk. Methods: Data was gathered via a survey conducted electronically and by telephone. Questions were asked to determine the proportion of retail establishments that use their own tenderizing equipment. Questions also asked about other industry practices such as current sanitization and labeling practices. Results: The results of this study were that 24% of surveyed establishments mechanically tenderize their meat products. Of these establishments, 33% have a label that states the meat has been tenderized mechanically and 17% provide cooking instructions on this label. An association was found between mechanically tenderizing meat and establishment type, which suggests that grocery stores are more likely to mechanically tenderize than other establishments, such as restaurants. On the other hand, no association was found between operator experience and their level of knowledge regarding the risks of mechanical tenderization. Conclusions: Overall, this study has demonstrated the likelihood is high that consumers purchase and consume beef that has been mechanically tenderized at the retail level. The results from this study can be used to aid public health officials in quantifying the risk of mechanical tenderization at a retail level and aid in the development and implementation of new legislation such as mandatory labeling of all mechanically tenderized meat.

AbstractQuarantine has been used for centuries in an effort to prevent the introduction, transmission, and spread of communicable diseases. While backed by legal authority, the public and even the health care worker community’s understanding of the term is murky at best and scientific evidence to support the use of quarantine is frequently lacking. The multiple interpretations and references to quarantine, the inconsistent application of public health quarantine laws across jurisdictional boundaries, and reports of ineffectiveness are further complicated by associated infringement of civil liberties and human rights abuses. Given the need to balance public safety with human rights, we must be more precise about the meaning of quarantine and consider the efficacy and negative secondary effects resulting from its implementation. This article explains quarantine terminology and then uses a case study from Taiwan during the 2002–2003 severe acute respiratory syndrome (SARS) outbreak to illustrate the key principles associated with quarantine measures taken during the 2014 Ebola outbreak and the potential hazards that can arise from quarantines. Finally, we provide a quarantine and isolation decision tree to assist policy makers and public health officials in applying medically defensible, outcomes-based data and legal authorities to optimize management of emerging infectious diseases. An investigation into support for restrictions on people testing seropositive for HIV is reported on. Data were collected during telephone interviews with two-hundred adults aged eighteen to sixty-five in the Chicago metropolitan area. Using the analytic technique of LISREL, six models which attempt to explain support for restrictions were tested. It was found that the model best supported by the data indicates that two groups contribute to support for restrictions on HIV carriers - one due to intolerance of homosexuality and one to mistrust of public health officials regarding their control and management of the AIDS epidemic. The relevance of these findings for public health policy makers is discussed.

Immunization programs today stand at crossroads. Even as COVID vaccine campaigns continue, inequity, concerns and confusion around them seems ever expanding. While vaccine hesitancy in some form or the other has existed since the inception of vaccination, the complex and dynamic world that we live in now has resulted in hesitancy to vaccines become an outcome of myriad interactions that we encounter in our day to day lives. Factors extraneous to health systems are major determinants and it is essentially the culmination of economics, politics, science, and technology impacting human behaviors and emotions which result in a parent, family or a community arrive at the decision of whether to or not to vaccinate. Vaccine hesitancy is on the rise, it is becoming more organized and now is not just a problem of high- income countries. It is imperative that as public health advocates, academicians, policy makers, managers and implementers we recognize it and adopt a non-judgmental and non-partisan approach built on empathy, respect and trust and not just scientific knowledge and theorems to address it. The current paper puts forward a novel approach to address hesitancy, the LEARN approach: and advocates that we need to ‘Listen’ to the voices of concern, ‘Engage’ with all stakeholders, ‘Articulate’ and communicate better in a language which is simple and comprehensible, ‘Reflect’ on what works and what does not and at the same time ‘Negate’ events and episodes which dent confidence in science and promote complacency.
