scholarly journals The evolution of depressive symptomatology across three waves of the COVID-19 pandemic: A 17-month representative longitudinal study of the adult population

This 17-month longitudinal study on a representative sample of 4,361 Norwegian adults employs an observational ABAB design across six repeated assessments and three pandemic waves to systematically investigate the evolution of depressive symptomatology across all modifications of viral mitigation protocols (VMPs) from their onset to termination. Using Latent Change Score Models to analyze 26,166 observations, the study empirically corroborates that critical fluctuations in depressive symptomatology within and across individuals occur during the first three months of the pandemic, after which symptom trajectories are predominantly consolidated throughout the pandemic period. Contrary to established belief, female sex, young age, lower education and preexisting psychiatric diagnosis only served as adequate predictors of the initial shocks to symptomatology observed during the onset of the pandemic, and did not adequately predict subsequent change and the critical fluctuations observed in symptoms within and across individuals. Population-level trajectories demonstrated that symptom levels increased in accordance with the presence and strictness of VMPs. Upon predominant termination of VMPs, population-level symptoms began declining, while large heterogeneity was present across the adult population. Detrimental long-term adversities were revealed by 10% of the adults. These individuals displayed chaotic adaptation to the pandemic and its VMPs, exhibiting substantial increases in clinical levels of symptomatology ensuing partial re-opening of society and through the remainder of the pandemic, with these deleterious symptoms further projected to remain heightened ahead. Number of times exposed to quarantine was incrementally tied with increases in contemporaneously experienced and long-term depressive adversities, while information obtainment through unmonitored sources was associated with contemporaneous but not long-term states of heightened symptomatology.

It is increasingly recognized that SARS-CoV-2 can produce long-term complications after recovery from the acute effects of infection. Here, we report the analysis of 32 self-reported short and long-term symptoms in a general adult population cohort comprised of 357 COVID-19+ cases, 5,497 SARS-CoV-2-negative controls, and 19,095 non-tested individuals. The majority of our COVID-19+ cases are mild, with only 9 of the 357 COVID-19+ cases having been hospitalized. Our results show that 36.1% of COVID-19+ cases have symptoms lasting longer than 30 days, and 14.8% still have at least one symptom after 90 days. These numbers are higher for COVID-19+ cases who were initially more ill, 44.9% at 30 days and 20.8% at 90 days, but even for very mild and initially asymptomatic cases, 21.3% have complications persist for 30 days or longer. In contrast, only 8.4% of participants from the general untested population develop new symptoms lasting longer than 30 days due to any illness during the same study period. The long-term symptoms most enriched in those with COVID-19 are anosmia, ageusia, difficulty concentrating, dyspnea, memory loss, confusion, chest pain, and pain with deep breaths. In addition to individuals who are initially more sick having more long-term symptoms, we additionally observe that individuals who have an initial symptom of dyspnea are significantly more likely to develop long-term symptoms. Importantly, our study finds that the overall level of illness is an important variable to account for when assessing the statistical significance of symptoms that are associated with COVID-19. Our study provides a baseline from which to understand the frequency of COVID-19 long-term symptoms at the population level and demonstrates that, although those most likely to develop long-term COVID-19 complications are those who initially have more severe illness, even those with mild or asymptomatic courses of infection are at increased risk of long-term complications.

LAY SUMMARY Little is known about the mental health of Veterans as they get older. How does the mental health and aging process of Veterans compare to people who have not served in the military? The Canadian Longitudinal Study on Aging (CLSA) is a long-term national study of the aging adult population in Canada. A total of 51,338 participants across Canada aged 45 to 85 years were recruited at the study baseline between 2011 and 2015. Of the CLSA participants, about 4,500 self-identified as Veterans, with military service dating back as early as 1941. The goal of this study was to describe the mental health, in particular posttraumatic stress disorder (PTSD), of Veterans in the CLSA at the study baseline and examine differences across Veteran sub-groups and compared to non-Veterans.

ObjectivesFrequent attenders (FAs) create a substantial portion of primary care workload but little is known about FAs’ sickness absences. The aim of the study is to investigate how occasional and persistent frequent attendance is associated with sickness absences among the working population in occupational health (OH) primary care.Setting and participantsThis is a longitudinal study using medical record data (2014–2016) from an OH care provider in Finland. In total, 59 676 patients were included and categorised into occasional and persistent FAs or non-FAs. Sick-leave episodes and their lengths were collected along with associated diagnostic codes. Logistic regression was used to analyse associations between FA status and sick leaves of different lengths (1–3, 4–14 and ≥15 days).ResultsBoth occasional and persistent FA had more and longer duration of sick leave than non-FA through the study years. Persistent FAs had consistently high absence rates. Occasional FAs had elevated absence rates even 2 years after their frequent attendance period. Persistent FAs (OR=11 95% CI 7.54 to 16.06 in 2016) and occasional FAs (OR=2.95 95% CI 2.50 to 3.49 in 2016) were associated with long (≥15 days) sickness absence when compared with non-FAs. Both groups of FAs had an increased risk of long-term sick leaves indicating a risk of disability pension.ConclusionBoth occasional and persistent FAs should be identified in primary care units caring for working-age patients. As frequent attendance is associated with long sickness absences and possibly disability pensions, rehabilitation should be directed at this group to prevent work disability.

Abstract Purpose of Review Anthropogenic activities can lead to the loss, fragmentation, and alteration of wildlife habitats. I reviewed the recent literature (2014–2019) focused on the responses of avian, mammalian, and herpetofaunal species to oil and natural gas development, a widespread and still-expanding land use worldwide. My primary goals were to identify any generalities in species’ responses to development and summarize remaining gaps in knowledge. To do so, I evaluated the directionality of a wide variety of responses in relation to taxon, location, development type, development metric, habitat type, and spatiotemporal aspects. Recent Findings Studies (n = 70) were restricted to the USA and Canada, and taxonomically biased towards birds and mammals. Longer studies, but not those incorporating multiple spatial scales, were more likely to detect significant responses. Negative responses of all types were present in relatively low frequencies across all taxa, locations, development types, and development metrics but were context-dependent. The directionality of responses by the same species often varied across studies or development metrics. Summary The state of knowledge about wildlife responses to oil and natural gas development has developed considerably, though many biases and gaps remain. Studies outside of North America and that focus on herpetofauna are lacking. Tests of mechanistic hypotheses for effects, long-term studies, assessment of response thresholds, and experimental designs that isolate the effects of different stimuli associated with development, remain critical. Moreover, tests of the efficacy of habitat mitigation efforts have been rare. Finally, investigations of the demographic effects of development across the full annual cycle were absent for non-game species and are critical for the estimation of population-level effects.

AbstractGlobally, bird migration is occurring earlier in the year, consistent with climate-related changes in breeding resources. Although often attributed to phenotypic plasticity, there is no clear demonstration of long-term population advancement in avian migration through individual plasticity. Using direct observations of bar-tailed godwits (Limosa lapponica) departing New Zealand on a 16,000-km journey to Alaska, we show that migration advanced by six days during 2008–2020, and that within-individual advancement was sufficient to explain this population-level change. However, in individuals tracked for the entire migration (50 total tracks of 36 individuals), earlier departure did not lead to earlier arrival or breeding in Alaska, due to prolonged stopovers in Asia. Moreover, changes in breeding-site phenology varied across Alaska, but were not reflected in within-population differences in advancement of migratory departure. We demonstrate that plastic responses can drive population-level changes in timing of long-distance migration, but also that behavioral and environmental constraints en route may yet limit adaptive responses to global change.

Abstract The recent reauthorization of the Older Americans Act adds language and definitions to current issues facing the aging population. Specifically, Title I includes definitions related to program adaptation and coordination, workforce and long-term care issues, nutrition and social isolation, as well as family caregivers. Different from the last authorization, these definitions span beyond the individual experience to include other entities impacted by an aging society, such as the workforce and families. Overall, the Title I reauthorization seeks to modernize policy to reflect the current influx of the older adult population and its consequences.
