Introduction: Vitamin D is an important micronutrient having crucial role in calcium and phosphate homeostasis but there is emerging evidence to suggest its role in prevention of infectious, inflammatory and neoplastic diseases. The vitamin D insufficiency/deficiency is a widely recognized problem among children in developed as well as developing countries.Objective: To find out the prevalence of vitamin D deficiency/insufficiency among children aged 2 months to 13 yrs and its correlation with serum calcium, phosphate and demographic factors.Methodology: Prospective, cross sectional, hospital based study conducted at Birat Medical College Teaching Hospital for a period of one year among children presented to pediatric OPD in whom pediatrician had a suspicion of Vitamin D Insufficiency /Deficiency. Venous blood Samples were taken for the estimation of 25(OH) Vitamin D, Calcium and Phosphate. Statistical analysis was done using SPSS software version 16. Results Among 174 children aged 2 months to 13 years, the prevalence of vitamin D insufficiency at the cutoff of <30ng/ml was 82.75% where as no cases of vitamin D deficiency was noted at cut off level of < 10 ng/ml. Children residing in urban area, samples taken in the winter season had higher prevalence of vitamin D insufficiency (p value <0.05). Serum calcium and phosphate level were found positively correlated with Vitamin D level (p<.05). However the relationship was very strong between Vitamin D and Calcium (r=0.402, p-value <0.001) and weak positive correlation was noted with phosphate(r=0.155, p value <0.05).Conclusion: There is very high prevalence of Vitamin D insufficiency among children. The vitamin D insufficiency was significantly higher in winter season as compared to summer. Children living in the urban area are more prone to Vitamin D insufficiency. Moderately strong positive correlation was observed between Vitamin D level and calcium but weak correlation existed with phosphate. BJHS 2018;3(1)5 : 381-384

ABSTRACT Introduction Hypovitaminosis D in pregnancy has been reported to cause various maternal effects, i.e., hypocalcemia, subclinical myopathy, increased risk of preeclampsia (PE), gestational diabetes mellitus (GDM), cesarean sections, and fetal effects, i.e., neonatal tetany, hyperbilirubinemia congenital rickets, infantile rickets, etc. Only few Indian studies are available in this regard. Objective To estimate serum vitamin D levels in pregnant women, cord blood, and study fetomaternal outcomes. Materials and methods A prospective observational study was conducted on 54 consecutive pregnant women and their newborn babies. Serum 25-hydroxy vitamin D [25(OH)D] level was estimated in all women at the time of admission in labor ward. They were followed up to delivery and 48 hours postpartum. Vitamin D was also estimated in cord blood collected during delivery. All results were recorded and analyzed statistically. Results The mean 25(OH)D level in pregnancy was 6.81 ± 7.38 ng/mL. The mean 25(OH)D level in their babies (cord blood) was 6.34 ± 7.05 ng/mL. There was very strong positive correlation between maternal and fetal serum 25(OH)D levels (p-value 0.001, r-value 0.9). Vitamin D deficiency was strongly associated with obesity, PE, and GDM (p-value 0.001). Neonatal jaundice and tetany were also significantly associated with severe vitamin D deficiency. Conclusion Low levels of vitamin D have been observed in pregnant women and their newborn babies. Hypovitaminosis D has been associated with adverse fetomaternal outcomes. As there is a strong correlation of maternal and neonatal levels, supplementing vitamin D in a pregnant women might improve these adverse pregnancy outcomes. Introduction: Vitamin D deficiency has profound adverse effects on health. Serum calcium, phosphorus and even alkalinephasphatase cannot predict underlying vitamin D deficiency. Objectives: 1. To determine the frequency of vitamin D deficiency in outpatientprivate clinic 2. To determine the relation of vitamin D deficiency with the presenting symptom of bone or body aches. To see the relation ofits deficiency to the serum calcium, alkaline phosphate and phosphate levels. Study design: Descriptive study. Setting: One of the local privateclinic. Materials and Methods: 800 patients who presented to the clinic due to any ailment, having presenting complains with bone or bodyaches or not, were included in this study. The duration of study was 7 months from June to December 2008. Results: The study showed amongtotal 33% (n=264) were male and 67% (n=536) were female. 33 patients (4.1 %) were below age of 20 years, 364 patients (45.2%) were havingage between 20-40 years, 252 (31.5%) were between 41 -60 years, 143 (17.8%) were between 61 -80 years and only 8 patients (1 %) were above80years. Over all vitamin D present in sufficient amount (>30 ng/ml) in only 4 patients (0.5%), reaming patients (n=796,99.5%) were deficientin vitamin D were further divided into those who were having deficiency (serum level <20 ng/ml) (n=636, 79%) and insufficiency (serum levelbetween 21-29 ng/ml) (n=160,20%). All the deficient patients were having normal serum calcium and phosphate levels and only 33 patientswere having modestly raised serum alkaline phosphate. Among the sample only 318 (39.7%) were having bone or body aches as a presentingfeature while remaining 482 (60.3%) were having no pains. More ever pain has got insignificant relation to any level of serum vitamin D level(p-value=0.201), however younger deficient patients were having lesser chance of bone or body aches as compared to age more then 60 year(p-value<0.001). Conclusions: Vitamin D deficiency is more prevalent in our community, as compared to published data, targeting youngpopulation. Vitamin D supplementation should be planned to decrease its varied and multidimensional ill effects on health.

AbstractBackgroundThe COVID-19 pandemic goes along with increased mortality from acute respiratory disease, and measures to limit the spread of the infection go along with increased risk of vitamin D deficiency, especially among high risk groups. It has been suggested that vitamin D3 supplementation might help to reduce respiratory disease mortality.MethodsWe assessed the prevalence of vitamin D insufficiency and deficiency, defined by 25(OH)D blood levels of 30-50 and <30 nmol/L, respectively, and their association with mortality from respiratory diseases during 15 years of follow-up in a cohort of 9,548 adults aged 50-75 years from Saarland, Germany.ResultsVitamin D insufficiency and deficiency were common (44% and 15%, respectively). Compared to sufficient vitamin D status, respiratory disease mortality was 2.1 (95%-CI 1.3-3.2)- and 3.0 (95%-CI 1.8-5.2)-fold increased, respectively. Although significant increases were seen in both women and men, they were much stronger among women, with 8.5 (95% CI 2.4-30.1) and 2.3 (95% CI 1.1-4.4)-fold increase of respiratory disease mortality in case of vitamin D deficiency among women and men, respectively (p-value for interaction =0.041). Overall, 41% (95% CI 20%-58%) of respiratory disease mortality was statistically attributable to vitamin D insufficiency or deficiency.ConclusionVitamin D insufficiency and deficiency are common and account for a large proportion of respiratory disease mortality in older adults, supporting suggestions that vitamin D3 supplementation might make a major contribution to limit the burden of the COVID-19 pandemic, particularly among women.

Objectives: Vitamin D deficiency is being studied all over the world and is a global health problem. Recent studies haveshown that Vitamin D may have a significant role in reducing the incidence of autoimmune diseases. This study was conducted to find out the level of Vitamin D in patients with thyroid disorders and compare Vitamin D levels in Anti Tpo positive and negative patients. Subject and Methodology: 60 patients from Chitwan Medical College outpatient clinic were included in the study from February to May 2016. A brief history and clinical examinations were taken from all patients along with laboratory tests for thyroid functions, TPO antibody level of 25(OH) Vitamin D3 level and Serum Calcium level. Results: Both patients of hypothyroidism (autoimmune and nonautoimmune) had severe Vitamin D deficiency. Most of the hyperthyroid cases had Vitamin D insufficiency. Anti TPO positive and negative patients both had Vitamin D deficiency. Conclusion: Our study shows there is significant Vitamin D deficiency in hypothyroidism and hyperthyroidism irrespective of its autoimmunity. This study certainly encourages us to screen Vitamin D and supplement Vitamin D to all thyroid disorders patients.

Objective: We aimed to find out the association of low serum vitamin D levels with the incidence of prostate cancer through a prospective case-control study. Material and methods: This study was carried out in the tertiary care hospital (India). All newly diagnosed patients of prostate cancer and age-matched controls were included. Serum vitamin D levels were measured in all of them. Vitamin D status (ng/mL) was classified as severe deficiency <10, moderate deficiency 10–<30, normal 30–100, and toxicity >100. Normality of the data was tested by the Kolmogorov–Smirnov test, statistical analysis was done with Statistical Package for the Social Sciences version 26.0, p-value of <0.05 was considered significant. Results: In our study, 320 cases and 320 controls were included. The mean vitamin D levels in cases and controls were 15.71 ± 6.5 (ng/mL) and 17.63 ± 4.54 (ng/mL), respectively, ( p-Value <0.01). Patients with severe vitamin D deficiency (73.58%) had a Gleason score ⩾8 on biopsy and 79.24% of them had a serum prostate specific antigen (PSA) level >20 ng/mL. Conclusion: We had concluded that there was no significant association between vitamin D deficiency and increased risk of prostate cancer, although patients with higher-grade prostate cancer and with higher PSA level had severe vitamin D deficiency. Level of evidence: Not applicable for this multicentre audit.

ABSTRACT Background : Roux-en-Y gastric bypass patients can experience changes in calcium metabolism and hyperparathyroidism secondary to vitamin D deficiency. Aim : To evaluate nutritional deficiencies related to the calcium metabolism of patients undergoing gastric bypass with a 10-year follow-up. Method : This is a longitudinal retrospective study of patients submitted to Roux-en-Y gastric bypass at a multidisciplinary clinic located in the Brazilian southeast region. The study investigated the results of the following biochemical tests: serum calcium, ionized calcium, vitamin D, and parathormone (PTH). The generalized estimating equations (GEE) determined the nutritional deficiencies using a significance level of 5%. Results : Among the patients who finished the study (120 months), 82.86% (n=29) had vitamin D deficiency, and 41.94% (n=13) had high PTH. Postoperative time had a significant effect on PTH (p=0.0059). The percentages of patients with vitamin D, serum calcium, and ionized calcium deficiencies did not change significantly over time. Conclusion : One of the outcomes was vitamin D deficiency associated with secondary hyperparathyroidism. These findings reaffirm the importance of monitoring the bone metabolism of patients submitted to Roux-en-Y gastric bypass. HEADINGS: Calcium deficiency. Vitamin D deficiency. AbstractThe incidence of vitamin D deficiency is high globally, and vitamin D supplementation draws particular attention. The objective of this study was to investigate the effects of stratified vitamin D supplementation in middle-aged and elderly individuals with vitamin D insufficiency in Beijing. A total of 448 subjects aged over 40 years old were selected from a community in Beijing. Among them, 100 middle-aged and elderly people with vitamin D insufficiency were randomly selected on a voluntary basis. They were further divided into control group and intervention group. The control group received health education and lifestyle guidance, and the intervention group received lifestyle guidance and vitamin D supplementation for nine months. The doses were stratified as follows: for vitamin D insufficiency, oral vitamin D3 supplement was given at 5000 IU/w; for mild vitamin D deficiency, oral vitamin D3 supplement was given at 10 000 IU/w; for severe vitamin D deficiency, oral vitamin D3 supplement was given at 15 000 IU/w. Safety evaluation was conducted after three-month treatment. The intervention group consisted of 8%, 62%, and 30% of cases who had vitamin D insufficiency, mild vitamin D deficiency, and severe vitamin D deficiency, respectively, which were similar with the control group. It showed that the blood 25(OH)D level increased significantly in the intervention group, from 14.30±4.30 ng/ml to 33.62±6.99 ng/ml (p<0.001), in contrast to insignificant change in the control group. Stratified vitamin D supplementation effectively increased the blood 25(OH)D level, as well as the number of cases with corrected vitamin D insufficiency or deficiency.

Psoriasis is an immuno-mediated chronic systemic disease involving cytokines of helper Th1 pathway. Vitamin D has an effect on keratinocyte proliferation, differentiation and immune modulation of immune system especially Th1 pathway, which is altered in psoriatic skin suggesting that Vitamin D may have a role in pathogenesis of psoriasis. To study correlation between psoriasis vulgaris and serum vitamin D. To evaluate serum vitamin D level in psoriasis cases and in control group and correlating vitamin D level with severity and duration of the psoriasis. 57 cases (&#62;15years of age) with psoriasis and 57 healthy subjects were recruited. Psoriasis was clinically diagnosed and severity evaluated by PASI scale. Vitamin D was analysed by enhanced chemilumine scence on vitrus Eci autoanalyzer of Orth clinical diagnostic. Vitamin D deficiency defined as &#60;20ng/ml, insufficiency 20-30ng/ml and sufficient 30-100ng/ml. Vitamin D deficiency in the study was 22.8% in patients and 14% in control group. Vitamin D insufficiency was found in 42.1% of cases and 19.3% of control. According to chi-square the p-value is 0.003 showing significant association. There was a tendency towards decrease in vitamin D level with increase in disease duration. There was negative correlation between vitamin D and PASI score. The study found a significant relationship between vitamin D and psoriasis. Further metanalysis involving larger study population will be required to establish whether vitamin D levels benefits patient with psoriasis vulgaris.
