Introduction: Studies have shown different views regarding the effect of music in vitals e.g Heart rate (HR), Blood pressure (BP) and atiention. The effect of preferred music with lyrics in vitals and reaction time in stroop test has not been performed in Nepalese students so, we conducted the study. Objective: To find out the change in HR, BP and reaction time in Stroop test before and after their preferred music with lyrics. Methodology Thirty male medical and paramedical students aged 25.27 ± 2.0 participated in study. The vital signs and reaction time in Stroop test before and after music was taken. Results Paired-t test was used to compare means before and after exposure to music. The means are expressed as Mean ± SD. Heart rate (HR) increased after exposure to music (66.33±9.51 Vs 67.2±8.44) (p<.05). The error in Stroop test was less after music (.66±.49 Vs.63±.66) (p<.05). The reaction time after error correction decreased post exposure to music (24.117±4.61Vs23.29±4.45) (p<.05). Conclusion The heart rate increased after exposure to music. The errors decreased after listening to music which also decreased reaction time.

Introduction and objectives: The impact of multiple subcutaneous (s.c.) esketamine injections on the blood pressure (BP) and heart rate (HR) of patients with unipolar and bipolar treatment-resistant depression (TRD) is poorly understood. This study aimed to assess the cardiovascular safety of multiple s.c. doses of esketamine in patients with TRD. Methods: Seventy TRD patients received 394 weekly s.c. esketamine injections in conjunction with oral antidepressant therapy for up to six weeks. Weekly esketamine doses were 0.5, 0.75 or 1.0 mg/kg according to each patient’s response to treatment. Participants were monitored before each treatment and every 15 minutes thereafter for 120 minutes. We assessed systolic blood pressure (SBP), diastolic blood pressure (DBP), and HR measurements for the entire treatment course. Results: BP increased after the first s.c. esketamine injection, reaching maximum mean SBP/DBP levels of 4.87/5.54 mmHg within 30–45 minutes. At the end of monitoring, 120 minutes post dose, vital signs returned to pretreatment levels. We did not detect significant differences in BP between doses of 0.5, 0.75, and 1 mg/kg esketamine. Mean HR did not differ significantly between doses or before and after s.c. esketamine injection. Conclusions: The BP changes observed with repeated s.c. esketamine injections were mild and well tolerated for doses up to 1 mg/kg. The s.c. route is a simple and safe method of esketamine administration, even for patients with clinical comorbidities, including obesity, hypertension, diabetes, and dyslipidemia. However, 14/70 patients experienced treatment-emergent transient hypertension (SBP >180 mmHg and/or a DBP >110 mmHg). Therefore, we strongly recommend monitoring BP for 90 minutes after esketamine dosing. Since s.c. esketamine is cheap, requires less frequent dosing (once a week), and is a simpler procedure compared to intravenous infusions, it might have an impact on public health.

AbstractBackground:Ivabradine is a unique medication that reduces the intrinsic heart rate by specifically blocking the inward funny current that controls the pacemaker activity of the sinus node. We conducted a retrospective cohort study to assess the efficacy of ivabradine in children suffering from postural orthostatic tachycardia syndrome.Methods:A chart review was conducted of patients less than 18 years of age who were diagnosed with postural orthostatic tachycardia syndrome who had received ivabradine as treatment from January 2015 to February 2019 at our institution. Twenty-seven patients (25 females, 92.5%) were identified for the study. The outcomes which were assessed included a change in the severity and frequency of symptoms, heart rate, and blood pressure before and after starting ivabradine.Results:There was an improvement in the symptoms of 18 (67%) out of 27 patients. The most notable symptom affected was syncope/presyncope with a reduction in 90%, followed by lightheadedness (85%) and fatigue (81%). The vital signs of the patients showed an overall significant lowering of the heart rate during sitting (89.7 ± 17.9 versus 73.2 ± 12.1; p-value <0.05) and standing (100.5 ± 18.1 versus 80.9 ± 10.1; p-value <0.05) without a significant change in the blood pressure. Two patients had visual disturbances (luminous phenomena). Severe bradycardia and excessive flushing were seen in two patients, respectively. Another one patient reported joint pain and fatigue.Conclusion:This study indicates that 67% of children treated with ivabradine report an improvement in symptoms.

Heart rate, mean blood pressure, adrenal blood flow, and adrenal plasma adrenaline and noradrenaline were compared before and after ligation of the anterior descending coronary artery in dogs anesthetized with chloralose. One group of 12 dogs responded to acute coronary occlusion with a sudden and marked decrease in mean blood pressure (mean, 31%) and heart rate (mean, 18%) followed by an early onset (mean, 227 sec) of ventricular fibrillation. Another group of nine dogs responded with slight decreases in mean blood pressure (mean, 13%) and heart rate (mean, 5%), during which time ventricular fibrillation occurred late (mean, 30 min) or not at all. While the two groups were statistically different in mean blood pressure and heart rate, the minute output of adrenal catecholamines in either group was not found to be related to the early or late occurrence of ventricular fibrillation.

Background:Music therapy is based on ancient cross-cultural beliefs that music can have a “healing” effect on mind and body. Research determined that listening to music can increase comfort and relaxation, relieve pain, lower distress, reduce anxiety, improve positive emotions and mood, and decrease psychological symptoms. Music therapy has been used greatly in various medical procedures to reduce associated anxiety and pain. Patients have a high level of anxiety when they are in the hospital, this is the case of patients with rheumatic diseases who consult regularly to have intravenous infusion of biological therapies.Objectives:The purpose of this study was to examine the effectiveness of music therapy on pain, anxiety, and vital signs among patients with chronic inflammatory rheumatic diseases during intravenous infusion of biological drugs.Methods:Fifty patients were divided into two groups: The experimental group G1 (n=25) received drug infusion while lestening to soft music (30 minutes); and the control group G2 (n=25) received only drug infusion. Measures include pain, anxiety, vital signs (blood pressure, heart rate and respiratory rate). The pain was measured using visual analogic scale (VAS). The state-trait anxiety inventory (STAI) was used for measuring anxiety, low anxiety ranges from 20 to 39, the moderate anxiety ranges from 40 to 59, and high anxiety ranges from 60 to 80. Vital signs (systolic blood pressure [SBP], diastolic blood pressure [DBP], heart rate [HR], and respiratory rate [RR]) were measured before, during and immediately after the infusion.Statistical package for social sciences (SPSS) was used for analysis.Results:The mean age in G1 was 44.45 years (26-72) with a sex ratio (M/F) of 0.8. Including the 25 patients, 12 had rheumatoid arthritis, 10 had ankylosing spondylitis and 3 had psoriatic arthritis. The mean disease duration was 8 years. In G2, the mean age was 46 years (25-70) with a sex ratio (M/F) of 0.75, 12 had rheumatoid arthritis, 11 had ankylosing spondylitis and 2 had psoriatic arthritis. The mean disease duration was 7.5 years. The biological drugs used were: Infliximab in 30 cases, Tocilizumab in 12 cases and Rituximab in 8 cases.Before the infusion, the patients of experimental group had a mean VAS of 5/10±3, a mean STAI of 50.62±6.01, a mean SBP of 13.6 cmHg±1.4, a mean DBP of 8.6 cmHg±1, a mean HR of 85±10 and a mean RR of 18±3. While in control group the mean VAS was 5.5±2, the mean STAI was 50.89±5.5, the mean SBP was 13.4±1.2, the mean DBP was 8.8±1.1, the mean HR was 82±8 and the mean RR was 19±2.During the infusion and after music intervention in G1, the mean STAI became 38.35±5 in G1 versus 46.7±5.2 in G2 (p value=0.022), the mean SBP became 12.1±0.5 in G1 versus 13±1 in G2 (p=0.035), the mean DBP became 8.1±0.8 in G1 versus 8.4±0.9 in G2 (p=0.4), the mean HR became 76±9 in G1 versus 78±7 in G2 (p=0.04) and the mean RR became 17.3±2.1 in G1 versus 18.2±1.7 in G2 (p=0.39).This study found a statistically significant decrease in anxiety, systolic blood pressure and heart rate in patients receiving music interventions during biological therapies infusion, but no significant difference were identified in diastolic blood pressure and respiratory rate.Conclusion:The findings provide further evidence to support the use of music therapy to reduce anxiety, and lower systolic blood pressure and heart rate in patients with rheumatic disease during biological therapies infusion.References:[1] Lin, C., Hwang, S., Jiang, P., & Hsiung, N. Physiological responses associated with the ratings of musical stimuli were investigated. For 101 university music and biology students in the experimental group, heart rate, blood pressure, and finger temperature were measured before and after listening to about 10 min. of music (Venus and Jupiter from Holst's The Planets) in an anechoic chamber. They also served as their own controls by sitting in silence for about 10 min. No significant differences were found on the three measures for the two musical stimuli between the musical or control conditions or between the two majors.

Abstract Background Hydrotherapeutic applications to the head and spine have shown to improve cardiovascular and autonomic functions. There is lack of study reporting the effect of either neutral spinal bath (NSB) or neutral spinal spray (NSS). Hence, the present study was conducted to evaluate and compare the effects of both NSB and NSS in healthy volunteers. Methods Thirty healthy subjects were recruited and randomized into either neutral spinal bath group (NSBG) or neutral spinal spray group (NSSG). A single session of NSB, NSS was given for 15 min to the NSBG and NSSG, respectively. Assessments were taken before and after the interventions. Results Results of this study showed a significant reduction in low-frequency (LF) to high-frequency (HF) (LF/HF) ratio of heart rate variability (HRV) spectrum in NSBG compared with NSSG (p=0.026). Within-group analysis of both NSBG and NSSG showed a significant increase in the mean of the intervals between adjacent QRS complexes or the instantaneous heart rate (HR) (RRI) (p=0.002; p=0.009, respectively), along with a significant reduction in HR (p=0.002; p=0.004, respectively). But, a significant reduction in systolic blood pressure (SBP) (p=0.037) and pulse pressure (PP) (p=0.017) was observed in NSSG, while a significant reduction in diastolic blood pressure (DBP) (p=0.008), mean arterial blood pressure (MAP) (p=0.008) and LF/HF ratio (p=0.041) was observed in NSBG. Conclusion Results of the study suggest that 15 min of both NSB and NSS might be effective in reducing HR and improving HRV. However, NSS is particularly effective in reducing SBP and PP, while NSB is particularly effective in reducing DBP and MAP along with improving sympathovagal balance in healthy volunteers.

Abstrak Terapi komplementer merupakan salah satu alternatif untuk mengatasi masalah kesehatan. Hidroterapi dan relaksasi Benson merupakan terapi komplementer yang efektif terhadap tekanan darah dan nadi, namun masih jarang masyarakat yang memanfaatkannya. Diketahui ada pengaruh kombinasi hidroterapi dan relaksasi Benson (disebut hidroson) terhadap tekanan darah dan nadi. Metode penelitian pra-eksperimen, pre- and post- design dengan teknik purposive sampel pada 32 responden usia 26-65 tahun. Penelitian dilakukan dari tanggal 1-18 Maret 2019 di RT 19 dan 20, Sungapan V Desa Wahyuharjo, Kecamatan Lendah, Kabupaten Kulon Progo dengan tiap responden diberikan intervensi selama 3 hari berturut-turut. Instrumen yang digunakan adalah sphygmomanometer digital untuk mengukur tekanan darah dan nadi 5 menit sebelum dan sesudah pemberian terapi. Tekanan darah dianalisis dengan wilcoxon test dan paired t-test untuk nadi. Hasil analisis menunjukkan tekanan darah sistolik sebelum terapi sebesar 118,25 mmHg dan setelah terapi sebesar 111,00 mmHg, sehingga disimpulkan terjadi penurunan sebesar 7,25 mmHg dengan ρ Value 0,0001. Tekanan darah diastolik sebelum terapi sebesar 81,25 mmHg dan setelah terapi sebesar 78,75 mmHg, sehingga disimpulkan terjadi penurunan sebesar 2,50 mmHg dengan ρ value 0,002. Nadi sebelum terapi sebesar 82,30 x/menit dan setelah terapi sebesar 80,64 x/menit, sehingga disimpulkan terjadi penurunan sebesar 1,66 x/menit dengan ρ value 0,003. Ada pengaruh yang signifikan terhadap pemberian kombinasi hidroterapi dan relaksasi Benson (hidroson) terhadap penurunan tekanan darah dan nadi di RT 19 dan 20, Sungapan V. Hydrotherapy and Benson's relaxation are effective complementary therapies for blood pressure and pulse, but still few patients utilize it. There is a known effect of hydrotherapy and Benson relaxation combination (called hydrosol) on blood pressure and pulse. The study method was pre-experimental with pre and post design with a purposive sampling technique on 32 respondents aged 26-65 years. The study was conducted from 1-18 March 2019 in neighborhood 19 and 20, Sungapan V, Wahyuharjo Village, Lendah District, Kulon Progo. Regency Each respondent was given intervention with hydrosol therapy for 3 consecutive days. The instrument used was a digital sphygmomanometer to measure blood pressure and pulse 5 minutes before and after the administration of therapy. Blood pressure was analyzed by the Wilcoxon test while pulsing by paired t-test. The analysis showed that systolic blood pressure before therapy was 118.25 mmHg and after therapy became 111,00 mmHg, so there was a decrease of 7,25 mmHg with a p-value of 0,0001. The diastolic blood pressure before treatment was 81,25 mmHg and after therapy became 78,75 mmHg, so there was a decrease of 2,50 mmHg with a p-value of 0,002 mm. The pulse before therapy was 82,30 x / min and after therapy became 80,64 x / min, so there was a decrease of 1,66 x / min with ρ-value 0,003. There is a significant effect of giving a combination of hydrotherapy and Benson relaxation (hydrosol) on reducing blood pressure and pulse among 32 subjects in neighborhood 19 and 20, Sungapan V. Objective: To assess dental anxiety in patients undergoing oral surgery, as well as its impact on blood pressure and heart rate. Material and Methods: A total of 233 patients answered a socio-demographic questionnaire and another one based on the Corah dental anxiety scale. Blood pressure and heart rate were assessed at three moments while: patients were in the waiting room, immediately before and after the procedure. Results: This study revealed a prevalence of anxiety of 77.3%. There was a statistically significant difference in mean systolic blood pressure and heart rate at the three moments of the evaluation. Anxiety was prevalent in the sample and was observed from the time in the waiting room until the time when local anesthesia was performed, causing variations in systolic blood pressure and heart rate, anxiety levels decreased after the end of the service. In conclusion, we observed that oral surgery is directly related to increased anxiety, and anxiety is mainly related to the change in heart rate.

Objective To investigate the effects of Shaolin Ba Duan Jin on patients with hypertension. Methods In the hypertensive patients detected in the national physical fitness test of public officials in Guiyang City, 40 patients with hypertension were selected. The experiment was started after Shaolin Ba Duan Jin has been practiced for seven days and the subjects had learned it. Experimental method: Frequency of subjects practicing Ba Duan Jin is 7 days a week, subjects practice once a day, practicing time is 6:00-7:30 am or 18:00 -19:30 pm, practicing lasts 1.5 hours each time (practicing has 3 groups, 14 minutes in each group;subjects rest 2 minutes between groups,;preparing part is 8 minutes;ending part is 5 minutes), record the blood pressure of the subjects before and after each experiment, and fill in the quality of life scale (WHOQOL-BREF scale) regularly for a period of 12 weeks. Questionnaire method: Fill in the quality of life scale every three weeks and measure heart rate and blood pressure before and after each exercise. Mathematical statistics: The paired sample t test was used to analyze the changes in blood pressure before and after the 12-week experiment. Results 1) The systolic and diastolic blood pressures of the subjects were significantly lower than before the experiment (P <0.01). 2) The WHOQOL-BREF scale after 12 weeks has improved significantly in all areas than before the experiment. Conclusions 1) Martial arts Ba Duan Jin has a good influence on the circulatory system of hypertensive patients. The performance is that the heart rate is slow and the systolic and diastolic blood pressures have a downward trend. 2) Martial arts Ba Duan Jin has significant improvement in the physiology, psychology, social relations, environment and other fields .
