The paper is aimed at optimal treatment of posttraumatic cervical pathology. 192 patients with cervical erosive ectropion (67 patients with CIN I) have been observed. The results of the conventional treatment made according to the clinical protocols (control group; n = 64) have been compared with the results of the patient's management according to the proposed algorithm (main group; n = 128). The algorithm includes microsurgical intervention, prolonged use of combined oral contraceptives or dienogest during 6–12 weeks, non-steroid anti-inflammatory drugs immediately after excision, and regenerative agents not earlier than at the fourth week after the excision. The six months after the minor operation the positive therapeutic effect was 1,7 times higher in the main group. The frequency of complications was significantly lower: implant cervical endometriosis – 3,6 times, retention cysts – 4,2 times, abnormal scarring – 3,8 times, relapses of endocervical ectopia and CIN – 2,2 times, postcoagulation hemorrhage – 2,75 times, leukoplakia – 2,6 times. The use of integrated algorithm in the treatment of the cervical erosive ectropion is clinical reasonable.

Purpose: To determine the therapeutic effect of Xuezhitong capsule in patients with microvascular angina (MVA), and its impact on vascular endothelial function.Methods: In total, 172 MVA patients treated in Beijing City Fengtai District Nanyuan Hospital from September 2017 to September 2019 were selected and randomized into control group which received conventional treatment, and treatment group which received Xuezhitong capsules plus. There were 86 patients in each group. Therapeutic effect, levels of inflammatory factors, i.e., high-sensitivity C- reactive protein (hs-CRP), interleukin-(IL-6), tumor necrosis factor-α (TNF-α), and vascular endothelial factors such as nitric oxide (NO), thromboxane B2 (TXB2) and endothelin (ET), were determined.Results: Markedly higher total treatment effectiveness was observed in the treatment group than in the control group (89.53 % vs. 72.94 %; p < 0.05). In both groups, treatment reduced the levels of hs-CRP, IL-6, TNF-α, TXB2 and ET, but elevated NO, with better results for treatment group than the control group (p < 0.05). Better optimizations of total cholesterol (TC), triglycerides (TGs), low-density lipoprotein cholesterol (LDL-C) and high-density lipoprotein (HDL) were observed in the treatment group, relative to the control group (p < 0.05). Patients in the treatment group experienced fewer (8.14%) adverse reactions than those in control group (21.18 %, p < 0.05).Conclusion: Xuezhitong capsule, when combined with conventional treatment, exerts high therapeutic effectiveness and safety in MVA patients by inhibiting inflammatory reactions, optimizing endothelialfunction, reducing blood lipid levels, and decreasing the incidence of cardiovascular adverse events. Thus, the combination therapy is a potentially superior therapeutic strategy to the conventional approach for the management of MVA patients.

In the case of radicular syndrome in patients with dorsopathy, degenerative dystrophic changes, which contribute to the creation of additional mechanisms of pain impulse and chronization of the pathological process, are diagnosed in the deep muscles. The study included 40 patients with dorsopathy of the lumbosacral spine and radicular syndrome, with the average age of 45,3±1,5years, who were divided into 2 groups comparable in age and clinical and functional characteristics: the main group of 20 patients who were treated with laser phoresis of biologically active substances and the control group of 20 patients who received standard pharmacotherapy. As a result of the study, a significantly more pronounced improvement in the state of microcirculation in the paravertebral muscles when using phytoextracts laser phoresis was proved, which was confirmed by radiothermometry and transcutaneous oximetry. The analysis of radiothermometry and oximetry parameters obtained 3 months after the end of the course showed the stability of the therapeutic effect in 96% of patients in the main group, while in the control group, 50% of patients showed a regression of the effect obtained as a result of treatment. In longer terms, the therapeutic effect in 90% of patients of the main group was maintained for 9–12 months of follow-up, while in the control group — no more than 3–4 months.

Efficacy of platelet-rich autoplasma (PRA) in treatment of osteoarthrosis was evaluated. Prospective study included 120 patients (83 females, 37 males, mean age 52.8±4.5 years) with clinical and roentgenologic diagnosis of II-IV stage osteoarthrosis by Kellgren-Lawrence classification. The patients from the main group (n=60) were on courses of PRA intraarticular injections (3 injections with 1-2 weeks interval). In the control group the patients received nonsteroidal anti-inflammatory drugs (NSAIDs) and periodically injections of glucocorticoids when indicated. Results were assessed by the verbal efficacy scale, Lequesne index and the pain visual analog scale. Follow up period made up 6 months. Use of PRA enabled to eliminate pain syndrome and improve knee joint function without adverse effects. In 6 months the number of patients who noted the efficacy of treatment was 31% higher in the main group than in the control one. The obtained results allow recommending PRA use for knee osteoarthrosis treatment as an alternative to NSAIDs especially in contraindications to the latter.

The purpose of the study was to evaluate the effect of a physical therapy program on the state of functioning of the maxillofacial region in patients with arthrosis of the temporomandibular joint. Materials and methods. The study involved 33 people with arthrosis of the temporomandibular joint of the II-III degrees, which were divided into 2 groups – control and main. The control group consisted of 18 people who received arthrosis therapy only with non-steroidal anti-inflammatory drugs. The main group consisted of 15 people who additionally underwent the developed program of physical therapy (therapeutic exercises, massage, post-isometric relaxation, kinesiotaping of the masticatory muscles, neck muscles, cervical-collar zone; paraffin therapy course for the temporomandibular joint area, patient education). Complaints of patients, the intensity of pain in the temporomandibular joint on a visual analogue scale, the amount of mouth opening were determined, and auscultation of the joint was performed. Results and discussion. After the completion of the treatment course (for patients of the control group) and physical therapy (for the patients of the main group), pain in the temporomandibular joint did not manifest itself at rest and during movement. This is explained, in particular, by the fact that the basis of conservative treatment of arthrosis is the use of non-steroidal anti-inflammatory drugs with analgesic effects. At the same time, the usage of physical therapy, in comparison with medications, in the main group was statistically significant (p <0.05) in relation to control patients, it improved the functional activity of the jaw area and the functioning of the temporomandibular joint, which was manifested by a decrease in the number of persons with complaints of pathological sounds during jaw movements and their auscultatory detection, discomfort when chewing, as well as an increase in the magnitude of opening the mouth. Restoration of the condition of patients with diseases of the maxillofacial region, and, in particular, the temporomandibular joint, is an urgent problem not only for dentists, but also for specialists in the field of rehabilitation, in particular, physical therapists. The objectives of physical therapy in patients with arthrosis of the temporomandibular joint are to reduce the load on the joint, eliminate and reduce pain, synovitis, myalgia, improve joint function, stimulate and activate metabolic processes in cartilaginous and bone tissues, normalize blood flow, and thus improve quality of life. Conclusion. Physical therapy means improve the functional ability of the temporomandibular joint in the complex recovery of patients with arthrosis statistically significantly better than monotherapy with non-steroidal anti-inflammatory drugs

Objective: Since endometriosis is an estrogen-dependent disease; therefore, combined oral contraceptives (COCs) may not be the best choice for the treatment of endometriosis. The objective of the present study was to investigate the effects of ethinyl estradiol (EE) and desogestrel (DSG) in COCs on cell proliferation and apoptosis in ectopic endometrial tissue as compared to DSG alone. Materials and methods: Forty-five women of reproductive age with at least one endometriotic cyst were recruited into this single-blind randomized controlled trial study and randomly divided equally into three groups. EE-DSG and DSG groups received EE (0.03 mg) and DSG (0.15 mg) or DSG alone daily for 28-35 days before surgery. The control group was prescribed nothing. Endometriotic cyst tissues were collected during ovarian cystectomy for immunohistochemistry. Results: Levels of Ki-67 positive cells in the ectopic endometrial tissue of the EE-DSG group were significantly higher than the DSG group (median [IQR]; 1.4[1.2] vs 0.6 [0.7], P <0.016). There were significantly more TUNEL-positive cells in the EE-DSG group compared to the DSG group (median [IQR]; 2.8[0.7] vs 1.8[1.4], P < 0.016, respectively). Moreover, the number of TUNEL-positive cells in the EE-DSG and DSG groups were significantly higher than the control (median [IQR]; 2.8[0.7] vs 0.2[0.2] and 1.8[1.4] vs 0.2[0.2], P <0.016). The levels of cells that positively stained for Bcl2 were not different among all groups. Conclusion: Progestin alone increased cell apoptosis in ectopic endometria. However, concurrent EE in COCs enhanced proliferation and promoted a greater apoptotic effect in ectopic endometria compared to progestin alone.

The purpose. To study the clinical efficacy of plasmapheresis in combination with ozone therapy in the prevention of ocular herpes recurrence.Patients and methods. There were 108 patients with relapsing forms of eye herpes (herpes keratitis, keratouveitis, uveitis) under our observation. 56 men and 52 women, aged between 37 and 62 old (47,6 ± 11,5) were involved into the research. The number of patients included into the main group were 54 (54 eyes), and also 54 patients (54 eyes) were in the comparison group. Results. In the main group, the patients undergone a course of anti — relapse therapy with plasmapheresis in combination with therapeutic ozone in combination with traditional treatment from 54 persons — the expressed therapeutic effect was registered in 48-and (88.8 %) patients, the partial therapeutic effect (decrease in the frequency of relapses) was observed in 6 (11.1 %) patients, the absence of effect was not registered in any patient. In the control group, consisting of 54 patients, which undergone anti-relapse therapy with plasmapheresis in combination with traditional treatment, expressed therapeutic effect (no exacerbations) was observed in 31 (57.4 %) patients, a partial therapeutic effect (decrease in the frequency of relapses) was observed in 17 (31.5 %) patients, and the absence of effect — in 6 (11.1 %) patients.Conclusions. For prevention of ophthalmic herpes recurrence and optimal results of anti-relapse therapy of patients with recurrent ophthalmic herpes were shown concurrently with the beginning of the standard antiherpetic treatment courses of plasmapheresis with ozone therapy and spend it as monotherapy at the end of treatment.

Purpose: to study the effectiveness of seydana (black cumin) oil in combined treatment of herpetic endothelial keratitis. Material and methods. The study included 68 patients (68 eyes) with herpetic endothelial keratitis, developed after cataract surgery. 35 men and 33 women aged 39 to 73 (58.3 ± 8.9) years were divided into two equal groups. The main group (34 patients, 34 eyes) received conventional treatment supplemented with black cumin oil, while the comparison group of 34 patients (34 eyes) received conventional treatment alone.Results. Cornea infiltration in the main group resorbed sooner than in the comparison group (18.43 ± 0.3 days and 23.2 ± 0.5 days respectively; p < 0.05). The treatment lasted shorter in the main group compared with the comparison group (22.1 ± 0.1 days vs. 25.2 ± 0.3 days; p < 0.05). Complete epithelialization was averagely noted on the 19th to the 21st day of treatment (respectively, 19.6 ± 0.9 and of 21.12 ± 0.6; p > 0.05). Visual acuity improved to achieve, respectively, 0.51 ± 0.24 and 0.44 ± 0.17 (p > 0.05).Conclusion. The patients who received seydana oil in addition to conventional treatment showed a significantly higher therapeutic effect in some parameters (resorption time of corneal infiltration and duration of treatment) than the comparison group.

Objective: To investigate the clinical effect of ambroxol in the treatment of asthmatic bronchitis. Methods: One hundred and twenty patients with asthmatic bronchitis who were admitted to our hospital from June 2017 to August 2018 were selected as the research subjects and divided into a control group and an observation group according to random number table method, 60 in each group. The control group was treated with conventional treatment, while the observation group was treated with ambroxol in addition to conventional treatment. The therapeutic effect, disappearance time of symptoms and signs and the recovery of pulmonary function were compared between the two groups. Results: The total effective rate of the observation group was 96.7%, and that of the control group was 73.3%. The control effect of the observation group was significantly better than that of the control group, showing a significant difference (P<0.05). The disappearance time of symptoms of the observation group was shorter than that of the control group, and the recovery of pulmonary function was better; the differences were statistically significant (P<0.05). Conclusion: For asthmatic bronchitis patients, addition of ambroxol to conventional treatment can improve the therapeutic effect, shorten the disappearance time of clinical signs and symptoms, and promote the recovery of patients, which is worth clinical application. doi: https://doi.org/10.12669/pjms.36.3.1607 How to cite this:Du X, Zhao C, Liu S, Su S. The article considers the influence of combined oral contraceptives on size and function of thyroid gland in women of reproductive age. We examined 215 women, taking low dose and microdose combined oral contraceptives for 12 months as monotherapy or in combination with potassium iodide. The first group consisted of 75 women with diffuse nontoxic goiter, second – of 70 women with autoimmune thyroiditis. Control group consisted of 70 women without pathology of thyroid gland. Prior to therapy with combined contraceptives and after 12 months of treatment, we determined serum levels of free triiodothyronine, free thyroxine, and thyrotropin releasing hormone, antibodies to thyroid peroxidase and antibodies to thyroglobulin (ELISA) and performed sonography of thyroid gland. Results of our study suggest that combined oral contraceptives may increase the size of thyroid gland among women with diffuse nontoxic goiter without changing its function. In women with autoimmune thyroiditis, application of combined oral contraceptive significantly decreases blood levels of autoantibodies to thyroid peroxidase and thyroglobulin. Application of combined oral contraceptives in healthy women doesn’t influence the size and function of thyroid gland.
