Background: Prehospital emergency department provides land, air, boat, and rail ambulance care. This study aimed to compare the situation of air and ground emergency casualties in road traffic accidents transferred to Shahid Beheshti Medical Center in Qom City, Iran, 2015-2018. Materials and Methods: The present study is a retrospective descriptive-analytical study. All road traffic accident victims transferred to Shahid Beheshti Medical Center in Qom by prehospital emergency land or air staff from 2015 to 2018 at 6 to 20 o’clock were included in this study (census method). The exclusion criteria included burns, death, transmission during cardiopulmonary resuscitation or by urban ambulances, and information deficiencies. We used a researcher-made checklist to examine victims’ age, sex, type of transmission, type of accident, type of trauma, distance, initial vital signs, time intervals, and day of the accident. SPSS software version 16 was used to analyze the data. Data were compared in two groups with the t test and the Chi-square test. Results: A total of 2057 casualties were included in the study: 566 casualties were transferred by air emergency and 1491 by ground emergency. The mean age of air emergency casualties was significantly lower than the ground emergency (P=0.008). Trauma to the neck, chest, abdomen and lower back was significantly higher in air emergency casualties, but hand and foot trauma were more common in ground emergency casualties. Most air missions took place on holidays or weekends, while most ground missions were done during weekdays (P<0.001). The mean distance of the air emergency missions was more than Beheshti Medical Center (P<0.001). Compared with ground emergency casualties, air emergency casualties had average systolic blood pressure, lower level of consciousness, and higher heart rate and respiration (P<0.001). The mean duration of transfer from the accident scene to the medical center in the air emergency was significantly shorter (P<0.001). Conclusion: Air emergency casualties were usually accompanied by more critical vital signs and severe injuries to the neck, chest, abdomen, and back. This study can provide clinical triage criteria that focus on key environmental factors and reduced transport time. Further studies are needed to investigate the consequences of traffic accident casualties at the medical center to determine which subgroups will benefit most from using air emergency.

Background: Chest trauma is responsible for 50% of deaths due to trauma. This kind of death usually occurs immediately after the trauma has occurred. Various therapeutic options have been reported for management of chest injuries like clinical observation, thoracocentesis, tube thoracostomy and open thoracotomy. Objective: To observe the pattern and outcome of management in chest trauma Methods: This is an observational study carried out in Casualty department of Chittagong Medical College Hospital (CMCH), Chittagong, between April 2015 to March 2016. Our study was included all patients, both sexes, following chest injury at Casualty units of Chittagong Medical College Hospital. All the data were recorded through the preformed data collection sheet and analyzed. Result: The mean age was found 37.7±18.1 years with range from 12 to 80 years. Male female ratio was 11.8:1. The mean time elapsed after trauma was found 6.1±3.1 hours with range from 1 to 72 hours. Almost one third (35.7%) patients was affecting road traffic accident followed by 42(27.3%) assault, 35(22.7%) stab injury, 15(9.7%) fall and 7(4.5%) gun shot . More than three fourth (80.5%) patients were managed by tube thoracostomy followed by 28(18.2%) observation and 2(1.3%) ventilatory support. No thoracotomy was done in emergency department. 42(27.2%) patients was found open pneumothorax followed by 41(26.6%) rib fracture, 31(20.1%) haemopneumothorax, 14(9%) simple pneumothorax, 12(7.8%) haemothorax, 6(3.9%) chest wall injury, 5(3.2%) tension pneumothorax, and 3(1.9%) flail chest. About the side of tube 60(39.0%) patients were given tube on left side followed by 57(37.0%) patients on right side, 9(5.8%) patients on both (left & right) side and 28(18.2%) patients needed no tube. Regarding the complications, 13(30%) patients had persistent haemothorax followed by 12(29%)tubes were placed outside triangle of safety, 6(13.9%) tubes were kinked, 6(13.9%) patients developed port side infection, 2(4.5%)tube was placed too shallow, 2(4.5%) patients developed empyema thoracis and 2(4.5%) patients developed bronchopleural fistula. The mean ICT removal information was found 8.8±3.6 days with range from 4 to 18 days. Reinsertion of ICT was done in 6(4.7%) patients. More than two third (68.2%) patients were recovered well, 43(27.9%) patients developed complication and 6(3.9%)patients died. More than two third (66.9%) patients had length of hospital stay 11-20 days. Conclusion: Most of the patients were in 3rd decade and male predominant. Road traffic accident and tube thoracostomy were more common. Open pneumothorax, rib fracture and haemopneumothorax were commonest injuries. Nearly one third of the patients had developed complications. Re-insertion of ICT needed almost five percent and death almost four percent. Journal of Surgical Sciences (2018) Vol. 22 (2) : 110-117

Background: In Bangladesh fatality rate due to road traffic accidents is rising sharply day by day. At least 2297 people were killed and 5480 were injured in road traffic accidents within 1st six months of 2017.Whereas in the previous year at 2016 at least 1941 people were killed and 4794 were injured within the 1st six months. No survey has been reported in Bangladesh yet correlating ADHD as a reason of impulsive driving which ends up in a road crash.

9238 traffic accidents data are collected in rural road of China. Through the data analysis, the main causes of rural road traffic accident are presented. The external environment, the participant features, road features and accident characteristics are involved. The regression analysis in SPSS is applied to find the relationship between the accident features. Overall, the rural road traffic accident was mainly due to in the rural area there are mass travel mode, lower grade roads, poorer safety awareness of traveler and the road is lack of traffic safety facilities and so on.

Introduction: Road traffic accidents (RTA) are the eighth leading cause of death globally, and the leading cause of death in Bangladesh. The road safety situation in Bangladesh has been deteriorating with increasing number of road accident deaths, largely as direct consequences of rapid growth in population, motorization, urbanization and lack of investment in road safety. Objectives: To describe the injury characteristics of road traffic accidents cases that reported at Combined Military Hospital (CMH), Dhaka. Materials and Methods: This descriptive cross-sectional hospital based study was conducted on all the RTA cases reported to medical inspection room CMH, Dhaka from 01 March to 30 June 2010. A total of 126 RTA victims irrespective of age and sex were included, data were collected by face to face interview using a pre-tested questionnaire. Results: A total of 126 road traffic accident victims both sexes were studied. The majority of the study subjects were men 103 (81.7%) and most of them were in 31-40 years group (27%), Muslims (98.41%) were the majority. In our study regarding educational qualification of RTA victims most of them were SSC qualified (44’4%) and primary level (15.1%). Military services holder (45.2%) was the majority of road traffic crash victims. Bus/Minibus (32.5%) was responsible for the majority of road traffic crashes and motor vehicle passengers (42.9%) were main victims. Laceration & cut injury were (38.1%) and multiple injury (18.3%) were the most common injury. Main roads (49.2%) were the commonest site of the accidents.  Regarding place of occurrence main roads 62 (49.2%) was the most common site and noon 41 (32.5%) was the most common time of occurrence of accidents. Conclusion: Road traffic crashes constitute a major public health problem in our setting and contribute significantly to unacceptably high morbidity and mortality. Urgent preventive measures targeting at reducing the occurrence of road traffic crashes is necessary to reduce the morbidity and mortality resulting from these injuries. Introduction: To date, no epidemiological studies on jaw fracture have examined its characteristics according to hospital status (public vs private). The aim of this study was to examine their differences in terms of patient- and injury-related variables. Methods: A retrospective study was conducted using patients' medical records in 2 types of hospital located in the northern area of Brittany (France). All patients hospitalized for jaw fracture between 2006 and 2017 were eligible. Comparisons between centres according to age, gender, anatomic location of fracture, mechanism of injury, and length of stay were undertaken using χ2, Fisher exact test, t-test, and Cochrane-Armitage trend test. Results: A total of 142 patients were included: 45.1% in the public and 54.9% in the private hospitals. Of them, 84.5% were men and the mean age was 32.0. Main causes of fractures were assault (56.3%), fall (30.3%) and road traffic accident (10%). A small number of patients had polytrauma (4.2%). The mean duration of stay was less than 2 days. Compared to patients who were admitted in private hospital, those who were likely to be admitted in public hospital were those: who had a jaw fracture in context of road traffic accident (18.8% vs 2.6%), with polytrauma (9.4% vs 0.0%) and maxilla fracture (9.4% vs 1.3%), and who had a longer stay (2.2 vs 1.1 days). Conclusion: This study showed that differences observed in characteristics of jaw fractures between public and private hospitals may be due to recruitment process: more severely affected patients are likely to be admitted in the public hospital.

Road traffic accidents are challenges the world is battling in recent times. Majority of increasing road fatalities occurs in developing countries. This study seeks to address part of the issues leading to mechanical failures and road traffic accidents in the developing countries with Ghana as a case study. The study model was validated with the use of a sample collected from 880 mechanics from six regions. The mechanics completed questionnaire measures of perception on used parts, brand new parts, mechanical failure, road accidents and demographic variables. In addition, observation and survey method were also used to collect the data on work quality. The result shows that mechanical failure and road traffic accident correlated positively with used parts and negatively correlated with brand new parts. The study model explained R2 = 0.46 and 0.65 of the variance in mechanical failure and road traffic accident respectively. The independent predictors of mechanical failure were age, gender, experience, perception about used parts and work quality. The predictors of road traffic accident were experience, mechanical failure, perception about brand new parts and work quality. We conclude that the mechanical failure and road traffic accidents could be attributed to non-use of genuine parts, poor work quality standard and poor supervision during and after repairs. Remedial measures addressing these issues could reduce road traffic accident in Ghana.

In this study, road traffic injury (RTI) was defined as any injury resulting from a road traffic accident irrespective of severity and outcome. Road traffic accident (RTA) was defined as any crash on the road involving at least one moving vehicle, irrespective of it resulting in an injury. This could include collision with a vehicle or any non`moving object while driving/riding a vehicle, collision with a moving vehicle while walking/running/standing/ sitting on the road, or fall from a moving vehicle. The burden of road traffic accidents (RTA) is a leading cause of all trauma admissions in hospitals worldwide. Road traffic injuries cause considerable economic losses to victims, their families, and to nations as a whole. These losses arise from the cost of treatment (including rehabilitation and incident investigation) as well as reduced/lost productivity (e.g. in wages) for those killed or disabled by their injuries and for family members who need to take time off work (or school) to care for the injured. Road traffic fatality in the Kingdom of Saudi Arabia (KSA) is the highest, accounts for 4.7% of all mortalities. Road injuries also are reported to be the most serious in this country, with an accident to injury ratio of 8:6. In this study, we try to focus on some causes of the accidents in KSA, so we can implement the prevention plan.

Purpose: Traumatic Brain Injury (TBI) is still considered as a leading cause of morbidity and mortality of victims by traffic accident. Despite a fact that many measurements and preventions have been applied, the rate of TBI is remained high. Aim of this study was to investigate treatment process of TBI caused by road traffic accident at Viet Duc Hospital after the Decree 100 which has been issued in Vietnam in early 2020. Materials and method: A retrospective and prospective study has been conducted in Viet Duc Hospital from Dec, 2020 to March, 2021. All the patients with TBI by road traffic accident were enrolled. Severity of TBI was identified by Glasgow Scale (GCS) and BAC (Blood Alcohol Concentration) test taken on arrival. The data was collected from medical record as well as autopsy report and analysed by SPSS.20.0 Results: A total of 150 patients with TBI by road traffic accidents were enrolled, age group from 21 to 60 years old accounted for 64%, male accounted for 86.7%. Associated injuries were maxillofacial lesions 48%, extremities 24.7%, chest accounted for 20%. Severity of TBI with GCS 6 - 8 was the highest rate 52%, from 3 - 5 GCS accounted for 30%. 40% were operated on emergency; BAC was positive 46.7%, of which over from 50 mg/L accounted for 32.6%. The patients with GCS from 6 to 8 were BAC positive accounted for a higher rate than groups with GCS above 9 or below 5. The most common TBI lesions were subarachnoid hemorrhage, subdural hematoma and cerebral edema accounted for 67.3%, 60% and 58% respectively. Overall mortality was 26% including in-hospital death and discharged to die. Conclusions and recommendations: Data from the study has shown that many young men with severe TBI by road traffic accident were BAC positive which can results in the high risk of death and disability. Thus, we highly recommend counterparts should constantly strengthen the propaganda and enforcement measures in order to reduce traffic accidents and TBI patients.
