ABSTRACT: FULFILLMENT OF URINE INCONTINENCE NEEDS FOR ELIMINATION WITH KEGEL EXERCISE AT BINJAI ELDERLY SOCIAL SERVICE UPT Background: Urinary incontinence means the spontaneous expulsion of urine at any time against the will (involuntary). Elderly is the process of slowly losing the ability of tissues to repair themselves/replace and maintain their normal function.Purpose: Kegel exercises are exercises to strengthen the pelvic muscles or exercises that aim to strengthen the pelvic floor muscles, especially the pubococcygeal muscles so that a woman can strengthen the urinary tract muscles.Methods: This research method is descriptive with a nursing process approach. The subject of the study was conducted on 2 patients with the same case, namely in patients who experienced urinary incontinence.Results: The results of the urinary incontinence assessment were associated with decreased bladder and detrusor muscle tone. The nursing interventions and implementation are monitoring the pattern and ability to urinate, identifying the causes of urinary incontinence, explaining the definition, types, and causes of urinary incontinence, recommending limiting fluid consumption 2-3 hours before bedtime, encouraging clients to do Kegel exercises and collaborating with medical and physiotherapists to overcome urinary incontinence. Kegel exercises were carried out in stages to the patient 2 days a day, morning and evening.Conclusion: The evaluation was carried out for 3 days and the results were that the patient was able to urinate and was controlled. The research is expected that the elderly UPT Binjai will improve more skills and always coordinate with other health teams in providing nursing care in accordance with Standard Operating Procedures (SOP). Keywords: Elimination Needs, Urinary Incontinence, Kegel Exercises INTISARI: PEMENUHAN KEBUTUHAN ELIMINASI PADA INKONTINENSIA URINEDENGAN KEGEL EXERCISE DI UPT PELAYANAN SOSIAL LANJUT USIA BINJAI Latar Belakang: Inkontinensia urin berarti pengeluaran urin secara spontan pada sembarang waktu diluar kehendak (involunter). Lansia adalah proses hilangnya secara perlahan-lahan kemampuan jaringan untuk memperbaiki diri/mengganti dan mempertahankan fungsi normalnya.Tujuan: Senam kegel adalah senam untuk menguatkan otot panggul atau senam yang bertujuan untuk memperkuat otot-otot dasar panggul terutama otot pubococcygeal sehingga seorang wanita dapat memperkuat otot-otot saluran kemih.Metode: Metode penelitian ini adalah deskriptif dengan pendekatan proses Keperawatan. Subjek penelitian dilakukan pada 2 orang pasien dengan kasu yang sama yaitu pada pasien yang mengalami inkontinensia urin.Hasil: Hasil pengkajian inkontinensia urin berhubungan dengan penurunan tonus kandung kemih dan otot detrusor. Intervensi dan implementasi keperawatanya itu memonitor pola dan kemampuan berkemih, mengidentifikasi penyebab inkontinensia urine, menjelaskan definisi, jenis dan penyebab inkontinensia urine, menganjurkan membatasi konsumsi cairan 2-3 jam menjelang tidur, menganjurkan klien untuk senam kegel dan melakukan kolaborasi dengan medis dan fisioterapi untuk mengatasi inkontinensia urine. Senam kegel dilakukan secara bertahap kepada pasien 2 hari sehari pagi dan sore hari.Kesimpulan: Evaluasi dilakukan selama 3 hari dan  didapatkan hasil yaitu pasien dapat berkemih dan dikontrol. Penelitian diharapkan UPT lanjut usia Binjai meningkatkan keterampilan yang lebih dan selalu berkoordinasi dengan tim kesehatan lainnya dalam memberikan asuhan keperawatan sesuai dengan Standart Operasional Prosedur (SOP). Kata Kunci: Kebutuhan Eliminasi, InkontinensiaUrin, Senam Kegel

Abstrak:Inkontinensia urine menjadi salah satu masalah yang meluas dan merugikan terutama bagi lansia. Salah satu bentuknya seperti lansia akan merasa rendah diri karena selalu basah akibat urine yang keluar, mungkin pada saat batuk, bersin, mengangkat barang berat dan ketidakmampuan menahan buang air kecil. Senam Kegel yang merupakan terapi non farmakologi untuk mengatasi inkontinensia urin. Kegiatan pengabdian yang dilakukan bertujuan untuk melatih lansia untuk melakukan senam Kegel sehingga dapat mengurangi gejala inkontinensia urin. Kegiatan dilakukan dengan pemberian edukasi, latihan senam Kegel, dan pendampingan selama tiga pertemuan. Hasil kegiatan menunjukkan bahwa mitra mengalami penurunan ketidakmampuan menahan kemih dari rerata 8,56 menjadi 6,3 dan penurunan frekuensi berkemih dari rerata 2,07 menjadi 1,44 serta rerata skor total inkontinensia urine menurun dari 10,63 menjadi 7,74. Abstract:Urinary incontinence is a widespread and detrimental problem especially for the elderly. One form such as the elderly will feel inferior because it is always wet due to urine coming out, maybe when coughing, sneezing, lifting heavy objects and the inability to resist urination. Kegel exercises which is a non-pharmacological therapy for overcoming urinary incontinence. Community service activities was aimed to train elderly for doing Kegel exercise thus can reduce symptoms of urinary incontinence. Service were carried out by providing education, Kegel exercises, and mentoring for three meetings. Introduction: The 200 million people in the world experience urinary incontinence. In general, with increasing age, the capacity of the bladder decreases. The remaining urine in the bladder tends to increase and irregular contraction of the bladder muscles becomes more frequent. This situation often makes the elderly experience problems with the fulfillment of urine elimination needs, namely urinary incontinence. Efforts to overcome urinary incontinence in the form of bladder training programs are aimed at developing muscle tone and bladder sphincter so that it can function optimally. Objective: Knowing the profile of muscle strength and physical performance with the incidence of urinary incontinence in elderly women. Methods: This research was a retrospective descriptive study based on the results of filling out the questionnaire and evaluating it directly. Based on tabulated data, 38 elderly women were selected as the study sample. Results: There were (42.1%) urinary incontinence on elderly with very poor left hand muscle strength and (50%) urinary incontinence on elderly with very poor right hand muscle strength. In physical performance with urinary incontinence as many as (57.8%) were normal and (42.1%) were poor. Conclusions: Muscle strength and physical performance can be used as a measurement to early detection of the urinary incontinence. Cilj istraživanja je da se dokaže djelotvornost programa Kegelovih vježbi na liječenje blage do umjerene stresne urinarne inkontinencije kod žena. Istraživanje je obuhvatilo 43 pacijentice, liječene u konsultativno-specijalističkoj ambulanti Javne zdravstvene ustanove Dom zdravlja Laktaši. Pacijenticama su uzeti: opšti podaci, indeks tjelesne mase, ginekološka anamneza, socijalna anamneza, svim pacijenticama je urađen ginekološki pregled na dan javljanja u konsultativno-specijalističku ambulantu, procijenjen defekt prednje stijenke rodnice prema POP-Q klasifikaciji. Bolji uspjeh u liječenju inkontinencije smo postigli kod žena u pre i perimenopauzi, u odnosu na žene u postmenopauzi. Kod 14 (35%) pacijentica nismo dobili povlačenje tegoba, izazvane inkontinencijom. Takođe, kod 10 (25%) pacijentica koje su rađale krupniju djecu program Kegelovih vježbi nije dao rezultate. Kod 30 (75%) pacijentica koje su imale visoku, višu ili srednju stručnu spremu nakon odrađenih Kegelovih vježbi, smo dobili prestanak inkontinencije. Kod 10 (25%) pacijentica koje su bile domaćice i obavljale teške fizičke poslove nije došlo do oporavka od inkontinencije.Na osnovu dobijenih rezultata, zaključili smo da su Kegelove vježbe efikasan način liječenja blage i srednje teške stres urinarne inkontinencije, ali da na efikasnost utiču brojni faktori, kao što su starosna dob žena, tjelesna težina, obrazovanje, broj poroda, težak fizički rad.

Objective: To survey in the literature the role of the nurse in the care of people with urinary incontinence (UI). Methods: Integrative literature review from the publications within the period of 2013 in the databases: BDENF, LILACS, Web of Science, PubMed and SciELO, in the Portuguese, English and Spanish languages. The sample consisted of 11 publications, most of them in English. Results: The data were categorized into three dimensions of care: educational (three articles), care (eight articles) and management (an article, which was also categorized as care). Educational: although nurses presented superficial and incomplete knowledge about incontinence, playful strategies were used for teaching. Assistance: the use of superabsorbent diapers and pads, 3% dimethicone impregnated wipes, spray for the formation of polymer film for incontinence-associated dermatitis, pelvic musculature, urethral massage, bladder training, and eating habits modification were recommended. Gerencial: the quality of nursing care for the elderly with UI was evaluated through protocols, standards and Standard Operational Procedures (SOPs). Conclusion: There is a shortage of studies on the subject and it was evidenced that the nurse assumes an assistance role in urinary incontinence, although its knowledge is incipient.

<p><em>Because of the increasing age, it causes the changes of anatomy and function of the urinary organs caused by weakening of the pelvic muscles. About 12 million people, 34% occurred in men and 60% in women. Indonesia 32.2% in the Elderlies suffers urinary incontinence. The aims of this study was to determine the effect of Kegel Exercise toward urinary in continence on the elderlies.The design of this study is Quasy experiment with design of one group pretest-posttest. This research was conducted in January-August 2015. The population in this research was 110 Elderlies, based on inclusion criteria were 12 people taken by total sampling techniques. The Data was collected by using questionaire and checklist sheets. Data were analyzed by using univariate and bivariate statistical dependent T-test with Shapiro Wilk normality test. The result showed that the frequency of urinary incontinence in the elderlies before doing Kegel exercises had the average about 10.58 with deviation 2,065. Freqency of urinary incontinence in the elderlies after doing Kegel exercises had the average about 8.00 with standard deviation 2,796. After doing the statistical, there is the effect of urinary incontinence before and after doing Kegel exercises in the elderlies. Kegel exercises can reduce urinary incontinence. It can be applied as the daily activities at social institution every day. </em></p>

Urinarsy incontinence involves difficulty in retaining urine, making it a problem with multiple consequences. The purpose of this chapter is to encourage reflection on this situation and identify potential action with a view to promoting self-care in the elderly with urinary incontinence. The authors undertook an ethnographic survey in convalescence units in which they observed and interviewed elderly patients with urinary incontinence and healthcare professionals. Implementing an “observation, reporting, and reflection” approach, they carried out a content analysis. Three cultural dimensions emerged affecting self-care. Functional dependence and vulnerability led to an impaired attitude to the body, which constituted the primary challenge in the process. The promotion of self-care was marked by difficulties, strategies, and modes of action where the nurses played a significant role in liaison with the multidisciplinary team. The preparation for discharge was marked by points of contention and negotiations that decided whether the patient would return home or be the subject of referral.

Introduction: BTL EMSELLA™ utilizes High-Intensity Focused Electromagnetic technology (HIFEM) to cause deep pelvic floor muscles stimulation and restoration of the neuromuscular control. Key effectiveness is based on focused electromagnetic energy, in-depth penetration and stimulation of the entire pelvic floor area. A single BTL EMSELLA™ session brings thousands of supramaximal pelvic floor muscle contractions, which are extremely important in muscle reeducation of incontinent patients. Objective: Prospective study to evaluate the safety and preliminary effectiveness of the use of BTL EMSELLA magnetic stimulation in urinary incontinence. Method: Thirty-two patients with light and moderate urinary incontinence were recruited to perform 6 sessions of BTL EMSELLA during three weeks of initial treatment. Follow-up after three months. The patients received sessions lasting 28 minutes, completing the different treatment protocols. Initially the patients underwent a quality of life test before and after treatment, evaluation with advanced ultrasound using elastography to measure the initial tissue's elasticity and be able to compare after treatment, clinical functional evaluation and urodynamic test. Results: No adverse reactions were observed. All the patients finished the treatment sessions. Two patients reported increased pain after treatment in the first session corresponding to a VAS scale greater than 5 with duration greater than three hours. The treatment was highly satisfactory in 84,4% of the patients. After the first three months the improvement was maintained in 77% of the patients. No muscle injuries were observed. Elastographic changes and improvement of muscle tone were detected by advanced ultrasound (elastography) in 100% of patients. Conclusions: BTL EMSELLA is safe, well tolerated and effective for the treatment of mild and moderate urinary incontinence. The observed elastographic changes demonstrate the improvement of pelvic floor muscle tone after treatment. A reduction in the symptoms of urinary incontinence was demonstrated. Recommendations: Continue increasing the number of cases for research and increase the variables that we have decided to incorporate in the next research section such as MRI and pressure calculation.

Intoxication as a ground to set aside a contract is not something that has proved to be easy for the law to regulate. This is perhaps not very surprising. Intoxication is a temporary condition of varying degrees of magnitude. Its presence does however raise questions of contractual autonomy and individual responsibility. Alcohol consumption is a common social activity and perceptions of intoxication and especially alcoholism have changed over time. Roman law is surprisingly quiet on the subject. In modern times the rules about intoxicated contracting in Scottish and English law is very similar. Rather more interestingly the law in these two jurisdictions has reached the current position in slightly different ways. This history can be traced through English Equity, the works of the Scottish Institutional writers, the rise of the Will Theory, and all leavened with a dose of judicial pragmatism.

Freedom is the focus of the first of the writings included in The Two Fundamental Problems of Ethics. The attention that Schopenhauer devotes to the subject does not stop here, however, since freedom appears recurrently in different parts of his system. It is linked to his theory of knowledge, metaphysics, aesthetics, and the denial of the will. This chapter follows that track and examines the presence in different contexts of Schopenhauerian thought of a freedom that is so undeniable as unexplainable. In this way will be shown Schopenhauer’s transition from the freedom of the voluntas to the freedom of noluntas [non-willing] and the state of great liberation that occurs because the will frees itself from itself. It is a transition that begins and ends at the same point: mystery
