Background: Liver involvement in dengue infection is one of the clinical presentations. Objective: This study was undertaken to see severity of liver dysfunction in children with dengue infection. Methods: It is a cross sectional observational study conducted in 100 consecutive children with serologically positive dengue infection aged between 2 months to 14 years of age. Liver function tests were done in all patients and were studied the severity of deranged LFT with severity of dengue infection. Results: Out of 100 admitted children with dengue infection, deranged liver function tests found in 57(57.0%). ALT was above normal in 10 (29.4% of DF, 19 (57.6%) cases of DHF and 19(57.6%) cases of DSS respectively. AST was above normal level in 12(35.3%) cases of DF, 23(69.7%) cases of DHF and 22(66.7%) cases of DSS respectively. Low albumin was found in 41.0% of cases, 15(45.5%) cases of DHF and 19(57.6%) cases of DSS. INR was high only 4 cases of DSS. The bilirubin was normal in all cases except one died due to ALF due to DSS. The mean ALT and mean AST statistically higher in DHF and DSS in compare to DF (P-value 0.011 and 0.037) respectively. However, there was no significant difference in meal ALT and AST between DHF and DSS respectively. Overall mortality in 2 cases with DHF and DSS. Conclusion: Dengue infection is associated with variable level of liver dysfunction. The severity of liver dysfunction can predict the severity of dengue infection. Cirrhosis of liver is a chronic process characterized by structurally abnormalnodules in the liver, with diffuse generalized increased fibrous tissue in the liver. Spontaneousbacterial peritonitis(SBP) is a serious complication occurring in 10 to 30% of patients hospitalizedwith cirrhotic ascites. Objective: Assess the frequency of SBP in asymptomatic patients of livercirrhosis. Study design: Cross sectional study. Setting: Hepatitis clinic medical unit- 1 Serviceshospital, Lahore. Duration of study: One year. Sample size: Total 100 patients of cirrhoticascites. Results: Ten patients were found to have SBP (10%, 95% CI, 4.1 to15.9) and 90 patientswere found to have no SBP (90%, 95%CI, 84.1 to 95.9). Seven SBP patients (70%) had pyurea(Fisher’s exact test p- value< 0.01), and abnormal liver function tests (Chi square-value<0.01).Conclusions: We concluded that 10% of asymptomatic patients with cirrhotic ascites had SBP.SBP patients had significantly abnormal liver function tests and pyuria.

The present study was conducted in Khartoum state (Sudan) as a cross-sectional study in 200 participants (100 smokers and 100 non-smokers), to investigate the effects of Cigarettes Smoking on Liver functions of Sudanese smokers. The smokers were categorized into either current smokers (70 participants) or past smokers (30 Participants). Parameters were analyzed by using spectrophotometer. From the results, the levels of liver function tests (TB, Albumin and globulin) were significantly higher in non-smokers (6.0, 3.6 and 2.43 g/dl) as compared to current smokers (4.4, 2.4 and 1.98 g/dl) and past smokers (4.7, 2.7 and 1.93 g/dl). Meanwhile, ALT was significantly increased in current and past smokers as compared to non-smokers, where as AST was significantly higher in current smokers as compared to non-smokers, while there was no significant difference between current and past smokers as well as between past and non-smokers for this parameter. In addition, heavy and long duration of smoking were both associated with low total protein, albumin and AST levels .Also liver function tests were not significantly differing in current smoker's non-diabetics and non-smokers diabetics except total protein. From the results when, liver enzymes raised that may lead to increase proteins metabolism and increase blood glucose. These signs may place cigarettes smoking one of the top causes of hepatitis, pancreatitis, insulin resistance and finally diabetes mellitus type II. On the other hand, smoking cessation in diabetics lead to decrease the risk of hepatitis, Also regular physical activity and using medication regularly in diabetics lead to the same actions above; in addition diabetes complications were decreased.

ObjectivesLiver disease is a major cause of morbidity and mortality in sub-Saharan Africa, but its prevalence, distribution and aetiology have not been well characterised. We therefore set out to examine liver function tests (LFTs) and liver fibrosis scores in a rural African population.DesignWe undertook a cross-sectional survey of LFTs. We classified abnormal LFTs based on reference ranges set in America and in Africa. We derived fibrosis scores (aspartate aminotransferase (AST) to Platelet Ratio Index (APRI), fibrosis-4, gamma-glutamyl transferase (GGT) to platelet ratio (GPR), red cell distribution width to platelet ratio and S-index). We collected information about alcohol intake, and infection with HIV, hepatitis B virus (HBV) and hepatitis C virus (HCV).SettingWe studied a population cohort in South-Western Uganda.ParticipantsData were available for 8099 adults (median age 30 years; 56% female).ResultsThe prevalence of HBV, HCV and HIV infection was 3%, 0.2% and 8%, respectively. The prevalence of abnormal LFTs was higher based on the American reference range compared with the African reference range (eg, for AST 13% vs 3%, respectively). Elevated AST/ALT ratio was significantly associated with self-reported alcohol consumption (p<0.001), and the overall prevalence of AST/ALT ratio >2 was 11% (suggesting alcoholic hepatitis). The highest prevalence of fibrosis was predicted by the GPR score, with 24% of the population falling above the threshold for fibrosis. There was an association between the presence of HIV or HBV and raised GPR (p=0.005) and S-index (p<0.001). By multivariate analysis, elevated LFTs and fibrosis scores were most consistently associated with older age, male sex, being under-weight, HIV or HBV infection and alcohol consumption.ConclusionsFurther work is required to determine normal reference ranges for LFTs in this setting, to evaluate the specificity and sensitivity of fibrosis scores and to determine the aetiology of liver disease.

Prospective studies concerning liver disease in pediatric cystic fibrosis patients are scarce. The present study aimed to describe the prevalence and clinical expression of cystic fibrosis - related liver disease, in a cohort of 62 pediatric patients. Descriptive study, resulting from the prospective evaluation, between 1994 and 2009, of 62 pediatric patients (age &lt;18 years) with cystic fibrosis. The follow-up protocol included a clinical assessment every 2 months, liver function tests every 6 months and annual liver ultrasonography. The cumulative prevalence of liver disease was 11.2% (7/62 cases). All patients had ΔF508 mutation and pancreatic insufficiency, none had meconium ileus. The liver involvement became clinically evident at a mean age of 8 years (3-15 years), revealed by hepatomegaly or hepatosplenomegaly (3 cases) and/ or abnormalities of liver function tests (3 cases) changes of liver ultrasound (7 cases) with evidence of portal hypertension (2 cases). Four patients were submitted to liver biopsy; biliary fibrosis was documented in one case, focal biliary cirrhosis in 2 cases and multilobular cirrhosis in another case. Within a median 11.6 years follow-up period (all patients under UDCA therapy after liver disease diagnosis), progression of liver disease was observed in 2 patients; one patient developed refractory variceal bleeding and progressive hepatic failure, requiring liver transplant. The results of the present study agree with those of previous pediatric studies, further documenting clinical expression of liver disease in CF patients, which is usually detected in the first decade of life and emphasize the contribution of ultrasound to early diagnosis of liver involvement. Moreover, although advanced liver disease is a relatively rare event, early isolated liver transplantation may have to be considered at this age group.

Chronic active hepatitis (CAH) and chronic persistent hepatitis (CPH), the two histologically distinct forms of viral hepatitis present with variable clinical features. So liver function tests play the key role in diagnosis and prognosis of these disorders. Among the liver function tests, determination of serum aminotransferases: aspartate aminotransferase (AST), alanine aminotransferase (ALT) and AST/ALT ratio are still popularly used. In fact over the transaminase tests no other enzymatic tests have any particular value. In infiltrative disorders there is damage to both mitochondrial and cytoplasmic membranes. Therefore there is a proportionally greater increase in plasma AST activity than ALT. This comparative study was carried out in the department of Biochemistry, Sir Salimullah Medical College, Dhaka from January 2007 to December 2007 to observe the role of aminotransferases and AST/ALT ratio in CAH and CPH. With prefixed inclusion and exclusion criteria a total of 44 age and sex matched subjects were purposively enrolled in the study. Out of them 20 were CAH and 24 were CPH. Serum AST, ALT and total bilirubin were measured and AST/ALT ratio were calculated. Mentioned parameters were compared between groups. Unpaired student's t-test and chi (x2) square test were done to detect the significant difference between the groups using SPSS version 12.0. Mean age of the study subjects in CAH and CPH group were 32.20 ± 10.65 years (20-50 years) and 32.83 ± 10.68 years (20-49 years) respectively. There was no significant difference regarding age and sex distribution between the groups (p > 0.05). Mean ± SD serum AST was 295.08 ± 153.77 IU/L and 73.38 ± 45.72 IU/L and ALT was 352.44 ± 206.95 IU/L and 121.01 ± 58.77 IU/L in CAH and CPH respectively and both were statistically highly significant (p < 0.01). Mean ± SD of AST / ALT ratio in CAH and CPH were 0.87 ± 0.14 and 0.59 ± 0.14 respectively and was highly significant (p < 0.01). There was no significant difference in serum bilirubin level between the groups (20.78 ± 14.16 ?mol/L vs. 19.25 ± 15.90 ?mol /L) (p > 0.05). Introduction: A randomized control trial To compare efficacy of oral rifampicin in terms of drying of macula and decrease macular thickness with observation alone in patients of acute central serous Chorioretinopathy done in Layton Rahmatulla Benevolent Trust (LRBT) Free Eye and Cancer Hospital, Lahore from January 2017 to June 2017 Methods: After getting approval from hospital Ethical committee 140 patients of CSR were included in the study. The demographic details were noted and patients were randomized by lottery methods in two groups (group A& B). Group-A was observed for spontaneous resolution, routine treatment started if no improvement noted after 6 weeks of observation alone. Group B was given oral rifampicin 600mg per day for four weeks with liver function tests being done before commencement of treatment and after 2 weeks. Oral rifampicin was stopped if patient developed deranged liver function tests. Patients were followed up at 4 weeks for macular dryness and decrease macular thickness on optical coherence tomography(OCT). All the readings were carried out and noted by single person in order to minimize bias. Results: A total of 140 patents, 70 in each group, were included In study with mean age 38.77+7.74 in Group-A and 39.14+7.97 years in Group-B. regarding gender distribution 65.71 %(n=46) in Group-A and 613.43%(n=43) in Group-B were male.   Comparison of outcome of treatment of acute central serous chorioretinopathy with oral rifampicin vs observation showed that 18.57 %(n=13) in Group-A and 41.43%(n=29) in Group-B had dry macula. Conclusion: We concluded that there is a significant difference in drying of macula in acute central serous chorioretinopathy with Rifampicin versus observation alone.

Background: Change of liver biochemical profiles is normal during pregnancy. It is almost impossible to understand disease processes that can threaten women during pregnancy without understanding normal physiological change. Aim of this study is to evaluate changes in serum liver function tests in normal pregnant women in first, second and third trimester. Material and Methods: The cross-sectional comparative study was carried out in the Department of Obstetrics & Gynaecology of Rajshahi Medical College Hospital, Bangladesh in 2019. This study consists of 90 pregnant women and 90 matched control. Among the 90 pregnant women, 30 were in first trimester, 30 were in second trimester and 30 were in third trimester. Blood samples were taken for routine liver function and protein profiles Results: Serum total and direct bilirubin concentrations were significantly lower in second and third trimester. The mean ALP level was slightly increased in 2nd trimester and drastically increased in 3rd trimesters during pregnancy. Serum ALT and AST activity was significantly increased in third trimester. No significant change in serum total proteins concentration, but serum albumin concentration was significantly lower and serum globulin concentration was significantly higher in all three trimester. Serum albumin/globulin ratio was significantly reduced in second and third trimester. Conclusion: Relative values of various liver function tests during gestational trimesters appear to be the best guide to confirm the diagnosis and treatment strategies. Thus, gynecologists should routinely monitor liver function tests in all gestational trimesters to avoid the future complications to mother and offspring. TAJ 2020; 33(1): 17-24

Abstract Background Acne is a chronic inflammatory disorder of the pilosebaceous unit. Oral isotretinoin is a highly effective treatment for patients with moderate and severe, but causes significant side effects. Objective To estimate homocysteine level in acne patients after treatment with isotretinoin. Patients and Methods The prospective study was conducted at Ain Shams University Faculty of Medicine, Egypt, from June 2017 to November 2017, including male or non-pregnant female patients more than 18 years of age, categorized by the Global Evaluation Acne (GEA) into 22 patients with moderate acne vulgaris and 22 patients with severe acne. Isotretinoin was started in a dosage of 0.5 mg/kg/day, and 1.0mg/kg/day in moderate and severe acne patients respectively. Baseline homocysteine level, liver function tests, serum cholesterol and lipid profile were tested before and after 3 months. Results There was significant difference (p = 0.001) in homocysteine level among all cases Moreover, homocysteine serum level in patient's group who received high dose isotretinoin (1 mg/kg/d) showed highly significant results (p = 0.001) in contrast to patient's group who received low dose isotretinoin (0.5 mg/kg/d) = 0.003). A significant increase in lipid profile and liver function test was seen in all cases. Conclusion low dose isotretinoin seems to have less side effects on basic laboratory data such as CBC, liver function tests, lipid profile and homocysteine serum levels than high dose.
