Introduction: Vitamin D deficiency has been linked to a whole spectrum of diseases including osteoporosis, cancer, diabetes, and cardiovascular and immune disorders. Though Bangladesh is low latitude country, vitamin D Deficiency is serious and wide spread problem in Bangladesh. Physicians of Bangladesh are more vulnerable to low vitamin D status due to long indoor work hours and lack of sunlight exposure in both government & private institute. So, the aim of this study to evaluate vitamin D status among the physicians working around the Bangladesh. Methods: This was a cross-sectional, multicenter study where we enrolled 1112 doctors from 9 different areas of Bangladesh from May 2018 to June 2019. A Blood sample was collected from all participants to measure Serum 25(OH)D and various patient data such as age, gender, BMI, physical activity level during leisure time, use of vitamins and medications, sunlight exposure time, tea/coffee drinking, smoking, H/O comorbidities etc. was collected. Results: Of the 1112 doctors assessed in the study,794 (71.4%) was male and 318 (28.6%) was female. Mean age of the participants was 45.5±11.1 Years. Vitamin D deficiency and insufficiency was found in 734 (65.8%) and 105 (9.4%) participants respectively. Mean vitamin D level was 18.9 ng/ml (±8.6 ng/ml). Less than 5% of participants of Rajshahi, Rangpur, Dhaka, Bogura and Sylhet had sufficient vitamin D level. Vitamin D deficiency was significantly associated with age, obesity, sunlight exposure, physical inactivity, hypertension and vitamin D supplementation. Conclusion: Vitamin D deficiency is very common among physicians of all over Bangladesh. The high prevalence of vitamin D deficiency in the present study points towards urgent need of an integrated approach to detect vitamin D deficiency among health care professionals and treat appropriately. Introduction: Despite abundant sunshine for cutaneous vitamin D synthesis, low levels of vitamin D have been documented among the Malaysian population. The aim of this study was to characterise the effects of occupational sunlight exposure and monsoon season on serum 25-hydroxyvitamin D [25(OH)D] concentration. Methods: A comparative cross-sectional study was performed among Malay outdoor (n=119) and indoor workers (n=119) in Kelantan. Two-point data were collected on the same participants, first during non-monsoon season and second during the northeast monsoon season. Data collection comprised of anthropometry measurements (body mass index and body fat), fasting blood test [serum 25(OH)D concentration], and questionnaire (physical activity level, sun exposure, sun protection use, and vitamin D intake). Vitamin D classification was based on the 2011 Endocrine Society Clinical Practice Guidelines. Results: This study found that serum 25(OH)D concentrations were significantly higher in outdoor compared to indoor workers irrespective of season (p<0.001) and sex (p<0.001). Monsoonal differences in serum 25(OH)D concentration was only observed in male outdoor workers (mean difference=10.39 nmol/l, p<0.001). Significant association between vitamin D status and occupation was also observed (p<0.001). However, no significant association was found between vitamin D status and monsoon season [χ2(1)=0.076, p=0.783]. Conclusion: Seasonal and occupational factors should be considered while evaluating individual serum 25(OH)D concentration and in comparing community studies, especially among workers.

AbstractVitamin D deficiency is prevalent all over the world, especially in tropical countries. In epileptics, antiepileptic drugs (AED) and associated comorbidities further impact vitamin D status. The aim of this study is to estimate the prevalence of 25(OH) vitamin D deficiency in epileptic children and evaluate probable risk factors. A cross-sectional study of 200 children between 1 and 18 years of age on AED was undertaken in the pediatric neurology clinic of a tertiary care center of Northern India. In all children, serum 25(OH) vitamin D levels, calcium, phosphorus, and alkaline phosphatase were estimated. The deficiency levels of vitamin D were categorized as: deficiency <20 ng/mL, insufficiency 20 to 30 ng/mL, and sufficiency >30 ng/mL. The potential risk factors for hypovitaminosis D, including type of epilepsy, AED regimen (specific medications, polytherapy vs. monotherapy), cerebral palsy, ambulatory status, intellectual disability, body mass index, gender, and vegetarianism were examined. Among the 200 enrolled children (60% boys), 106/200 (53%) were vitamin D deficient. There was no significant relation of vitamin D levels with gender, type of epilepsy, neurological deficit, and type of AED. An inverse relationship of higher phenytoin doses during monotherapy associated with lower vitamin D levels in monotherapy was found, thereby suggesting adverse effect of high doses of phenytoin on vitamin D levels. VDD is common among epileptic children on AED therapy. Vitamin D deficiency is common prevalent among epileptic children on AED therapy, and its detection and treatment correction should be an integral part of epilepsy management.

ObjectivesIn Kuwait, as in many Arab states in the Gulf region, there are limited data on the prevalence of vitamin D deficiency among healthy adolescents. This study aimed to estimate the prevalence of vitamin D deficiency in a nationally representative sample of adolescents and investigate factors associated with vitamin D status.MethodsA cross-sectional study was conducted on 1416 adolescents aged 11–16 years, who were randomly selected from middle schools in all governorates of Kuwait. Data were collected from parents through self-administered questionnaire and from adolescents through face-to-face interview. Vitamin D was measured using liquid chromatography-tandem mass spectrometry. Logistic regression was used to investigate the independent factors associated with vitamin D status.ResultsThe prevalence of vitamin D deficiency was 81.21% (95% CI 71.61% to 90.81%), while severe deficiency was 39.48%. Only 3.60% of adolescents were vitamin D-sufficient. The prevalence of vitamin D deficiency was significantly higher among girls compared with boys (91.69% vs 70.32%; p<0.001). There was a significant inverse correlation between vitamin D and parathyroid hormone (Spearman correlation=−0.35; p<0.001). In the final model, gender, age, governorate, parental education, body mass index, vitamin D supplement and the number of times adolescents walk to schools per week were all significantly related to vitamin D deficiency.ConclusionHigh prevalence of vitamin D deficiency was noted among adolescents in Kuwait despite the abundant sunshine, which may reflect strong sun avoidance behaviour. Adequate outdoor daytime activities should be encouraged especially for girls. We call for locally tailored guidelines for vitamin D supplement in which girls should have a higher dose compared with boys.

Previous research suggests prevalent vitamin D deficiency in pregnant women residing in South Australia and the Eastern Seaboard, however recent data from Perth, Western Australia (WA) is lacking. This cross-sectional study ofn=209 pregnant women (36–40 weeks of gestation, 84% white Caucasian) reports on the vitamin D (25[OH]D) status of a contemporary population of pregnant women in Perth, WA, with a focus on the relative contributions of supplemental vitamin D and ambient ultraviolet (UV) radiation to 25(OH)D levels. Mean (SD) season-adjusted 25(OH)D levels were 77.7 (24.6) nmol/l. The prevalence of vitamin D deficiency (25[OH]D<50 nmol/l) was 13.9%. Ambient UV radiation levels in the 90 days preceding blood draw were significantly correlated with serum 25(OH)D levels (unstandardized coefficient 2.82; 95% CI 1.77, 3.86,P<0.001). Vitamin D supplementation expressed as dose per kg of body weight was also positively correlated with serum 25(OH)D levels (unstandardized coefficient 0.744; 95% CI 0.395, 1.092,P<0.001). In conclusion, this study finds that vitamin D deficiency in a predominantly white Caucasian cohort of pregnant women is less prevalent than has been reported in other studies, providing useful information relating to supplementation and screening in this, and similar, populations.

Purpose. As the vitamin D status of Algerian postmenopausal women was poorly described, this cross-sectional study investigated the prevalence of low vitamin D status in a sample population. Secondarily, predictive factors of this hypovitaminosis D were explored.Methods. All the 336 selected women ≥ 45 years from Douera were interviewed to get anthropometric and lifestyle data, reproductive and medical history, medications, and calcium/vitamin D intakes. A blood sample was collected to measure 25-hydroxyvitamin D (25(OH)D) concentrations.Results. Approximately 86% of subjects had low vitamin D status (<20 ng/mL). Mean 25(OH)D level was 14.4 ± 5.3 ng/mL with a clear seasonal dynamic and a significant negative correlation with PTH levels (r  = −0.15,p=0.006). A multiple regression analysis using the 25(OH)D cutoff value of 17 ng/mL instead of the generally admitted level of 20 ng/mL was performed to increase statistical power. Other seasons than summer (OR 4.159 and 95% CI 2.456–7.043), obesity (≥30 kg/m2, OR 1.826, 95% CI 1.081–3.083), and veiling (OR 3.526, 95% CI 1.090–11.400) were significantly associated with 25(OH)D concentrations <17 ng/mL.Conclusions. In North Algeria, the abundant sunlight appears insufficient to fully offset hypovitaminosis D risk factors in postmenopausal women, especially obesity and veiling. It suggests the major need to increase vitamin D supplementation in this subpopulation.

Background: The possibility of vitamin D deficiency is high among mothers and their premature newborns. Objectives: Due to the high rate of vitamin D deficiency in Iranian women, this study aimed to determine the association between the serum levels of vitamin D in the blood of premature newborns and their mothers in the Iranian population. Methods: This cross-sectional study was conducted on 324 mothers and their 324 newborns with gestational age less than 37 weeks, who were referred to Ghaem Hospital affiliated to Mashhad University of Medical Sciences, Mashhad, Iran, during 2016-2020. After clamping, 1.5 cc blood samples were obtained through the umbilical cord to determine the neonates’ vitamin D levels. Finally, the serum levels of vitamin D were measured, and the correlation between the levels of vitamin D of mothers and their newborns was calculated. Results: The mean vitamin D levels of newborns and mothers were 15.75 ± 11.18 and 20.16 ± 13.41 ng/mL, respectively. Moreover, there was a high correlation between the vitamin D levels of mothers and their newborns (r = 0.672; P < 0.001). Furthermore, a significant correlation was observed between the vitamin D levels of mothers, who used vitamin D supplementation during pregnancy and their neonates’ levels (r = 0.773; P < 0.001). Conclusions: The findings suggest a positive correlation between neonatal and maternal vitamin D levels. Moreover, it seems that maternal vitamin D levels can predict neonatal vitamin D deficiency.

Objective While vitamin D regulates immune cells, little is known about it in autoimmune Addison’s disease (AAD). We investigated the vitamin D status in AAD patients from five European populations to assess its deficiency. In addition, we studied two case-control cohorts for vitamin D metabolism and pathway genes. Design Cross-sectional study. Methods A total of 1028 patients with AAD from Germany (n = 239), Italy (n = 328), Norway (n = 378), UK (n = 44) and Poland (n = 39) and 679 controls from Germany (n = 301) and Norway (n = 378) were studied for 25(OH)D3 (primary objective). Secondary objectives (1,25(OH)2D3 and pathway genes) were examined in case-controls from Germany and Norway correlating 25(OH)D3 and single nucleotide polymorphisms within genes encoding the vitamin D receptor (VDR), 1-α-hydroxylase (CYP27B1), 25-hydroxylase (CYP2R1), 24-hydroxylase (CYP24A1) and vitamin D binding protein (GC/DBP). Results Vitamin D deficiency (25(OH)D3 10–20 ng/mL) was highly prevalent in AAD patients (34–57%), 5–22% were severely deficient (<10 ng/mL), 28–38% insufficient (20–30 ng/mL) and only 7–14% sufficient (>30 ng/mL). Lower 25(OH)D3 and 1,25(OH)2D3 levels were observed both in Norwegian and German AAD (P = 0.03/0.003 and P = 1 × 10-5/< 1 × 10-7, respectively) the former was associated with CYP2R1 (rs1553006) genotype G. Whereas controls achieved sufficient median 25(OH)D3 in summers (21.4 to 21.9 ng/mL), AAD patients remained largely deficient (18.0 to 21.2 ng/mL) and synthesize less 1,25(OH)2D3. Conclusion Vitamin D deficiency and insufficiency are highly prevalent in AAD patients. The vitamin D status of AAD may be influenced by genetic factors and suggests individual vitamin D requirements throughout the year.

Background. A vitamin D deficiency is a global wide health problem. Inadequate vitamin D status leads to serious medical, social and economic consequences and requires timely diagnosis and adequate correction.Aim: to assess the incidence of vitamin D deficiency among the population living in regions of the Russian Federation located at latitudes from 45° to 70° and to study its relationship to various factors (demographic, socio-economic, geographical, etc.).Materials and methods. A multicenter, cross-sectional, randomized study of individuals of both sexes aged 18 to 50 years. The study of 25ОНD level in the blood serum was carried out by INVITRO LLC laboratories using chemilumescent analysis in two stages: in the period from March to May 2020 and from October to November 2020. The first stage of the study involved 500 volunteers (77% women, 23% men). Demographic and socioeconomic characteristics, factors associated with vitamin D levels, were assessed using a specially designed questionnaire.Results. In this article we present the results of the first stage. A countrywide high prevalence of vitamin D deficiency (56.4%) and insufficiency (27.9%) was revealed, that is, 84.3% of the population aged 18–50 years requires cholecalciferol supplementation. The lowest vitamin D status was found in St. Petersburg, Arkhangelsk, Kyzyl and Rostov-on-Don (over 90% of participants had inadequate levels). A decrease in the level of 25ОНD was observed to a greater extent in men than in women (p <0.05), as well as in young people aged 18–25 years (p <0.05). No major association of vitamin D status with anthropometric parameters, the number of days of disability, were not found. However, those who went to tanning beds or were in direct sunlight for at least 30 days a year had higher levels of 25ОНD, and those using sunscreens had lower levels.Conclusion. The data obtained confirms the widespread prevalence of vitamin D deficiency and insufficiency in the country (84.3% of participants), generally not related to the geographical latitude of residence, which indicates the need for vitamin D supplementation in a significant part of the population.

Background: Dyslipidemia is one of the most common metabolic syndrome among diabetic patients due to several factors include insulin insufficiency, resistance, and central obesity. Furthermore both vitamin D deficiency and diabetes are most public health worldwide problems. Therefore the aim of the present work to study the dyslipidemia and vitamin D status in diabetes patients and also to study the relation between vitamin D status and lipid profile in diabetic patients. Methods: A Cross sectional study conducted on randomly selected diabetic patients whether have vitamin D deficiency with dyslipidemia, vitamin D deficiency with no dyslipidemia or dyslipidemia with no vitamin D deficiency. A total sample 165 patients enrolled in the study with serum lipid profile, vitamin D and glycemic control measured at beginning and end of the study. The data analysis was done through Chi-square or T test at α< 0.05. Result: The data collected on 165 patients revealed that, patients aged 41-60 years were most common, and female gender was twice as male. This study include both types of diabetes with vitamin D deficiency and dyslipidemia or vitamin D deficiency with no dyslipidemia or dyslipidemia with not vitamin D deficiency as control. Therefore, the result of this work confirmed that vitamin D deficiency significant implicated in elevated serum levels of TG, TC, LDL, VLDL, FPG and HbA1C (P< 0.05). However, vitamin D deficiency has linked to slight increased serum HDL levels. In compared to man, vitamin D deficiency linked significantly to dyslipidemia and abnormal high levels blood glucose and HbA1C par in women. Conclusion: The present study revealed that, vitamin D deficiency associated negatively with serum levels of TC, TG, VLDL, LDL, FPG and HbA1C whereas the deficiency of vitamin D linked to elevated HDL levels. In gender distribution lower vitamin D values associated with elevated serum FPG, HbA1C, lipid profile with exception HDL in women. The data of this study suggested that, diabetic patients with dyslipidemia may improve their lipid profile and glucose hemostasis through vitamin D supplementation

Background and Aims: Vitamin D plays an important role in bone health, cardiovascular health, brain development, immune system regulation, mood regulation, and cancer prevention. This study aimed to evaluate the vitamin D status and the prevalence of vitamin D deficiency in Jiroft city, Kerman Province, south of Iran. Materials and Methods: In this cross-sectional study, 5243 people were selected by random sampling during 2015-2019.  Serum 25 (OH) D level was measured by VIDAS 25-OH vitamin D total testing kits. Levels used for definitions were deficient, D3 < 20 ng/ml; insufficient, D3 20-30 ng/ml; adequate, D3 30-100 ng/ml, and potential toxicity, >100 ng/ml.   Results: The mean of vitamin D was 26.03 ng/ml and 24.19 ng/ml in men and women, respectively (p=0.003). The prevalence of vitamin D deficiency was 41.55% and 51.7% in men and women, respectively. A significant relationship was seen between age and level of vitamin D (p=0.001). Conclusions: The results of our study showed that vitamin D deficiency is common among the general population, especially in women in Jiroft city, Kerman Province, south of Iran. Serious plans are needed to improve the status of vitamin D in the people living in this region.
