Background: Osteoarthritis of knee joint is one of the most common problem which cause pain, loss of function even joint disability and increases misery and morbidity in old age. Now a day’s intra-articular injection of single Hyaluronic acid or corticosteroids (Methylprednisolone) are using for knee osteoarthritis cases. But the combined therapy by injection of both Hyaluronic acid and Methyl prednisolone are not so much practiced and still under debate. Objectives: To compare the efficacy of intra-articular injection of combined Hyaluronic acid and Methyl prednisolone with single Hyaluronic acid alone. Methods: A prospective, comparative study was done in Northern International Medical College Hospital, Dhaka, during May 2017 to April 2018 at Out Patient Department with 80 patients (132 knees). Patients were randomized into two groups: Group-A with 40 patients with 15 unilateral and 25 bilateral knee cases (65 knees) were treated with combined Hyaluronic acid and Methyl prednisolone and Group-B with 40 patients with13 unilateral and 27 bilateral knee cases (67 knees) were infiltrated with single Hyaluronic acid. Results were evaluated with Knee Society Score (KSS) and Visual analogue score (VAS) at the 1st week, 1st month and 3rd month after injection. Result: In both Group (A and B) patients of age 61–70 years were highest, 50% and 45% respectively. Male and female ratio was 1:2.2. At the 1st week KSS for pain in Group-A (mean 65.6) was better than Group-B (mean 57.4). At the 1st month mean pain scores (Group-A 72.4, Group-B 64.4) and the 3rd month (Group-A 81.4, Group-B 75.6) and mean function scores at 1st week (Group-A 61.3, Group-B 49.6)), 1st month (Group-A65.1, Group-B 53.6) and 3rd month (Group-A68.4, Group-B 59.4). The KSS for pain and function improved in combination therapy group.VAS decreased in both groups. Conclusion: As combination therapy by Hyaluronic acid and Methyl prednisolone showed better efficacy than single Hyaluronic acid. Therefore, combination of these two drugs in a single vial can be used as an effective and easy nonoperative treatment of knee osteoarthritis. Objective: To compare the clean intermittent self-catheterization (CISC) with continuous indwelling catheterization (CIDC) in relieving acute urinary retention (AUR) due to benign enlargement of prostate (BEP). Materials and Methods :A total 60 patients attending in urology department of Dhaka Medical college hospital were included according to inclusion criteria ,Patients were randomized by lottery into two groups namely group –A and group –B for CISC and IDC drainage respectively . Thus total 60 patients 30 in each group completed study. Results : Most men can safely be managed as out-patients after AUR due to BPH. The degree of mucosal congestion and inflammation within the bladder was found to be lower in those using CISC and the bladder capacity in these patients was also found higher.Patients with an IDC had a high incidence of UTIs then that of patients with CISC. During the period of catheterization the incidence of UTI was 43.3% in group B in comparison to 40% in group A; before TURP 36% in group B in comparison to 10% incidence in group A.According to patient’s opinion CISC is better than IDC in the management of AUR. Experiencing bladder spasm, reporting blood in urine, management difficulties, incidence and severity of pain were less in CISC group, and the method of CISC was well accepted by patients as well as their family members. Conclusion: From the current study it may be suggested that CISC is better technique for management of AUR patient due to BPH than IDC. Background: Transurethral resection of prostate (TURP) is considered as the gold standard surgical treatment for Benign prostatic Hyperplasia (BPH). Among many post TURP complications, urethral stricture is one of the most dreaded urological complications, which may result in lifelong misery. The present study aimed to report the outcomes of Otis urethrotomy to prevent urethral stricture before TURP.Method: This was a comparative study between two equal groups of 100 patients who met inclusion criteria. Study conducted in the department of surgery, Urology unit, Nepalgunj Medical College from 2014 May to 2016 April. In group A, included all the patients underwent TURP without Otis urethrotomy and in group B , included all patients underwent TURP after doing Otis Urethrotomy. The patients were evaluated for evidence of urethral stricture formation.Result: In Group A, 20 patients out of 100(20%) developed urethral stricture whereas in Group B, 8 patients out of 100(8%) developed urethral stricture .The rate of Urethral Stricture in Group A is significantly more than in Group B (p<0.05).Conclusion: The incidence of urethral stricture formation can be significantly reduced by doing Otis urethrotomy before TURP than TURP alone.JNGMC Vol. 14 No. 1 July 2016, Page: 5-7

Objective: Diarrhea is a common problem in our country especially children. There were many treatment options for acute watery diarrhea in addition to oral rehydration saline for reducing the severity of acute watery diarrhea. The aim of this study was to compare the efficacy of zinc & probiotics combination therapy to zinc alone therapy in reducing the severity of acute diarrhea.Method: This was a randomized controlled trial type study, conducted at department of pediatrics Rajshahi Medical College Hospital from July 2014 to June 2016. A total of 110 numbers of children ages 6 months to 5 years with acute watery diarrhea were enrolled those who were fulfilled the selection criteria. They were divided into two groups. Group A (n=55) received zinc-probiotics combination therapy and group B (n=55) received zinc only. Measurement of disease severity was based on the frequency of diarrhea (times/day) and duration of diarrhea (hours) after initial drug consumption.Result: Among the study population 56% male &44% female in group A and 58% male & 42% female in group B. Duration of acute watery diarrhea was significantly reduced in group A than group B(56.22 versus 70.69 hours respectively), (P-0.002)and frequency of stool also reduced in group A than group B(3.92 versus 7.15 times/day) on day 2 (P-0.002). Consistency of stool also improved in group A than group B[liquid stool12(21.8%) versus 25(45.5%), (P-0.015) on day 1, semi-liquid stool 4 (8.2%) versus 16 (30.8%), (P-0.004) on day 2, formed stool 29 (52.73%) versus 16(29.09%), P-0.004 on day 2 respectively].Conclusion: Combination therapy was more effective in reducing the severity of acute watery diarrhea than zinc alone therapy in children.TAJ 2017; 30(2): 32-38

Background: Prevalence of the episiotomy varies around the world depending on whether it is used as a routine or a restricted procedure. The rates are still higher in developing countries, like ours, since the use of restricted episiotomy is not being practiced widely in primigravidas. Methods: In our randomized comparative study of 222 women who were admitted to the Department of Obstetrics and Gynaecology, SMS Medical College Jaipur, the enrolled women were divided into 2 groups. In Group-A: Vicrylrapide (polygalactin-910) suture material was used in 111 patients and in Group-B chromic catgut suture material was used in another 111 patients, for repair of episiotomies. Results: In our present study in vicryl rapide group at 24-48 hrs pain was present in 49.55% of patients, redness was found in 8.10% of patients, swelling was found in 3.60% patients and analgesic used by 72.97% of patients. In chromic group at 24-48 hrs pain was present in 69.36% of patients, redness was found in 21.62%, swelling was found in 7.20% and analgesic was used by 89.18% of patients. Conclusion: Our study recommends use of vicryl rapide for episiotomy repair in the care of parturient women. Keywords: Complication, Chromic Catgut, Vicryl Rapide, Episiotomy

A comparative study between Single Layer versus Double Layer Intestinal Anastomosis,was undertaken at Department of Surgery,Bankura Sammilani Medical College & Hospital,Bankura from April 2019 – September 2020,which included 74 patients, comprising 2 groups: Group A-Single layer and Group B- Double layer with equal number of patients randomly allotted in each group. More number of patients had anastomotic leak in Group B than Group A, though not statistically significant. Difference of Mean Duration of Anastomosis with both groups is statistically significant.Mean Duration of Hospital Stay with both groups is statistically insignificant. Although more number of patients had anastomotic leaks in Group B than Group A,it was statistically insignificant.

A clinical trial was carried out for the duration of six months from September' 2012 to February' 2013 in the Department of Dermatology and Venereology, Shaheed Monsur Ali Medical College, Uttara Dhaka and patients attending private clinical chamber. To evaluate the effectiveness of oxiconazole cream in comparison to the ketoconazole cream for the treatment of inguinocrural dermatophytoses.A total number of 60 patients with inguinocrural dermatophytoses were included in the study of which 30 patients were treated with oxiconazole (Group A) and the rest 30 patients were treated with ketoconazole (Group B)once daily for 21 days and weekly the outcome of lesions were clinically evaluated and recorded.In group A, male and female were 17 (56.7%) cases and 13 (43.3%) cases respectively. In group B, male and female were 16 (53.3%) cases and 14 (46.7%) cases respectively.The mean age with SD in group A and group B were 28.93 ± 8.29 years and 31.36 ± 8.36 years respectively. The mean scoring with SD in group A and group B were 6.26 ± 2.22 minutes and 6.53 ± 1.81 minutes respectively at the time of observation and 4.23 ± 1.50 minutes and 5.13 ± 1.45 minutes respectively after 1 week and 2.00 ± 1.22 minutes and 3.25 ± 1.07 minutes respectively after 2 weeks. The difference between the mean score of the two group is significant (p=0.006). The mean scoring with SD in group A and group B were 0.00 ± 0.00 minutes and 1.75 ± 0.95 respectively after 3 weeks.Sotopical treatment oxiconazole has revealed itself to be as efficient as ketoconazole and it seems more effective and better tolerated than ketoconazole.Medicine Today 2016 Vol.28(2): 57-61

Background: This study was undertaken to evaluate and compare the efficacies of two different regimen for induction of labour in IUFD in an effort to find a better management of woman with IUFD.Methods: It was a hospital based prospective comparative study taken up in the Department of Obstetrics and Gynaecology, Assam Medical College and Hospital (AMCH), Dibrugarh for a period of 1 year. A total number of 90 cases were selected and randomly divided into 2 groups. The two groups were induced using regimen recommended by the RCOG. Group A was induced with Mifepristone - Misoprostol Combined Regimen while Group B was induced with only Misoprostol.Results: Both the groups were found to be comparable with respect to mean Pre – induction Bishops score. The mean Induction to Onset of Labour Interval and Induction to Delivery Interval were significantly less in Group A as compared to Group B. Mean dose of Misoprostol required was less in Group A compared to Group B. In terms of side effects tolerance to Group A was better than Group B.Conclusions: It was observed in the study that both the regimen were equally safe, easy to administer and affordable but the Combination Regimen had a slight edge over misoprostol alone regimen in terms of tolerance, fewer side effects and efficacy with regard to early onset of labour, shorter Induction to Delivery Interval and relatively less dose of misoprostol than misoprostol alone regimen.

Context: Osteoarthritis is primarily a disease of cartilage as it is characterized by the degradation of hyaline cartilage in the joints. It is believed to be a dynamic disease that reflects the balance between destruction and repair. Clinically, there is pain, swelling of joints and limitation of motion. Pathological disease is characterized by focal erosive lesions, cartilage destruction, subchondral sclerosis, cyst formation and large osteophyte at the margin of the joints. The objectives of management of osteoarthritis (OA) of the knee are to relieve pain, maintain or improve mobility, and minimize disability. Treatment options include non -pharmacologic intervention, drug therapy, and surgery. Different modalities of physical therapy have been shown to help improve clinical symptoms and function of knee OA with fewer adverse effects. Transcutaneous electrical nerve stimulation (TENS) is among these non invasive therapies which have been used to treat a variety of painful acute and chronic conditions including osteoarthritis. Material and Methods: This randomized clinical trial was conducted on 60 (sixty) patients attending in Physical Medicine and Rehabilitation department of Dhaka Medical College Hospital, who were suffering from knee osteoarthritis. The patients were randomly divided into two groups, Group-A and Group-B. The patients were evaluated clinically and data was collected from both groups in a pre designed data collection sheet for visual analogue scale (VAS) on Pain, 50 feet walking time in seconds and tenderness index in every two weeks interval from the first visit for up to 6 weeks. All the data were analyzed by SPSS version 16.1. Result : The present study showed pain, tenderness and walking time were significantly improved in Group A who were treated with TENS, NSAID & ADL instructions than in Group B who were treated with NSAID & ADL instructions after 6 weeks( P<0.05). Conclusion: The results of this study suggest that application of TENS along with NSAIDs and ADL instructions is more effective in reduction of pain and improving functional performances in patients with knee osteoarthritis than the drug only treatment. The clinical study was carried out to find out the efficacy of Tankana & Sphatika bhasma with Madhu pratisarana in Tundikeri (Chronic Tonsillitis).The study was an Open Labelled Randomized Comparative study with a pre- and post-test design at the outpatient level of S.V.M. Ayurvedic Medical College & R.P.K Ayurvedic Hospital, Ilkal, Karnatak, India. In accordance to inclusion and exclusion criteria 30 patients were treated with Tankana bhasma (Purified borax powder) along with Madhu (honey) pratisarana and Sphatika bhasma (purified potash alum) along with Madhu pratisarana on alternative days for 13 days to Group A and Group B respectively. Each group comprised with 15 patients. After assessment of all data, the result conforms that both the drug Tankana bhasma with Madhu pratisarana and Sphatika bhasma with Madhu pratisarana on Tundikeri have significant result. However, in overall assessment, the efficacy of Tankana bhasma pratisarana is more significant than Sphatika bhasma pratisarana in the management of Tundikeri. Background: Verruca is one of the common dermatopathologies which has multiple therapeutic options but with variable success rates, refractory cases and high recurrence rates. Nowadays, treatment with intralesional injections has gained recognition due to its effectiveness in clearing verrucae. These act by stimulating the cell-mediated immunity. Out of scores of options available for intralesional therapeutics, Vitamin D3 appears to be more promising but least evaluated. Therefore, we planned to evaluate the efficacy of intralesional Vitamin D3 in various types of cutaneous verrucae. Simultaneously the results were compared with intralesional bleomycin, also. Methods: A total of 200 patients of cutaneous verrucae with varying size and duration were included in the experimental randomized comparative study. We divided them into two groups. Group A, comprising of 100 patients, received 0.2-0.5 ml intralesional Vitamin D3 (600,000 IU, 15mg/ml) and Group B, also of hundred subjects, received intralesional Bleomycin (1 mg/ml) into the base of verrucae. A maximum of 5 verrucae were injected per session at 3 weeks interval until resolution or for a maximum of 4 sessions. Patients were followed up for 6 months after the last injection to assess the clearance status and detect any recurrence. Results: In Group A (Vitamin D3), 'Complete response', 'Partial response' and 'No response' were observed in 85.07%, 6.74% and 8.17% respectively after 4 sessions. Recurrence rate was 0.81% after 6 months. In Group B (Bleomycin), 'Complete response', 'Partial response' and 'No response' were found in 77.99%, 10.47% and 11.53% in the series. Recurrence rate was 1.71%, comparatively higher in group B. Conclusion: The efficacy of intralesional Vitamin D3 was found significantly higher as compared to intralesional Bleomycin in the treatment of cutaneous verrucae with less recurrence rates. Vitamin D3 has an additional advantage of cost-effective treatment over Bleomycin. We purpose its use, as a primary mode of treatment in various types of cutaneous verrucae.
