Purpose: To develop a photographic classification for cilioretinal arteries and to investigate its association with myopic macular degeneration (MMD).Methods: One thousand six hundred ninety-two highly myopic eyes of 1,692 patients were included. The presence of a cilioretinal artery was determined by fundus photographs, and a photographic classification was proposed. MMD was classified according to the International META-PM Classification. Associations of the cilioretinal artery and its classifications with MMD and visual acuity were analyzed.Results: Of the eyes tested, 245 (14.5%) had a cilioretinal artery. The cilioretinal arteries were classified into four categories (temporal “cake-fork,” 35.92%; temporal “ribbon,” 53.47%; “multiple,” 6.53%; “nasal,” 4.08%) and 3 distributions based on whether its visible branches reached the central foveal area. Eyes with cilioretinal arteries had significantly less MMD of grade ≥3 and better visual acuity than those without (P < 0.01). Multiple linear regression analysis showed that younger age, male sex, shorter axial length, and the presence of a cilioretinal artery were associated with better visual acuity in highly myopic eyes (all P < 0.05). The “nasal” category presented more MMD with grade ≥3 and worse visual acuity than the other categories (P < 0.05), whereas the “multiple” category contained no eyes with MMD grade ≥3. The cilioretinal arteries reaching the central foveal area showed less MMD of grade ≥3 and better visual acuity than those not (P < 0.05).Conclusions: We propose a photographic classification for cilioretinal arteries that has good clinical relevance to visual functions. The cilioretinal artery may potentially afford protection against MMD.

PurposeTo evaluate whether the presence of dome-shaped macula (DSM) is a protective factor for visual acuity after cataract surgery in patients with high myopia.MethodsIncluded were 891 highly myopic cataract eyes (600 patients) that were examined by optical coherence tomography (OCT) through the central fovea and underwent cataract surgery in our hospital. DSM was defined as an inward bulge >50 µm in horizontal or vertical OCT sections. The incidences of various maculopathies were compared between eyes with and those without DSM. The influences of age, sex, eye laterality, axial length and DSM on postoperative visual acuity were evaluated by multivariate linear regression.ResultsOf the 891 eyes, 123 (13.8%) had DSM. There was a greater association of DSM with extrafoveal retinoschisis (RS) than with other vision-threatening complications such as foveal RS and choroidal neovascularisation. In addition to axial length and age, sex was associated with the presence of DSM (p=0.016). In bilateral high myopia, the incidence of DSM increased with the degree of anisometropia and was more common in the longer eye of patients with anisometropia. Younger age, male sex, shorter axial length and the presence of DSM were associated with better postoperative visual acuity in highly myopic cataract eyes (β=0.124, p=0.002; β=0.142, p<0.001; β=0.275, p<0.001 and β=−0.088, p=0.038, respectively).ConclusionAssociated with fewer visual threatening macular complications, presence of DSM may be a protective factor for visual function after cataract surgery in highly myopic eyes.

We examined professional quality of life (ProQOL) and organizational climate among 122 postretired university academics from 19 universities, who were randomly selected from all 4 regions in Thailand. Participants completed measures of ProQOL, work ability, organizational climate, and psychological morbidity. Results indicated that the mean score for compassion satisfaction (CS) was 42.4, for burnout (BO) was 29.7, and for secondary traumatic stress (STS) was 20.3, whereas the proportions of participants with high levels of CS, BO, and STS were 21.3%, 28.7%, and 16.4%, respectively. Multiple linear regression analysis results revealed that younger age (60–65 years), excellent work ability, and an organizational climate of warmth and standards were significantly associated with CS; no psychological distress and lack of responsibility were significantly associated with BO; and lack of responsibility was significantly associated with STS. These results may help health managers at universities to improve ProQOL among their retired workforce.

Background: Age-related macular degeneration (AMD) is the leading cause of blindness in ageing western societies and accounts for greater than 50% of all US visual disability. This report describes the 25-year history of a 66-year-old optometrist who has successfully endured AMD.  Case Report: Visual acuity and serial retinal photographs from 1983 to 2009 as various nutritional modalities and non-dietary lifestyle changes were introduced. After starting lutein-based nutritional supplements beginning at approximately 15 years from diagnosis, the optometrist’s Snellen visual acuity improved in his right eye from 20/40 to 20/25 with a subjective improvement in distortion, but eventually regressed to 20/70-20/80 with some increase in metamorphopsia. The left eye, initially 20/30, improved to 20/15 and has remained stable at 20/20 with complete resolution of metamorphopsia and near complete resolution of a parafoveal scotoma. Fundus photographs demonstrate a reduction in soft and hard drusen count over time in each retina and possible parafoveal repigmentation of atrophic areas with later addition of higher dose zeaxanthin. Conclusions: AMD is a nutritionresponsive disease. The carotenoids, lutein and zeaxanthin appear to be particularly robust therapeutic components of nutritional supplement formulations.

Abstract Background To probe the change of macular choroidal thickness (MCT) in diabetic retinopathy (DR) by using enhanced depth imaging optical coherence tomography (EDI-OCT). Methods A retrospective research was conducted among 136 eyes. Patients’ mean age was 57.34±9.42 years. 67 eyes were suffered from diabetic retinopathy (DR) without macular edema (ME), 69 eyes with ME and age matched 64 healthy eyes were as control. The choriocapillaris and the medium vascular layers (Sattler’s layer) were defined as the choroidal inner layer, and the large vessel layer (Haller’s layer) was defined as the choroidal outer layer. Results The whole, inner and out layers of choroid in macular area of the eyes with DR were significantly thinner compared to that of control (P<0.001). The whole and out choroidal layers in DR with ME were significantly thinner than that in DR without ME (P<0.001). The best-corrected visual acuity (BCVA, LogMar) was 0.41±0.37and 0.70±0.45 for DR without ME and DR with ME, respectively. Univariate linear regression analysis showed a positive correlation between MCT and BCVA in either DR with or without ME groups (P=0.023,P=0.006). A stepwise multiple linear regression analysis demonstrated that the elucidative variables strong association with MCT were BCVA and central macular thickness (CMT) in DR eyes. Conclusion In the eyes with DR, MCT was thinner compared to that of healthy control and this change was more substantial in ME eyes. MCT had a positive correction with BCVA. Our results indicate that the pathological change occurred in choroidal layer of the eyes with DR and this change correlated with impairment of BCVA.

Background and aims: Oxidative stress (OS) induces the production of fibroblast growth factor 21 (FGF21). Previous data have revealed that FGF21 protects cells from OS injury and death, making it a potential therapeutic option for many diseases with increased OS. However, the association of this growth factor with OS markers in humans with chronic kidney disease (CKD) remains unknown. This study aims to evaluate the association of serum FGF21 with serum total antioxidant capacity (TAC) and oxidized low-density lipoproteins (OxLDL) in subjects in different stages of kidney disease. Methods: This is a cross-sectional study that included 382 subjects with different stages of CKD, irrespective of type 2 diabetes (T2D) diagnosis. Associations of serum FGF21 with OxLDL, TAC, sex, age, body mass index (BMI), fasting plasma glucose, estimated glomerular filtration rate (eGFR), T2D, and smoking, were evaluated through bivariate and partial correlation analyses. Independent associations of these variables with serum FGF21 were evaluated using multiple linear regression analysis. Results: Serum FGF21 was significantly and positively correlated with age ( r = 0.236), TAC (lnTAC) ( r = 0.217), and negatively correlated with eGFR ( r = −0.429) and male sex ( r = −0.102). After controlling by age, sex, BMI, T2D, smoking, and eGFR; both TAC and OxLDL were positively correlated with FGF21 ( r = 0.117 and 0.158 respectively, p < 0.05). Using multiple linear regression analysis, eGFR, male sex, T2D, OxLDL, and TAC were independently associated with serum FGF21 (STDβ = −0.475, 0.162, −0.153, 0.142 and 0.136 respectively; p < 0.05 for all) adjusted for age, BMI, smoking, and fasting plasma glucose. Conclusion: A positive association between serum FGF21 and OS has been found independently of renal function in humans. Results from the present study provide novel information for deeper understanding of the role of FGF21 in OS in humans with CKD and T2D; mechanistic studies to explain the association of serum FGF21 with oxidative stress in CKD are needed.

Cerebral blood flow (CBF) becomes pulsatile in response to pulsatile change in perfusion pressure which is regulated by cerebrovascular impedance. In this study, we aimed to characterize age-related differences in cerebrovascular impedance across the adult lifespan. Carotid artery pressure (CAP, via applanation tonometry) and CBF velocity in the middle cerebral artery (CBFV, via transcranial Doppler) were measured in 148 healthy adults (21-79 years, 62% women). Cerebrovascular impedance was quantified using transfer function analysis. Coherence between changes in CBFV and CAP was higher than 0.90 in the frequency range of 0.78-2.73 Hz, suggesting linear dynamic relationship between these two variables. Impedance modulus at the first harmonics of CBFV and CAP oscillations (Z1, 0.78-1.56 Hz), reflected mainly heart rate frequency, was 20 % higher in the old (> 64 years, P=0.002) and 13 % higher in the middle-aged (45-64 years, P=0.08) than young individuals (<45 years). In addition, Z1 was 24% higher in men than women (P<0.001). Multiple linear regression analysis revealed that Z1 is negatively associated with systolic (β=-0.470), diastolic (β=-0.418), pulsatile (β=-0.374), and mean CBFV (β=-0.473) (P<0.001 for all) after adjustment for age, sex, and body mass index (BMI). These results suggest that older age and male sex are associated with higher cerebrovascular impedance than young individuals which may contribute to brain hypoperfusion.

Background:We have recently developed a novel ultrasonographic Halo Score to quantify the extent of vascular inflammation in GCA [1]. High Halo Scores were associated with a high rate of ocular ischaemia among patients with GCA. Earlier studies have shown that GCA patients with intimal hyperplasia in their temporal artery biopsy (TAB) are at the highest risk of neuro-ophthalmic, ischaemic complications [2,3]. We therefore questioned whether the ultrasonographic Halo Score might be linked to the presence of intimal hyperplasia in patients with GCA.Objectives:To investigate the relationship between the ultrasonographic Halo Score and intimal hyperplasia.Methods:This is a prospective study including 92 patients suspected of having GCA, who underwent both ultrasound of temporal/axillary arteries and TAB at diagnosis. Ultrasonographic halo counts and Halo Scores were determined [1]. An experienced pathologist determined whether or not the TAB findings were consistent with GCA. TABs were systematically evaluated for the presence of a transmural infiltrate and intimal hyperplasia. Multiple linear regression analysis was performed with either halo counts or Halo Scores as the dependent variable. Predictive variables included the presence of a transmural TAB infiltrate, intimal hyperplasia and male sex.Results:The TAB was consistent with GCA in 27 patients. The TAB revealed transmural inflammation in 18 patients and giant cells in 24 patients. Intimal hyperplasia was found in 20 patients with a positive TAB. Patients with a positive TAB showed higher halo counts and Halo Scores than patients with a negative TAB. Overall, patients with a positive TAB and intimal hyperplasia presented with the highest halo counts and Halo Scores (Figure). Among patients with a positive TAB, only intimal hyperplasia and male sex were predictive of higher halo counts and Halo Scores in the multiple linear regression analysis. Ocular ischaemia was present in 14% of patients with a positive TAB without intimal hyperplasia. Purpose: To investigate visual function and vision-related quality of life (VR-QoL) changes in patients with myopic choroidal neovascularization (mCNV) after ranibizumab treatment.Methods: Quantitatively evaluate the objective tests of visual function (visual acuity, microperimetry, and metamorphopsia by m-Charts) before and after 3+prn (pro re neta) ranibizumab treatment for 1 year. The National Eye Institute 25-Item Visual Function Questionnaire (VFQ-25) was performed to evaluate the VR-QoL.Results: A total of 57 eyes of 57 patients were included in this study. The median average metamorphopsia score was 0.65 before treatment and improved to 0.45 after treatment (p = 0.0003). There was also a significant difference in the average threshold, macular integrity, and proportion of patients with stable fixation by the microperimetry (p &lt; 0.000, p &lt; 0.0001, and p = 0.03, respectively). After treatment, the VR-QoL composite, general vision subscale, and vision-related mental health subscale score were increased with borderline or statistical significance (p = 0.088, p = 0.0038, and p = 0.012, respectively). Subgroup analysis demonstrated parallel improvement of the VR-QoL score, metamorphopsia, average macular threshold, and fixation stability in patients with or without visual acuity increase. By multiple linear regression analysis, the VFQ-25 score after anti-VEGF treatment was only associated with the baseline VFQ-25 score and macular integrity. Improvements in the VFQ-25 score were only associated with changes in the metamorphopsia score.Conclusions: Integral lifting in several aspects of visual function was observed in mCNV after ranibizumab treatment. Macular integrity and metamorphopsia, but not visual acuity, were associated with VR-QoL.

This research goal is to identification and evaluation influence of work motivation and work environment to employee’s performance in BAPPEDA Kabupaten Wonosobo. The object of this research is 37 employees of Badan Perencanaan Pembangunan Kabupaten Wonosobo. And the location of this research is at Badan Perencanaan Pembangunan Kabupaten Wonosobo. The analysis used is test validity, reliability testing, and test the hypothesis, with the help of the computer program SPSS version 17, using multiple linear regression analysis. Based on calculations of data and analysis used, the regression equation is obtained: Y = 11.733 + 0.320 X1 +0.334 X2 + ε, by using the equation regression analytical method can conclude that (X1) take effect positively against employees performance. With t value in amount of 2,219 (bigger than t in table in amount of 1,690) and significance value in amount of 0,33. By applying significance limited value in amount of 0,05, it means, hypothesis that claim if work motivation take effect against employees performance can be accepted. There is a positive and significant correlation between work environment variables (X2) against employees. With t value in amount of 2,219 (bigger than t in table in amount of 1,690) and significance value in amount of 0,33 (smaller than 0,5). Simultaneously, work motivation take effect positively and significantly against employees performance with the F value in amount of 11,562 (bigger than 0.05), then obtained significance value 0.000. It can be concluded that the work motivation and work environment has a positive and significant influence on employee performance in BAPPEDA Kabupaten Wonosobo.

The study aims to analyze the influence of internal banking factors in the form of: Capital Adequency Ratio (CAR), Financing to Deposit Ratio (FDR) and Total Assets (TA) to Non Performing Financing at Sharia Banks. This research method used multiple linear regression analysis with the help of SPSS 16.00 software which is used to see the influence between the independent variables in the form of Capital Adequacy Ratio (CAR), Financing to Deposit Ratio (FDR) and Total Assets (TA) to Non Performing Financing. The sample of this study was 3 Islamic Commercial Banks, so there were 36 annual reports obtained through purposive sampling, then analyzed using multiple linear regression methods. The results showed that based on the F Test, the independent variable had an effect on the NPF, indicated by the F value of 17,016 and significance of 0,000, overall the independent variable was able to explain the effect of 69.60%. While based on the partial t test, showed that CAR has a significant negative effect, Total assets have a significant positive effect with a significance value below 0.05 (5%). Meanwhile FDR does not affect NPF.
