Background and Aims: Plant-forward dietary patterns have been associated with cardiometabolic health benefits, which, in turn, have been related to cognitive performance with inconsistent findings. The objective of this study was to examine the relationship between baseline adherence to three a priori dietary patterns (Mediterranean, DASH, and MIND diets) with 2-year changes in cognitive performance in older adults with overweight or obesity and high cardiovascular disease risk.Methods: A prospective cohort analysis was conducted within the PREDIMED-Plus trial, involving 6,647 men and women aged 55–75 years with overweight or obesity and metabolic syndrome. Using a validated, semiquantitative 143-item food frequency questionnaire completed at baseline, the dietary pattern adherence scores were calculated. An extensive neuropsychological test battery was administered at baseline and 2-year follow-up. Multivariable-adjusted linear regression models were used to assess associations between 2-year changes in cognitive function z-scores across tertiles of baseline adherence to the a priori dietary patterns.Results: Adherence to the Mediterranean diet at baseline was associated with 2-year changes in the general cognitive screening Mini-Mental State Examination (MMSE, β: 0.070; 95% CI: 0.014, 0.175, P-trend = 0.011), and two executive function-related assessments: the Trail Making Tests Part A (TMT-A, β: −0.054; 95% CI: −0.110, − 0.002, P-trend = 0.047) and Part B (TMT-B, β: −0.079; 95% CI: −0.134, −0.024, P-trend = 0.004). Adherence to the MIND diet was associated with the backward recall Digit Span Test assessment of working memory (DST-B, β: 0.058; 95% CI: 0.002, 0.114, P-trend = 0.045). However, higher adherence to the DASH dietary pattern was not associated with better cognitive function over a period of 2 years.Conclusion: In older Spanish individuals with overweight or obesity and at high cardiovascular disease risk, higher baseline adherence to the Mediterranean dietary pattern may be associated with better cognitive performance than lower adherence over a period of 2 years.

Background: Limited data are available on the association of major dietary patterns and predicted risk of cardiovascular disease (CVD) using the Framingham Risk Score (FRS). Aim: To investigate the association between major dietary patterns and the predicted 10-year CVD risk in an Iranian population Methods: This cross-sectional study was conducted on 522 apparently healthy adults referred to health centers in Tehran. Usual dietary intake was assessed using a validated semi-quantitative food frequency questionnaire. The association between dietary patterns and the predicted risk of development of CVD was evaluated using linear multiple regression. Principal component analysis was used to identify major dietary patterns. Results: In total, 41% of the men ( n = 95) and 46% of women ( n = 134) were in the low risk (<10%) and 6% of men ( n = 14) and 1% of women ( n = 3) were in the high risk (> 20%) category of the FRS. A significant decrease was found for body mass index ( p < 0.001), systolic blood pressure (SBP) ( p = 0.003), diastolic blood pressure ( p = 0.005), and triglyceride ( p = 0.02) in the higher tertile of healthy dietary pattern. Adherence to an unhealthy dietary pattern was associated with a significant increase in body weight ( p = 0.03) and a significant decrease in high-density lipoprotein cholesterol ( p < 0.001). The healthy dietary pattern score was negatively associated with SBP ( p = 0.04) and FRS ( p < 0.001). Conclusion: Although we observed improvements in CVD risk factors with greater adherence to healthy dietary patterns, there was no association between identified dietary patterns and the predicted risk of 10-year CVD.

ABSTRACTBackground:Evidence from observational studies to date suggests that healthy dietary patterns are associated with better cognitive performance in later life. We examined the extent to which childhood intelligence quotient (IQ) and socioeconomic status account for this association.Methods:Analyses were carried out on 882 participants in the Lothian Birth Cohort 1936 Study. Four dietary patterns were extracted using principal components analysis of a food frequency questionnaire, namely “Mediterranean-style,” “health aware,” “traditional,” and “sweet foods.” Cognitive function was assessed at the age of 70 years, including general (g) cognitive ability, processing speed, memory, and verbal ability.Results:Before adjustment for childhood IQ and socioeconomic status, the “Mediterranean-style” dietary pattern was associated with significantly better cognitive performance (effect size as partial eta-square (ηp2) range = 0.005 to 0.055), and the “traditional” dietary pattern was associated with poorer performance on all cognitive domains measured in old age (ηp2 = 0.009 to 0.103). After adjustment for childhood IQ (measured at the age of 11 years) and socioeconomic status, statistical significance was lost for most associations, with the exception of verbal ability and the “Mediterranean-style” pattern (National Adult Reading Test (NART) ηp2 = 0.006 and Wechsler Test of Adult Reading (WTAR) ηp2 = 0.013), and the “traditional” pattern (NART ηp2 = 0.035 and WTAR ηp2 = 0.027).Conclusions:Our results suggest a pattern of reverse causation or confounding; a higher childhood cognitive ability (and adult socioeconomic status) predicts adherence to a “healthy” diet and better cognitive performance in old age. Our models show no direct link between diet and cognitive performance in old age; instead they are related via the lifelong-stable trait of intelligence.

Objectives: The purpose of this study was to assess the relationship between dietary patterns and the 10-year risk of cardiovascular disease (CVD) in the elderly population in Korea. Methods: Cluster analysis was conducted on the data of 1687 elderly participants (797 men and 890 women) aged ≥65 years from the 2014–2016 Korea National Health and Nutrition Examination Survey (KNHANES), using a 24-h dietary recall survey to assess dietary patterns. Dietary patterns were classified into clusters 1 (typical Korean diet), 2 (high carbohydrate diet), and 3 (healthy diet). The 10-year risk of CVD was calculated based on age, total and HDL-cholesterol levels, systolic blood pressure level, antihypertensive medication use, smoking status, and presence of diabetes. A complex sample general linear model was applied to determine the association between dietary patterns and the 10-year risk of CVD. Results: In total, 275 (33.7%), 141 (17.9%), and 381 (48.3%) men, and 207 (22.6%), 276 (30.9%), and 407(46.6%) women were included in clusters 1, 2, and 3, respectively. The 10-year risk of CVD was lower in men in cluster 3 (healthy diet) than in those in cluster 1 (typical Korean diet) (t = 2.092, p = 0.037). Additionally, the 10-year risk of CVD was lower in men who performed strength training than in those who did not (t = 3.575, p < 0.001). There were no significant differences in women. Conclusions: After adjusting for sociodemographic variables, men who consumed a healthy diet had a lower 10-year risk of CVD than those who consumed a typical Korean diet. When organizing nutrition education programs to improve dietary habits in the elderly, content on diets that consist of various food groups to prevent CVD is required. In particular, it is necessary to develop content that emphasizes the importance of healthy eating habits in men.

Abstract Objectives The impact of habitual diet on chronic diseases has not been extensively characterized in South America. We aimed to identify major dietary patterns (DP) in a population adult cohort in Uruguay (Genotype Phenotype and Environment of Hypertension Study - GEFA-HT-UY) and to assess associations with metabolic and anthropometric markers of cardiovascular disease risk. Methods In a subsample (n = 295), DP were derived by principal component analysis based on 27 food groups (food frequency questionnaire). Total cholesterol, triglycerides (TG), low and high-density lipoproteins cholesterol (LDL, HDL), fasting glucose and insulin (HOMA), 25(OH)D, neutrophils and lymphocytes were measured in blood. Body weight, height, waist and blood pressure (BP) were measured. Multivariable linear regression models were used to estimate by tertile of DP load the adjusted changes of each outcome variable (relative to tertile 1), according to age (splitting by median age, 54 y). The models included sex, smoking, alcohol drinking, BMI, and season for 25(OH)D, as covariables. Results Three DP were identified: Meat (MDP), Prudent (PDP), and Cereal and Mate (CMDP), explaining 22.6% of total variance. MDP was characterized by higher loads for red, processed and barbecued meat; PDP by higher loads for vegetables, fish and nuts; and CMDP by higher loads for cereals and mate (traditional infused drink). Protein, sodium and alcohol intake increased, and fibre, mono and polyunsaturated fatty acids (MUFA, PUFA) intake decreased, by MDP tertile. Protein, fat (MUFA), fibre and calcium intake increased by PDP tertile. Carbohydrate, sodium and energy intake increased, and PUFA and calcium intake decreased, by CMDP tertile. MDP was associated with lower HDL (−3.1 mg/dl) and 25(OH)D (−3.5 ng/ml), and higher TG/HDL ratio (0.8) and HOMA (0.5), in the older group (P &lt; 0.05). PDP was associated with higher 25(OH)D in both age groups (2.9 and 7.7 ng/ml; P &lt; 0.002). CMDP was associated with higher neutrophils/lymphocytes ratio (0.35) in the younger group, and higher LDL (18.2 mg/dl), systolic BP (5.9 mm Hg) and waist/height ratio (0.024) in the older group (P &lt; 0.05). Conclusions Meat DP was associated with worse blood lipid profile in younger adults, Cereal and Mate DP with worse cardiovascular risk markers in older adults, and Prudent DP with higher 25(OH)D in both age groups. Background: Elevated cardiovascular disease risk factor burden is a recognized contributor to poorer cognitive function; however, the physiological mechanisms underlying this association are not well understood. We sought to assess the potential mediation effect of left ventricular (LV) remodeling on the association between lifetime systolic blood pressure and cognitive function in a community-based cohort of middle-aged adults. Methods: Nine hundred sixty participants of the Bogalusa Heart Study (59.2% women, 33.8% black, aged 48.4±5.1 years) received 2-dimensional echocardiography to quantify relative wall thickness, LV mass, and diastolic and systolic LV function; and a standardized neurocognitive battery to assess memory, executive functioning, and language processing. Multivariable linear regression assessed the association of cardiac structure and function with a global composite cognitive function score, adjusting for traditional cardiovascular disease risk factors. Mediation analysis assessed the effect of LV mass index on the association between lifetime systolic blood pressure burden and cognitive function. Results: There were 233 (24.3%) and 136 (14.2%) individuals with concentric LV remodeling and concentric LV hypertrophy, respectively. Each g/m 2.7 increment in LV mass index was associated with a 0.03 standardized unit decrement in global cognitive function ( P =0.03). Individuals with concentric LV remodeling and isolated diastolic dysfunction had the poorest cognitive function, and a greater ratio between early mitral inflow velocity and early diastolic mitral annular velocity (E/e’) was associated with poorer cognitive function, even after adjustment for LV mass index (B=−0.12; P =0.03). A total of 18.8% of the association between lifetime systolic blood pressure burden and midlife cognitive function was accounted for by LV mass index. Conclusions: Cardiac remodeling partially mediates the association between lifespan systolic blood pressure burden and adult cognition in individuals without dementia or clinical cardiovascular disease. Slowing or reversing the progression of cardiac remodeling in middle-age may be a novel therapeutic approach to prevent cognitive decline.

AbstractCardiovacular disease (CVD) is the leading cause of death among older adults and is often accompanied by functional decline. It is unclear what is driving this co-occurrence, but it may be behavioral, environmental and/or genetic. We used a family-based study to estimate the phenotypic and shared genetic correlation between CVD risk factors and physical and cognitive functional measures. Participants (n = 1,881) were from the Long Life Family Study, which enrolled families based on their exceptional longevity (sample mean age = 69.4 years, 44% female). Cardiovascular disease risk factors included carotid vessel measures [intima-media thickness and inter-adventitial diameter], obesity [body mass index (BMI) and waist circumference], and hypertension [systolic and diastolic blood pressures]. Function was measured in the physical [gait speed, grip strength, chair stand] and cognitive [digital symbol substitution test, retained and working memory, semantic fluency, and trail making tests] domains. We used SOLAR to estimate the genetic, environmental, and phenotypic correlation between each pair adjusting for age, age2, sex, field center, smoking, height, and weight. There were significant phenotypic correlations (range |0.05–0.22|) between CVD risk factors and physical and cognitive function (all P < 0.05). Most significant genetic correlations (range |0.21–0.62|) were between CVD risk factorsand cognitive function, although BMI and waist circumference had significant genetic correlation with gait speed and chair stand time (range |0.29–0.53|; all P < 0.05). These results suggest that CVD risk factors may share a common genetic-and thus, biologic-basis with both cognitive and physical function. This is particularly informative for research into the genetic determinants of chronic disease.
