This article aims to answer how a commissioning body can steer health services based on value in an environment where the commissioner is responsible for the health services of a population with varying health service needs. In this design science study, we constructed a value-based steering model consisting of three parts: (1) the principles of steering; (2) the steering process; and (3) Value Steering Canvas, a concrete tool for steering. The study is based on Finland, a tax-funded healthcare system, where healthcare is a public service. The results can be applied in any system where there is a commissioner and a service provider, whether they are two separate organizations or not. We conclude that steering can be done based on value. The commissioning body can start using value-based steering without changes in legislation or in the present service system. Further research is needed to test the model in practice.

As fiscal pressures mount, health-planning and decision-making at smaller geographics scales must be more effective. Involving local constituents in needs assessments, it is believed, would lead to better identification and serving of regional demands and needs for health services. This article examines needs assessment as a tool to determine a community's service needs and establish priorities for the creation of programs. Various approaches used in needs assessments are described, including survey methods, structured groups and geographic information systems.


The benefits of a fully-digitalised public healthcare system are significant. Digital health is an essential tool in order to improve efficiency, provide coordinated care, and make real health improvements. However, the National Health Service (NHS) has yet to provide a fully digitalised system to patients and providers despite technological advances in recent years. This chapter will thus describe the progress which has already been made in providing remote health services within the NHS. It will also explore problems arising from digitalising health services and the management of health both within the institution and through more informal networks beyond the NHS.

Background: The Republic of the Sudan located in north-east of Africa and is considered to be a lower-middle income country. The country has well established healthcare system with many drawbacks mainly due to economic and managerial reasons followed by prolonged political instability and sanctions.Objective: The aim of this study is to give an insight over the health services system in Sudan and to analyze the strength, weakness, opportunities, and threats (SWOT).Materials and Methods: The search was done from two electronic databases: MEDLINE/Pubmed and from public search engines: Google Scholar and Google with key Search words used mainly as “Healthcare system in Sudan “. Additionally, SWOT analysis of healthcare system in Sudan was carried out based on the Roemer’s model of health service system. Results: The Sudanese healthcare system was analyzed for different components of the system: The system in Sudan has full package of strategic plans and policies be it in a long term or short. Despite this there is poor implementation and organization along with frail health information system. The main external factors that drawback the system is the overall economic instability which resulted in cutting of the health expenditure.Conclusion: The Sudan is a rich country in terms of natural resources and population. Its health service system has strengths and weaknesses. It needs to build on its qualified human work force, stress on its well-designed short and long-term strategies on health care system and the partnership with external funding institutions, while overcoming the challenges on creating the proper health information system, economic support system and centralization of health service and professionals. Keywords: Health Care System, Sudan, SWOT analysis

The Classification and Statistical Manual of Mental Health Concerns ( CSM) is a proposed alternative to the Diagnostic and Statistical Manual of Mental Disorders ( DSM) and International Classification of Diseases ( ICD). In contrast to the DSM/ ICD overarching concept of “mental disorders,” the CSM’s overarching concept is “mental health concerns.” A mental health concern occurs when a person seeking mental health services expresses to a mental health service provider a concern about any of these topics: behavior, emotion, mood, meaning of life, death, dying, managing chronic pain, addiction, work, relationships, education, eating, cognition, sleep, and challenging life situations. The CSM begins from the perspective of the person seeking services and that is what would be classified in its manual. In addition to classifying mental health concerns, the CSM would describe a collaborative approach between the person expressing the concern and the mental health service provider for creating a psychological formulation narrative that eschews the DSM/ ICD pathologizing jargon. Compared with the DSM/ ICD approach, the use of the CSM potentially would be less stigmatizing, as well as more practical. Moreover, it would be more consistent with principles of science, eliminate the monopoly of the DSM/ ICD mental disorder approach for accessing mental health services, provide a new choice to both mental health service users and providers, challenge old ideas, stimulate fresh perspectives, and open new avenues of research.

Abstract Introduction: Healthcare service is an essential determinant to population health. This qualitative case study aims to explore health service users’ perspective of effective health services delivery and the current challenges affecting the management and delivery of health services at a primary healthcare facility in Madang Province, Papua New Guinea (PNG). Methods: Qualitative data were collected using semi-structured interviews with key informants (KI) representing academics, undergraduate students, administration staff, patients and healthcare workers. The interviews covered three main areas: users’ views of effective health services, current challenges affecting effective planning and management of primary healthcare services, and interventions to improve health services planning and delivery. Results: The services users associated effective health services delivery with increased availability, accessibility, acceptability, and quality healthcare. Many factors exist to influence the effective planning and delivery of health services. The results show that health systems and personal factors have a major influence on the planning and health services delivery. Conclusion: The findings from this study call for an evaluation of the current healthcare system, particularly at the primary healthcare level, as the primary point of contact to the formal healthcare system, and the need for developing a contextual model of healthcare that meet the needs of the service users. We concluded that if health services users’ perspectives are considered in health policy, the local community may experience significant improvement in health status.

This study will examine the system of implementing the Banyuwangi Regional healthcare program, especially in the implementation of pro-poor budgeting in poverty reduction. The method used in this study is descriptive qualitative with respondents are resource persons who are beneficiaries of regional healthcare, health services and technical support officers at the village level. The results of this study indicate that the implementation of public healthcare in Banyuwangi Regency has been running well overall. Regional healthcare in Banyuwangi Regency, as usual, called poor statement letters (SPM). In terms of implementation, the comparison between manual and online management of SPM is very different. The arrangement of SPM online in accordance with the statement of the village head and health service only requires a maximum of one day. Whereas in reality, online SPM management can reach three working days, faster than the time needed for manual management because it takes approximately one week. The difference in the management of the SPM needs to be minimized so that all communities are able to obtain healthcare benefits appropriately. Most of the respondents, 95 percent stated that the regional healthcare program provided considerable benefits for the community, especially the poor.

Founder and central leaders of policymakers and development in the health sector said several obstacles must be addressed in the health service system, one of which is the low level of hospital services in Indonesia. The health service system in Indonesia still needs to be improved by the government, both in terms of equity, regulation, and system integration. To achieve the goals of sustainable health services, it is necessary to implement an integrated system (internal control system) in the hospital. An effective internal control system can anticipate various environmental changes that occur quickly, increase operational effectiveness and efficiency, and anticipate law violations. To improve the quality of sustainable health services, apart from implementing an internal control system, hospitals also need to carry out various innovations to provide changes in the bureaucratic climate. Innovations are carried out based on learning on all processes that have been carried out by the hospital. Therefore, the objective to be achieved in this study is to determine the effect of internal control systems and organizational culture (innovation and learning culture) on the quality of sustainable public services at BLUD RSUD dr. Slamet, Garut Regency. The data was collected using a survey method using a questionnaire. Data were analyzed using multiple regression methods. The results showed no influence on the internal control system on the quality of sustainable public services, but there was an influence of organizational culture on the quality of sustainable public services.

In cases of tooth loss caused by extraction, further treatment must be carried out immediately, namely the use of dentures. Making dentures can be done by medical personnel such as dentists, dental specialists, but many people make dentures at dental artisans. The purpose of this study was to determine the factors that influence people's interest in making removable dentures to dental technicians in Siantan Hulu Village, North Pontianak in 2015. This research method was through a survey, explanatory research type with a cross sectional approach. Samples taken by people who use the services of dental artisans to make removable dentures are 40 respondents. The results of the Product Moment Correlation Test obtained the perception of the value of rs = 0.047 and the value of /Probability = 0.773. and the distance traveled with a value of rs = 0.243 and a value of /Probability = 0.131 so that it does not have a relationship with public interest in making removable dentures to dental artisans. For motivation with a value of rs = 0.704 and a value of /Probability = 0.000, health service facilities with a value of rs = 0.431 and a value of /Probability = 0.005 and a tariff with a value of rs = 0.520 and a value of /Probability = 0.001, has a relationship with public interest to make dentures to a dentist. The conclusion of the study is that perception and distance do not affect people's interest, while motivation, health service facilities and tariffs affect people's interest in making dentures for dental artisans. People are expected to choose health services wisely. Kasus kehilangan gigi yang disebabkan oleh pencabutan, harus segera dilakukan perawatan lebih lanjut yaitu dengan pemakaian gigi tiruan. Pembuatan gigi tiruan dapat dilakukan oleh tenaga medis seperti dokter gigi, dokter gigi spesialis, namun banyak masyarakat yang membuat gigi tiruan di tukang gigi. Tujuan penelitian ini adalah untuk mengetahui faktor-faktor yang mempengaruhi minat masyarakat untuk membuat gigi tiruan lepasan ke tukang gigi di Kelurahan Siantan Hulu Pontianak Utara tahun 2015. Metode penelitian ini melalui survey, jenis explanatory research dengan pendekatan cross sectional. Sampel yang diambil masyarakat yang menggunakan jasa tukang gigi untuk membuat gigi tiruan lepasan yaitu berjumlah 40 responden. Hasil dari Uji Korelasi Product Moment didapatkan hasil persepsi dengan nilai rs = 0.047 dan nilai /Probabilitas = 0.773. dan jarak tempuh dengan nilai rs = 0.243 dan nilai /Probabilitas = 0.131 sehingga tidak memiliki hubungan terhadap minat masyarakat untuk membuat gigi tiruan lepasan ke tukang gigi. Untuk motivasi dengan nilai rs = 0.704 dan nilai /Probabilitas= 0.000, sarana pelayanan kesehatan dengan nilai rs = 0.431 dan nilai /Probabilitas = 0.005 dan tarif dengan nilai rs = 0.520 dan nilai /Probabilitas = 0.001, memiliki hubungan terhadap minat masyarakat untuk membuat gigi tiruan ke tukang gigi. Kesimpulan dari penelitian ialah persepsi dan jarak tidak berpengaruh terhadap minat masyarakat, sementara motivasi, sarana pelayanan kesehatan dan tarif berpengaruh terhadap minat masyarakat untuk membuat gigi tiruan ke tukang gigi.  Masyarakat diharapkan dapat memilih pelayanan kesehatan dengan bijak.
