Global increases in both population size and ageing have led to a drastic expansion in the demand for healthcare services. The shortage of nursing workforce capacity continues, posing immense challenges for the global healthcare system. We aimed to identify the antecedents and contextual factors that contribute to the decisions of occupational turnover from the clinical duties of registered nurses in public hospitals in Hong Kong. A qualitative descriptive design was used in this study. A total of 18 registered nurses who had resigned from public hospitals in Hong Kong and changed their occupations were recruited via convenience and snowball sampling methods. Data were collected through individual, semi-structured, and face-to-face interviews and were analyzed according to the content analysis approach. The antecedents and contextual factors that contributed to the registered nurses’ decisions regarding occupational turnover were identified from the collected data. These factors were classified into three overarching categories: (1) job dissatisfaction due to a tense work environment, (2) low motivation due to limited career opportunities, and (3) inadequate communication due to ineffective leadership. The identification of these antecedents and contextual factors could help healthcare service providers to develop strategies to enhance nurses’ commitment and engagement in their positions and eventually improve their retention. Based on these factors, healthcare sector policy makers could consider incorporating appropriate strategies into healthcare system policy.

The revolution in the Internet of Things (IoT) is redesigning and reshaping the healthcare system technologically, economically and socially. The emerging and rapidly growing IoT-based Smart Healthcare System (SHCS) is seen as a sustainable solution to reduce the burden on the existing healthcare system due to increasing diseases and limited medical infrastructure. IoT-based SHCS plays a vital role in delivery of healthcare services in rural and remote areas where the essential medical amenities, necessary infrastructures and qualified medical practitioners are not available. Therefore, in this paper, a comprehensive investigation of futuristic IoT-based SHCS and its constituents is presented. This paper provides exhaustive review on different techniques and technologies dealing with smart healthcare framework, physiological sensing, signal processing, data communication, cloud computing and data analytics used in IoT-based SHCS. A comparative analysis of existing literature has been carried out to identify the recent trends and advancements in this very dynamic field of global importance. In addition to this, it highlights different issues and challenges, along with the recommendation for further research in the field. The prime objective of this paper is to deliver the state-of-the-art understanding and update about IoT-based SHCS and its constituents by providing a good source of information to the researchers, service providers, technologists, medical practitioners and the general population.

Research on service quality has been performed by researchers on various research objects. In many previous studies, service quality provided by service providers became the factor that affects customer satisfaction and other various research variables. The study was conducted at public hospitals that offer healthcare services with a focus on service quality. The purpose of this study was to analyze the service quality provided by public hospitals, and to test their effect on the satisfaction and trust of patients or attendants. The objects of this research are the public hospitals in Bandung with the patients or attendants as the unit of analysis. Primary data were obtained through the dissemination of questionnaires on patients or attendants to obtain data on their evaluation of service quality of public hospitals, and their satisfaction and trust towards public hospitals. Obtained data was processed using structural equation modeling to confirm causality of related research variable. The results depict the service quality provided by public hospitals in Bandung covering infrastructure, personnel quality, clinical care process, administrative procedures, safety indicators, and social responsibility. Moreover, the results of hypothesis testing show that service quality has an influence on patient satisfaction, and ultimately affects their trust in the hospital.

Assessing the effectiveness of the Georgian healthcare system in the modern period and conditions, takes into account the results achieved, as well as the main problems that hinder the effective functioning of this important field; At the same time, it is important to develop the main directions of their solution, where we consider the improvement of the state policy-making process during the implementation of reforms in the healthcare sector, which should take into account the state of health of the population, quality of healthcare services, results, health care; furthermore disease prevention, equality, financial provision, access to health care, efficiency, rational allocation of health care system resources and other key features of the health care system.


The quality of human life has improved with the technological advancement and miniaturization of sensors. Healthcare sector has adopted the new technology with the results of related research. Still, healthcare services are not easily affordable in developing countries. This paper aims to reduce the expenses of healthcare system through designing a remote healthcare system comprising of three modules. The first module deals with detection of patient’s vitals using sensors. Second module is to collect and analyze the data and can be sent back to the doctor or guardian through Email and/or SMS alerts in case of any emergencies using Arduino. Third module implemented by K nearest neighbor classification algorithm is used to make runtime decisions if the alert message is not answered by doctors. By implementing these three modules, we achieved a better results in prediction and accuracy than the Attribute based credential and Amrita IoT-based Medical.

Abstract Background: There is a growing interest in redesigning healthcare systems to increase access to and coordination across care settings for people with chronic conditions. We aim to promote patient-centredness by gaining a better understanding of the barriers faced by (1) children with chronic bronchial asthma, (2) adults with non-specific chronic back pain, and (3) older people with pre-existing mental illness/es while addressing the problem of fragmentation in Austria’s healthcare system.Methods: Using a qualitative design, we conducted semi-structured interviews face-to-face and by telephone with health service providers, researchers, experts by experience (e.g., patient advocates, family members/carers), and employees in public health administration between July and October 2019. The analysis and interpretation of data were guided by Levesque’s model of access, a conceptual framework used to evaluate access broadly according to five dimensions of accessibility to care: approachability, acceptability, availability and accommodation, affordability, and appropriateness.Results: The findings from the 25 expert interviews were organised within Levesque’s conceptual framework in four dimensions of barriers. They highlight the lack of coordination and defined patient pathways, particularly at the onset of the condition, when seeking a diagnosis, but also later on. Barriers related to issues such as geography (e.g., longer distances and travel times as well as fewer healthcare services in remote rural regions), coordination of care (e.g., structural barriers and inconsistent care pathways), socioeconomic status (e.g., the ability to pay for healthcare services but also basic health literacy), and comorbidity (e.g., consideration of the patients’ state of mental health and holistic therapy approaches).Conclusions: Barriers to healthcare access are of particular concern for patients with chronic conditions, resulting in an urgent need to improve health services according to patients’ specific needs. For health services to be properly accessible, timely and integrated care independent of social and economic resources, continuity of care, and significant improvements in patient-centred communication and coordination of care are paramount. This study has important implications for policy and practice as it highlights provider, expert, and researcher perspectives on access to the healthcare system in Austria and offers a broader look at the changes required by stakeholders in the future.

The healthcare industry is among the most rapidly growing service industries in the world economy. In recent years, the government of Malaysia has taken various initiative to increase funding for the healthcare sector to improve the quality of life amongst citizens. However, the continuing complaints regarding patients’ dissatisfaction towards the quality of healthcare services lead to a huge challenge to the service providers. Thus, the main objective of this research is to measure patient satisfaction with healthcare service quality. The selected Malaysian public hospitals were involved in this research. A questionnaire was used as a research instrument distributed to the patient's experienced service delivery at Malaysian public hospitals. The convenience sampling technique was employed to gather the data. Data collected were analyzed using SERVQUAL-gap analysis. The descriptive results revealed that the patients were only moderately satisfied with healthcare service quality rendered byMalaysian public hospitals. Based on the SERVQUAL-gap analysis results, the empathy dimension showed the lowest mean value. This indicates that service quality offered by Malaysian public hospitals still needs improvement in terms of the level of caring and needs as well as individualized attention that the hospital needs to provide to their patients. This research benefits the managerial in the healthcare industry to deliver excellent service experience to the patients by considering the relevant indicators of service evaluation.

Abstract This study critically evaluates the adoption of a universal healthcare system recently introduced by the Indonesian government in 2014. Our study is driven by the lack of critical analysis of social and political factors and unintended consequences of New Public Management, which is evident in the healthcare sector reforms in emerging economies. This study not only examines the impact of economic and political forces surrounding the introduction of a universal health insurance programme in the country but also offers insights into the critical challenges and undesirable outcomes of a fundamental reform of the healthcare sector in Indonesia. Through a systematic and detailed review of prior studies, legal sources and reports from government and media organizations about the implementation and progress of an UHC health insurance programme in Indonesia, the authors find that a more democratic political system that emerged in 1998 created the opportunity for politicians and international financial aid agencies to introduce a universal social security administration agency called Badan Penyelenggara Jaminan Sosial (BPJS). Despite the introduction of BPJS to expand the health services’ coverage, this effort faces critical challenges and unintended outcomes including: (1) increased financial deficits, (2) resistance from medical professionals and (3) politicians’ tendency to blame BPJS’s management for failing to pay healthcare services costs. We argue that the adoption of the insurance system was primarily motivated by politicians’ own interests and those of international agencies at the expense of a sustainable national healthcare system. This study contributes to the healthcare industry policy literature by showing that a poorly designed UHC system could and will undermine the core values of healthcare services. It will also threaten the sustainability of the medical profession in Indonesia. The authors offer several suggestions for devising better policies in this sector in the developing nations.

An attempt has been made in this article to shed some light on the decisions of patients on their institutional choice in accessing healthcare services within micro empirical framework in the state of West Bengal, India. The determinants of such decisions are also recognized in this article. In conscientious, statistical methods are anticipated to frame the socio-economic and decisional variables, that influence the decision. Principal component analysis and ordered probit analysis have been deployed to scrutinize the same. It is observed that the economic status of the patients, their level of schooling, their income and some other decision variables have a strong influence on their access of healthcare institutions. The study has projected that intra-category and inter-category competition among available alternative service providers needs to be introduced and social entrepreneurship model in state-owned hospitals can be pioneered with due considerations.

With growing globalisation and an increasing number of people on the move across boundaries, it has become vital that service providers, policy makers and mental health professionals are aware of the different needs of the patients they are responsible. One of the most fundamental barriers for migrants, refugees and asylum seekers in accessing health services are inadequate legal entitlement and, mechanisms for ensuring that they are well known and respected in practice. Access to the healthcare system is impeded by language and cultural communication problems. Qualified language and cultural mediators are not widely available, and moreover, are not regularly asked to attend. This can lead to misunderstandings, misdiagnosis and incorrect treatment, with serious consequences for the afflicted. The language barrier represents one of the main barriers to access to the healthcare system for people who do not speak the local language; indeed, language is the main working tool of psychiatry and psychotherapy, without which successful communication is impossible. Additionally, the lack of health literacy among the staff of institutions, which provide care for refugees and asylum seekers means that there is a lack of knowledge about the main symptoms of common mental health problems among these groups. The healthcare services, which are currently available, are not well prepared for these increasing specific groups. In dealing with ethnic minorities, including asylum seekers and refugees, mental healthcare professionals need to be culturally competent.In this talk, main models for providing mental health care for migrants and refugees will be presented and discussed.Disclosure of interestThe author has not supplied his declaration of competing interest.

The objective of this paper is to measure the worthiness which the compliance of governance norms will provide to public organizations. We introduce the principles of corporate governance that should characterize the function of public sector and our analysis focuses on public hospitals. Sending appropriate designed questionnaires to an adequate sample of Greek public hospitals, we measure and analyze the impact that the implementation of a corporate governance code would have to hospitals’ administration, control system and communication with stakeholders. According to research’s results the implementation of a corporate governance code by public hospitals could add value to the provided healthcare services.
