scholarly journals Opposite Effects of Work-Related Physical Activity and Leisure-Time Physical Activity on the Risk of Diabetes in Korean Adults

The object of this study was to examine the effects of domestic and work-related physical activity (DWPA) and leisure-time physical activity (LTPA) on the risk of diabetes, by categorizing fasting blood glucose (FBG) levels into normal, Impaired Fasting Glucose (IFG), and diabetes. The sample consisted of 4661 adults aged 30 years or above, and was chosen from the 2017 Korean National Health and Nutrition Examination Survey (KNHANES) data. Of all the subjects, 14.6% engaged in high-intensity DWPA and 6.25% in moderate-intensity DWPA; while 11.68% and 24.80% engaged in high- and moderate-intensity LTPA, respectively. The effects of both types of physical activities on the risk of diabetes were analyzed using a Bayesian ordered probit model. For those with high-intensity DWPA, the probability of the FBG level being normal was 5.10% (SE = 0.25) lower than for those with non-high-intensity DWPA, and the probabilities of IFG and diabetes were 3.30% (SE = 0.15) and 1.79% (SE = 0.09) higher, respectively. However, for those with high-intensity LTPA, the probability of the FBG level being normal was 2.54% (SE = 0.09) higher, and the probabilities of IFG and diabetes were 1.74% (SE = 0.07) and 0.80% (SE = 0.03) lower, respectively, than those with non-high-intensity LTPA. Likewise, for moderate-intensity DWPA and LTPA, the results were the same compared to low-intensity physical activities though the magnitude of the effects were smaller than for high-intensity. Thus, the activities related to work have a negative effect and those related to leisure have a positive effect. The criteria for physical activities to reduce the risk of diabetes should be set by separating these domains of physical activity, and new management strategies for diabetes are needed for people with moderate- or high-intensity DWPA.

Abstract Background According to the World Health Organization, non-communicable diseases or chronic diseases, account for 71 percent of all deaths globally, affecting individuals of all age groups and countries. Epidemiological studies have shown an association between a lack of physical activity (PA) and the risk of raised blood pressure, increased body mass index and obesity that can lead to chronic diseases. Methods The aim of the study was to assess the relationship between leisure-time PA and the risk of chronic diseases among adult population. We conducted a cross-sectional study of 470 men and 641 women who participated in “The Impact of Sustainable Mobility, Physical Activity and Environmental Factors on Urban Population Health Study (ISMA)” in Lithuania. Leisure-time PA was self-reported and divided into three levels: low, moderate and high. Logistic regression was used to estimate adjusted odds ratios (ORs) of chronic diseases according to leisure-time PA. Results More than half (55%) of the study population engaged in a moderate intensity PA and 12% of participants engaged in a high intensity leisure-time PA. Men 1.6 times more often than women reported engaging in a high intensity PA in leisure time (p = 0.004). Adults who engaged in low intensity leisure-time PA had almost 3 times (OR = 2.91; 95% confidence interval (CI) 1.62-5.24) higher risk of chronic diseases, whereas those adults who reported moderate intensity leisure-time PA had an OR of 1.63 (95% CI 0.92-2.87) compared to individuals who engaged in a high-level PA. Conclusions Our findings suggest that high levels of leisure-time PA could reduce the risk of chronic diseases and this effect was significantly stronger in men. Key messages Global decline in PA is related to inaction during leisure time and sedentary behaviour at work and at home that has a major health impact worldwide. High levels of leisure-time PA are important factor for chronic disease prevention.

Introduction: Latinos engage in higher levels of occupational physical activity, yet lower levels of leisure-time physical activity (LTPA), than other racial/ethnic groups. Limited research has examined specific work-based activities that contribute to lower LTPA among Latinos. The purpose of our study was to examine associations between frequency of work-related exertion and standing/walking with LTPA among Latinos. Hypothesis: We hypothesized that higher frequency of work-related exertion and standing/walking would be associated with lower levels of LTPA among Latinos. Methods: We used cross-sectional 2015 National Health Interview Survey (NHIS) data on employed Latinos ≥18 years of age (n=3,305). We examined two types of work-based activities: frequency of exertion and frequency of standing/walking. Frequency of exertion was assessed based on reported frequency of engaging in repeated lifting, pulling, pushing, or bending at work. Frequency of standing/walking was assessed based on frequency with which their work involved standing or walking around. Responses to work-based activity items were on a Likert scale (0=never, 1=seldom, 2= sometimes, 3=often, 4=always). LTPA was measured based on minutes of moderate-to-vigorous LTPA aerobic physical activity per week, which were categorized into none (0 min/week), some LTPA (>0 min/week of activity but less than recommendation), and meeting the LTPA guideline (≥150 min of moderate-vigorous activity or ≥75 min of vigorous activity or 150 min of moderate activity/week). Multinomial logistic regression models that accounted for the complex survey design of NHIS were used to estimate the associations between frequency of work-related exertion and standing/walking with LTPA. All models were adjusted for age, sex, education, shift work, and acculturation proxies (nativity, years living in the US). Results: In adjusted models, compared with those who reported never engaging in exertion at work, individuals reporting always exerting themselves at work were significantly less likely to meet the LTPA guideline (Odds Ratio [OR]: 0.68, 95% Confidence Interval [CI]: 0.50-0.94). In addition, compared with those that reported never standing/walking at work, only those who reported that their job seldom involved standing/walking at work were significantly less likely to meet the LTPA guideline (OR: 0.52, 95% CI: 0.30-0.92). Conclusions: In conclusion, these findings suggest that work-related physical activities may contribute to LTPA levels among Latinos, and associations vary based on the type of physical activities performed at work. Future physical activity promotion efforts among Latinos should consider the role of worksite policies to promote wellness.

Aim: In spite of the many health-related benefits of regular physical activity, fatiguing work may be a barrier to performing leisure-time physical activity. This study investigates the association between work-related fatigue and the duration of low- and high-intensity leisure-time physical activity in workers with sedentary and physically demanding jobs. Methods: From the 2010 round of the Danish Work Environment Cohort Study, currently employed wage earners from the general working population ( N=10,427) replied to questions about work-related fatigue (predictor) and duration of low- and high-intensity leisure-time physical activity (outcome). Associations were modelled using general linear models controlling for various confounders. Results: Among workers with physically demanding jobs, higher levels of work-related fatigue were associated with gradually lower levels of leisure-time physical activity – for low, moderate and high levels of work-related fatigue the duration of high-intensity leisure-time physical activity was 133 (95% confidence interval (CI) 127–178), 134 (95% CI 109–160) and 113 (95% CI 86–140) min per week, respectively (trend test p<0.001). The duration of high-intensity leisure-time physical activity was lower among older workers (≥50 years) compared to younger workers (<50 years) (132 ± 126 vs 168 ± 150 min per week) ( p<0.0001). Conclusions: The duration of high-intensity leisure-time physical activity gradually decreases with increased work-related fatigue in workers with physically demanding jobs. Older workers perform less high-intensity physical activity than younger workers. Workplaces should consider initiatives to allow workers with physically demanding jobs and older workers to perform physical exercise during working hours and thereby increase physical capacity to meet the job demands.

Abstract Background The association between leisure-time physical activity and cardiovascular mortality has been previously studied, but few studies have focused on specific activities and intensities. Methods The association between different leisure-time physical activities and cardiovascular mortality was investigated among 25,876 individuals without diabetes or cardiovascular disease from the population-based Malmö Diet and Cancer Study cohort. The individuals estimated the average duration spent on 17 physical activities at baseline in 1991–1996 and after 5 years. Cardiovascular mortality was obtained from a register during a mean of 20 years of follow-up. Results A total leisure-time physical activity of 15–25 metabolic equivalent task (MET) hours/week was associated with a decreased risk of cardiovascular mortality (HR 15–25 vs < 7.5 MET-h/week =0.80, 95% CI 0.69–0.93), with no further risk reduction at higher levels. Several high-intensity activities (i.e., lawn tennis and running) and moderate-intensity activities (i.e., golf, cycling and gardening) were associated with a reduced risk. Individuals who engaged in high-intensity physical activity for an average of 2.29 MET h/week (30 min/week) had an 18% (95% CI 0.72–0.93) reduced risk of cardiovascular mortality compared with non-participants, and no further risk reductions were observed at higher levels. Decreased risk was observed among individuals who had started (HR 0.56, 95% CI 0.32–0.97) or continued (HR 0.49, 95% CI 0.36–0.66) high-intensity activities at the five-year follow-up. Conclusions Moderate- and high-intensity leisure-time physical activities reduced the risk of cardiovascular mortality. With regard to total leisure-time physical activity, the largest risk reduction was observed for 15–25 MET-h/week (equivalent to walking for approximately 5 h/week).

Background. Work is reported as one of the main sources of psychological stress. Because of its role in the onset of burnout and impact on economic and health systems, work-related stress (WS) has become an issue of much concern. Among modifiable factors capable of reducing WS, two categories of physical activity (PA), namely leisure-time and moderate-to-vigorous physical activity (LTPA and MVPA), show promising evidence. Previous findings suggest that LTPA and MVPA allow adults to experience psychological detachment from job demands and restore their depleted resources at work. However, the optimal independent doses of LTPA and MVPA associated with a lower WS level has not yet been established. Methods. The aim of this study was therefore to address this gap using a cross-sectional, nationally representative sample of 4 200 Canadian workers. MVPA was measured through accelerometry and a self-reported assessment was conducted to collect data on WS and LTPA. Results. Generalized additive models indicated that one hour a day on average spent doing a LTPA of 8.5 METs-hour was associated with the highest benefits on WS (p < 0.001, Adjusted R2 = 0.04) while the optimal average daily dose of MVPA was around 90 minutes (p < 0.001, Adjusted R2 = 0.04). Noteworthy, first signs of WS reduction appear long before optimum is reached (e.g. 30 minutes of MVPA), stressing the relevance of merely doing an LTPA/MVPA regardless of the dose. Conclusion. Findings offer practical recommendations for public health policies on the optimal doses of MVPA/LTPA associated with decreased WS.

We examined the association between objective and perceived neighborhood characteristics and self-reported leisure-time physical activity (PA) in older Japanese residents living in areas ranging from metropolitan to rural in 2016. Objective measures used were walkability and the numbers of parks/green spaces and sports facilities within 500 or 1000 m of subjects’ homes, calculated using geographic information systems. Subjective measures were the subjects’ perceptions of their neighborhoods, assessed using a structured questionnaire. All variables were divided into three groups, and the lowest tertile was used as the reference. We assessed the location and frequency of strolling or brisk walking, moderate-intensity PA, and vigorous-intensity PA (sports) using a self-reported questionnaire and defined as performing a certain type of PA 3–4 times/week as a habit. Living in a neighborhood in the highest tertile for walkability and number of parks/green spaces as well as perception of having good access to recreational facilities, observing others exercising and the presence of walkable sidewalks was associated with walking and sports habits (multivariable odds ratios (ORs): 1.33–2.46, all p < 0.05). Interestingly, objective measures of PA-friendly environmental features were inversely associated with moderate-intensity PA habits, potentially because moderate-intensity PA consisted predominantly of gardening. In conclusion, living in an environment supportive of PA, whether objectively or subjectively measured, is related to leisure-time PA habits among older Japanese adults.

Aims: To investigate the associations between both high- and low-intensity leisure-time physical activity on physical-work ability and chronic musculoskeletal complaints among manual workers aged 50–70 years. Methods: In this cross-sectional study, 1763 manual workers (mean age 57.1, SD ± 4.7 years) replied to questions about leisure-time physical activity, physical-work ability, chronic musculoskeletal complaints as well as health and lifestyle factors. Ordinal and binomial logistic regression models were constructed to assess the influence of the duration of both low- and high-intensity physical activity on physical-work ability and chronic musculoskeletal complaints, respectively. Models were adjusted for gender, age, smoking, alcohol intake, body mass index, physical-work demand, work experience, chronic disease and, regarding physical-work ability, also for chronic musculoskeletal complaints. Results: A statistically significant association between greater physical-work ability and high-intensity physical activity was found for workers engaged in 3–4 h/wk and ⩾ 5 h/wk (OR 1.59, 95% CI 1.15–2.19 and OR 1.56, 95% CI 1.10–2.22, respectively). For both high- and low-intensity physical activity, a duration of 3–4 h/wk was associated with lower odds of reporting chronic musculoskeletal complaints in the knees (OR 0.65, 95% CI 0.42–1.00 and OR 0.61, 95% CI 0.38–0.99, respectively). Conclusions: Engaging in ⩾ 3 h/wk of high-intensity leisure-time physical activity was associated with greater physical-work ability among manual workers aged 50–70 years. Both high- and low-intensity physical activity related to lower odds of having chronic musculoskeletal complaints in the knees.

Using the Theory of Planned Behavior as a framework, the Attitude to Leisure-time Physical Activity, Expectations of Others, Perceived Control, and Intention to Engage in Leisure-time Physical Activity scales were developed for use among high school students. The study population included 20 boys and 68 girls 13 to 17 years of age (for boys, M=15.1 yr., SD=1.0; for girls, M= 15.0 yr., SD= 1.1). Generation of items and the establishment of content validity were performed by professionals in exercise physiology, physical education, and clinical psychology. Each scale item was phrased in a Likert type format. Both unipolar and bipolar scales with seven response choices were developed. Following the pilot testing and subsequent revisions, 32 items were retained in the Attitude to Leisure-time Physical Activity scale. 10 items were retained in the Expectations of Others scale, 3 items were retained in the Perceived Control Scale, and 24 items were retained in the Intention to Engage in Leisure-time Physical Activity scale. Coefficients indicated adequate stability and internal consistency with α ranging from .81 to .96. Studies of validities are underway, after which scales would be made available to those interested in intervention techniques for promoting positive attitudes toward physical fitness, perception of control over engaging in leisure-lime physical activities, and good intentions to engage in leisure-time physical activities. The present results arc encouraging.
