As adolescents spend the majority of their time focused on exams and assignments, they do not have sufficient time to engage in physical activity; this lack of physical activity is an important public health concern. This study aimed to investigate how school-based physical activity programs affect the health-related physical fitness of adolescents in the Republic of Korea. For this study, a total of 120 high school students participated in a school-based physical activity program that included badminton and table tennis for 15 weeks each (35 min/day, three times a week), with a total of 30 weeks for one academic year. The parameters for health-related physical fitness measured muscle strength (handgrip strength), power (standing long jump), cardiorespiratory fitness (shuttle run test), flexibility (sit and reach), body mass index (BMI), and the total score. The results revealed a statistically significant improvement in muscle strength (p < 0.001), power (p < 0.001), cardiorespiratory fitness (p < 0.001), flexibility (p = 0.005), and the overall health-related physical fitness score (p = 0.001). However, students’ BMI showed no significant difference before and after participation (p = 0.825). The results of this study indicated that school-based physical activity programs can have a positive effect on the health-related physical fitness of adolescents.

Abstract Background Physical fitness is a marker of health and is associated with health-related quality of life (HRQoL). Identifying which components of physical fitness are associated with HRQoL in people with fibromyalgia may contribute to the development of more specific therapeutic strategies. Objective The 2 aims of this study were to examine the association of different components of physical fitness (ie, flexibility, muscle strength, speed and agility, and cardiorespiratory fitness) with HRQoL and to determine the extent to which any association between the components of physical fitness and HRQoL were of clinical relevance to women with fibromyalgia. Design A cross-sectional design was used. Methods This study included 466 women with fibromyalgia from southern Spain (Andalusia). The Senior Fitness Test battery and the handgrip test were used to assess physical fitness, and the 36-Item Short-Form Health Survey (SF-36) was used to assess HRQoL. Tender points, cognitive impairment, anthropometric measurements, and medication usage were also measured. First, multivariate linear regression was used to assess the individual relationship of each physical fitness test with the 8 dimensions of the SF-36. Second, a standardized composite score was computed for each component of physical fitness (flexibility, muscle strength, speed and agility, and cardiorespiratory fitness). A 1-way analysis of covariance to assess the differences in each of the 8 dimensions of the SF-36 across each physical fitness composite score was conducted. Forward stepwise regression was performed to analyze which components of physical fitness were independently associated with the SF-36 physical and mental component scales. Results Overall, higher levels of physical fitness were associated with higher levels of HRQoL (regardless of the SF-36 subscale evaluated). The effect sizes for HRQoL between participants with the lowest and the highest physical fitness levels ranged from moderate to large (Cohen d = 0.53–0.90). The muscle strength composite score was independently associated with the SF-36 physical component scale, whereas the flexibility composite score and cardiorespiratory fitness were independently associated with the SF-36 mental component scale. Limitations A limitation was that the cross-sectional design precluded the establishment of causality. Additionally, only women were included in the study, because fibromyalgia predominantly affects women. Conclusions High levels of physical fitness were consistently associated with better HRQoL in women with fibromyalgia; clinically relevant differences were demonstrated between those at extreme physical fitness levels. Muscle strength, flexibility, and cardiorespiratory fitness were independent indicators of HRQoL. These results warrant further prospective research on the potential of fitness to predict HRQoL in this population.

Methods Physical fitness was assessed as cardiorespiratory fitness and muscle strength. Fitness was measured using the six-minute-walk-test and muscle strength was assessed by hand grip and standing lung jump. Overweight (≥ 1 SD) and obesity (≥ 2 SD) were determined by body mass index. A cross sectional study was done in 452 children (185 boys and 267 girls), age range (7–9 years). Cardio-metabolic-risk (serum glucose, triglycerides, HDL, insulin) and waist for height ratio was expressed as z score. Data was analyzed using bivariate analysis and multiple and logistic regression. Results A 21% were obese and 27% overweight. Children with high body fat and low cardiorespiratory fitness and muscle strength had an OR of 6.8-fold (IC 95% 3.4 to 13.9) cardiometabolic risk z-score, compared to those most fit. Conclusion School children with higher body fat and lower physical fitness had increased cardio-metabolic-risk-score. No significant difference in cardio-metabolic risk in the non-obese children was found independent of their physical fitness.

This thesis is part of the School in Motion study. School in Motion was initiated by the Norwegian government because of the political demand for more knowledge regarding possible approaches to increase adolescent physical activity. The reason for the demand was a report depicting 15-year-olds as among the most sedentary populations in Norway. This is concerning because physical activity can have a preventive function against non-communicable diseases, psychological difficulties and mental disorders. Norwegian schools are given extended responsibilities for their students’ physical and mental health. Physical activity can contribute in this purpose, therefore, finding feasible approaches to increase physical activity in school is an important research topic. The School in Motion study was a cluster randomized controlled trial involving 29 lower secondary schools in Norway. The participants were 2084 9th graders (14-15 years old) from schools located in western, south western, southern, and eastern parts of the country. The study aimed to implement two separate intervention models for increased physical activity and physical education in school, in order to evaluate their effects on physical activity, physical health, mental health, learning and learning environment. Intervention model 1 included weekly, 30 minutes of physical activity, 30 minutes of physically active academic lessons, and one additional physical education lesson. Intervention model 2 included two weekly physical activity lessons in which students were encouraged to pursue their own activity interests, in groups they formed themselves. The aims of this thesis were to elucidate different aspects of the relationship between school-based physical activity interventions, physical fitness and mental health among adolescents, and the feasibility of school-based PA interventions in lower secondary school. The data that are presented in this thesis are the basis of four separate papers. The first paper analyzed baseline data to examine the cross-sectional association between physical fitness and mental health. The results showed a small, inverse association between cardiorespiratory fitness and mental health, while no association was found between mental health and body composition or muscular strength. The second paper presents data from a qualitative study that aimed to elucidate how intervention model 2 was implemented, and what influenced the implementation. The results showed large differences in how the intervention was implemented. Specifically, two out of four investigated schools implemented the intervention adequately, one school implemented the intervention partially adequate and the last school failed at several implementation aspects. Four main factors were responsible for influencing implementation: frame factors, intervention-, participant-, and provider characteristics. The results elucidate the challenge of implementing a complex physical activity intervention in schools, i. e., complex contexts, and expecting the implementation process to be similar for the schools. The third paper examined the effect the interventions had on mental health, by using a cluster randomized controlled trial design. The results showed no intervention effects on the overall population. However, subgroup analyses indicated that intervention model 1 was beneficial for those with high levels of psychological difficulties at baseline, and both intervention models were beneficial for the immigrant subgroup. The results also indicated that intervention model 2 had a negative effect on a subscale of psychological difficulties, peer relationships, within the subgroups non-immigrant girls, and those with “borderline” psychological difficulties at baseline. The fourth paper presents associations between the one-year change in physical fitness components and mental health status. Subgroup analyses showed an inverse association between change in cardiorespiratory fitness and mental health status among boys; an inverse association between change in muscular strength and mental health status among immigrants; and an association between change in cardiorespiratory fitness and mental health among girls with high socioeconomic status. The results indicate that the associations between physical activity, physical fitness and mental health among adolescents depend on different mechanisms that are specific to certain subgroups. The findings of this thesis add nuance to a research field characterized by relatively few studies, small sample sizes, cross-sectional designs and little qualitative knowledge. In summary, this thesis suggests that improving cardiorespiratory fitness can be beneficial for mental health promotion among adolescent boys; and that improving muscular strength can be beneficial for mental health promotion among adolescent immigrants. Moreover, school-based PA programs can be useful for mental health promotion among adolescents with high levels of psychological difficulties and among adolescent immigrants. Regarding the implementation of physical activity programs in schools, this is feasible. However, a successful implementation depends to a large degree on schools’ facilities and frame factors, thus, the physical activity programs should have flexible designs to secure an optimal fit in each individual school. The new Core curriculum in Norway for primary and secondary education specifies that physical and mental health is to be an important interdisciplinary topic in school. The knowledge generated through this thesis can contribute to the development of this interdisciplinary topic, and to the ongoing discussion about the role of physical activity and mental health in school.

The aim of this study was to identify the relationship between health-related physical fitness components (aerobic fitness, muscle strength, flexibility, and body fat) and resting heart rate (RHR) in Brazilian adolescents. The study included 695 schoolchildren (14–19 years) from public schools of the city of São José, Brazil. RHR was evaluated using an automated oscillometric sphygmomanometer. Aerobic fitness was assessed by the modified Canadian Aerobic Fitness Test; muscle strength was measured by handgrip dynamometer; flexibility was assessed by the sit-and-reach test; and body fat was assessed indirectly by sum of two skinfold thicknesses (triceps and subscapular). Sociodemographic variables, habitual physical activity, sexual maturation, and body mass index were the covariates. Cardiorespiratory fitness (β = -0.11; 95%CI: -0.14, -0.08) and handgrip strength (β = -0.10; 95%CI: -0.18, -0.01) were inversely associated with RHR in boys. For girls, cardiorespiratory fitness (β = -0.09; 95%CI: -0.12, -0.06) was inversely associated with RHR. In both sexes, body fat (β = 0.50; 95%CI: 0.25, 0.75 for boys; β = 0.17; 95%CI: 0.36, 2.72 for girls) was directly associated with RHR. The RHR is measured more easily than the physical fitness tests, so it is recommended to assess adolescent’s heath in large surveillance systems.

BACKGROUND Childhood obesity accompanied with lower levels of health-related physical fitness (HRPF) is a major threat to public health both internationally and locally. Children with intellectual disability (ID), especially adolescents, have a higher risk of being overweight/obese and having poor HRPF levels. More interventions, therefore, are needed to help this population attain their optimal health levels. However, there has been limited research on this population compared with studies designed for their typically developing (TD) peers. OBJECTIVE The proposed study aims to fill this knowledge gap by developing and examining the success of a physical activity (PA) intervention for the target population. METHODS The proposed study will be a school-based randomised controlled trial lasting for 12 weeks. The participants (N=48) will be recruited from special schools for students with mild ID and then randomly allocated to either the intervention group (IG) or the waiting-list control group (CG). During the intervention period, the participants in the IG will receive a fun, game-based moderate-to-vigorous physical activity (MVPA) training programme (twice/week, 60-min/session, a total of 24 sessions). The intensity of the activities will increase in a progressive manner. Those in the CG will receive no intervention. To observe and evaluate the sustaining effects of the intervention, follow-up testing will be scheduled for the participants 12 weeks after the intervention concludes. The study outcomes will include primary outcomes (fatness-related outcomes and fitness-related outcomes) and secondary outcomes (blood pressure). All of the measurements will be taken three times. After the follow-up tests, the same PA training programme will be provided to the participants in the CG. RESULTS This study is ongoing. The participants were recruited from October 2020 to November 2020. Total duration of the study is 13 months. Study results are expected at the end of 2021. CONCLUSIONS The proposed study is expected to reduce fatness and improve HRPF levels in children with ID. If proven effective, the intervention will be made accessible to more special schools and mainstream schools with students with ID. Furthermore, the study can serve as an example for international researchers, policymakers and the public who are seeking to tackle the problem of obesity and poor HRPF among children with ID. Background. Studies on physical activity, which is positively associated with physical fitness, reveal that children’s activity is not sufficient (BHFNC, 2010; Chen, Zheng, Yi, & Yao, 2014; Currie et al., 2012), and their physical fitness deteriorates (Volbekienė & Kavaliauskas, 2002; Мирошниченко & Астраханцев, 2005; Синявский, Власов, & Сергеев, 2009). Non-formal physical education (NFPE) is one of the means to increase children’s physical activity and physical fitness. The aim of this study was to determine the influence of NFPE on the physical fitness of 6th grade pupils. Methods. The research was conducted in May, 2013; 356 six-graders (48.0% of girls) from four Klaipėda city comprehensive schools participated in the research. As many as 56.5% of children (48.3% of girls) participated in NFPE in school and out of school. The participants completed five physical fitness tests. Results. Independent t test revealed that the results of boys who attended the NFPE group of cardiorespiratory fitness, t(180) = −2.093, p = .038; upper body muscular strength and endurance, t(182) = 2.413, p = .017; abdominal muscular strength and endurance, t(186) = 3.282, p = .001; explosive leg power, t(183) = 1.967, p = .049, and girls’ results of abdominal muscular strength and endurance, t(172) = 2.687, p = .008 were significantly higher than those in the NFPE non attended group. Conclusion. Non-formal physical education is a meaningful educational form for increasing children’s, particularly boys’, health related physical fitness; therefore it is purposeful to encourage children to participate in physical activities in school and after classes.
