Purpose: To investigate the clinical significance of vessel density (VD) on visual field (VF) progression regarding the severity of glaucoma. Methods: A total of 130 eyes were recruited in this retrospective and longitudinal study. Superficial and deep VDs in circumpapillary and macular regions were measured via ImageJ. The rate of VF progression was defined as the mean deviation (MD) slope (dB/year). Linear regression was used to verify factors affecting deterioration of VF. The eyes with lower superficial VD were further analyzed. Results: Fifty patients with early glaucoma (EG) (MD > −6 dB) and 52 patients with moderate-to-advanced glaucoma (MAG) (MD ≤ −6 dB) were included. A faster progression rate was found in MAG (p = 0.049). Superficial VD was noticeably related to the VF progression rate in total eyes and in MAG (Both Ps ≤ 0.007, respectively). With patients in the lower half of the superficial VD, the VD was significantly associated with the rate of progression (B, 0.049, p = 0.021). This association was independent of the baseline MD and OCT parameters. Conclusion: Decreased superficial VD might conversely affect the progression of glaucoma even in MAG, which suggests superficial VD could be used as a potential marker to foresee the disease progression even in progressed eyes.

In this paper, the impact of manufacturing variations on performance of an axial compressor rotor are evaluated at design rotational speed. The geometric variations from the design intent were obtained from an optical coordinate measuring machine and used to evaluate the impact of manufacturing variations on performance and the flow field in the rotor. The complete blisk is simulated using 3D CFD calculations, allowing for a detailed analysis of the impact of geometric variations on the flow. It is shown that the mean shift of the geometry from the design intent is responsible for the majority of the change in performance in terms of mass flow and total pressure ratio for this specific blisk. In terms of polytropic efficiency, the measured geometric scatter is shown to have a higher influence than the geometric mean deviation. The geometric scatter around the mean is shown to impact the pressure distribution along the leading edge and the shock position. Furthermore, a blisk is analyzed with one blade deviating substantially from the design intent, denoted as blade 0. It is shown that the impact of blade 0 on the flow is largely limited to the blade passages that it is directly a part of. The results presented in this paper also show that the impact of this blade on the flow field can be represented by a simulation including 3 blade passages. In terms of loss, using 5 blade passages is shown to give a close estimate for the relative change in loss for blade 0 and neighboring blades.

This study aims to test the influence of the senior management’s support as a moderating variable on the relationship between the independent factors (Training, Empowerment, Motivation and Communication) and the dependent variable (Performance of Employees). (SEM-AMOS) is used to test the impact of the moderating variable. Where it is depended on the method of sampling or analysis of what is known as multiple-groups analysis. The paragraphs of the senior management’s support variable are collected and divided into two groups according to the mean of the total paragraphs. In addition, according to the relative weights given to the paragraphs of the questionnaire, using a five- point’s Likert scale: 1= strongly disagree to 5 = strongly agree. The first group consisted of the grades less than the mean and it is considered as the group which is non-supporters of the existence of support. While the second group consisted of the grades higher than the mean and considered as the group which is a supporter of the existence of support. The study found that the model of study in the presence of the support of the senior management’s is appropriate for the second group and inappropriate in light of the lack of support by the senior management’s support for the first group.

Abstract Purpose: To evaluate the relationship between structure and function in moderate and advanced primary open-angle glaucoma (POAG) and to determine the accuracy of structure and vasculature for discriminating moderate from advanced POAG.Methods: In this cross-sectional study 25 eyes with moderate and 40 eyes with advanced POAG were enrolled. All eyes underwent measurement of the thickness of circumpapillary retinal nerve fiber layer (cpRNFL) and macular ganglion cell complex (GCC), and optical coherence tomography angiography (OCTA) of the optic nerve head (ONH) and macula. Visual field (VF) was evaluated by Swedish interactive threshold algorithm and 24-2 and 10-2 patterns. The correlation between structure and vasculature and the mean deviation (MD) of the VFs was evaluated by a partial correlation coefficient. The area under the receiver operating characteristic curve (AUC) was applied for assessing the power of variables for discrimination moderate from advanced POAG.Results: Superior cpRNFL, superior GCC, whole image vessel density (wiVD) of the ONH area, and vessel density in inferior quadrant of perifovea had the strongest correlation with the mean deviation (MD) of the VF 24-2 (r= .351, .558, .649 and .397; p< .05). The greatest AUCs belonged to inferior cpRNFL (.789), superior GCC (.818), vessel density of the inferior hemifield of ONH area (.886), and vessel density in inferior quadrant of perifovea (.833) without statistically significant difference in pairwise comparison.Conclusion: Vasculature has a stronger correlation than the structure with MD in moderate and advanced POAG and is as accurate as structure in discrimination moderate from advanced POAG.

Surviving Vietnam: Psychological Consequences of the War for U.S. Veterans presents a unique combination of historical material, military records of combat exposure, clinical diagnoses of post-traumatic stress disorder (PTSD), and interviews with representative samples of veterans surveyed both a little over decade after the war’s conclusion in the National Vietnam Veterans Readjustment Study (NVVRS), and again nearly four decades after the war’s conclusion in the National Vietnam Veterans Longitudinal Study (Longitudinal Study). It focuses specifically on veterans’ war-zone experiences and the development in some of PTSD, a relatively new and controversial diagnosis. The monograph begins with a brief history of the Vietnam war that provides context for the discussions of the relevance to their mental health outcomes of the severity of veterans’ exposure to combat, their personal involvement in harm to civilians and prisoners, their race/ethnicity, and their military assignments. It discusses nurses’ experiences in Vietnam and the psychological impact on families of veterans’ chronic war-related PTSD. The monograph then examines factors affecting surveyed veterans’ post-war readjustment, including the effects of changing public attitudes toward the war and the veterans’ own appraisals of the impact of the war on their lives after the war. It concludes by discussing the policy implications of its research findings.

Aim. To assess the degree of satisfaction among hemodialysis patients and the factors influencing this satisfaction.Methods. Patients were recruited from 3 Saudi dialysis centers. Demographic data was collected. Using 1 to 10 Likert scale, the patients were asked to rate theoverall satisfactionwith, andthe overall impactof, their dialysis therapy on their lives and to rate the effect of the dialysis therapy on 15 qualities of life domains.Results. 322 patients were recruited (72.6% of the total eligible patients). The mean age was 51.7 years (15.4); 58% have been on dialysis for 3 years. The mean Charlson Comorbidity Index was 3.2 (2), and Kt/V was 1.3 (0.44). The mean satisfaction score was (7.41 2.75) and the mean score of the impact of the dialysis on the patients' lives was 5.32 2.55. Male patients reported worse effect of dialysis on family life, social life, energy, and appetite. Longer period since the commencement of dialysis was associated with adverse effect on finances and energy. Lower level of education was associated with worse dialysis effect on stress, overall health, sexual life, hobbies, and exercise ability.Conclusion. The level of satisfaction is affected by gender, duration on dialysis, educational level, and standard of care given.

280 Background: MRI is increasingly used in follow-up of men undergoing AS for localized prostate cancer. Large cohorts predating the MRI era have shown that the risk of disease progression or stopping an AS protocol is ~20-30% at 5 years, but the impact of MRI on outcomes in AS is unclear. Methods: We studied men who initiated AS at Dana-Farber Cancer Institute between 2001-2017 for Gleason 6 prostate cancer diagnosed on transrectal ultrasound-guided biopsy, and who underwent a prostate MRI as part of their AS protocol. Progression on AS was defined as finding Gleason ≥7 cancer on repeat biopsy or at radical prostatectomy. Early MRI was defined as one performed within 1 year of diagnosis. Results: A total of 148 men were identified from a prospective IRB-approved database. Median PSA at diagnosis was 4.8 (range 0.7-14.3), median number of positive biopsy cores was 1 (1-7), median maximal core involvement by cancer was 10% (1-65), and most men (87%) had T1c disease. Overall, 54 (36%) progressed on AS over a median follow-up of 5.6 years (95% CI 5.1-6.0), with the majority (n=44, 81%) progressing at repeat biopsy; the 5-year progression rate was 32% (25-41). There were no major differences in baseline clinicopathologic characteristics between men undergoing early (n=103, 70%) or delayed (n=45, 30%) MRI, but men who had an early MRI underwent fewer subsequent biopsies (median 2 vs. 3, p=0.010) and more targeted biopsies (47% vs. 36, p=0.212). Men who underwent early MRI had a higher rate of progression compared to those who had a delayed MRI (5-year rate 43% [33-55] vs. 12% [5-26], log-rank p=0.001). However, when immortal time bias was accounted for by considering MRI receipt as a time-dependent covariate, early MRI was not associated with progression (HR=0.74 [0.50-1.09], p=0.129). Conclusions: The 5-year progression rate on AS was ~30% in the MRI era, a rate similar to that reported in series that predate the adoption of MRI. A higher rate of progression was seen in men undergoing MRI within 1 year of diagnosis, but MRI timing did not influence risk of progression when accounting for the time interval before an MRI was performed. Longer follow-up is needed to determine the impact of MRI on cancer-specific survival in men undergoing AS.

The objective of the present review was to establish levels of conserved fodder wastage when feeding livestock (sheep, beef cattle, dairy cattle) under various conditions and using various feed-out systems, and to determine the factors affecting wastage. The mean wastage of hay recorded in the literature reviewed was 17% of the DM offered, but the range was from 4 to 77%. The main factors affecting the degree of wastage were storage method, packaging method, method of feeding out, amount of fodder on offer and its palatability and/or quality and the impact of wet weather. Although the emphasis was on hay, the principles should also apply to silage. If wastage was 40% rather than 5%, the cost of feeding conserved fodder to livestock would be a third greater than producers might expect or budget on.

AbstractBackground and purposePain is a common symptom associated with multiple sclerosis (MS), and has lasting effects on an individual’s functional capacity and quality of life. A wide range of prevalence rates of pain (between 23% and 90%)have been reported in MS and this is mainly due to the methodological differences amongst the studies such as variability in patient sources, method of sampling and the definition of pain used. Chronic pain in MS, defined as pain lasting for greater than 3–6 months, can have a significant impact on their biopsychosocial health, including negative impact on activities of daily living, relationships and social participation. The long-term course of MS-related pain and its impact in an Australian cohort over a 7-year period has been investigated earlier. The aim of this longitudinal study was to describe the impact of chronic pain, pain-related disability and carer burden in persons with MS over a 10-year period. The aim of this longitudinal study was to describe the impact of chronic pain, pain-related disability and carer burden in persons with MS over a 10-year period.MethodsThis was a prospective longitudinal study conducted at the Rehabilitation Department of Royal Melbourne Hospital (RMH), a tertiary referral hospital in Victoria and Australia. The source of participants was from the RMH MS database and contains detailed MS patient information including demographic data, diagnosis details (using McDonald’s criteria), pain characteristics. Structured face-face interviews and validated measures were used, which include the visual analogue scale (VAS); chronic pain grade (CPG); the assessment of quality of life (AQoL) and the carer strain index (CSI). The mean age of the participants (n = 70) was 55.3 years and majority (70%) were female.ResultsThe mean age of the participants (n = 70) was 55.3 years and majority (70%) were female. The findings show that over time (10 years), participants report having greater bilateral bodily pain and greater description of pain as ‘worse as it could be’. Pain types were similar to 7-years follow-up but remained higher than baseline. There was a significant deterioration in quality of life in those with more severe CPG over time. Almost half of the participants 31 (44%) required care either from a private carer, institution or from a family member. Although fear of taking medications and side effects were common barriers to treatment for pain, there was an increase in the use of pharmacological treatment over time and an increase in the use of healthcare services, mainly neurologists and general practitioners.ConclusionsThe pain measures reported by the participants were similar to those at the 7-year follow-up except there was a greater representation of bilateral pain locations (limb, trunk and facial pain) compared to baseline and 7-year follow-up. At 10-year follow-up, more participants used medications compared tc 7-year follow-up and there was an increase in the use of health professionals at the 10-year follow-up At the 10-year follow up QoL of the participants deteriorated significantly and more participants had progressed to higher CPGIII and CPGIV. This study demonstrates that chronic pain is a significant issue over time in MS, with clinical and health implications, impact on quality of life, disability and healthcare utilization.ImplicationsGreater awareness of chronic pain in pwMS, cognitive classifications and an interdisciplinary approach is required to improve long-term patient outcomes and well-being.Crown Copyright © 2017 Published by Elsevier B.V. on behalf of Scandinavian Association for the Study of Pain. Background/AimThis study aimed to compare central visual sensitivity under monocular and binocular conditions in patients with glaucoma using the new imo static perimetry.MethodsFifty-one consecutive eyes of 51 patients with open-angle glaucoma who were affected with at least one significant point in the central 10° were examined in this cross-sectional study. Monocular and binocular random single-eye tests were performed using the imo perimeter and the Humphrey field analyser (HFA) 24-2 and 10-2 tests. The eyes were assigned to ‘better’ and ‘worse’ categories based on the visual acuity and central visual thresholding. Central visual sensitivity results obtained by monocular, binocular random single-eye tests and binocular simultaneous both eye test were compared.ResultsThe average mean deviation with the HFA 24-2 was −5.5 (–1.5, –14.6) dB (median, (IQR)) in the better eyes and −18.0 (–12.9, –23.8) dB in the worse eyes. The mean sensitivity in the central 4 points of the visual field (VF) of the worse eyes was lower when measured under the binocular eye condition than under the monocular condition. Conversely, this value of the better eyes was greater when measured under the binocular eye condition than under the monocular condition.ConclusionsThe central sensitivity of the better eyes was better and that of the worse eyes poorer with binocular testing than with monocular testing in patients with glaucoma. Although monocular VF testing is still the most straightforward means to monocularly monitor glaucoma at clinical settings, binocular testing, such as provided with imo perimetry, may be a useful clinical tool to predict the effect of VF impairments on a patient’s quality of visual life.
