Autosomal dominant polycystic kidney disease (ADPKD) is the most common genetic nephropathy, and tolvaptan is the only therapy available. However, tolvaptan slows but does not stop disease progression, is marred by polyuria, and most patients worldwide lack access. This and recent preclinical research findings on the glucose-dependency of cyst-lining cells have renewed interest in the dietary management of ADPKD. We now review the current dietary recommendations for ADPKD patients according to clinical guidelines, the evidence base for those, and the potential impact of preclinical studies addressing the impact of diet on ADPKD progression. The clinical efficacy of tolvaptan has put the focus on water intake and solute ingestion as modifiable factors that may impact tolvaptan tolerance and ADPKD progression. By contrast, dietary modifications suggested to ADPKD patients, such as avoiding caffeine, are not well supported and their impact is unknown. Recent studies have identified a chronic shift in energy production from mitochondrial oxidative phosphorylation to aerobic glycolysis (Warburg effect) as a contributor to cyst growth, rendering cyst cells exquisitely sensitive to glucose availability. Therefore, low calorie or ketogenic diets have delayed preclinical ADPKD progression. Additional preclinical data warn of potential negative impact of excess dietary phosphate or oxalate in ADPKD progression.

Migraine is characterized by recurrent attacks of disabling headaches, often accompanied by sensory and motor disturbances. Clinical manifestations of migraine are influenced by dietary behaviors and dietary elements. Several dietary triggers for migraine have been identified, leading to the definition of strategies such as elimination diets, ketogenic diets, and comprehensive diets, mainly to help prevent migraine. Although inconsistency is present in the literature and no consensus exists, the available data are promising in supporting beneficial dietary interventions for some migraine patients. Several factors influence the net outcome, including age, sex, genetics, and environmental factors. Advancement in understanding the underlying mechanisms of migraine pathogenesis and how dietary factors can interfere with those mechanisms has encouraged investigators to consider diet as a disease-modifying agent, which may also interfere with the gut–brain axis or the epigenetics of migraine. Future work holds potential for phenotyping migraine patients and offering personalized recommendations in line with biopsychosocial models for the management of migraine. Diet, as an important element of lifestyle, is a modifiable aspect that needs further attention. Well-designed, systematic, and mechanism-driven dietary research is needed to provide evidence-based dietary recommendations specific to migraine. This narrative review aims to present the current status and future perspective on diet and migraine, in order to stimulate further research and awareness.

ABSTRACT Use of high-fat, ketogenic diets (KDs) to support physical performance has grown in popularity over recent years. While these diets enhance fat and reduce carbohydrate oxidation during exercise, the impact of a KD on physical performance remains controversial. The objective of this work was to assess the effect of KDs on physical performance compared with mixed macronutrient diets [control (CON)]. A systematic review of the literature was conducted using PubMed and Cochrane Library databases. Randomized and nonrandomized studies were included if participants were healthy (free of chronic disease), nonobese [BMI (kg/m2) <30], trained or untrained men or women consuming KD (<50 g carbohydrate/d or serum or whole-blood β-hydroxybutyrate >0.5 mmol/L) compared with CON (fat, 12–38% of total energy intake) diets for ≥14 d, followed by a physical performance test. Seventeen studies (10 parallel, 7 crossover) with 29 performance (13 endurance, 16 power or strength) outcomes were identified. Of the 13 endurance-type performance outcomes, 3 (1 time trial, 2 time-to-exhaustion) reported lower and 10 (4 time trials, 6 time-to-exhaustion) reported no difference in performance between the KD compared with CON. Of the 16 power or strength performance outcomes, 3 (1 power, 2 strength) reported lower, 11 (4 power, 7 strength) no difference, and 2 (power) enhanced performance in the KD compared with the CON. Risk of bias identified some concern of bias primarily due to studies allowing participants to self-select diet intervention groups and the inability to blind participants to the study intervention. Overall, the majority of null results across studies suggest that a KD does not have a positive or negative impact on physical performance compared with a CON diet. However, discordant results between studies may be due to multiple factors, such as the duration consuming study diets, training status, performance test, and sex differences, which will be discussed in this systematic review.

The article is devoted to the characterization of the models and the stages of formation of education systems in African countries and of their current status. The aim of the work was to analyze the impact of various factors on the development of education in Africa. In particular, the impact of military conflicts and the activities of terrorist organizations on the field of education were investigated. It also analyzes the increasing role of the world community in the development of education in African countries in recent decades. The main examples are the aspects of the activities of the terrorist organization Boko Haram, aimed at destabilizing and directly destroying the education system of Nigeria, and the impact of the Mali conflict of 2012-2013 on education. The activities of Boko Haram are widely covered in the scientific literature, but the article focuses on its aspects that are connected precisely with the negative impact on the educational system in Nigeria. The work broadens the perception of the challenges currently facing educational systems in Africa.

Abstract Indonesia is a tropical country that experiences the impact of climate change. The utilization of taro as an amphibian crop might sustain the food availability for the most vulnerable farmers against the negative impact of climate change. The study aims to evaluate the current status of taro utilization and production as local food concerning food diversification, sustainable agriculture, and climate change issues. Data were obtained from a focus group discussion with primary stakeholders of the taro value chain and literature review. Results show that taro is an important secondary food in some regions with three main uses as local consumption, local trading, and export. Prominent taro consumers existed in West Papua, Papua, Maluku, and Central Sulawesi Provinces with consumption rates 10.6, 5.3, 2.2, and 2.0 kg/capita/year, respectively. Taro was intensively cultivated in Banten, West Java, and South Sulawesi provinces, for fresh and processed products. The respondents agree with taro as a prospective adaptive crop to climate change of both drought and excess precipitation. There are some issues in the biodiversity conservation, crop improvement, link-match industry, and stakeholder capacity building for competitive, quality, quantity, and sustainable production, as future works in the taro value chain in Indonesia.

ObjectiveTo describe the development and current status of training and certification in clinical neurophysiology (CNP); to explore the impact of the newer subspecialties in sleep medicine, neuromuscular medicine, and epilepsy; and to obtain information about aspects of practice in the subspecialty.MethodsInformation about training programs and certification was obtained from the records of the Accreditation Council for Graduate Medical Education and the American Board of Psychiatry and Neurology, and diplomates were surveyed about their CNP practice activities and attitudes toward certification/recertification.ResultsIn the years since the first examination was administered, a robust number of CNP training programs developed, but recently, there has been a decrease in the number of programs and fellows, although the number of programs and fellows in the subspecialties of epilepsy, neuromuscular medicine, and sleep medicine has increased. A diplomate survey indicated that most respondents devoted significant practice time to CNP procedures, especially to EEGs and EMGs. Although more diplomates performed EEGs than EMGs, a substantial portion performed both. Most diplomates were planning to or had maintained certification in CNP.ConclusionOver 3,000 neurologists, child neurologists, and psychiatrists have obtained certification in CNP, and the majority are participating in recertification. Although the newer and overlapping subspecialties of epilepsy, neuromuscular medicine, and sleep medicine may be having a negative impact on CNP, it continues to have a relatively large number of programs and attracts a relatively large number of fellows.

Purpose The emerging situation of pandemic due to COVID-19 has not only influenced the daily life but also the society and travel activities around the world thereby depriving tourists (especially who are already on the move) of facilities and even making them to look for desperate alternatives. Such situation in fact may draw a long-term impact on guest–host relationship while residents’ behavior reflects hostility toward the tourists. Such kind of interactions contributes toward tourists’ perception and experience about the destination and its services thereby affecting their level of acceptance and tolerance for tour and travel activities (Armenski et al., 2011). Moreover, the guest–host relationship has mostly been studied with perspective of and focus on residents/host and not enough on tourists (Skipper, 2009; Vargas-Sánchez et al., 2014). Keeping this in view, this paper aims to analyze the impact of pandemic situation on guest–host relationship and its future impact on travel intentions among the tourists in India. Design/methodology/approach The study undertakes the help of in-depth interviews and extracting themes to understand the guest–host relationship and the perspective of tourists in challenging times like COVID-19 and its impact on the relationship. The secondary sources have been adopted to retrieve the data related to current status of travel industry in India. In-depth interviews were conducted online to gather data for the qualitative analysis regarding the research. Further, the data has been analyzed for retrieving a dimensional approach to subject area. Findings The data from participant observation showed that the hosts displayed panic, mistrust and irresponsible behavior toward the guests, and this clearly indicates that the pandemic situation has a highly negative impact on the image of the community and the destination. This ultimately affects the guest–hosts relationships in the long term. Most of the tourists showed that they were okay with following the rules and respect local culture but were expecting support from local community during distress. It was found that there were two female tourists who were asked to vacate the accommodation, which can be considered as an inappropriate and extreme behavior. Thus, COVID-19 is not only causing a threat to the tourism presently but will have a prolonged influence on guest–host relationship as negative interaction or experiences are supposed to be frequently radiated by the tourists (de Albuquerque and McElroy, 2001). Originality/value There is no dearth of studies focusing on travel behavior dimensions, whereas the linkage of residents’ behavior toward it still requires much consideration and analyzing simultaneously. The study looks into the area of guest–host relationship and tries to explore it from the perspective and significance of tourist (guest) rather than the much read and researched resident (host) perception at the center. The findings of the study could be helpful in drawing the strategic framework for the industry to handle and sustain the guest–host relation so as to safeguard the future of tourism and sustain potential travel market reiterating the significance of tourists/guests and their perspective about the hosts, in developing and growing the tourism of a destination.

Objective: To identify and detail physiological factors that influence cognition in military personnel. Background: Maintenance of cognitive and task performance is important under several scenarios, none more so than in a military context. Personnel are prepared for and trained to tolerate many of the stressors they encounter; however, consideration of stressors typically extends only as far as the physical, psychological, and environmental requirements of a given task. While considering these factors certainly characterizes the broader picture, several physiological states and traits can influence cognition and thus, should also be considered. Method: A systematic review of the electronic databases Medline (PubMed), EMBASE (Scopus), PsycINFO, and Web of Science was conducted from inception up to January 2019. Eligibility criteria included current military personnel, an outcome of cognition, and the assessment of a physiological factor. Results: The search returned 60,564 records, of which 60 were included in the review. Eleven studies examined the impact of demographic factors on cognition, 16 examined fatigue, 10 investigated nutrition, and 24 the impact of biological factors on cognitive performance. Conclusion: Factors identified as having a positive impact on cognition include aerobic fitness, nutritional supplementation, and visual acuity. In contrast, factors identified as having a negative impact include fatigue arising from sustained operations, dehydration, undernutrition, and an exaggerated physiological stress response to a cognitive task or a stressor. A further subset of these factors was considered modifiable. Application: The modifiable factors identified provide avenues for training and preparation to enhance cognition in ways previously unconsidered.

The field of learning analytics has evolved over the past decade to provide new ways to view, understand and enhance learning activities and environments in higher education. It brings together research and practice traditions from multiple disciplines to provide an evidence base to inform student support and effective design for learning. This has resulted in a plethora of ideas and research exploring how data can be analysed and utilised to not only inform educators, but also to drive online learning systems that offer personalised learning experiences and/or feedback for students. However, a core challenge that the learning analytics community continues to face is how the impact of these innovations can be demonstrated. Where impact is positive, there is a case for continuing or increasing the use of learning analytics, however, there is also the potential for negative impact which is something that needs to be identified quickly and managed. As more institutions implement strategies to take advantage of learning analytics as part of core business, it is important that impact can be evaluated and addressed to ensure effectiveness and sustainability. In this editorial of the AJET special issue dedicated to the impact of learning analytics in higher education, we consider what impact can mean in the context of learning analytics and what the field needs to do to ensure that there are clear pathways to impact that result in the development of systems, analyses, and interventions that improve the educational environment.

Abstract While recent research has greatly enhanced understanding of the pathology of the disease, and new treatment modalities have been developed, little is known about myelodysplastic syndromes (MDS) from the patient’s perspective. The purpose of this qualitative study was to explore the impact of MDS on patients’ quality of life (QOL). Five focus groups were convened at different geographic locations throughout the United States; 70 MDS patients attended and 62 actively participated in the discussions (89%). The framework for the study was based on Ferrell’s work exploring the impact of cancer on QOL (Oncology Nursing Forum, 1996). Exploratory in nature, the discussions often proceeded in differing directions, but the core open-ended questions were asked at each session. Sessions were tape recorded and professionally transcribed. Transcripts were coded and emerging themes identified using thematic analysis methods aided by the qualitative analysis program N5 (QSR International). The patient sample was 93% Caucasian, 51% male, with a mean age of 69 ± 9 years; 26% lived alone. Known MDS subtype was: 19 RA, 19 RARS, 11 RAEB, 3 5q-, 2 other (16 unknown); median time since diagnosis was 26 months (3 – 276 months). MDS treatment varied considerably: 73% of patients received growth factors, 61% transfusions, 19% azacitidine, 16% thalidomide, 14% iron chelation; 29% all other; many patients received multiple (often concurrent) therapies. Findings from the focus groups revealed a multifaceted description of how MDS can affect one’s QOL. QOL was typically defined by these patients as maintaining functional ability and independence, sustaining positive relationships, and maintaining control over one’s life. In contrast, MDS was found to cause a substantial and sustained decrease in one’s ability to function in a variety of areas, in part due to fatigue (reported by 81% of the patients). QOL was adversely affected by the work expended on managing the disease: frequent office visits, monitoring symptoms, administering therapy, and managing side effects. Family functioning was also disrupted, with family members resuming patients’ roles, limiting activities, and providing direct care. Interestingly, the emotional impact from MDS was often viewed as more problematic than the physical impact; emotional reactions included shock, anger, depression, and anxiety. In contrast, spiritual well-being was often enhanced, with a renewed appreciation for life, relationships, and faith (29% of patients). Many patients (50%) voiced concern about their future and the inability to plan for it effectively. Frustration with their current status caused the majority of patients to continue to seek out additional information about MDS and potential treatment options, as well as to make attempts to take some control over a seemingly uncontrolled situation. Data from this study suggest that MDS has a substantial, often negative impact on patients’ lives, and clinicians should be cognizant of this impact. Moreover, attention must be directed at providing more comprehensive support for the patient, including utilizing ancillary support services, throughout the illness trajectory. In so doing, the adverse impact of MDS on patients’ QOL may be diminished.

Concerns about anthropogenic climate change have resulted in promotion of renewable energy sources, especially wind energy. A concern raised against widespread windfarm development is that it may negatively impact bird populations as a result of bird collision with turbines, habitat loss and disturbance. Using systematic review methodology bird abundance data were synthesized from 19 globally-distributed windfarms using meta-analysis. The effects of bird taxon, turbine number, power, location, latitude, habitat type, size of area, time since operation, migratory status of the species and quality of evidence were analysed using meta-regression. Although the synthesized data suggest a significant negative impact of windfarms on bird abundance, there is considerable variation in the impact of individual windfarm sites on individual bird species, and it is unclear if the negative impact is a decline in population abundance or a decline in use owing to avoidance. Anseriformes experienced greater declines in abundance than other taxa, followed by Charadriiformes, Falconiformes and Accipitriformes, and Passeriformes. Time since windfarms commenced operation also had a significant impact on bird abundance, with longer operating times resulting in greater declines in abundance than short operating times. Other variables, including turbine number and turbine power either had very weak but statistically significant effects or did not have a significant effect on bird abundance. Windfarms may have significant biological impacts, especially over longer time scales, but the evidence-base is poor, with many studies being methodologically weak, and more long-term impact assessments are required. There is clear evidence that Anseriformes (wildfowl) and Charadriiformes (waders) experience declines in abundance, suggesting that a precautionary approach should be adopted to windfarm development near aggregations of these taxa in offshore and coastal locations. The impact of windfarm developments on bird populations must also be viewed in the context of the possible impact of climate change in the absence of windfarms.
