LABORATORY PREDICTORS OF GLAUCOMA IN PATIENTS WITH MIDDLE AND OLD AGE - A TOOL TO IMPROVE THE QUALITY OF LIFE (LITERATURE REVIEW)

В мире насчитывается около 1,3 млрд человек с такими нарушениями зрительного анализатора, как катаракта и глаукома, распространенность которых значимо выше у лиц пожилого возраста. Заболевания глаз остаются глобальной медикосоциальной и экономической проблемой, что связано как с прогрессированием болезни вплоть до слепоты, так и с отсутствием методов патогенетической терапии. Вторичной профилактикой глаукомы остается своевременное выявление патологии. В настоящее время разработан ряд диагностических приемов, включающих, в основном, инструментальные методики. Несомненно, в качестве скрининговых методов донозологической диагностики могут выступать лабораторные показатели. Сегодня выявлена группа биомаркеров, обладающих разной степенью чувствительности и специфичности. Однако широкое использование индикации данных маркеров затруднено, в том числе и по организационным причинам. Встает вопрос поиска следов присутствия патологии в таких рутинных исследованиях, как общий анализ крови. Глаукома признана системным заболеванием, что может найти отражение в изменении морфофункциональных свойств форменных элементов крови. There are about 1,3 billion people worldwide with visual disorders such as cataract and glaucoma, the prevalence of which is significantly higher in elder persons. Eye diseases remain a global medical, social and economic problem, associated with both the progression of the disease up to blindness and the lack of pathogenetic therapy methods. Timely detection of pathology is secondary prevention of glaucoma. At present, a number of diagnostic techniques have been developed, including mainly instrumental techniques. Undoubtedly, laboratory indicators should be used as screening methods of early diagnosis. Today, a group of biomarkers with different degrees of sensitivity and specificity has been identified. However, these markers have some difficulty in indicating. Finding signs of glaucoma in complete blood count is an important task. Glaucoma is recognized as a systemic disease, which should be reflected in the change in morphofunctional properties of blood elements.

Проведен ретроспективный анализ многолетней динамики и структуры случаев офтальмологических заболеваний у пациентов, проходивших лечение в многопрофильной больнице. Описана возрастная и нозологическая структура офтальмологических заболеваний. Выявлены закономерные особенности структуры заболеваний относительно разных возрастных групп, также обнаружены закономерности в их динамике. A retrospective analysis of the long-term dynamics and structure of cases of ophthalmological diseases of patients treated in a large multidisciplinary hospital was carried out. The age and nosological structure of eye diseases is described. Natural features of the structure of diseases relative to different age groups have been revealed, as well as patterns in dynamics have been found.

HIV-exposed uninfected infants (HEU) represent a growing population in developing countries including Gabon. Several studies have shown the vulnerability of these infants toward infectious diseases. The aim of the study was to contribute to the global effort to understand how HIVexposure or anti retroviral therapy affects infants’ blood elements. We assessed HEU infants’ complete blood count using a blood analyzer instrument. Our investigations showed that among the observed clinically relevant hematological abnormalities events, thrombocytosis was the most prevalent clinically relevant hematological abnormality associated with HEU infants’. We showed that HEU infants had significantly higher platelets count than HUinfants. Therefore, higher level of platelets seems to characterize HEU infants when compared to HU infants.

Background: The credibility of the result of a complete blood count (CBC) is closely connected with the preanalytical phase. Aim of the study: This study evaluated the impact of improper filling of microtubes and assessed the effect of storage conditions on results of CBC. Materials and methods: Blood samples collected into microtubes provided to the laboratory were studied. To determine the effects of sample volume on CBC result, blood from 14 adult volunteers were drawn, and test-tubes were filled with 200, 300, 400, 500, 600 and 700 μl. In the stability studies, overfilled samples stored at ambient temperature were analysed at: 1, 2 and 3 hours after phlebotomy. The analyses were made with the Sysmex KX-21N and XT-1800i analysers. Results: More than 75% of the analysed samples for CBC were incorrectly filled and mainly overfilled (60%). An excess collection of blood volume resulted in the decrease of haemoglobin, haematocrit, red blood cells and mean corpuscular volume as well as in an increase in the number of platelets. The storage of overfilled microtubes for CBC for 3 hours at room temperature had no further effect on the results of this test. Conclusion: Medical staff does not keep the instructions of the manufacturers. It might lead directly to a reduction of the results credibility and a decrease of the quality of the analysis.

The case study presents a 51-year-old woman diagnosed with a schizoaffective disorder who was initially treated with risperidone. After 7 days of hospitalisation, the patient was discharged at her own request. On the same day, after a few hours, she returned to the emergency room, reporting severe anxiety and auditory hallucinations. Risperidone was replaced by paliperidone. During the treatment, a decrease in leucocyte count after paliperidone administration was observed. During further hospital stay, the treatment was switched to olanzapine in depot form. Correct symptomatic response to the drug used was observed. The leucocyte counts were gradually compensated, which confirms the drug-related cause of previously observed complete blood count abnormalities. The case indicates the possibility of side effects in the treatment with certain drugs, and underlines the impact of the dosage form of the drug being taken on the patient’s quality of life.

Multiple recurrent somatic mutations have recently been identified in association with myeloproliferative neoplasms (MPN). This meta-analysis aims to assess the pooled prevalence of TET2 gene mutations among patients with MPN. Six databases (PubMed, Scopus, ScienceDirect, Google Scholar, Web of Science and Embase) were searched for relevant studies from inception till September 2020, without language restrictions. The eligibility criteria included BCR-ABL-negative MPN adults with TET2 gene mutations. A random-effects model was used to estimate the pooled prevalence with 95% confidence intervals (CIs). Subgroup analyses explored results among different continents and countries, WHO diagnostic criteria, screening methods and types of MF. Quality assessment was undertaken using the Joanna Briggs Institute critical appraisal tool. The study was registered with PROSPERO (CRD42020212223). Thirty-five studies were included (n = 5121, 47.1% female). Overall, the pooled prevalence of TET2 gene mutations in MPN patients was 15.5% (95% CI: 12.1–19.0%, I2 = 94%). Regional differences explained a substantial amount of heterogeneity. The prevalence of TET2 gene mutations among the three subtypes PV, ET and MF were 16.8%, 9.8% and 15.7%, respectively. The quality of the included studies was determined to be moderate–high among 83% of the included studies. Among patients with BCR-ABL-negative MPN, the overall prevalence of TET2 gene mutations was 15.5%.
