Research of matrix metalloproteinases in therapeutic dentistry

This article the properties of matrix metalloproteinases, their classification, function and role in the development of dental diseases are presented. The analysis of matrix metalloproteinases has been performed, which proves their important role in the physiological and pathological processes of the oral cavity. The possibility of using matrix metalloproteinases as part of one of the methods for diagnosing diseases and using them to evaluate the effectiveness of treatment has been considered. The structure of different types of metalloproteinases, as well as a model of the catalytic domain of the MMP-8 molecule has been presented.

Цель. Изучение клеток иммунной системы полости рта у пациентов до проведения операции лифтинга лица, а также сопоставление этих данных с показателями, выявленными при осложнениях в послеоперационном периоде. Методика. Исследованы клетки иммунной системы полости рта 100 женщин (23-68 лет), которым перед операцией лифтинга лица проведено стоматологическое обследование и необходимое лечение при наличии кариеса дентина, клиновидного дефекта эмали зуба, хронического периодонтита или пульпита. Выделенные путем смывов десенной борозды клетки окрашивали моноклональными антителами и анализировали на проточном цитометре. Определяли уровень спонтанного и индуцированного апоптоза. Оценивали уровень фагоцитоза и генерацию супероксидного аниона нейтрофилами. Результаты. Показано, что хронические воспалительные заболевания зубов оказывают негативное влияние на состояние иммунных клеток ротовой полости, сопровождаются повышением генерации супероксидного аниона нейтрофилами, повышением уровня спонтанного и церамид-индуцированного апоптоза клеток десенной борозды. Заключение. Наличие хронической патологии зубов, даже после санации, оказывает негативное влияние на течение послеоперационного периода при проведении операции лифтинга лица, в частности, способствует увеличению времени заживления операционной раны, инфицированию раны с появлением очагов некроза в области операционного шва. The object. The study of the cells of the immune system of the oral cavity in patients before the operation of face lifting, as well as a comparison of these indicators with complications in the postoperative period. Methods. The immune cells from the oral cavity were studied in 100 women (23-68 years), who underwent a dental examination and necessary treatment if they had the dentin caries, wedge-shaped defect of tooth enamel, chronic periodontitis or pulpitis before the facial lifting operation. The immune cells have been isolated by a lavage of gingival sulcus around the damaged tooth. Results. It has been shown that chronic dental diseases made a negative contribution to the oral cavity immune cells. It has been accompanied by the elevated levels of superoxide originating from neutrophils as well as the increased levels of spontaneous and ceramide-induced apoptosis of immune cells isolated from the gingival sulcus. Conclusion. The presence of chronic pathology of teeth even in the case of the preoperative dental sanation has a negative impact on the postoperative period after face lifting, in particular, contributes to the lengthening of the surgical wound healing time, wound infection as well as the partial necrosis of suture.

Relevance. The relevance of the problem is in the high prevalence of the herpes simplex virus. According to WHO, nearly 90% of the population are carriers. Relapses can occur several times a year, which leads to disability and impairs the quality of life. Currently, there are a large number of drugs for general and local treatment. However, they are toxic and can cause allergic reactions. In recent years, a large number of studies have been devoted to the use of lasers in the treatment of dental diseases. Therefore, the question of the use of a laser in the treatment of herpetic lesions of the oral cavity remains relevant.Purpose. The research purpose was to determine the effectiveness of the use of a diode laser in the treatment of herpetic lesions of the lips.Materials and methods. We conducted a comparative assessment of the effectiveness of the use of a diode laser in monotherapy of herpetic lip lesions in 18 patients. The comparison group consisted of 18 patients, in the local treatment of which antiviral ointments were used.Results. The results of the study showed that the use of a diode laser can reduce treatment time, accelerate the epithelization of foci.Conclusion. The results of our study allow us to recommend the use of a diode laser in the complex treatment of herpetic lesions of the oral cavity.

Aim. Evaluation of dental diseases prevalence in professional athletes engaged in power sports and of the influence of biologically neutral products based on propolis on the level of their dental health. Methods. The examination of organs and tissues of the oral cavity of professional athletes engaged for a long time in power sports was carried out. With the use of the developed specialized questionnaire dental status of the athletes was determined. Based on the analysis of the results of clinical and laboratory studies anti-inflammatory and anti-bacterial effectiveness of products based on propolis were evaluated. Results. The results of the survey showed that not all athletes are informed about the possibility of aggravation of pathological processes in soft and hard tissues of oral cavity during active training, mainly, in power sports. Lack of awareness of surveyed athletes about the need for timely individual oral hygiene procedures and regular dentist visits was revealed, especially during intense training before and after competitions. On evaluation of dental status of professional combat athletes high prevalence and intensity of inflammatory periodontal disease and caries were found. High anti-inflammatory efficiency of the product «Pomegranate Balm» for correction of dental status of professional athletes was proven. Conclusion. High incidence of inflammatory periodontal diseases of mild to moderate severity was revealed in athletes during prolonged and intense physical training; efficacy of biologically neutral products based on propolis was established.

Background.The oral an perioral piercing has a long history as part of religious, tribal,cultural or sexual symbolism and nowdays there is a high incidence of oral and perioral piercing in the adolescent population. This practice has a long history as part of religious, tribal, cultural or sexual symbolism. This article reviews current knowledge on injuries or diseases that might be produced by piercing in the oral cavity. We propose a classification to diagnosed the pathologies related to oral an perioral piercingMethods.A search was conducted of articles in PubMed, Scielo published between 1997 and 2007, using the key words ``oral and perioral, piercing ´´, ``oral, piercing and disease”, ``recessions and oral piercing´´. It has reviewed about twentythree articles 17 were narrative reviews and 6 case seriesResults.A review was carried out on the origins of oral and perioral body piercing and its local implications, classifying the different alterations like recessions, systemic implications that it can produce in the oral and perioral cavity.Conclusion.Patients with oral and perioral piercing should be regularly followed up because of the possible development of different types of adverse effects.Clinical implications.Adverse effects of oral and perioral piercing can be systemic, with transmission of infectious diseases such as hepatitis B or C, or can be local, with alteration of oral mucosae or even of dental structures.

The considerable prevalence of dental diseases in the patients with hemophilia is due to the absence of prevention and the proper care of the oral cavity, as well as regular medical examination. This category of patients reluctantly turns to the dentists for fear of bleeding, and the doctors for the same reason, avoid interventions in the mouth. Dental aspects of hemophilia are relevant problem of modern medicine due to the high intensity of organs and tissues of the oral cavity, a real risk of complications during dental procedures of therapeutic, surgical and orthopedic profiles. Epidemiological studies on lesions of the oral cavity in the patients with hereditary coagulative pathologies show a high prevalence of various dental disease, poor hygienic condition of the mouth, which entails a considerable need for dental treatment and prevention activities. To ensure timely quality dental care, control over the hygienic condition of oral cavity it is necessary to conduct regular examination of the state of the oral cavity in the patients with hemophilia. Dentists refuse to the patients in need of assistance because of fear of bleeding and the risk of infection with hepatitis and HIV infection. The high level of dental diseases at hemophilia may be due to the poor hygienic condition of the mouth, change of physicochemical properties of saliva and local immunity. Most patients with hemophilia brush their teeth not regularly and unsatisfactory. A vast number of microorganisms contained in this RAID, low local immunity can cause various diseases of the oral cavity. In this group of patients it is more important to prevent the development of dental disease than to treat them, for fear of complications.

The therapeutic and diagnostic activity is a collective work that is carried out jointly by specialists in different directions of dentistry and support units. The main goal of this type of activity is to improve the dental health by preventing dental diseases. It includes the treatment and rehabilitation using various ways of replacement of the lost integrity of the dentition and restoration of the teeth using a prosthetic. In order to do this, it is necessary to prepare the teeth and the pulp of the oral cavity for prosthetics. The sequence of preparatory activities includes a series of steps carried out in sequence: preventive, hygienic, therapeutic (including periodontal), surgical and orthopedic (including orthodontics). Each of these stages includes a series of manipulations, the sequence and number of which is determined by the individual dental status of each patient. The inspection and preparation of a comprehensive plan for the treatment involves a sequence of all these actions carried out by a team of experts in agreement with the patient, evidenced by the «informed consent» subscription. After the preparatory activities, it is possible to go to the final stage, that is to make a prosthetic appliance to the patient. The total duration of all phases of preparatory activities preceding the prosthetics depends on the complexity of the steps themselves, and is determined by the severity of the related dental diseases. The treatment of these diseases is the main part of the preparatory activities.

The review summarizes studies that used the timalin immunomodulator in many dental diseases. Issues related to its immunopharmacological action and effective clinical use in inflammatory, traumatic, and other pathological processes of the maxillofacial region were considered. Special attention was paid to their use in medical complexes for diseases of tissues and organs of the oral cavity in various age groups.

Regardless of the demonstrable success of fixed appliances like implants for the replacements of missing natural teeth in partially and fully edentulous patients, countable hindrances still persist. A crucial role is played by impressions used in the fabrication of dental implants for the success, durability and outcome of the restoration. An impression is a three dimensional negative likeness of the future position of the implant in the oral cavity. In view of the absence of periodontal ligaments that serve as natural compensators of the slight but significant details of the impression surface, therefore, impressions in implants are very crucial for implant success. Taking this factor into consideration, the present review article emphasizes on the different types of impression techniques used in implant dentistry.

Context: Matrix metalloproteinases (MMPs) are involved in the remodelling of the glomerular basement membrane (GBM) by tightly regulating the metabolism of extracellular matrix (ECM) of the GBM. Evidence Acquisitions: Directory of Open Access Journals (DOAJ), Google Scholar, PubMed, EBSCO, Scopus and Web of Science have been searched. Results: Gelatinases (MMP-2 and MMP-9) are mainly found involved in the remodelling of GBM and therefore this review focuses on these two MMPs and their action in nephrotic syndrome (NS), which is a protein losing enteropathy occurring due to the loss of integrity of GBM. In addition to the blood corpuscles, glomerular epithelial cells and mesangium are also expressing MMPs, and various cytokines and growth factors are involved in addition to tissue inhibitors of metalloproteinases (TIMPs) in regulating the metabolism of ECM via MMPs. While examining the results of MMP activity and expression in NS, except diabetic nephropathy (DN), membranoproliferative glomerulonephritis (MPGN) and hereditary NS where there was a clear down-regulation of MMP, all the other types of NS showed conflicting results. Both suppression and induction of MMPs are finally leading to GBM thickening, loss of integrity and proteinuria. Enhanced MMP activity leads to increase in matrix turnover and accumulation of ECM remnants and apoptotic cells leading to fibrosis. On the other hand, diminished expression of MMPs prevents the normal ECM turnover and matrix accumulation. The review compiled the mechanisms of action of both downregulation and upregulation of MMPs. Conclusions: Imbalance of ECM metabolism due to varied expression levels and activities of MMPs in different types of primary NS might contribute to the progression of nephropathies. Further studies are required to identify the potential and usage of MMPs as a diagnostic/prognostic/ therapeutic tool.
