The role of the oral cavity immune cells in the postoperative complications after face lifting

Цель. Изучение клеток иммунной системы полости рта у пациентов до проведения операции лифтинга лица, а также сопоставление этих данных с показателями, выявленными при осложнениях в послеоперационном периоде. Методика. Исследованы клетки иммунной системы полости рта 100 женщин (23-68 лет), которым перед операцией лифтинга лица проведено стоматологическое обследование и необходимое лечение при наличии кариеса дентина, клиновидного дефекта эмали зуба, хронического периодонтита или пульпита. Выделенные путем смывов десенной борозды клетки окрашивали моноклональными антителами и анализировали на проточном цитометре. Определяли уровень спонтанного и индуцированного апоптоза. Оценивали уровень фагоцитоза и генерацию супероксидного аниона нейтрофилами. Результаты. Показано, что хронические воспалительные заболевания зубов оказывают негативное влияние на состояние иммунных клеток ротовой полости, сопровождаются повышением генерации супероксидного аниона нейтрофилами, повышением уровня спонтанного и церамид-индуцированного апоптоза клеток десенной борозды. Заключение. Наличие хронической патологии зубов, даже после санации, оказывает негативное влияние на течение послеоперационного периода при проведении операции лифтинга лица, в частности, способствует увеличению времени заживления операционной раны, инфицированию раны с появлением очагов некроза в области операционного шва. The object. The study of the cells of the immune system of the oral cavity in patients before the operation of face lifting, as well as a comparison of these indicators with complications in the postoperative period. Methods. The immune cells from the oral cavity were studied in 100 women (23-68 years), who underwent a dental examination and necessary treatment if they had the dentin caries, wedge-shaped defect of tooth enamel, chronic periodontitis or pulpitis before the facial lifting operation. The immune cells have been isolated by a lavage of gingival sulcus around the damaged tooth. Results. It has been shown that chronic dental diseases made a negative contribution to the oral cavity immune cells. It has been accompanied by the elevated levels of superoxide originating from neutrophils as well as the increased levels of spontaneous and ceramide-induced apoptosis of immune cells isolated from the gingival sulcus. Conclusion. The presence of chronic pathology of teeth even in the case of the preoperative dental sanation has a negative impact on the postoperative period after face lifting, in particular, contributes to the lengthening of the surgical wound healing time, wound infection as well as the partial necrosis of suture.

Relevance. Prognostic value of PD-L1 expression in oral cavity squamous cell carcinoma (OCSCC) and its effect on survival is still controversial. It should be to determine the prognostic role of PD-L1 expression on tumor and immune cells of OCSCC and assess their effect on overall survival (OS) and progression-free survival (PFS).Materials and methods. A prospective study included 145 patients, first diagnosed with OCSCC. PD-L1 expression on tumor and immune cells, infiltrating tumor and its microenvironment, was assessed in all tumor samples by IHC, CPS was calculated. Cut-off values were determined by ROC analysis for identification of PD-L1 expression effect on OS and PFS.Results. Most patients with oral mucosa squamous cell carcinoma showed positive expression of PD-L1 on tumor (77.2%) and immune cells (92.4%). The median PD-L1 expression on tumor cells was 13.5% [1.0-40.0], the median PD-L1 expression on immune cells was 5.0% [1.0-11.0], and the median CPS – 18.0 [3.0-7.8]. Univariate and multivariate analyses revealed a significant negative effect of PD-L1 expression on immune cells ≤ 7% on OS (HR 0.66; 95% CI 0.45-0.93; p = 0.0498); PD-L1 expression in tumor cells ≤ 15% (HR 0.65; 95% CI 0.43-0.98; p = 0.0416) and CPS ≤ 21 (HR 0.62; 95% CI 0.44-0.92; p = 0.0183) for PFS. PD-L1 expression in tumor cells ≤ 6% (HR 0.71; 95% CI 0.47-1.08; p = 0.1096) and CPS ≤ 7 (RR 0.67; 95% CI 0.44-1.01; p = 0.0575) had a confident tendency to negative impact on OS.Conclusion. Positive PD-L1 expression in tumor and immune cells as well as CPS are effective additional factors in the prognosis of the disease course, OS and PFS in patients with OCSCC.

In the last few decades, the incidence of caesarean section is increasing in the world, especially in Indonesia. One of the way to treat tissue scar is through biologic and synthetic dressing where nowadays, amnion has been used as biologic dressing frequently. This study was conducted to determine the effect of the use of fresh amniotic membrane on wound incision Caesarean section compared with Caesarean section incision wound covered using regular gauze bandages and fixated with plaster in RS. Dr. Reksodiwiryo Padang. The design of this study is an experimen-tal study with Post test design with control group design. Sampling was done using a formula consecutive sampling two different test samples obtained an average of 72 people for each group. The analysis used include univariate and bivariate analyzes. The average wound healing time the difference was statistically significant (p value <0.05) in the treatment and control groups. There was highly significant difference in the proportion of local infection on day 3 between the treatment and control groups (p value <0.05). There were very significant differences in the proportion of local allergic reactions at day 3, and 5 between the treatment and control group (p <0.05). There are significant differences in terms of the cost of care per day between treatment and control groups (p <0.05). Relevance. The relevance of the problem is in the high prevalence of the herpes simplex virus. According to WHO, nearly 90% of the population are carriers. Relapses can occur several times a year, which leads to disability and impairs the quality of life. Currently, there are a large number of drugs for general and local treatment. However, they are toxic and can cause allergic reactions. In recent years, a large number of studies have been devoted to the use of lasers in the treatment of dental diseases. Therefore, the question of the use of a laser in the treatment of herpetic lesions of the oral cavity remains relevant.Purpose. The research purpose was to determine the effectiveness of the use of a diode laser in the treatment of herpetic lesions of the lips.Materials and methods. We conducted a comparative assessment of the effectiveness of the use of a diode laser in monotherapy of herpetic lip lesions in 18 patients. The comparison group consisted of 18 patients, in the local treatment of which antiviral ointments were used.Results. The results of the study showed that the use of a diode laser can reduce treatment time, accelerate the epithelization of foci.Conclusion. The results of our study allow us to recommend the use of a diode laser in the complex treatment of herpetic lesions of the oral cavity.

This article the properties of matrix metalloproteinases, their classification, function and role in the development of dental diseases are presented. The analysis of matrix metalloproteinases has been performed, which proves their important role in the physiological and pathological processes of the oral cavity. The possibility of using matrix metalloproteinases as part of one of the methods for diagnosing diseases and using them to evaluate the effectiveness of treatment has been considered. The structure of different types of metalloproteinases, as well as a model of the catalytic domain of the MMP-8 molecule has been presented.

Background: It is known that hormonal imbalances during pregnancy make women more susceptible to dental problems. High levels of progesterone and estrogen during pregnancy, lead to an increased inflammatory response to dental plaque thus causing predisposing to gum diseases such as gingivitis. If untreated, gingivitis leads to chronic periodontitis which may manifest systemically in form of cardiovascular, endocrine or even respiratory disorders. Also, hyperacidity in the oral cavity due to gastric reflux and vomiting leads to decreased pH thus damaging the tooth enamel making the oral cavity more prone to tooth decay and tooth loss. Studies also show that periodontal disease can also lead to adverse pregnancy outcomes such as pre-term and low birth weight babies. Objectives: We sought to understand the role of oral health in pregnancy. Methods: We identified major articles of interest in the field of oral health in pregnancy and drafted a mini-symposium based on relevant information. Conclusion: Regular dental visits and cognizant efforts to sustain a healthy oral environment can help women in the prevention and treatment of dental issues during pregnancy. The paper highlights the common oral manifestations during pregnancy and their local and systemic impact on the body during pregnancy. Furthermore, it also emphasizes the importance of good oral health practices to counteract the oral complications and the significance of oral health awareness in pregnant women.

Abstract Background Hemorrhoids are common. Hemorrhoidectomy should typically be offered to patients whose symptoms result from external hemorrhoids or combined internal and external hemorrhoids with prolapse (grades III/IV). However, none of the currently used surgical methods could be considered an ideal surgical option that is effective, safe, and painless. We hypothesized that a combination of Ruiyun procedure for hemorrhoids (RPH) and simplified Milligan–Morgan hemorrhoidectomy (sMMH) will increase the safety and effectiveness of surgical treatment hemorrhoids. This study aimed to evaluate the efficacy of Ruiyun procedure for hemorrhoids combined simplified Milligan–Morgan hemorrhoidectomy with dentate line-sparing (RPH + sMMH) to treat grade III/IV hemorrhoid. Methods Total 452 patients with hemorrhoids of grade III/IV were retrospectively reviewed in China-Japan Friendship Hospital, 244 cases were assigned to RPH + sMMH group, and 208 cases in MMH group. The primary efficacy outcome was rate of curative at 3 month after operation, and the recurrence rate within 12 months post operation. Secondary efficacy outcomes included wound healing time, time required to resume normal work, constipation symptom, quality of life, and pain post operation was also evaluated. The safety outcome included postoperative complications. Results There were no differences between the two groups in demographic characteristics. There was no statistically significant difference between the two groups in the curative rate. The recurrence rate after 12 months post operation in the RPH + sMMH (3.0%) was significantly lower than the sMMH group (7.8%) (P = 0.032). The wound healing time was significantly shorter in RPH + sMMH group than that in MMH group (P < 0.001). The time required to resume normal work in the RPH + sMMH group was significantly shorter than MMH group (P < 0.001). Compared with the MMH group, the RPH + sMMH therapy preserve better life quality and lower constipation symptom (all P < 0.05). Patients who underwent RPH + sMMH had significantly less postoperative pain than MMH therapy. The total rate of patients with postoperative complications in the RPH + sMMH group (8.6%) was significant lower than the MMH group (16.3%) (P = 0.012). Conclusion RPH + sMMH may more effective in treating patients with III/IV hemorrhoids, which indicated lower recurrence rate, lower postoperative complications and pain, shorter recovery and return to normal life.

ABSTRACT Objective: There are no specific criteria that define the level of amputation in diabetic patients. The objective of this study was to assess the influence of clinical and laboratory parameters in determining the level of amputation and the wound healing time. Methods: One hundred and thirty-nine diabetic patients were retrospectively assessed. They underwent surgical procedures due to infection and/or ischemic necrosis. Type of surgery, antibiotic use, laboratory parameters and length of hospital stay were evaluated in this study. Results: The most common amputation level was transmetatarsal, occurring in 26 patients (28.9%). The wound healing time increased with statistical significance in individuals undergoing debridement, who did not receive preoperative antibiotics and did not undergo vascular intervention. Higher levels of amputation were statistically related to limb ischemia, previous amputation and non-use of preoperative antibiotics. Conclusion: Patients with minor amputations undergo stump revision surgery more often, but the act of always targeting the most distal stump possible decreases energy expenditure while walking, allowing patients to achieve better quality of life. Risk factors for major amputations were ischemia and previous amputations. A protective factor was preoperative antibiotic therapy. Level of Evidence III, Retrospective Study.

Subject. The scientific works of recent years show an increase in the degree of negative impact of vitamin deficiency and vitamin-like substances on the state of health of the population. With the deficiency of most vitamins, synthetic processes and regeneration of oral tissues are reduced, so often the initial signs of hypovitaminosis are stomatitis, gingivitis, glossitis, and therefore, it is dentists who are the first to diagnose deviations in the body associated with vitamin deficiency. This justifies the relevance and practical value of studying and describing clinical cases of manifestations of deficient vitamin conditions in the oral cavity. The object – is to study the effect of B vitamins on the oral mucosa in order to increase the effectiveness of diagnosis of vitamin-deficient conditions of the human body. Methodology. These clinical examples illustrate the management experience of patients with manifestations of deficient vitamin conditions on the oral mucosa. Clinical and laboratory methods of diagnosing the analysed conditions of the organism were applied. Statistical processing of materials was carried out using the STATISTICA 10 software (StatSoft, Inc., USA). Results. The deficient condition of the organism in relation to vitamins B2, B6, B12 in patients 18-75 years old has been studied. Characteristic clinical changes on the oral mucosa of the examined patients were established. So, in most patients with vitamin B2 deficiency, the classic Sebrel triad was found: dermatitis, glossitis, cheilitis. In those examined with a lack of vitamin B6, language desquamations (smoothed, polished language) were determined in the 83.6%, often combined with glossodinia. Patients with vitamin B12 deficiency were characterized by a lesion in the form of Meller-Gunter glossitis in 74.9% of cases, moreover, 67.6% of patients showed paresthesia in the area of tongue and oral mucosa. Conclusions. The study found that the first clinical symptoms of deficient conditions of the presented vitamins of group B were found from the oral cavity. Thus, it is the dentist who is the first to diagnose the pathological states of lack of group B vitamins in the human body, which emphasizes the importance and relevance of continuing to study these states of the body.

Background: Fistula in ano is a silent menace in human beings, the treatment is a challenging one even for experienced surgeons, fistula in ano forms a good majority of treatable benign lesions of rectum and anal canal. Aim was to study the efficacy of fistulotomy and fistulectomy in the treatment of low anal fistulae.Methods: A randomized control study was conducted to compare fistulotomy with fistulectomy in patients with low anal fistulae.Results: The operating time in fistulotomy group was 12.13minutes±2.11minutes and in fistulectomy group was 22.23±3.36minutes. The post-surgery hospital stays in Group I was 1.80±0.66days and in Group II was 2.60±0.563 days. The wound healing time in Group I was 24.20±2.95 days which was considerably less when compared to patients in Group II where it was 31.50±4.34 days.Conclusions: Fistulotomy has a slight edge over fistulectomy in the treatment of low anal fistulas since it has shorter operating time, less post-operative pain, less complications, quicker wound healing time, less incontinence and a comparable recurrence rate.

Melanoma is one of the most immunogenic tumors and has the highest potential to elicit specific adaptive antitumor immune responses. Immune cells induce apoptosis of cancer cells either by soluble factors or by triggering cell-death pathways. Melanoma cells exploit multiple mechanisms to escape immune system tumoricidal control. FKBP51 is a relevant pro-oncogenic factor of melanoma cells supporting NF-κB-mediated resistance and cancer stemness/invasion epigenetic programs. Herein, we show that FKBP51-silencing increases TNF-related apoptosis-inducing ligand (TRAIL)-R2 (DR5) expression and sensitizes melanoma cells to TRAIL-induced apoptosis. Consistent with the general increase in histone deacetylases, as by the proteomic profile, the immune precipitation assay showed decreased acetyl-Yin Yang 1 (YY1) after FKBP51 depletion, suggesting an impaired repressor activity of this transcription factor. ChIP assay supported this hypothesis. Compared with non-silenced cells, a reduced acetyl-YY1 was found on the DR5 promoter, resulting in increased DR5 transcript levels. Using Crispr/Cas9 knockout (KO) melanoma cells, we confirmed the negative regulation of DR5 by FKBP51. We also show that KO cells displayed reduced levels of acetyl-EP300 responsible for YY1 acetylation, along with reduced acetyl-YY1. Reconstituting FKBP51 levels contrasted the effects of KO on DR5, acetyl-YY1, and acetyl-EP300 levels. In conclusion, our finding shows that FKBP51 reduces DR5 expression at the transcriptional level by promoting YY1 repressor activity. Our study supports the conclusion that targeting FKBP51 increases the expression level of DR5 and sensitivity to TRAIL-induced cell death, which can improve the tumoricidal action of immune cells.
