Abstract: Backgrounds Dysmenorrhea is the most common gynecological symptom reported in adolescents. Prevalence estimates vary from 45% to 75%. Absenteeism from work and school as a result of dysmenorrhoea is common (13% to 51% women have been absent at least once and 5% to 14% are often absent owing to the severity of symptoms). Dysmenorrhea association with psychologist and behavior factors and its effects their physical activities and school performance. Some studies had shown that association between school performance with dysmenorrhea.Objective To assess the comparison between school performance with and without dysmenorrhea in pubertal adolescents. Methods A cross sectional study was conducted on Juni 2010.The samples were recruited with simple randomization in adolescents aged 10 until 18 years old at school of Musthafawiyah Mandailing Natal districs. All subjects who met the inclusion criteria were assessed their school performance on 2 consecutive semester in one year. In order to assessed the comparison between school performance with or without dysmenorrhea used X2 test. Results One hunderd and sixteen subject were participated in this study. After measured the school performance. No significant different on school performance with and without dysmenorrhea in two group (P=0.176 and P=008, 95%CI -0.05 to 0.05). Conclusion: There was no significant difference school performance between with and without dysmenorrheal. Keywords: Dysmenorrhea, school performance, pubertal adolescent.     Abstrak: Latar belakang Nyeri haid merupakan nyeri yang terjadi selama masa menstruasi.Angka kejadian nyeri haid berkisar antara 45% sampai 75% dari seluruh remaja perempuan pubertas. Dimana ketidak hadiran di sekolah berkisar antara 13% sampai 51% serta 5% sampai 14% ketidak hadiran tersebut disebabkan beratnya gejala yang terjadi. Nyeri haid berhubungan dengan faktor perilaku dan psikologis. Selain itu dapat menyebabkan pembatasan aktifitas sekolah dan prestasi akademik. Beberapa studi telah menunjukkan adanya kaitan prestasi akademik dengan nyeri haid. Tujuan Untuk mengetahui perbandingan prestasi akademik dengan dan tanpa nyeri haid pada anak perempuan pubertas. Metode Studi ini merupakan studi cross sectional. Pemilihan sampel secara acak sederhana yang dilakukan pada anak remaja perempuan yang berusia 10 sampai 18 tahun di SMP St Theresia Malalayang dan SMA St Theresia Malalayang Manado yang dilakukan pada bulan Desember 2011. Semua subjek yang memenuhi kriteria inklusi dinilai prestasi akademik selama dua semester berturut-turut.Untuk menilai perbandingan prestasi akademik dengan atau tanpa nyeri haid digunakan uji X2. Hasil Seratus enam belas subjek berpartisipasi dalam penelitian ini. Setelah dinilai prestasi akademik. Ditemukan bahwa tidak terdapat perbedaan yang bermakna antara prestasi akademik dengan atau tanpa nyeri haid (P 0.176 vs P: 0.08). IK 95%:-0.05; 0.05. Simpulan: Tidak ditemukan adanya hubungan prestasi akademik dengan dan tanpa nyeri haid. Kata kunci: Nyeri haid, prestasi akademik, perempuan pubertas

Objective: Fever alone can lead to rare serious complications in children, such as febrile seizures. The aim of this study is to assess the knowledge, beliefs, and behavior of parents toward fever and its management. Methods: A cross-sectional study using an online questionnaire was applied over a period of 3 months, from January to March 2018, to parents who were living in Saudi Arabia. The inclusion criteria were a parent who is a resident of Saudi Arabia, with at least one child aged 6 years or less, while incomplete questionnaires, having a child aged more than 6 years, or parents who were not living in Saudi Arabia were excluded. Results: A total of 656 parents completed the questionnaire. More than two-thirds of the subjects were female, the majority of whom were aged between 25–33 years old. The best-reported place to measure the temperature of children was the armpit (46%), followed by the ear (28%) and the mouth (10.7%). More than half of the parents considered their children feverish at a temperature of 38 °C. The majority of parents (79.7%) reported that the most serious side effects of fever were seizure, brain damage (39.3%), coma (29.9%), dehydration (29.7%), and death (25%). The most common method used to measure a child’s temperature was an electronic thermometer (62.3%). The most common antipyretic was paracetamol (84.5%). Conclusions: Our study demonstrates the good knowledge of parents in identifying a feverish temperature using the recommended route and tools for measuring body temperature.

Background and objectives: Although laparoscopic sleeve gastrectomy (LSG) is effective for obesity management, postoperative vitamin B12 (B12) deficiency is of major concern. In this cross-sectional study, we assessed the levels of B12 and its related functional biomarkers, namely, total homocysteine (tHcy), methylmalonic acid (MMA), folate, methylcitric acid (MCA), and hemoglobin (Hb), in one-year postoperative LSG patients and matched controls. Materials and Methods: Plasma B12, tHcy, MMA, folate, and MCA were measured in matched controls (n = 66) and patients (n = 71) using validated liquid chromatography-tandem mass spectrometry techniques and protocols in the United Arab Emirates (UAE). Results: The median B12 concentration in patients (177 pmol/L) was significantly lower (p < 0.001) than in the controls (334.7 pmol/L). The tHcy and MMA levels were significantly increased (p < 0.001 and p = 0.011, respectively) and folate levels were significantly decreased (p = 0.001) in the LSG patients compared to the controls. Interestingly, no significant difference in MCA levels were observed between the two groups. The levels of tHcy and MMA were concomitantly increased with the decreased folate levels in postoperative LSG patients when compared with the controls. The Hb levels were significantly lower in males and females in the patient group compared with those in the control group, respectively (p = 0.005 and p = 0.043). Conclusions: This is the first report of serum levels of B12 and its functional biomarkers in postoperative LSG patients among a local population from the UAE. Our findings revealed significant alterations of the B12 biomarkers, total B12, MMA, and tHcy in one-year postoperative LSG patients.

BACKGROUND: Measles is highly endemic disease in Pakistan that can be prevented by vaccinating the child.Patients with rash and fever are the major syptoms of this infection. The aim of this study was to find the associationof clinical and serological outcome of measles and rubella with demographic profiles in patients hospitalised withclinical suspicion of these conditions. METHODS: A descriptive cross sectional study was conducted in children hospital of PIMS Islamabad where allchildren presenting with maculopapular rash in outdoor patient, indoor patient and accident and emergencydepartment were enrolled. The study duration was one year or the completion of required sample size. A total of 73children who fulfilled the inclusion criteria were enrolled in this study. A purposive sampling technique was used toselect cases. Data was collected through a structured proforma especially designed for this study. Permission wastaken from the Hospital Ethical Committee before the commencement of the study. A written consent was also takenfrom the parents of children prior to conduct the study. RESULTS: All the children in this study was 46.5 (± 40.1) months of age, the mean (SD) age was 4 years and 10months (± 3 years and 4 months). The median age of all children was 24 months (2 years). The youngest child was 7months old while the eldest child was 12 years (144 months) of age. Out of 73 children enrolled in the current studywho presented with maculopapular rash and their serological outcome was assessed, 50 (68.5 percent) were boyswhereas 23 (31.5 percent) were girls. Koplik's spots were present among 56 (76.7 percent) children whereas 17(23.3 percent) did not have Koplik's spots at the time of enrolment. lymphadenopathy was assessed in 35 (47.9percent) children while 38 (52.1 percent) did not have lymphadenopathy at the time of enrolment. CONCLUSION: Study concluded that a higher proportion of children presenting with maculopapular rash werediagnosed to have measles on the basis of serology.

Adolescent females with and without dysmenorrhea were recruited for this study. All participants completed questionnaires including age of menarche, length of menstrual cycle, length of bleeding, number of sanitary napkins used daily and school absences. School reports from two consecutive semesters in one year were used to evaluate subjects’ academic performance. An academic score of higher than 7.5 was considered good performance while scores of less than 7.5 were considered poor. We used the chi-square test to analyze differences in school performance between girls with and without dysmenorrhea.Results One hundred and sixteen participants were divided into 2 groups, those with and without dysmenorrhea, of 58 subjects each. We found no significant difference in school performance between the two groups, P=0.176 (95% CI -0.009 to -0.048 and P=0.08 (95%CI -0.052 to 0.024).Conclusion There was no significant difference in school performance of girls with and without dysmenorrhea.

Objective: To compare the temperamental characteristics of children who stutter with those who do not stutter. Study Design: Comparative cross-sectional study. Place and Duration of Study: Ayub Medical Complex, Abbottabad, from Jun to Nov 2018. Methodology: We recruited 120 children of both genders aged 3-8 years. Sample recruited included two groups including 60 children with stuttering (CWS) and 60 children with no stuttering (CWNS), using consecutive sampling. After taking consent, data was gathered using demographic sheet and Children Behavioral Questionnaire (CBQ) from the sample population. Statistical analysis was done using SPSS-21. Results: The sample included 82 (68.3% males and 38 (31.7%) female children. t-test results of children with stuttering and children with no stuttering showed statistically significant difference for effortful control (p<0.05) including dimension of inhibitory control, low intensity pleasure and perceptual sensitivity. However, the values for Surgency Extraversion and Negative affectivity were not statistically significant though results showed higher and lower mean scores respectively for stutterers compared to non-stutterers. However, the dimensions of anger, frustration, discomfort and falling reactivity showed statistically significant difference (p<0.05). Conclusion: Children with stuttering and children with no stuttering differ in their temperamental characteristics with statistically significant difference for effortful control with lower control in stutterers.

Background: Although sexual death during intercourse occurs rarely in patients with cardiovascular diseases (CVDs), most such patients avoid it because they fear a dangerous event. Given the significance of awareness about this issue among patients with CVDs, we sought to compare sexual knowledge between 2 groups of patients prepared for cardiac surgery and patients prepared for cardiac rehabilitation (CR). Methods: This cross-sectional study, conducted between April and July 2016, recruited 157 patients with CVDs (107 candidates for surgery and 50 patients prepared for CR) in Imam Ali Hospital, in the Iranian city of Kermanshah. The cases, selected through entire counting according to our inclusion criteria, responded to a standard sexual knowledge inventory. Sexual knowledge and professionals responsible in providing sexual rehabilitation from the perspective of patients were compared using the independent t-test and the χ2 test. Results: The participants’ mean age was 55.39±9.82 years (male: 58.6%). Overall, the 2 groups had poor sexual knowledge. Although the CR program started 57.85±13.92 days after surgery, the sexual knowledge of this group of patients was not significantly different from that of the patients prepared for surgery (P=0.904). This difference was not significant between the 2 genders (P=0.077). Finally, concerning the professionals responsible in providing sexual rehabilitation, the patients selected psychologists (P=0.006) and nurses (P=0.012) more frequently in the initial phase of CR program.   Conclusion: Sexual knowledge was poor in our CR patients at the outset of the program. Given the poor knowledge in these patients 2 months after surgery and the lack of significant difference in knowledge between these patients and those prepared for surgery, it is advisable that they be provided with the necessary information in this regard in this golden time before hospital discharge.

Objectives: This study aimed to examine Musculoskeletal Pains (MPs) among newly admitted university students and investigate the effect of gender differences on the prevalence of MPs. Methods: This cross-sectional descriptive study was conducted on 2641 university students (1303 men, 1338 women). The study data were collected from the Health Center of the University of Tehran in the fall of 2019. Musculoskeletal pains in students were examined using the Nordic questionnaire. The data were analyzed by descriptive statistics and the Chi-squared test. Results: The highest prevalence rates of MPs in the past 12 months, past 7 days, and problems that inhibited routine activities in the past 12 months were reported in the neck area (13.4%, 5.8%, and 10.5%, respectively), and the lowest rates in the elbow (1.5%, 0.6%, and 1%, respectively). Findings also indicated a significant difference between men and women regarding MPs in the neck and shoulders in all three items of the Nordic questionnaire (P<0.05). Discussion: Based on these results, it is necessary to prevent musculoskeletal pains, especially in the neck and back, among the newly admitted students to the university.

Background: In evaluation of non-traumatic subarachnoid hemorrhage CT angiography (CTA) has 97-98% sensitivity and near 100% specificity. Objective: This study was conducted to evaluate the CTA findings of CT positive non traumatic subarachnoid hemorrhage. Methodology: This is an observational cross sectional study performed in Neurology department of National Institute of Neurosciences and Hospital, Dhaka over one year period (January 2019 to December 2019). Total 87 CT positive subarachnoid hemorrhage cases were purposively included in this study. All CT positive patients underwent CTA of Cerebral vessels for further evaluation. The angiography were evaluated by competent neuro-radiologists blinded about the study. Result: Among 87 patients, 40.2% were male and 59.8% were female. The average age was 53.33±11.1 years. Among the studied patient the source of bleeding was found 78.16% aneurysmal and 21.84% non-aneurysmal. 85.30% patients had single aneurysm and 14.70% had multiple aneurysm. The highest number of patient had Acom aneurysm (41.17%) followed by MCA (22.05%), ICA (13.23%), ACA (7.35%) and vertebral artery (1.14%) in order of frequency. Among the multiple aneurysm group most of the patients had combination of Acom, MCA and ICA aneurysm. Conclusion: From this study, we can conclude that CTA can be used as the primary diagnostic tool in evaluation of spontaneous SAH. Pleural effusion is present when there is >15ml of fluid is accumulated in the pleural space. It can be divided into two types; exudative and transudative pleural effusion. Tuberculosis and parapneumonic effusion are the common cause of exudative pleural effusion whereas heart failure accounts for most of the cases of transudative pleural effusion. This study was a hospital based cross sectional study performed at Nepal Medical College during the period of January 2016-December 2016. A total of 50 patients who fulfilled the inclusion criteria were enrolled. Pleural effusion was confirmed by clinical examination and radiology. After confirmation of pleural effusion, pleural fluid was aspirated and was analysed for protein, LDH, cholesterol. The Heffner criteria was compared with Light criteria to classify exudative or transudative pleural effusion. Among 50 patients, 30 were male and 20 were female. The mean age of patient was 45.4±21.85 years. The sensitivity and specificity of using Light criteria to detect the two type of pleural effusion was 100% and 90.9%, whereas using Heffner criteria was 94.87%, 100% respectively(P<0.01). There are variety of causes for development of pleural effusion and no one criteria is definite to differentiate between exudative or transudative effusion. In this study Light criteria was more sensitive whereas Heffner criteria was more specific to classify exudative pleural effusion. Hence a combination of criteria might be useful in case where there is difficulty to identify the cause of pleural effusion.
