Abstract: When the pandemic corona virus disease (COVID-19) occurs, emergency services and hospitals are finally disrupted. One of the medical emergencies which can cause death and disability as well as other problems if being ignored, especially in productive age patients is stroke. This study was aimed to obtain the management of ischemic stroke during COVID-19 pandemic. This was a literature review study using three databases, namely Clinical Key, PubMed and Google Scholar. The keywords used in data search were acute AND stroke AND pandemic. The results showed that the treatment of acute ischemic stroke during COVID-19 pandemic consisted of three stages, namely pre-hospitalization (public education, contacting medical assistance, use of personal protective equipment, pre-notification), hospitalization (emergency room, imaging room, neuroradiology unit, ICU and IMCU), and post hospitalali-zation (integrated team approach). In conclusion, the management of acute ischemic stroke during COVID-19 pandemic begins before the medical personnel take action until the action is carried out. The main things in the management of acute ischemic stroke during COVID-19 pandemic are handling fast and precisely, using adequate personal protective equipment (PPE), and using communication tools or long-distance medical care (telemedicine).Keywords: acute ischemic stroke, COVID-19, pandemic Abstrak: Saat terjadi pandemi corona virus disease (COVID-19), layanan kegawatdaruratan dan rumah sakit megalami perubahan. Salah satu kasus kegawatdaruratan medik yang menjadi penyebab kematian dan kecacatan serta dapat menimbulkan masalah lainnya jika diabaikan terutama pada pasien usia produktif ialah stroke. Penelitian ini bertujuan untuk mengetahui penanganan stroke pada masa pandemic COVID-19. Jenis penelitian ialah literature review yang menggunakan tiga database yaitu Clinikal key, PubMed dan Google Scholar. Kata kunci yang digunakan dalam pencarian data ialah acute AND stroke AND pandemic. Hasil penelitian mendapatkan penanganan stroke iskemik akut di masa pandemi COVID-19 terdiri dari 3 tahapan yakni prahospitalisasi (edukasi masyarakat, menghubungi bantuan medis, penggunaan alat pelindung diri, pranotifikasi rumah sakit), hospitalisasi (ruang gawat darurat, ruang pencitraan, unit neuroradiologi, ICU dan IMCU), dan pasca hospitalisasi (pendekatan tim terpadu). Simpulan penelitian ini ialah penanganan stroke iskemik akut di masa pandemi COVID-19 dimulai dari sebelum tenaga medis melakukan tindakan sampai setelah tindakan dilaksanakan. Hal utama dalam penanganan stroke iskemik akut di masa pandemi COVID-19 ialah penanganan yang cepat dan tepat, penggunaan alat pelindung diri (APD) yang memadai, dan pemanfaatan alat komunikasi atau perawatan medis jarak jauh.Kata kunci: stroke iskemik akut, pandemi, COVID-19

Kondisi pandemi Corona virus disease (Covid-19) yang semakin memburuk bukan hanya masalah medis saja, namun juga masalah kerahasiaan pasien yang juga berpengaruh terhadap usaha menekan penyebaran Covid-19. Timbul pertanyaan apakah pasien Covid-19 berhak memperoleh perlindungan hak kerahasiaan atau hak kerahasiaan ini dapat dikesampingkan untuk kepentingan yang lebih luas dalam pencegahan penyebaran Covid-19. Jurnal yang digunakan dalam literature review didapatkan melalui database penyedia jurnal Scientific melalui google scholar. Dari literatur yang didapatkan secara etik adanya kewajiban moral dalam hal mencegah stigmatisasi, diskriminasi, dan kesalahan dalam menyebarluaskan informasi terkait Covid-19. Kewajiban untuk melindungi rahasia pasien tidak bersifat mutlak, dan dapat dikecualikan bila ada kondisi yang membahayakan masyarakat luas yang diatur dalam etika kesehatan masyarakat. Undang-undang kesehatan di beberapa negara seperti di Amerika memberikan pengecualian terhadap kerahasiaan data pasien apabila berhubungan dengan kepentingan umum maka kerahasiaan pasien dapat dibuka. Hal ini akan sejalan dengan kondisi saat ini dikarenakan penyebaran virus Covid-19 telah menjadi bencana. Telaah pustaka ini dilakukan karena sangat pentingnya suatu pedoman etik, dan peraturan kerahasiaan pasien khususnya pasien Covid-19 yang melindungi kedua belah pihak, dan adanya kepastian hukum perikatan dokter dan pasien.

AbstractThere are numbers of Indonesian Youtube Videos that show steps to wear and remove Personal Protective Equipment (PPE) for Corona Virus Disease (COVID19). However, the conformity with the current guidelines remains unknown. This paper aims to determine the validity of these videos based on the World Health Organization (WHO) guidelines. We searched on the Youtube website for videos in donning and doffing PPE for droplet precaution and selected the videos using inclusion and exclusion criteria. Included videos were then evaluated with a checklist derived from WHO course on donning and doffing PPE for COVID-19 and WHO recommendation on PPE for Covid-19. The search that was undertaken resulted in 66 videos, and 40 videos were included for evaluation. There is no significant difference in the donning and doffing score between account types (personal, organizational/institutional, news). The average number of viewers of all videos is more than 2700 viewers. The average score of donning is less than 70% of the total score, and the average doffing score is under 65%. The vast majority of the videos do not follow the WHO recommendation on the PPE type and use more equipment than recommended. There are several contamination risks shown by the videos. Indonesian Youtube videos on PPE procedures for COVID-19 must be selected carefully to be used as an instructional or educational media since most of it presents a high risk of cross-contamination.Keyword: personal protective equipment, audiovisual, cross-contamination, trainingAbstrakTerdapat video dalam Bahasa Indonesia yang terkait cara memakai dan melepaskan Alat Pelindung Diri (APD) untuk Corona Virus Disease (COVID19). Namun, kesesuaian video terhadap pedoman terkini belum diketahui. Penelitian ini bertujuan untuk menentukan validitas video berdasarkan pedoman World Health Organization. Kami menelusuri Youtube untuk memperoleh video instruksi mengenakan dan melepas APD pencegahan kontaminasi droplet dan menyeleksi video tersebut berdasarkan kriteria inklusi dan eksklusi. Video yang telah diseleksi kemudian dievaluasi menggunakan checklist yang diturunkan dari langkah-langkah yang ditunjukan dalam kursus APD COVID-19 yang disediakan oleh WHO, serta rekomendasi APD dari WHO. Penelusuran Youtube menampilkan 66 video, dan dipilih 40 video yang sesuai dengan kriteria kelayakan. Tidak terdapat perbedaan signifikan dalam skor mengenakan dan melepaskan APD antara akun personal, organisasi/institusi, dan berita. Rata-rata jumlah penonton video adalah lebih dari 2700. Skor rata-rata pemakaian APD kurang dari 70% dari skor total, dan skor pelepasan APD kurang dari 65%. Sebagian besar video tidak mengikuti rekomendasi WHO dalam pemilihan tipe PPE dan menggunakan alat yang lebih banyak dari yang direkomendasikan. Terdapat sejumlah risiko kontaminasi yang dapat diobservasi dari langkah-langkah yang ditunjukan dalam video. Video Youtube prosedur APD untuk COVID-19 perlu diseleksi dengan cermat untuk digunakan sebagai media instruksi maupun pendidikan karena sebagian besar menunjukan risiko tinggi kontaminasi silang.Kata Kunci: alat pelindung diri, audiovisual, kontaminasi silang, pelatihan

AbstractThe COVID-19 virus is very easy to spread, especially through droplets from infected people to others. This makes health workers vulnerable to infection. To overcome this problem. it is necessary to use personal protective equipment (PPE) which aims to protect health workers from the COVID-19 virus. To describe the compliance level of health workers with the personal protective equipment (PPE) in hospitals during the COVID-19 pandemic. A research literature review of 5 articles accessed from online databases such as Google Scholar and PubMed published from 2020-2021. The research used the Systematic Reviews and Meta-Analysis (PRISMA) checklist. The results of this literature review showed that, from a total of 1496 respondents, 1158 respondents (77.4%) had a good level of compliance with personal protective equipment (PPE) usage. Based on the results, it can be concluded that the health workers have a good level of compliance with the PPE usage during the COVID-19 pandemic. This literature review research is expected to be new knowledge, insight, and reference for health workers regarding the compliance with the use of personal protective equipment (PPE), especially during the COVID-19 pandemic. AbstrakVirus COVID-19 sangat mudah menyebar terutama melalui droplet dari orang yang terinfeksi kepada orang lain. Hal ini mengakibatkan petugas kesehatan rawan untuk terinfeksi. Untuk mengatasi masalah tersebut maka diperlukan penggunaaan alat pelindung diri (APD) yang bertujuan untuk melindungi petugas kesehatan dari virus COVID-19. Untuk mengetahui bagaimana tingkat kepatuhan penggunaan alat pelindung diri (APD) tenaga kesehatan di rumah sakit pada masa pandemi COVID-19. Penelitian literature review dari 5 artikel mengakses database online seperti Google Scholar dan PubMed yang di publikasi dari tahun 2020-2021. Instrumen telaah kritis yang digunakan yaitu instrument PRISMA checklist. Hasil penelitian literature review ini menunjukan diketahui dari total responden sebanyak 1496 orang, sebanyak 1158 responden dengan presentase (77,4%) memiliki kepatuhan yang baik dalam penggunaan alat pelindung diri (APD). Pada penelitian literature review kali ini didapatkan hasil berupa tingkat kepatuhan penggunaan APD yang baik oleh petugas kesehatan di masa pandemi COVID-19. Penelitian literature review ini diharapkan dapat menjadi pengetahuan, wawasan dan referensi baru untuk tenaga kesehatan terkait kepatuhan penggunaan alat pelindung diri (APD) terutama pada masa pandemi COVID-19.Kata kunci: Alat Pelindung Diri (APD), COVID-19, Kepatuhan, Tenaga Kesehatan.

Background: Personal protective equipment (PPE) is defined by the Occupational Safety and Health Administration as specialized clothing or equipment worn by an employee for protection against infectious materials. They include gloves, gowns, masks, respirators, googles and face shields. The CDC has issued guidelines on appropriateness of when, what, and how to use PPE. Despite these guidelines, compliance with PPE remains challenging. Methods: We implemented a massive hospital-wide rapid education program on PPE donning and doffing of all employees and staff. This program included an online video, return demonstration and just-in-time training. To develop the program, we recorded PPE training video, reviewed PPE validation checklist, developed new isolation precaution signage with quick response (QR) code to video, developed a nutrition tray removal video and a equipment cleaning video, developed family and visitor guidelines for isolation precautions, and created an audit tool for PPE donning and doffing practices. The program required interdisciplinary collaboration including administration, infection prevention, nursing education, central supply, environmental services, facility maintenance, and security. Results: The first phase of the program was implemented through 30 separate 4-hour PPE skills fair offered over 48 hours. In total, 500 staff members were trained in the first 48 hours; 6 additional 3-hour sessions were provided on site in the following 3 month. Additionally, training was provided in off-site clinics, physician leadership meetings, new-hire orientation for nursing staff, and monthly resident and fellow training through graduate medical education. As needed, training was provided by infection prevention, nursing education, and floor nurses. In total, 5,237 staff members were trained within 3 months after implementation. Actual audit results (50 audits per week) showed improved and sustained compliance to >94%. Conclusions: A massive hospital-wide educational program including online video, return demonstration, and just-in-time training is a feasible and very effective method to improve compliance with PPE donning and doffing. A multidisciplinary team approach, administration support, and continuous education and audits are key factors in successful implementation.Funding: NoneDisclosures: None

In normal conditions, discarding single-use personal protective equipment after use is the rule for its users due to the possibility of being infected, particularly for masks and filtering facepiece respirators. When the demand for these protective tools is not satisfied by the companies supplying them, a scenario of shortages occurs, and new strategies must arise. One possible approach regards the disinfection of these pieces of equipment, but there are multiple methods. Analyzing these methods, Ultraviolet-C (UV-C) becomes an exciting option, given its germicidal capability. This paper aims to describe the state-of-the-art for UV-C sterilization in masks and filtering facepiece respirators. To achieve this goal, we adopted a systematic literature review in multiple databases added to a snowball method to make our sample as robust as possible and encompass a more significant number of studies. We found that UV-C’s germicidal capability is just as good as other sterilization methods. Combining this characteristic with other advantages makes UV-C sterilization desirable compared to other methods, despite its possible disadvantages.
