Analisis Faktor yang Berpengaruh terhadap Kualitas Tidur Pasien Gagal Jantung di RSUD Kota Tasikmalaya

Pendahuluan: Penyakit gagal jantung adalah penyakit sindrom klinis yang ditandai oleh sesak nafas dan fatique saat istirahat atau aktivitas yang disebabkan oleh kelainan struktur atau fungsi jantung. Gejala yang paling umum yaitu ngantuk sepanjang hari dan kesulitan tidur. Kondisi tersebut menyebabkan peningkatan parahnya penyakit jantung, metabolik dan kognitif pasien. Penelitian ini bertujuan menganalisis faktor – faktor yang berpengaruh terhadap kualitas tidur pasien gagal jantung. Metode: Desain penelitian adalah korelasi analitik dengan cross sectional. Tekhnik sampling adalah consecutive sampling berjumlah  80. Analisa data dilakukan dengan metode univariat, bivariat dan multivariat dengan uji regresi logistik ganda. Hasil: Hasil penelitian menunjukkan bahwa kebiasaan minum kopi (p=0,001), kebiasaan merokok (p=0,004) dan tingkat keparahan penyakit (p=0,028) mempunyai pengaruh yang signifikan terhadap kualitas tidur pasien gagal jantung. Hasil multivariat didapatkan tingkat keparahan penyakit merupakan faktor paling dominan yang berpengaruh terhadap kualitas tidur pasien gagal jantung p = 0,001 (p < 0,05). Diskusi: Efek kafein dan nikotin menyebabkan peningkatan aktivitas kardiovaskuler seperti peningkatan denyut jantung dan tekanan darah sehingga menyebabkan pasien dalam keadaan terjaga. Perubahan pada kualitas tidur pada pasien gagal jantung juga dipengaruhi oleh tingkat keparahan penyakit yang menyebabkan perubahan secara patologis pada pasien seperti timbulnya sesak nafas dan adanya pembatasan pada saat aktivitas.Kata Kunci : Gagal Jantung, Kualitas Tidur Introduction: Heart failure is a clinical syndrome disease characterized by shortness of breath and fatigue at rest or on exertion caused by abnormalities of structure or function of the heart. The most common symptoms found in patients with heart failure are sleepy during the day and difficulty sleeping. The condition causes an increase in the severity of heart disease increases, metabolic and cognitive patients. This study aimed to analyze factors that affect the quality of sleep of heart failure patients in Tasikmalaya hospital. Methods: The study design was cross-sectional analytic correlation. Sampling techniques by consecutive sampling are 80 samples. The data analysis was conducted using univariate, bivariate and multivariate multiple logistic regression to find the dominant factor. Results: The results showed that coffee drinking habits (p = 0.001), smoking (p = 0.004) and severity of disease (p = 0.028) had a significant influence on the quality of sleep of heart failure patients. Multivariate results obtained severity of the disease is the most dominant factor affecting the quality of sleep of heart failure patients p = 0.001 (p <0.05). Discussion: The effects of caffeine and nicotine cause an increase in cardiovascular activity such as increased heart rate and blood pressure, causing the patient is awake. Changes in the quality of sleep in patients with heart failure also affected by the severity of the disease that causes pathological changes in patients such as the onset of shortness of breath and the current restrictions on activity. The study aimed to describe the reports of heart failure patients on the factors that cause difficulties to sleep and the association of these factors with the quality of sleep. This cross-sectional study involved a non-probabilistic sample of 400 patients (mean age 57.8 years, 64.8% were men, average education of 6.1 years, 82.5% in functional class II or III) with heart failure. The main factors associated with sleeping difficulty were: nocturia, interrupted sleep at night and breathing difficulty. Sleeping difficulties in heart failure patients are diverse and there is an association between these difficulties and quality of sleep. Most of these disorders warrant professional nursing interventions.

Background: Heart failure is an escalating health problem around the world. Despite significant scientific advances, heart failure patients experience multiple physical and psychological symptoms that can impact the quality of life. Aims: To determine the (1) symptom prevalence, severity, distress and symptom burden in patients with heart failure; (2) impact of age and gender on symptom prevalence, severity, distress and symptom burden; and (3) impact of symptom prevalence and symptom burden on health-related quality of life (HRQOL) in patients with heart failure. Methods: A convenience sample of 53 heart failure patients participated in this descriptive, cross-sectional design. Symptoms and HRQOL were measured using the Memorial Symptom Assessment Scale—Heart Failure and the Minnesota Living with Heart Failure Questionnaire. Results: Patients experienced a mean of 15.1 ± 8.0 symptoms. Shortness of breath and lack of energy were the most prevalent. Difficulty sleeping was the most burdensome symptom. Lower age, worse functional status, total symptom prevalence and total symptom burden predicted 67% of the variance in HRQOL. Conclusion: Patients with heart failure experience a high level of symptoms and symptom burden. Nurses should target interventions to decrease frequency, severity, distress and overall symptom burden and improve HRQOL.

Abstract Heart failure has now become a global cardiovascular problem with high mortality and disability. Sleep disturbance is a common problem in heart failure patients. Sleep disorders that occur in patients with heart failure will cause increased levels of urine catecholamine, plasma leptin, and inflammatory markers. These changes are related to the development of hypertension, activation of the sympathetic nervous system, and blunting of chemoreflex, which will increase the heart burden and ultimately lead to a poor prognosis. Sleep disorders in heart failure patients can be treated with psychological interventions and behaviors including sleep hygiene and Benson relaxation. Nurses, as one of the health professionals, have an important role in this matter. This is because the scope of nursing is meeting basic human needs, namely sleep. Through intervention in sleep hygiene and relaxation, Benson is expected to be able to improve the patient's sleep quality. Background: Heart failure is a major problem in western societies. Sleep Disorders maintain a bidirectional relationship with heart failure, as shown by studies conducted in other countries. This study aims to describe the quality of sleep in Spanish patients with heart failure. Materials and methods: We carried out a cross-sectional study to analyze the quality of sleep in a sample of 203 patients with a diagnosis of heart failure admitted to an Internal Medicine Service. The Pittsburg Sleep Quality Index (PSQI) was used to evaluate sleep quality in our sample over a one-month period. Results: 75% of the sample presented sleep disorders. The most common problems included the interruption of sleep (73.5% nocturia and 30% breathing difficulties); 35% had poor sleep efficiency; 33% showed a decrease in daytime performance; 84% had used hypnotics at some point to induce sleep and 35% used them regularly. Conclusions: This is the first study to report on the perceived sleep quality of patients with heart failure in Spain. Self-perception of sleep quality differed from that estimated by the PSQI. The prevalence of the use of sleep-inducing medication was very high. The diurnal dysfunction generated by sleep disorders in a heart failure environment can contribute to the development of self-care and cognitive deterioration problems.

Introduction: The quality of life of patients with heart failure is influenced by several factors, one of which is spiritual factors. Tri Hita Karana (THK) is one of the concepts of the life order of the Balinese people, which basically teaches about maintaining harmony between humans and God (prahyangan), humans and humans (pawongan), humans with the environment (palemahan). This study aimed to determine the relationship between THK implementation on the quality of life of heart failure patients at Regional Hospital of Mangusada. Methods: A cross-sectional design study was used. In total 41 heart failure patients who visited Mangusada Hospital between April to May 2020, selected using a purposive sampling technique. The research instrument used was the attitude and behavior implementation of THK and the Minnesota Living with Heart Failure Questionnaire (MLHFQ). The Spearman rank test was used in this study. Results: There were a significant correlation between Tri Hita Karana's Implementation and Life Quality of Heart Failure Patients in Mangusada Regional Hospital with p-value (0.001), where p < α (0.05). Conclusion: The better the implementation of THK, the better the quality of life for patients with heart failure. The results of this study were expected to be used in providing education to patients by implementing THK to improve the quality of life for patients with heart failure.

Background: An increasing number of people around the world suffer from cardiovascular diseases such as atrial fibrillation (AF) and heart failure (HF). Atrial fibrillation (AF) has been linked to poor prognosis in heart failure (HF) patients with mild to moderate LV dysfunction, and it is most prominent in these patients. Despite advancements in care and treatment options like catheter ablation, managing AF is still a therapeutic challenge even with these advances. Aim/Objective: To determine the frequency of atrial fibrillation in heart failure patients. Subjects and Methods: This cross-sectional study was conducted on 246 patients at Adult Cardiology, NICVD, Karachi for Six months after approval from 31-08-2019 till 29-02-2020. Patients' consent was obtained verbally before data were collected prospectively. There were 246 patients with heart failure who had been diagnosed and met the diagnostic criteria. An overview of the event's history was gathered, as well as demographic data. Qualitative variables were presented as frequency and percentages, while quantitative data were given as simple descriptive statistics such as mean and standard deviation. A p-value of 0.05 or lower is considered statistically significant. Results: A total of 246 patients with heart failure were included in this study. The mean age in our study was 56.78±2.81 years. Whereas, mean BNP and ejection fraction in our study was 1144±87.81 pg./ml and 34.65±4.14 %. 136 (55.3%) were male and 110 (44.7%) were female. Out of 246 patients with heart failure, 52 (21.1%) had atrial fibrillation and 194 (78.9%) did not have atrial fibrillation. Conclusion: The results of this research demonstrated that a large percentage of heart failure patients had atrial fibrillation. Thus, it is essential to focus on a comprehensive way of management of heart failure and its comorbidities rather than primarily treating the cardiac symptoms. Introduction. Heart failure is a health problem in Indonesia. The 2013 Basic Health Research Data showed that the estimated heart failure in Indonesia was 530,068 people. Echocardiography examination which has been a routine examination of heart failure patients is not necessarily available in all hospitals, so that a more applicable and inexpensive alternative examination is needed. Previous studies have shown an increase in C3c levels associated with improved survival and better cardiac remodeling. On this basis, this research needs to be performed in order to determine the correlation between C3c complement levels, NT-proBNP and LVEF in heart failure patients. Method. A cross-sectional study was conducted at the Dr Soetomo Hospital between August 2018 to September 2018 with 30 samples. Samples were taken consecutively from patients with heart failure who were treated at the integrated heart service center. Examination of C3c complement, NT-proBNP and echocardiography (LVEF data) was carried out in all patients. Result. The result of this study showed no significant correlation between C3c complement and NT-proBNP (r = -0.253, p = 0.177). The correlation between C3c complement and LVEF was also not significant (r = -0.074, p = 0.696). A significant moderate correlation between NT-proBNP and LVEF was found (r = -0.444, p = 0.014). Conclusion. The C3c complement could not be used as an alternative examination for NT-proBNP and LVEF. The limitations of the study were heterogeneous sample characteristics. A further study with more stringent criteria is needed to minimize the bias of examination results.

Heart failure occurs with symptoms associated with an impaired myocardial ventricular function other than that heart failure manifests as vascular congestion in the pulmonary and systemic circulation resulting in symptoms of insufficient circulation. The main treatment in patients with heart failure with sufficient oxygenation needs. Objective: to find out how to optimize the oxygen needs of heart failure patients. Method: This study uses a literature review method that focuses on fulfilling the oxygen needs of heart failure patients by searching Google Scholar, Elsevier, and Pubmed by using keywords heart failure, oxygenation, respiratory, and heart rate in the period 2015 - 2020. Results: the search found more than 500 case reports according to keywords and then filtered, obtained 5 case report articles. The journal describes the case of heart failure patients and how to cope with oxygen demand in heart failure patients who often experience shortness of breath or hypoxemia. Conclusion: oxygenation therapy for heart failure patients can be done by using ventilation aids and adjusting the patient's position according to the patient's condition and complications
