Maxillary labial and buccal frenum are considered as normal anatomic structures in the oral cavity. However, they may exist intraorally as a thick broad fibrous attachment and/or become located near the crest of the residual ridge, thus interfering with proper denture border extension resulting in inferior denture stability, retention and overall patient satisfaction. This case report highlights the importance of clinical examination and treatment planning which may mandate pre prosthetic surgery prior to fabrication of a new conventional partial denture. Adequate patient satisfaction with conventional partial dentures can be significantly increased after frenectomy.

Dentists are disappointed after delivery of cast removable partial dentures because their patient refuses or is unable to wear the denture and the treatment is therefore deemed unsuccessful. When so many patients do not comply with treatment, it is instructive to reflect on why and how the treatment is performed. Appropriate, comprehensive treatment planning should precede and eliciting the patient's chief complaint, as well as his or her expectations of treatment before treatment planning. The case presentation provides for the practitioner to discuss the patient's expectations and to outline both favourable and unfavourable short and long term outcomes. By providing this information, the practitioner ensures that the patient is fully informed before giving consent and that he or she understands the associated benefits and risks. Leukoplakia is a potentially malignant disorder that clinically presents as white lesion that cannot be scrapped off. It is associated with the habit of tobacco use, however, other factors have also to shown increase the risk of occurrence. Fordyce granules are not a disease per se but does appear as yellow spots in the oral cavity which can be easily identified clinically. The present article reports an interesting case of a patient with Leukoplakia and Fordyce granules underlying the white patch which was muffled in clinical examination and evident in histological diagnosis.

Introduction: The Invisalign system has been used in the orthodontic treatment of numerous types of malocclusions. However some clinicians have reported difficulty incorrecting rotations particularly in canines and premolars using the Invisalign appliance system. This case report describes the orthodontic treatment of an adult patient with severe canine rotation (more than 45°) and crowding on upper and lower tooth.Methods: The clinical examination included extra-oral and intra-oral photographs, panoramic radiographic, lateral cephalometric, stone casts, and upper and lower arch analysis obtained from ClinCheck 3.0.The treatment planning was resolved the crowding in both upper and lower arches and the severe rotation of 33 tooth (46°) using Invisalign system as well as the canine and molar relationship, dental verticalization, adequate over jet, overbite and dental midline using the same system. The duration of the treatment was approximately eight months.Results: In Post-treatment extra oralphotographs, no significant changes were observed at the end of the treatment. Intraoral photographs showed an important and notable improved aesthetics. The canine relationship improved slightly and molar Class Irelationship was maintained. An increase in transverse diameter was observed at the level of first premolars, second premolars and first molars. The overbite was improved. The crowding and the severe canine rotation were corrected. No obvious root resorption was radiographically evident and slight cephalometric changes.Conclusion: The treatment with the Invisalign system resolved the complaint of our patient so this system could be considered as an option that offered good results in the treatment of upper and lower crowding of arches involving severe canine rotation.

Abstract Implant-supported rehabilitations show higher patient satisfaction when compared with conventional prostheses. Systemic conditions, however, can affect treatment planning of dental implant supported rehabilitations. The oral structures that are commonly affected in patients with Down syndrome include the tongue (macroglossia), teeth (number and shape), and poor quality alveolar and jaw bones (osteoporotic-like). This clinical case presents an interdisciplinary team approach aimed at achieving functional, phonetic, and esthetic outcomes in a patient with Down syndrome.

Nowadays one of the prime concern in dentistry is on preservation of remaining natural teeth. Many patients require replacement of missing teeth and associated structures to enhance appearance, improve masticatory efficiency, prevent unwanted movement of teeth (overeruption/ drifting), and/ or improve phonetics. Presence of few teeth in oral cavity helps in preserving alveolar ridge integrity, maintain the proprioception, and gives psychological benefit to the patient. This case report presents a case of a 54-years-old female with few remaining teeth, one remaining tooth in maxilla and two remaining teeth in mandible. Patient wants to get her masticatory function and aestethic back with maintain her remaining teeth. Cu-sil denture is one of the simplest removable partial denture that can replace missing teeth where only few teeth remain in oral cavity. A Cu-sil denture is a denture with holes, lined by a gasket, the holes that providing space for remaining natural teeth to emerge into the oral cavity through the denture.

Even though dental attachments have a long successful history, many of the dental practioners are unaware about its benefits in daily clinical practice. It highly improves the comfort, aesthetic, function and patient satisfaction, especially in the patients with long span edentulism where implants and fixed partial denture (FPD) are not indicated and cast partial dentures are barely satisfactory. Use of attachment in providing retention to removable prosthesis is an old treatment modality with better success. It highly improves the comfort, aesthetic, function, and patient satisfaction, especially in the patients with long span edentulism where implants and fixed partial denture (FPD) are not indicated and cast partial dentures are barely satisfactory. Proper diagnosis and treatment planning is necessary for selection of appropriate attachment type. Successful restoration can be done with plenty of contemporary and conventional treatment planning. Attachment retained partial dentures are one such kind of treatment modality in prosthodontics. Studies have also demonstrated that precision-attachment partials last longer, wear less, need fewer adjustments, look better, work better, less destructive, protect abutment teeth, and are easier to clean. In this case report patients functional and aesthetic expectations were met by using a combination of Porcelain fused to metal (PFM) restorations and semi-precision attachments.

Aim: The aim of this report is to describe the management of a prosthodontic patient expressing unrealistic expectations with respect to the transition to edentulousness. Objectives: To outline (1) the diagnosis and explicit expectations of the patient on presentation (2) considerations made during treatment planning to address the wishes of the first time prosthodontic patient (3) a sequential treatment plan utilizing transitional partial removable dentures to manage the change to edentulousness (4) functional and aesthetic result achieved. Results: Delivery of immediate removable partial dentures retaining key abutment teeth in upper and lower arches was a viable prosthodontic solution in the transition to edentulousness of a patient expecting unrealistic treatment outcomes. Conclusions: Addressing impractical expectations and devising a treatment plan amenable to both clinician and patient is difficult. Strategies to manage these wishes in prosthodontic dentistry can include transitional partial dentures. Clinical relevance: Practitioners who encounter similar situations may consider this report valuable.

Obturator prostheses are considered to be the preferred choice for the restoration of maxillary defects. Patients with palatal insufficiency, speech and deglutition becomes impaired, thereby requiring prosthodontic rehabilitation. The prosthodontic rehabilitation of patients with acquired defects of the maxilla after surgical resection is the complete responsibility of a maxillofacial prosthodontist. He has to recreate an artificial barrier between the cavities and thus restore the functional capabilities of speech, mastication and swallowing. Palatal obturator is the only substitute which covers the defect and contributes to normal speech production.
