This study aims to determine the effect of discipline on the performance of employees of the Community Health Center (Puskesmas) of Raya Pematangsiantar City. This research used the design of literature and field research. The research method used was the causality method with a quantitative approach. The type of data used in this study was qualitative and quantitative data. Data sources consisted of primary and secondary data. The population and sample used in this study were 34 permanent employees. Data collection was done by questionnaire, interview, and documentation. The analysis technique used was qualitative descriptive and quantitative descriptive analysis consisting of multiple linear regression, correlation test, the test of determination and hypothesis. The results of this study concluded that employee discipline and performance was good. Regarding the results of the hypothesis, it shows that discipline has a positive and significant effect on employee performance.

The objective of this study was to determine the effect of organizational culture and commitment to employee performance at the Education Office of Pematangsiantar City. This study used literature and field research designs. The research method used was the causality method with a quantitative approach. The type of data used in this study was qualitative data and quantitative data. The data sources consisted of primary data and secondary data. The population and sample used in this study were 81 permanent employees. The data collection was carried out by questionnaire, interview and documentation. The analysis technique used was qualitative descriptive and quantitative descriptive analysis consisting of multiple linear regression, correlation test and determination and hypothesis tests. The results of this study concluded that the organizational culture, commitment and employee performance were good. Besides, the organizational culture had a positive and significant effect on employee performance. Furthermore, the commitment had a positive and significant effect on employee performance.

This research aims to analyze and determine 1) The application of income tax article 21 on business subject to final income tax at CV RPT, 2) The potential for underpayment of article 21 income tax after calculating according to applicable tax regulations. CV RPT is a company engaged in the construction service business. For taxpayers who are subject to final income tax, benefit in kind given to employees is the object of income tax article 21. The data used in this research are secondary data obtained through observation and documentation. This research uses a quantitative descriptive analysis. Based on the results of the research, there is a potential for underpayment of income tax article 21 on CV RPT, amounting to Rp9.212.540 for permanent employees and Rp2.750.000 for final income tax article 21 on pension benefits. Besides, it also resulted in the potential for the imposition of administrative sanctions in the form of interest due to the still unfulfilled income tax article 21 obligation, namely Rp1.289.756 for underpayment of article 21 income tax for permanent employees and Rp385.000 for final income tax article 21 on pension benefit.

AbstrakMasalah pelayanan sebenarnya bukanlah hal yang sulit atau rumit, tetapi apabila hal ini kurang diatasi maka dapat menimbulkan masalah serta peningkatan kinerja pegawai yang bagus merupakan kunci sukses untuk membangun keberhasilan Puskesmas dalam melayani pasien. Untuk meningkatkan kualitas pelayanan dan kinerja pegawai, Puskesmas Dinoyo Kota Malang akan mampu mendapatkan profitabilitas yang diperoleh dari kepuasan pasien oleh sebab diharapkan mampu meningkatkan kualitasnya puskesmas. Tujuan dari penelitian ini adalah: untuk menganalisis pengaruh pelayanan Puskesmas dan kinerja pegawai terhadap kepuasan pelanggan di Puskesmas Dinoyo Kota Malang Jawa Timur. Sampel yang digunakan dalam penelitian ini adalah pasien atau pelanggan dari Puskesmas Dinoyo selama satu minggu pada bulan Nopember tahun 2017. Hasil penelitian menunjukkan bahwa pengaruh pengaruh Puskesmas dan kinerja pegawai terhadap kepuasan pelanggan di Puskesmas Dinoyo Kota Malang  Jawa Timur  negatif dan tidak signifikan. Dalam penelitian ini juga mengindikasikan bahwa secara simultan pelayanan Puskesmas dan kinerja pegawai memiliki pengaruh terhadap kepuasan pelanggan Puskesmas.Kata kunci : pelayanan puskesmas, kinerja pegawai, kepuasan pelanggan Abstract            The problem of service is not difficult and complicated, but if this matter is not be overcame so it can make some problems and a good employee performance is a key of success  to build the success of Community Health Center on servicing the patient. To develop the quality of service and employee performance, Community Health Center Dinoyo Malang City will be able to get the profitability that obtained from the satisfaction customer, so for that reason it expected to develop the quality of Community Health Center.The objectives of this research is: to analyze the effect of the service of Community Health Center and the Employee Performance to Customer Satisfaction in Community Health Center Dinoyo Malang City East Java. Using of sample from this study is the patient of Community Health Center Dinoyo for a week on November at 2017. The result of this research shows that the service of Community Health Service and Employee Performance to Customer Satisfaction in Community Health Center in Dinoyo Malang City East Java is negative and not significant. In this research also indicates that the service of Community Health Center and Employee Performance have the effect to the satisfaction customer of Community Health Center simultaneously.Keyword : service of community health center, employee performance, satisfaction performance

ENGLISHLow Birth Weight is one of the health problems during neonatal period. The purpose of this study is to describe Low Birth Weight (LBW) incidences in Pati Regency, include time, sex, place, and contributing for neonatal mortalities. This research used descriptive and quantitative approaches. Data sources derived from secondary data, i.e. Low Birth Weight records in Pati Regency 2011-2014. Data were analyzed descriptively. Results of study are the percentage of Low Birth Weight (LBW) incidence in Pati Regency in 2014 was 3.19% and it tended to increase from the previous year. Based on infant’s sex, there was slight different percentage between male infant and female infant (about 0.09% to 0.17%). The highest percentage of LBW in 2014 was found in Community Health Center (Puskesmas) of Winong 2, i.e. 6.69%. Period 2011-2014, LBW was the prior determinant of neonatal mortality in every year with the percentage ranged 32-45%. Threathment and prevention for LBW should become priority to decrease neonatal mortality. INDONESIABerat Badan Lahir Rendah (BBLR) merupakan salah satu masalah kesehatan pada masa neonatal. Tujuan penelitian untuk menggambarkan kejadian BBLR di Kabupaten Pati berdasarkan waktu, jenis kelamin, tempat dan kontribusinya terhadap kematian neonatal. Penelitian ini menggunakan pendekatan deskriptif kuantitatif. Sumber data berasal dari data sekunder berupa data BBLR di Kabupaten Pati tahun 2011-2014 kemudian dianalisis secara deskriptif. Hasil penelitian menunjukkan angka kejadian BBLR di Kabupaten Pati tahun 2014 sebesar 3,19% dan ada kecenderungan meningkat dari tahun sebelumnya. Persentase BBLR pada bayi laki-laki berbeda dengan persentase BBLR pada bayi perempuan, meskipun perbedaannya tidak terlalu jauh yaitu sekitar 0,09% sampai 0,17%. Angka kejadian BBLR tertinggi tahun 2014 terjadi di Puskesmas Winong 2 yaitu sebanyak 6,69%. BBLR menjadi penyebab kematian neonatal terbesar yaitu sekitar 32-45% setiap tahun. Penanganan dan pencegahan BBLR menjadi prioritas dalam rangka menurunkan angka kematian neonatal.

Based on WHO, diarrhea kills about 2 million children in the world each year. Referring to Palembang health service in 2017, the number of diarrhea suspects reached 48.000 people. Meanwhile, diarrhea cases tend to increase in the working areas of a community health center 4 Ulu Palembang every year. In 2016, there were about 1.341 children suspected of diarrhea and increased to 1.361 in 2017. This research aims to know about mothers' behavior using the descriptive method. Variables include the mother’s knowledge, attitudes, and actions in dealing with children under five years with diarrhea. This thesis will be developed based on primary and secondary data. Data will be analyzed with univariate statistics. The analysis shows that 62% of respondents have good knowledge, 58% have a good attitude, and 64% have done proper action in caring for children under five years with diarrhea in community health center 4 Ulu working area 2018. Based on the result, there are many mothers who didn’t have good knowledge and attitude in care of children under five years with diarrhea in working area community health center. It’s suggested that intensive promotion is necessarily given which integrating society participation. To learn more factors influencing a mother’s behavior in caring for children under five years with diarrhea needs further research.                                               Keywords: Diarrhea, TheChildren Under The Age of Five

The purpose of this study was to analyze motivation, organizational culture and performance. This research is conducted in Community and Village Empowerment Office, it used sample of 33 ASN consist of Office Secretary, Head of Section, Head of Subdivision, Section Head and Staff, data is collected using questioners. This research is to analyze motivational variable data, organizational culture and performance using quantitative descriptive analysis. Data is divided into primer data and secondary data. The scoring of this research uses a Likert scale. To describe the figure of each variable studied, the data is tabulated and uses a statistical descriptive analysis by calculating the mean (mean). The analysis result present that variable motivation of State Civil Apparatus in the Office of Community Empowerment and Village of Sleman Regency generally get a good category with mean 3,79. While organizational culture variables generally also get a good category with mean 3,83. The ASN Performance variable in the Community and Village Empowerment Service, although it is not yet maximal, it also has a good category with mean 3, 49. Even though the performance variable obtains the lowest mean compared to the motivation variable and organizational culture variable, it remains in the good category. Abstract. This paper analyzed the Policy Implementation of Community Health Center (CHC) as Report Obligation Recipient Institution for Narcotic Addicts in Jakarta Province in 2014. The design of the study employed a mixed methods approach of Sequential Explanatory. The implementation analysis employed the model developed by Donald Van Meter and Carl Van Horn. Within the model, policy implementation was associated with performance. The relationship was influenced by six variables: standards and targets, resources, communication among implementing agencies, implementing agency characteristics, attitudes of the implementers, as well as social, economic, and political factors. We found that variations of those six variables influenced policy performance linearly.Keywords: descriptive analysis, a variation of variables, the performance of the policy

The purpose of this study is: to determine the effect of consumer perceptions on purchasing decisions at Perum Bulog Sub Divre Pematangsiantar. This study uses a library research design and field research. This type of data uses qualitative and quantitative data. Data sources are primary data and secondary data. Data collection was carried out by means of questionnaires, interviews, and documentation. Test the research instrument using the validity test and reliability test. The data analysis technique used is normality test, qualitative descriptive analysis, and quantitative descriptive analysis. The results of this study can be concluded as follows: Consumer perceptions have a positive and significant effect on purchasing decisions either simultaneously or partially. There is a strong and positive relationship between consumer perceptions of purchasing decisions. The level of purchase decisions can be explained by consumer perceptions. Consumer perceptions have a positive and significant effect on purchasing decisions at Perum Blog Sub Divre Pematangsiantar

The purpose of this research is to know the implementation of e-Puskesmas service at Ambacang Community Health Center of Kuranji Subdistrict, Padang City, to know the supporting and inhibiting factors in the implementation of e-Puskesmas service at Ambacang Community Health Center. This research is a qualitative research with descriptive method. This research was conducted at Puskesmas Ambacang Kecamatan Kuranji Kota Padang. The research by purposive sampling. The data collected are secondary data and primary data through interview, observation and documentation study. Test data validity is done with trianggulasi then data analyzed by reducing data, display data the results of this study indicate that the implementation of e-Puskesmas service has not been implemented properly so that the implementation of the service becomes not effective. Supporting factors were found to be sufficient budget in applying e-Puskesmas, a change of paradigm of staff at Ambacang Community Health Center. Inhibiting factors are lack of adequate infrastructure availability, internet connectivity level is not appropriate, lack of socialization about e-Puskesmas service.
