In order to provide a novel analytically traceable free space optics (FSO) channel model for describing turbulence based irradiance fluctuations, following basic scintillation theory principles, we have derived closed-form expression for probability density function (PDF) of a new statistical Gamma–Chi-square model. Further, capitalizing on provided model, error performances of FSO system over on–off keying (OOK) transmission scheme both in the presence of atmospheric turbulence and misalignment fading (pointing error) is investigated. For both cases, the average bit error rate (ABER) at the receiving side of the system is determined in an analytically closed form. The results are graphically presented in order to analyze the impact of different levels of turbulence, as well as other relevant parameters, on the quality of the received signal in the OOK modulated FSO system.

In this paper we will propose new analytically traceable probability density function (PDF) model for free space optics (FSO) turbulence, obtained as a generalization of double Ricean turbulence model, that encompasses both large-scale and small-scale turbulence eddy effects along by taking into account performance decreasing influence of misalignment introduced through boresight pointing error model. Consequently, after delivering the closed-form expressions for the newly introduced double FSO model, we obtain the analytical expressions for the bit error rate (BER) performance for the Double Rician distribution affected by misalignment. Numerical results will show the impact of system parameters on FSO link performance and we will provide full performance analysis. © 2021.

Abstract Introduction Poor handover between shifts can result in patient harm. This study was designed to evaluate the impact of implementing a handover protocol on the quality of information exchanged in the trauma handover meetings in a UK hospital. Method A prospective single-centre observational study was performed at an NHS Trust. Ten consecutive weekday trauma meetings, involving 43 patients, were observed to identify poor practices in handover. This data was used in conjunction with the Royal College of Surgeons’ recommendations for effective handover (2007) to create and implement a standard operating protocol (SOP). Following its implementation, a further 8 consecutive meetings, involving a further 47 patients, were observed. The data was analysed using t-test for quantitative variables and chi-square or Fisher’s exact tests for categorical variables. Results An improvement was demonstrated in multiple aspects of trauma handover including past medical history, injury date, results, diagnosis, consent, mark, and starvation status (all p < 0.001). Subgroup analyses showed that handover of neck-of-femur fracture patients including information on baseline mobility (p = 0.04), Nottingham-Hip-Fracture Score (p = 0.01), next-of-kin discussion (p = 0.075) and resuscitation status (p = 0.001) all improved following the intervention. Conclusions These results demonstrate that the implementation of a well-structured handover protocol can improve the transmission of critical information in trauma meetings.

ABSTRACTObjective: To study the impact of patient counseling on medication adherence and quality of life (QOL) in epileptic patients and to assess the factorsaffecting medication adherence.Methods: This study is a prospective observational study involving 100 patients with an age limit of 8-60 years and those taking Antiepilepticdrugs for at least 3 months. The study population received patient counseling during their first visit. The impact of patient counseling on QOL andmedication adherence was assessed using self-reported questionnaire QOLIE-31 and MMAS-8 between the first visit and the second visit. Statisticalanalysis (Paired t-test and Paired Chi-square test) was performed to analyze the impact of patient counseling on QOL and medication adherence inepileptic patients.Results: A total of 100 patients were included in the study. After providing patient counseling, it was observed that there was a statistically significant(p<0.05) improvement in all domains of QOLIE-31 and MMAS-8 scores. Before counseling, mean overall T-score of QOLIE-31 was 44.08±2.07whichwas changed to 49.14±1.27 after patient counseling with a mean change of 5.06 in overall T-score. In the case of medication adherence, beforecounseling 77% subjects were nonadherent to therapy, after counseling it was reduced to 41%. The common reasons for medication adherence wereforgetfulness, unawareness, therapy related, and economics related. Out of which forgetfulness along with unawareness was the major one.Conclusion: The study described that patient counseling plays a major role in improving QOL and medication adherence.Keywords: QOLIE-31, MMAS-8, Epilepsy, Antiepileptic drugs, Patient counseling.

Objectives: To determine the association between types of malocclusion and quality of life in children between 8-10 years of age and establish correlations between the severity of the malocclusion and particular bio-psychosocial variables. Study design: The sample was made up of 102 schoolchildren aged 8-10 years. Clinical exams were performed using the criteria of the Dental Aesthetic Index (DAI) to determine the presence and severity of malocclusions. The impact on quality of life was assessed using the Child Perceptions Questionnaire (CPQ8-10). Statistical analysis involved the chi-square test, Fisher's exact test and Spearman's correlation analysis. Results: Malocclusions affected 61% of the children examined. There was a positive correlation between total CPQ8-10 and DAI scores (P = 0.034). The following types of malocclusion had a significant effect on the quality of life of the children: upper anterior irregularity ≥ 2 mm, anterior open bite ≥ 2mm and diastema ≥ 2mm. Children with malocclusion experienced a greater negative impact on quality of life in comparison to those without malocclusion. Conclusions: Malocclusions had a negative influence over the quality of life of children between 8-10 years of age. More severe malocclusions had a greater impact with regard to social, emotional and functional aspects.

The outage performance with best relay selection is proposed for cognitive relay networks with multiple primary users in independent nonidentical distributed Nakagami-m fading channels. Specifically, we take the interference temperature and fading severity into consideration. Exact closed-form expression of outage probability is derived. Based on the exact closed-form expression, we can evaluate the impact of interference temperature, fading severity, number of relays, and number of primary users on the secondary network. Finally, the effects of fading severity, number of relays, number of primary users, and interference temperature on the system performance are examined through some representative numerical plots, and the Monte Carlo results match perfectly with theory results which validates our theory analysis.

The study examined the impact of the New Public Management Paradigm on the operation of Federal establishments in Borno State, Nigeria. To achieve this objective, the collected data were analysed using Chi-square, Correlation and ANOVA. The study revealed that there is a significant improvement in the performance of the staff and the quality of service delivery in the Federal establishments under study. The study concluded that the outsourcing of services has improved the quality of service delivery. However, the policy was not being properly implemented because in some of the Federal establishments under study, the same services outsourced were being carried out by very few retained staff. They were not enough to do the job and the outsource firms given the contract, did not provide enough qualified staff to augment the short-fall. The study recommended that qualified service providers (outsourcing firms) in the relevant cadres be allowed to do the job or as an alternative, the Federal government can encourage the setting up of Independent Units in all its establishments to compete with the outsourcing firms in carrying out outsourcing services at a fee, in order to attain qualitaty service delivery.   Keywords: New Public Management, Public service, Outsourcing and Service delivery.


This study obtains a closed-form solution for the discrete-time global quadratic hedging problem of Schweizer (1995) applied to vanilla European options under the geometric Gaussian random walk model for the underlying asset. This extends the work of Rémillard and Rubenthaler (2013), who obtained closed-form formulas for some components of the hedging problem solution. Coefficients embedded in the closed-form expression can be computed either directly or through a recursive algorithm. The author also presents a brief sensitivity analysis to determine the impact of the underlying asset drift and the hedging portfolio rebalancing frequency on the optimal hedging capital and the initial hedge ratio.

Purpose. To evaluate the impact of a nudge program on food pantry clients’ self-reported selection and use of healthy foods. Method. A convenience sample of clients of six urban food pantries in Utah were surveyed about their experience with the Thumbs Up for Healthy Choices nudge program. Chi-square tests were used to identify associations between demographic characteristics and self-reported program impact. Results. Ninety-four percent (n = 158) of respondents agreed that the program made it easier to make healthy choices. Sixty-five percent reported healthier diets since its implementation. Additionally, Hispanic respondents were more likely to report positive impacts than non-Hispanic respondents. Conclusions and Implications. Nudge programs are effective in increasing the selection of healthy foods among pantry clients in Utah. Impacts seemed to be particularly positive for Hispanic pantry users in Utah. Nutrition programs should consider implementing these low-cost strategies to improve dietary quality of pantry users.

AbstractIn this work, the integration of free-space optics (FSO) technology for implementing long-haul high-altitude platform (HAP)-to-ground transmission system is proposed. Mode division multiplexing (MDM) and orthogonal frequency division multiplexing (OFDM) are incorporated for carrying high-speed signals without interference. The impact of increasing input power, divergence angle, antenna diameter, and attenuation on the system performance is further investigated. In addition, the performance of the MDM-OFDM-HAP-to-ground FSO transmission system is improved by using a square root module at the receiver for link reach enhancement. The results exhibit a reliable transportation of 40 Gbps information over 20 km FSO distance with good quality of the received signal.

AbstrakPeningkatkan mutu asuhan keperawatan di rumah sakit memerlukan restructuring, reengineering, dan redesigning sistem pemberian asuhan keperawatan melalui implementasi Model Praktik Keperawatan Profesional (MPKP). Tujuan penelitian ini ialah untuk menilai dampak implementasi MPKP terhadap mutu asuhan keperawatan di rumah sakit. Disain penelitian adalah kuasi eksperimen dalam bentuk pre and post test with control group. Uji statistik yang digunakan adalah uji Chi-square dan uji t. Hasil penelitian menunjukkan bahwa pada kelompok intervensi (MPKP) terdapat peningkatan kepuasan klien dan keluarga (OR=114,24) yang lebih besar dibandingkan dengan peningkatan kepuasan klien dan keluarga pada kelompok kontrol (OR=3,78). Demikian juga terdapat peningkatan kepatuhan perawat terhadap standar yang lebih tinggi pada kelompok intervensi (OR=235,5) dibandingkan dengan kelompok kontrol (OR=0). Lama hari rawat lebih pendek pada kelompok intervensi. Angka infeksi nosokomial juga lebih rendah pada kelompok intervensi dibandingkan dengan kelompok kontrol. Penelitian ini menyimpulkan bahwa implementasi MPKP dapat meningkatkan mutu asuhan keperawatan di rumah sakit. Berdasarkan hasil tersebut, untuk memicu implementasi MPKP di berbagai rumah sakit, disarankan agar implementasi MPKP menjadi salah satu kriteria penilaian pada akreditasi rumah sakit khususnya pelayanan keperawatan AbstractIn order to improve the quality of nursing care in the hospital, nursing care delivery system need restructuring, reengineering, and redesigning through the implementation of Professional Nursing Practice Model (PNPM). The study was aimed to evaluate the impact of the PNPM on the quality of nursing care in the hospital. The design used was quasi experiment in pre and post test with control group. The statistic used are Chi-square and t-test. The result showed that in the intervention group, the improvement of client/family satisfaction with nursing care (OR = 114,28) was higher than the improvement in the control group (OR = 3,78). It was founded there was an improvement of the compliance of nurses to the standard was higher in the intervention group (OR = 235,5) compare to control group (OR = 0). The length of stay shorter in the intervention group, and also the nosocomial infection rate is lower in the intervention group compare to control group. The study concluded that the implementation of the PNPM could improve the quality of nursing care in the hospital. Based on this result, in order to challenge the implementation of PNPM in the hospital it was suggested that the implementation of PNPM becomes an evaluation criteria for hospital accreditation specifically for nursing service.
