AbstractThis commentary is intended to take China as a case to discuss the mission of the family planning program under low fertility scenario. After a brief review of the initiation of family planning program in the 1970s, as well as the reorientation of family planning program since ICPD in 1994, it will focus on the new mission for the family planning program under low fertility scenario in the twenty-first century, in particular concerning the issue of induced abortion among the others. Given the enormous evidence of unmet needs in reproductive health as identified in the discussion, it is argued that family planning programmes are in fact even more needed than ever before under low-fertility scenario, and should not be abandoned but strengthened, which clearly has nothing to do to call back to the program for population control in the 1970s–1980s, and nor even go back to the program for “two reorientations” in the 1990s, but to aim to serving the people to fulfill their reproductive health and reproductive rights in light of ICPD and SDGs, and to become truly integral component of “Healthy China 2030” Strategy.

This study is intended to pursue the previous attempts in examining the relationship between low fertility regime and the case of births which had been delivered unintendedly in Yogyakarta. Using an unweighted sample of 575 married women in the Indonesian Demographic and Health Survey 1991, this study found that 75 cases out of 474 last birth children were bom unintendedly (beyond the range of ideal number of children). Most of the 75 cases were bom by women who have 2 or 3 ideal number of children and by a devoted Family Planning acceptors. Most of the women have limited accesses in education and economic activities. The case of unwantednes were found predominantly among women older than 25 years. Even though only a tentative findings, this study shed some light to the fact that Family Planning program to some extent may contribute to the rate of unwantedness. Apolicy is needed especially in reducing the risk of unwantedness among the low-income women.

The countries of the Indian subcontinent provide some of the world's most evident case studies of the deleterious effects of population pressure. They also have undertaken some of the world's most massive family planning programs. India was the first country to declare a national population policy and to mount a nationwide family planning program. Pakistan and present day Bangladesh had a prolonged period during which the national family planning program had a separate organization with extremely high priority and official support. Continuing famines and two major wars in 25 years have contributed to high mortality. Nonetheless, population growth in these countries continues its inexorable upward curve. On the other hand, these programs must also be credited with some real successes and the birth rates in several Indian states are falling.

One of the indicators determine the wellness of Family Planning Program is the high number of participants enrolling, or commonly known as the Contraceptive Prevalence Rate (CPR) and the number of children born known as the Total Fertility Rate (TFR) of a maximum of 2 children. There are several regions in East Java where the situation of TFR and CPR does not show the ideal conditions. This shows that there are problems in the current family planning program. This research was conducted to map districts and cities in East Java based on TFR and CPR data respectively in quadrant form. This type of research is on-reactive research which utilizes secondary data from Indonesia - National Socio-Economic Survey in 2015. The research method used is recapitulating data and classifying districts and cities in the form of quadrants by comparing to data on achievement of TFR and CPR in East Java using SPSS. The results of the study have mapped districts and cities in East Java which are described in quadrant form. The regions that need major attention from the government are regions in quadrants I and II. Quadrant III is included in an abnormal condition and quadrant IV is a quadrant that has an area where TFR and CPR conditions are ideal. The conclusions of this study are the areas in quadrant I, namely: Sampang (district), Blitar (city), Blitar (district), Tulungagung (district), Ponorogo (district), Madiun (city), and Pasuruan (city) are the priority in getting the FP program improvements. The advice that can be given is to evaluate the ongoing family planning program to find out the factors that cause the TFR and CPR conditions are not in line with government expectations.

With regional autonomy in Indonesia, central government policy is not necessarily carried out by the local government direct becaause become priority not necessarily in daerah.so role of community leaders and NGOs to review very improve society participation hearts Family Planning Program. In addition is the volunteers have the role of highly positioned mainly to review that-majority region is the rural social and economic on underclass.

The family planning program also aims to improve the quality of the family in order to generate a sense of security, peace and hope of a better future in realizing the prosperity of birth and inner happiness. Allegedly the factor causing EFA participation in the family planning program is characteristic. The purpose of this research is to know the relationship of attitude and characteristic of Elderly Age Couple (PUS) with participation in family planning program at UPT Puskesmas Sungai Raya Sungai Raya District, East Aceh regency 2018. The research design used was analytic survey with cross sectional design. The population of this study is all Pairs Age of Fertile located in Work Area UPT Sungai Raya Public Health Service Center in January to December 2017 which amounted to 1897 people. Sampling using Slovin formula, obtained as many as 95 samples. The study was conducted from 7-17 July 2018 using questionnaires by interview. Statistical test using chi-square test. Result of research indicate that majority of fertile couple couples (PUS) participate in family planning program as much as 67 respondents (70,5%). Statistically there is relationship of attitude and characteristic of Elderly Age Couple (EFA) with non participation in family planning program in Working Area of UPT Puskesmas Sungai Raya Sungai Raya District of East Aceh Regency 2018 with p value <0,1. It is recommended that the family planning program holders in UPT Puskesmas Sungai Raya Sungai Raya District of East Aceh District to invite cross-sectoral figures to hold meetings to create mini workshop plans at least once a month to increase the participation of the Elderly Age Couple (PUS) in family planning programs. Tujuan penelitian ini untuk mengetahui hubungan sikap dan karakteristik Pasangan Usia Subur (PUS) dengan keikutsertaan dalam program KB di Wilayah Kerja UPT Puskesmas Sungai Raya Kecamatan Sungai Raya Kabupaten Aceh Timur tahun 2018. Desain penelitian yang digunakan adalah survei analitik dengan rancangan bedah lintang. Populasi dari penelitian ini adalah seluruh Pasangan Usia Subur yang berada di Wilayah Kerja UPT Puskesmas Sungai Raya pada bulan Januari sampai dengan Desember tahun 2017 yang berjumlah 1.897 orang. Pengambilan sampel menggunakan rumus Slovin, didapatkan sebanyak 95 sampel. Penelitian dilaksanakan dari tanggal 7-17 Juli tahun 2018 menggunakan kuesioner dengan cara wawancara. Uji statistik menggunakan uji chi-square. Hasil penelitian menunjukkan bahwa mayoritas Pasangan Usia Subur (PUS) ikut serta dalam program KB yaitu sebanyak 67 responden (70,5%). Secara statistik ada hubungan sikap dan karakteristik Pasangan Usia Subur (PUS) dengan ketidakikutsertaan dalam program KB di Wilayah Kerja UPT Puskesmas Sungai Raya Kecamatan Sungai Raya Kabupaten Aceh Timur tahun 2018 dengan p value < 0,1. Sebaiknya pemegang program KB di UPT Puskesmas Sungai Raya Kecamatan Sungai Raya Kabupaten Aceh Timur agar mengajak tokoh lintas sektor agar mengadakan pertemuan untuk membuat rencana loka karya mini setidaknya satu bulan sekali untuk meningkatkan keikutsertaan Pasangan Usia Subur (PUS) dalam program KB.Kata Kunci : Program KB, Sikap, Karakteristik
