Evaluation of the Self-Fitting Process with a Commercially Available Hearing Aid

Background: Hearing aids and personal sound amplification products that are designed to be self-fitted by the user at home are becoming increasingly available in the online marketplace. While these devices are often marketed as a low-cost alternative to traditional hearing health-care, little is known about people’s ability to successfully use and manage them. Previous research into the individual components of a simulated self-fitting procedure has been undertaken, but no study has evaluated performance of the procedure as a whole using a commercial product. Purpose: To evaluate the ability of a group of adults with a hearing loss to set up a pair of commercially available self-fitting hearing aids for their own use and to investigate factors associated with a successful outcome. Research Design: An interventional study that used regression analysis to identify potential contributors to the outcome. Study Sample: Forty adults with mild to moderately severe hearing loss participated in the study: 20 current hearing aid users (the “experienced” group) and 20 with no previous amplification experience (the “new” group). Twenty-four participants attended with partners, who were present to offer assistance with the study task as needed. Data Collection and Analysis: Participants followed a set of written, illustrated instructions to perform a multistep self-fitting procedure with a commercially available self-fitting hearing aid, with optional assistance from a lay partner. Standardized measures of cognitive function, health literacy, locus of control, hearing aid self-efficacy, and manual dexterity were collected. Statistical analysis was performed to examine the proportion of participants in each group who successfully performed the self-fitting procedure, factors that predicted successful completion of the task, and the contributions of partners to the outcome. Results: Fifty-five percent of participants were able to successfully perform the self-fitting procedure. Although the same success rate was observed for both experienced and new participants, the majority of the errors relating to the hearing test and the fine-tuning tasks were made by the experienced participants, while all of the errors associated with physically customizing the hearing aids and most of the insertion errors were made by the new participants. Although the majority of partners assisted in the self-fitting task, their contributions did not significantly influence the outcome. Further, no characteristic or combination of characteristics reliably predicted which participants would be successful at the self-fitting task. Conclusions: Although the majority of participants were able to complete the self-fitting task without error, the provision of knowledgeable support by trained personnel, rather than a fellow layperson, would most certainly increase the proportion of users who are able to achieve success. Refinements to the instructions and the physical design of the hearing aid may also serve to improve the success rate. Further evaluation of the range of self-fitting hearing aids that are now on the market should be undertaken.

Ensuring consistent audibility is an important objective when fitting hearing aids to children. This article reviews the factors that could affect the audibility of the speech signals to children. These factors range from a precise determination of the child's hearing loss to an accurate specification of gain in the chosen hearing aids. In addition, hearing aid technology and features such as multichannel processing, directional microphones, and feedback cancellation that could affect the achievement of consistent audibility are reviewed.

Fitting hearing aids to infants with hearing loss can be completed in a systematic and evidence-based manner. This is because there are clinical protocols and technologies available so that pediatric audiologists can fit hearing aids to their patients with hearing loss during the early months of life. Early hearing aid fitting supports speech and language development, however, the way in which the hearing aid is fitted can significantly impact the infant’s progress. A four-stage process for infant hearing aid fitting is offered as a framework with which to execute the essential components of the procedure. Key aspects of the pediatric hearing aid fitting process are described with a focus on elements that may impact an infant’s progress with hearing aids if not implemented.

Hearing Aids have dominated the audiological market for decades. While the costs of the electronic components have reduced substantially, the cost of a hearing aid has risen steadily to the point that it has become unaffordable for the vast majority of the population with Age Related Hearing Loss (ARHL). Here, we present an ultra-low-cost, affordable and accessible hearing aid device (‘LoCHAid’), specifically targeted for ARHL in elderly patients. The LoCHAid costs only 98 cents (< $1) to mass manufacture and can be personalized for each user through a 3D-printable case. It is designed to be an over-the-counter (OTC) self-serviceable solution for elderly individuals with ARHL. Electroacoustic measurements show that the device meets most of the targets set out by the WHO Preferred Product Profile and Consumer Technology Association for Hearing Aids. The frequency response of the hearing aid shows selectable gain in the range of 4-8 kHz, and mild to moderate gain between 200-1000 Hz, and shows very limited total distortion (1%). Simulated gain measurements show that the LoCHAid is well fitted to a range of ARHL profiles for males and females between the ages of 60-79 years. Overall, the measurements show that the device has the potential to benefit individuals with ARHL. Thus, our proposed design addresses a long-standing and grand challenge of affordable and accessible hearing technology for every elderly person on this planet.

Self-adjustment of hearing aid gain can provide valuable information about the gain preferences of individual listeners, but these preferences are not well understood. Listeners with mild-to-moderate hearing loss used self-adjustment to select amplification gain and compression parameters in real time on a portable touch screen device while listening in quiet and noisy backgrounds. Adjustments to gain prescribed by the National Acoustics Laboratories' non-linear fitting procedure (NAL-NL2) showed large between-subject variability. Known listener characteristics (age, gender, hearing thresholds, hearing aid experience, acceptable noise level, and external ear characteristics) and listener engagement with the self-adjustment software were examined as potential predictors of this variability. Neither listener characteristics nor time spent adjusting gain were robust predictors of gain change from NAL-NL2. Listeners with less than 2 years of hearing aid experience and who also had better hearing thresholds tended to select less gain, relative to NAL-NL2, than experienced hearing aid users who had poorer thresholds. Listener factors explained no more than 10% of the between-subject variance in deviation from NAL-NL2, suggesting that modifying prescriptive fitting formulae based on the factors examined here would be unlikely to result in amplification parameters that are similar to user-customized settings. Self-adjustment typically took less than 3 min, indicating that listeners could use comparable technology without a substantial time commitment.

Purpose: To calculate the real-ear insertion gain (REIG) according to the types of hearing aids, we evaluated the preferred REIGs of Korean hearing-impaired listeners and psychoacoustic differences between open-canal fitting (OF) and closed-canal fitting (CF).Methods: The subjects with sensorineural hearing loss were divided into OF group (4 monaural fittings, 15 binaural fittings with 34 ears), and CF group (8 monaural fittings, 13 binaural fittings with 34 ears). There were no statistical differences in hearing threshold level (HTL) at each octave frequencies, word recognition score (WRS) and Korean Adaptation of the International Outcome Inventory for Hearing Aids (K-IOI-HA) between the two groups. When there was no need for further electroacoustic adjustment of the hearing aid, sharpness and occlusion of amplified sound, clarity of sentence, and loudness of noise were evaluated based on the everyday experience. And REIGs with international speech test signal and WRS in sound-treated room were measured.Results: Preferred REIG for HTL was low for OF compared to CF in 0.5 kHz and 0.75 kHz at input SPL of 55 dB, 0.5 kHz at 60 dB SPL, 0.25 kHz and 0.5 kHz at 80 dB SPL. However, there were no differences in aided WRS and psychoacoustics between the two groups.Conclusion: In this study, the preferred REIG of OF and CF groups showed a difference under 0.75 kHz although there were no psychoacoustic differences between the two groups. Therefore, the results of this study should be considered when formulating Korean-type hearing aid fitting formula.

Purpose Hearing loss self-management refers to the knowledge and skills people use to manage the effects of hearing loss on all aspects of their daily lives. The purpose of this study was to investigate the relationship between self-reported hearing loss self-management and hearing aid benefit and satisfaction. Method Thirty-seven adults with hearing loss, all of whom were current users of bilateral hearing aids, participated in this observational study. The participants completed self-report inventories probing their hearing loss self-management and hearing aid benefit and satisfaction. Correlation analysis was used to investigate the relationship between individual domains of hearing loss self-management and hearing aid benefit and satisfaction. Results Participants who reported better self-management of the effects of their hearing loss on their emotional well-being and social participation were more likely to report less aided listening difficulty in noisy and reverberant environments and greater satisfaction with the effect of their hearing aids on their self-image. Participants who reported better self-management in the areas of adhering to treatment, participating in shared decision making, accessing services and resources, attending appointments, and monitoring for changes in their hearing and functional status were more likely to report greater satisfaction with the sound quality and performance of their hearing aids. Conclusion Study findings highlight the potential for using information about a patient's hearing loss self-management in different domains as part of clinical decision making and management planning.

Purpose This study examined the current pediatric amplification practice with an emphasis on hearing aid verification using probe microphone measurement (PMM), among audiologists in Klang Valley, Malaysia. Frequency of practice, access to PMM system, practiced protocols, barriers, and perception toward the benefits of PMM were identified through a survey. Method A questionnaire was distributed to and filled in by the audiologists who provided pediatric amplification service in Klang Valley, Malaysia. One hundred eight ( N = 108) audiologists, composed of 90.3% women and 9.7% men (age range: 23–48 years), participated in the survey. Results PMM was not a clinical routine practiced by a majority of the audiologists, despite its recognition as the best clinical practice that should be incorporated into protocols for fitting hearing aids in children. Variations in practice existed warranting further steps to improve the current practice for children with hearing impairment. The lack of access to PMM equipment was 1 major barrier for the audiologists to practice real-ear verification. Practitioners' characteristics such as time constraints, low confidence, and knowledge levels were also identified as barriers that impede the uptake of the evidence-based practice. Conclusions The implementation of PMM in clinical practice remains a challenge to the audiology profession. A knowledge-transfer approach that takes into consideration the barriers and involves effective collaboration or engagement between the knowledge providers and potential stakeholders is required to promote the clinical application of evidence-based best practice.

Background: For children with hearing loss, the benefits of FM systems in overcoming deleterious effects of noise, distance, and reverberation have led to recommendations for use beyond classroom settings. It is important that audiologists who recommend and fit these devices understand the rationale and procedures underlying fitting and verification. Objectives: This article reviews previousguidelines for FM verification, addresses technological advances, and introduces verification procedures appropriate for current FM and hearing-aid technology. Methods: Previous guidelines for verification of FM systems are reviewed. Those recommendations that are appropriate for current technology are addressed, as are procedures that are no longer adequate for hearing aids and FM systems utilizing more complex processing than in the past. Technological advances are discussed, and an updated approach to FM verification is proposed. Conclusions: Approaches to verification andfitting of FM systems must keep pace with advances in hearing-aid and FM technology. The transparency approach addressed in this paper is recommended for verification of FM systems coupled to hearing aids.

Persons with a high frequency hearing loss are difficult cases for whom to find suitable amplification. We have experienced some success with this problem in our Hearing Clinics using a specially designed earmold with a hearing aid. Thirty-five cases with high frequency hearing losses were selected from our clinical files for analysis of test results using standard, vented, and open earpieces. A statistical analysis of test results revealed that PB scores in sound field, using an average conversational intensity level (70 dB SPL), were enhanced when utilizing any one of the three earmolds. This result was due undoubtedly to increased sensitivity provided by the hearing aid. Only the open earmold used with a CROS hearing aid resulted in a significant improvement in discrimination when compared with the group’s unaided PB score under earphones or when comparing inter-earmold scores. These findings suggest that the inclusion of the open earmold with a CROS aid in the audiologist’s armamentarium should increase his flexibility in selecting hearing aids for persons with a high frequency hearing loss.

BACKGROUND Underuse of hearing aids is caused by several factors, including the stigma associated with hearing disability, affordability, and lack of awareness of rising hearing impairment associated with the growing population. Thus, there is a significant opportunity for the development of direct-to-consumer devices. For the past few years, smartphone-based hearing-aid apps have become more numerous and diverse, but few studies have investigated them. OBJECTIVE This study aimed to elucidate the electroacoustic characteristics and potential user benefits of a selection of currently available hearing-aid apps. METHODS We investigated the apps based on hearing-aid control standards (American National Standards Institute) using measurement procedures from previous studies. We categorized the apps and excluded those we considered inefficient. We investigated a selection of user-friendly, low-end apps, EarMachine and Sound Amplifier, with warble-tone audiometry, word recognition testing in unaided and aided conditions, and hearing-in-noise test in quiet and noise-front conditions in a group of users with mild hearing impairment (n = 7) as a pilot for a future long-term investigation. Results from the apps were compared with those of a conventional hearing aid. RESULTS Five of 14 apps were considered unusable based on low scores in several metrics, while the others varied across the range of electroacoustic measurements. The apps that we considered “high end” that provided lower processing latencies and audiogram-based fitting algorithms were superior overall. The clinical performance of the listeners tended to be better when using hearing aid, while the low end hearing-aid apps had limited benefits on the users. CONCLUSIONS Some apps showed the potential to benefit users with limited cases of minimal or mild hearing loss if the inconvenience of relatively poor electroacoustic performance did not outweigh the benefits of amplification.
