As one of the deadliest diseases in the world, heart disease requires serious treatment. The weaknesses of providing services for heart disease in Bali Province are that there is no online diagnostic system to make it easier for people to check their health conditions to find out whether they have heart disease. Based on this research, the design and implementation of a web-based online heart disease diagnosis system are carried out. The diagnostic system uses Artificial Intelligence and inputs data from the user based on several questions posed by the system. This research uses Case-Based Reasoning (CBR) algorithm with Design Science Research Methodology (DSRM) and a case study qualitative research method. The test results show that the system designed and implemented can run well and perform accurate diagnostics according to the design and user needs.

The grinding process is a typical complex nonlinear multivariable process with strongly coupling and large time delays. Based on the data-driven modeling theory, the integrated modeling and intelligent control method of grinding process is carried out in the paper, which includes the soft-sensor model of economic and technique indexes, the optimized set-point model utilizing case-based reasoning, and the self-tuning PID decoupling controller. For forecasting the key technology indicators (grinding granularity and mill discharge rate of grinding process), an adaptive soft-sensor modeling method based on wavelet neural network optimized by the improved shuffled frog leaping algorithm (ISFLA) is proposed. Then, a set point optimization control strategy of grinding process based on case-based reasoning (CBR) method is adopted to obtain the optimized velocity set-point of ore feed and pump water feed in the grinding process controlled loops. Finally, a self-tuning PID decoupling controller optimized is used to control the grinding process. Simulation results and industrial application experiments clearly show the feasibility and effectiveness of control methods and satisfy the real-time control requirements of the grinding process.

Parkinson disease is a major public health problem all around the world. This paper proposes an expert disease diagnosis system for Parkinson disease based on genetic algorithm- (GA-) wavelet kernel- (WK-) Extreme Learning Machines (ELM). The classifier used in this paper is single layer neural network (SLNN) and it is trained by the ELM learning method. The Parkinson disease datasets are obtained from the UCI machine learning database. In wavelet kernel-Extreme Learning Machine (WK-ELM) structure, there are three adjustable parameters of wavelet kernel. These parameters and the numbers of hidden neurons play a major role in the performance of ELM. In this study, the optimum values of these parameters and the numbers of hidden neurons of ELM were obtained by using a genetic algorithm (GA). The performance of the proposed GA-WK-ELM method is evaluated using statical methods such as classification accuracy, sensitivity and specificity analysis, and ROC curves. The calculated highest classification accuracy of the proposed GA-WK-ELM method is found as 96.81%.

A remote fault diagnosis method for ultrahigh speeding grinding based on multi-agent is presented. The general faults of ultrahigh speed grinding are analyzed and diagnosis model based on multi-agent is established, the dialogue layer, problem decomposition layer, control layer and problem solving layer in the process of diagnosis are studied and the knowledge reasoning model of fault diagnosis is set up based case-based reasoning (CBR) combining rule-based reasoning (RBR). Based on theoretical research, a remote fault diagnosis system of ultrahigh speed grinding is developed. Results of the system running prove the theory is correctness and the technology is feasibility.

Hydraulic descaling system is the key device to ensure the surface quality of billet. However, traditional control methods lead to the stability problem in hydraulic descaling system. To solve the problem, the construction of the hydraulic descaling computer control system is studied, the working principle of the system is analyzed, and the high pressure water bench of hydraulic descaling is designed. Based on it, the corresponding computer control software is developed. The application shows that the designed system is stable in practice, which is helpful for enterprise production.

Whipple's disease is a rare systemic infectious disorder caused by the bacterium Tropheryma whipplei. We report the case of a 61-year-old male patient who presented to emergency room complaining of asthenia, arthralgia, anorexia, articular complaints intermittent diarrhea, and a 10-kg weight loss in one year. Laboratory tests showed the following results: Hb = 7.5 g/dL, albumin = 2.5 mg/dL, weight = 50.3 kg (BMI 17.4 kg/m²). Upper gastrointestinal endoscopy revealed areas of focal enanthema in the duodenum. An endoscopic biopsy was suggestive of Whipple's disease. Diagnosis was confirmed based on a positive serum polymerase chain reaction. Treatment was initiated with intravenous ceftriaxone followed by oral trimethoprim-sulfamethoxazole. After one year of treatment, the patient was asymptomatic, with Hb = 13.5 g/dL, serum albumin = 5.3 mg/dL, and weight = 70 kg (BMI 24.2 kg/m²). Whipple's disease should be considered a differential diagnosis in patients with prolonged constitutional and/or gastrointestinal symptoms. Appropriate antibiotic treatment improves the quality of life of patients.

INTRODUCTION: Gastroesophageal reflux disease (GERD) is probably one of the most prevalent diseases in the world that also compromises the quality of life of the affected significantly. Its incidence in Brazil is 12%, corresponding to 20 million individuals. OBJECTIVE: To update the GERD management and the new trends on diagnosis and treatment, reviewing the international and Brazilian experience on it. METHOD: The literature review was based on papers published on Medline/Pubmed, SciELO, Lilacs, Embase and Cochrane crossing the following headings: gastroesophageal reflux disease, diagnosis, clinical treatment, surgery, fundoplication. RESULTS: Various factors are involved on GERD physiopathology, the most important being the transient lower esophageal sphincter relaxation. Clinical manifestations are heartburn, regurgitation (typical symptoms), cough, chest pain, asthma, hoarseness and throat clearing (atypical symptoms), which may be followed or not by typical symptoms. GERD patients may present complications such as peptic stenosis, hemorrhage, and Barrett's esophagus, which is the most important predisposing factor to adenocarcinoma. The GERD diagnosis must be based on the anamnesis and the symptoms must be evaluated in terms of duration, intensity, frequency, triggering and relief factors, pattern of evolution and impact on the patient's quality of life. The diagnosis requires confirmation with different exams. The goal of the clinical treatment is to relieve the symptoms and surgical treatment is indicated for patients who require continued drug use, with intolerance to prolonged clinical treatment and with GERD complications. CONCLUSION: GERD is a major digestive health problem and affect 12% of Brazilian people. The anamnesis is fundamental for the diagnosis of GERD, with special analysis of the typical and atypical symptoms (duration, intensity, frequency, triggering and relief factors, evolution and impact on the life quality). High digestive endoscopy and esophageal pHmetry are the most sensitive diagnosctic methods. The clinical treatment is useful in controlling the symptoms; however, the great problem is keeping the patients asymptomatic over time. Surgical treatment is indicated for patients who required continued drug use, intolerant to the drugs and with complicated forms of GERD.
