<p><strong>Introduction.</strong> Stress and burnout are common among healthcare professionals, including doctors and nurses. Work-related stress rates among the general working population average 18%, while among doctors the rate is reported to be around 28%. Stress in doctors can result in multiple negative consequences. Detecting stress early may have positive outcomes for doctors, their families and the patients they care for. There is growing concern about stress in doctors working in public hospitals, yet there is a paucity of studies on stress among these doctors in South Africa.</p><p><strong>Methods.</strong> A cross-sectional, descriptive study using a self-administered, standardised questionnaire (12-item General Health Questionnaire (GHQ-12)) was conducted among medical doctors working in four hospitals of the Ngaka Modiri Molema (NMM) district, North West province. The research questionnaire was distributed and returned anonymously to ensure confidentiality.</p><p><strong>Results.</strong> Of the 67 doctors in the study, 34 (51%) were found to be stressed; 18 (27%) of the participants were highly stressed (morbidly stressed). This result was compared with figures obtained by Govender in an earlier study conducted among private general practitioners in KwaDukuza, KwaZulu-Natal, in which 38% were stressed according to the GHQ-12; 23% of the subjects were morbidly stressed.</p><p><strong>Conclusion.</strong> The rate of stress among doctors working in the four hospitals of the NMM district is higher than that found in other studies, which report a stress prevalence of 28 - 38% among doctors.</p>

The work-related stress rate among the general working population is 18% whilst among doctors it is around 28%. Stress in general practitioners (GPs) can result in multiple negative consequences. Detecting stress early may have positive outcomes for doctors, their families and the people they care for at their practice.Method: A cross-sectional, descriptive study using a self-administered, standardised questionnaire (12-item General Health Questionnaire [GHC]) was performed on the 30 general practitioners in Kwa-Dukuza. Confidentiality and anonymity were maintained.Results: 26 of the 30 GPs (87%) responded to the survey. 10 GPs (38%) were stressed as per the GHQ, six of whom were severely stressed. 22 reported that they felt stressed at work (subjectively).Conclusion: The results indicated that stress among Kwa-Dukuza GPs is slightly higher (38%) than found in other studies that indicate a prevalence of 28% among doctors.

Background: According to a Cochrane review, continuous support during childbirth increases the mother’s chances of a vaginal birth without identified adverse effects. However, this evidence-based practice is not universally implemented. The objective of the study was to identify challenges encountered in implementing continuous support during childbirth in public hospitals in the North West Province of South Africa. Method: An explorative, descriptive and contextual qualitative approach was used. The data were collected during 2013 by conducting focus group interviews with 33 registered midwives who had worked in maternity units in three selected public hospitals in the North West Province for at least two years. Results: Midwives identified challenges that negatively impacted the implementation of continuous support during childbirth at organisational and interpersonal levels. At the organisational level, challenges included human resources, policies and guidelines as well as the architectural outlay of the maternity units. The personal challenges related to communication and attitudes of nurses, patients and their families.  Conclusions: Organisational and personal challenges had a negative impact on the provision of continuous care during childbirth.

AbstractObjectiveIt is known that stunting and obesity affect a large proportion of children in the world, and these can be affected by the physical activity levels of the children. In the present study, we evaluated the association between physical activity, physical development and body composition in black adolescent children.DesignCross-sectional study.SettingBlack township schools in the North West Province, South Africa.MethodsThree-hundred and thirteen grade 8 children were included in the Physical Activity in Youth study. Anthropometric measurements, body composition measures and maturity level as assessed by Tanner stages were determined in these children. In addition, Previous Day Physical Activity Recall questionnaires were administered on the children to record the various activities they undertake daily.ResultsThe demographic characteristics of the children showed a high level of homogeneity. A high prevalence of stunting (16.3%) was observed in the children, which was higher in boys than in girls (21.6 vs. 12.3%). Also prevalent was overweight/obesity (8.6%), but this was higher in girls than in boys (13.4 vs. 1.6%). The children also showed a reduction in levels of physical activity with advancement in maturity; furthermore, boys showed a more central form of fat deposition whilst girls showed more gynoid deposition.ConclusionsThe study revealed that physical activity plays a role in determining body composition, and further indicated that physical activity is associated with favourable body composition measures. Children who were more active were likely to have less fat deposits.

Using a cross-sectional dataset, this study examines the factors influencing the income and willingness to pay for indigenous plants by rural households in the North West Province of South Africa. The method of data analysis was based on a two-stage Heckman model. Based on empirical data, the majority (93%) of the households are willing to pay for the indigenous plants. Furthermore, factors such as the age of the household’s head, marital status, size of households, financial returns and economic benefit of indigenous plants significantly influenced income and willingness to pay for indigenous plants by the households. Thus, indigenous plants have the potential to penetrate local and international markets and can be used to improve the economies, livelihood, and food security status of rural households in South Africa. Indigenous plant cultivation can increase agro-food system species diversity while conserving plant species indigenous to this area of South Africa. Encouraging both consumption and production of indigenous plants can also help diversify local economies and communities.

Background: Little has been done to evaluate clinical competence of community service nurses (CSNs) during the 12-month compulsory community service in South Africa. Evaluating clinical competence of CSNs would be of benefit as it might improve quality patient care and promote patient satisfaction. It therefore became of paramount importance for the researcher to establish some method of evaluating the CSNs’ clinical competence during their compulsory service in the North West province (NWP), South Africa.Aim: To evaluate the clinical competence evaluation tool (CCET) for CSNs for reliability and validity.Setting: A selected regional level 2 hospital.Methods: Ten experts participated in the validation process. The tool was tested at one of the public hospitals in the NWP and 11 out of 13 CSNs participated in this process. Statistical Package for the Social Sciences version 25 was employed and the reliability of the tool was measured using Cronbach’s alpha.Results: This tool’s content validity index has exceeded 0.80 and is indicated at 0.98, which reflects excellent content validity. The higher the content validity ratio score the greater the agreement amongst the experts. The Cronbach’s alpha coefficients in the six competencies are all greater than 0.7 implying that the tool developed in this study is reliable. All the experts indicated that the tool is clear, simple, general, accessible and important.Conclusion: From the above-mentioned results, a CCET for CSNs was proven to be valid and reliable.Contribution: This was the first tool to be developed in NWP of South Africa.

Objectives. To determine the prevalence of overweight and obesity among grade 1 learners, taking into account age, gender, race and school socioeconomic profile. Methods. The study is based on the baseline prevalence of overweight and obesity as part of the ongoing 6-year longitudinal NW Child Health Integrated Learning and Development (NW-CHILD) study. Stratified random sampling was used to identify 20 schools from four educational districts across all five quintiles of schools in the North West Province (NWP) of South Africa (SA). A total of 816 (419 boys, 397 girls) grade 1 learners with a mean age of 6.80 (standard deviation 0.39) years participated in the study, including 567 black learners, 218 white learners and 31 learners from other race groups. Standard anthropometric techniques and international body mass index (BMI) cut-off points for children were used to determine overweight and obesity. Results. A combined prevalence of overweight and obesity (Ow/Ob) of 11.1% was found, of which 7.5% were overweight and 3.6% obese. Although 7-year-old children showed a significantly higher prevalence of overweight (10.5%) than 6-year-old children (6.3%; p<0.05), obesity levels were more prevalent among 6-year-old children (4.7%) than 7-year-old children (2.3%). Although not significant, a higher prevalence of overweight (9.3 v. 6.4%) and obesity (4.3% v. 3.3%) was found in girls than boys. For the whole sample, white children displayed a significantly higher prevalence of Ow/Ob compared with black children (19.7% v. 8.8%; p<0.05), and the prevalence of Ow/Ob was higher in 4th- (18.1%) and 5th- (21.2%) quintile schools (which are associated with higher socioeconomic status (SES)) than in quintile 1 to quintile 3 schools (5.1%, 8.7%, 6.1%, respectively; p<0.05). Conclusions. More than 10% of grade 1 learners in the NWP of SA were overweight or obese, with the prevalence being significantly lower in lower SES schools and among black children. Prevention strategies are needed to combat overweight and obesity, and the associated negative consequences of these health risks. Obesity levels in black children should also be monitored because of continuing economic transition, and taking into account increasing overweight and obesity in black South African adults.

Psychiatrists in particular represents a high risk group among doctors for experiencing burnout, alcohol and drug use, posing suicide risk and other forms of work related stress.MethodThe study comprised of a cross sectional questionnaire survey which included measure of stress (General Health Questionnaire), burnout (Maslach Burnout Inventory), and satisfaction with medicine as a career and personality (Big Five). During October to December 2019, core trainee and specialty trainee doctors in CNWL were asked to complete an online survey via emails.ResultWe collected data from 50 CNWL psychiatry trainees. The sample consisted of 20 females (40%) and 30 males (60%). Ages varied from 26–58 years old, with a median age of 28. Core trainees (CT1–3) were recorded as 72% and specialty trainees at 28%.Of those who responded, around half of the trainees (52%) experienced high levels of stress outside of work in their personal life. The most common causes that trainees felt makes psychiatry a stressful profession were violence and fear of violence, limited resources, dealing with confrontational patients, inability to affect systemic change and increasing culture of blame. Around half of respondents (54%) felt that they have experienced burnout but only 26% of respondents knew where to go to find resources to help cope with burnout. Physical exercise and speaking to colleagues were the most common coping mechanisms used by trainees to deal with stress.Free text responses on what can be improved in workplace to enhance a positive experience of work included improving multidisciplinary interactions, easily accessible resources and increasing staffing levels. 74% of respondents felt they continued to care about what happens to patients regardless of working conditions.ConclusionHalf of CNWL trainee doctors who responded have experienced burnout. Some factors associated with stress and burnout in doctors are unique to psychiatry profession. Free text responses were useful in identifying areas for improvement in work places and useful coping mechanisms, which can be used to inform prevention and implement interventions to tackle burnout.

Background: Job satisfaction and burnout have been recurring problems amongst nurses in the Republic of South Africa (RSA). As a result, nurses are still leaving the rural public sector in search of lucrative work at the urban and private sectors and in developed countries. Accordingly, Occupational Specific Dispensation (OSD) was introduced as a strategy to ircumvent the problem. However, since the implementation of OSD in 2007, there have been no studies conducted regarding the level of job satisfaction amongst nurses after the implementation of OSD in the North-West Province, especially because each province has its own challenges that are unique to the area.Objectives: The study had two objectives: to describe the level of job satisfaction amongstprofessional and other category nurses (OCNs) at a public hospital in the North-West Provinceand describe the perceptions of nurses about OSD and their intention to continue working inthe hospital.Method: A quantitative descriptive cross-sectional design was used. Stratified random sampling was used to select a sample of 92 professional nurses (PNs), 90 enrolled nurses and enrolled nursing assistants (Nursing Act 2005), which made a total of 182 participants. For the purpose of the study, the enrolled nurses and enrolled nursing assistants were referred to as OCNs. Data were collected using the Minnesota Satisfaction Questionnaire–short form and OSD statements and analysed with Statistical Package for Social Science (SPSS, version 18).Results: The majority of PNs (79.3%, n = 73) and OCNs (86.7%, n = 77) were dissatisfied with the working conditions and salary: PNs (80.4%, n = 74) and OCNs (87.8%, n = 79). The nurses mostly discredited the current state of the OSD implementation. Majority of the PNs (84%, n = 77) and OCNs (91%, n = 82) disagreed with the statement that ‘level of job satisfaction has improved after the implementation of OSD’.Conclusions: The National Department of Health should consider a holistic approach to address all work-related conditions for nurses in order to curb the attrition rates. Policy makers and unions should urgently review OSD for all categories of nurses.
