We examined how obese children perceive a maximal cardiorespiratory fitness test compared with a submaximal cardiorespiratory fitness test. Twenty-one obese children (body mass index ≥95th percentile, ages 10–17 years) completed maximal and submaximal cardiorespiratory fitness tests on 2 separate occasions. Oxygen consumption (VO2) and overall perceived exertion (Borg 15-category scale) were measured in both fitness tests. At comparable workloads, perceived exertion was rated significantly higher ( P < 0.001) in the submaximal cardiorespiratory fitness test compared with the maximal cardiorespiratory fitness test. The submaximal cardiorespiratory fitness test was significantly longer than the maximal test (14:21 ± 04:04 seconds vs. 12:48 ± 03:27 seconds, P < 0.001). Our data indicate that at the same relative intensity, obese children report comparable or even higher perceived exertion during submaximal fitness testing than during maximal fitness testing. Perceived exertion in a sample of children and youth with obesity may be influenced by test duration and protocol design.

BackgroundLow level of cardiorespiratory fitness has been recognized as an important independent and modifiable risk factor of increased morbidity and mortality. However, in standard outpatient settings, patients are not routinely screened for fitness and advantages of such testing for the management of type 2 diabetes have not been defined.AimTo describe the toleration of a fast, simple and practicable fitness test (2-min step-in-place test) by overweight/obese type 2 diabetics and their performance indicated by 2-min step-in-place test score (STS). To study short-term anthropometric, functional and metabolic changes following the implementation of the test in the selected population.MethodsA total of 33 overweight/obese type 2 diabetics underwent, besides routine examination at the outpatient clinic, the fitness test (group A). Patients were asked to increase their regular physical activity with focus on walking without change in diet and chronic medication. Three to four months later, the subjects were tested again. An identical number of age- and sex-matched obese diabetics followed in our outpatient clinic (without fitness testing), was randomly selected from the Hospital Information System (control group B).FindingsAll patients subjected to fitness testing completed the protocol successfully. STS score was found to have a considerable range with differences between males and females at the borderline of statistical significance. The data are compliant with lower aerobic endurance of obese diabetics compared with healthy population. Within study period, the tested group presented with improvements in STS (referring especially to the males) as well as in several laboratory parameters of glucose and lipid homeostasis, glomerular function and subclinical inflammation with no reflection in anthropometry. Group B demonstrated no significant change. In conclusion, 2-min step-in-place test is fast, undemanding and well-tolerated by patients and personnel. Following its validation based on cardiopulmonary exercise testing, the test may prove recommendable for screening or self-monitoring purposes.

This study aimed to determine how California secondary physical education teachers perceive the state mandated youth fitness testing for the 5th, 7th, and 9th grades using Fitnessgram. The participants were secondary school physical education teachers (N = 323). A previously validated attitudinal instrument (Keating & Silverman, 2004a) was used to collect the data. The means and standard deviations for each attitude subdomain and the overall attitudes were computed. MANOVA and ANOVA were employed to test the differences in attitudes by demographic and profession-related variables. Teachers’ overall attitudes toward the Fitnessgram were slightly higher than a neutral attitude, indicating slightly positive attitudes on a 7-point Likert scale (M = 4.47, SD = 1.06). The mean scores for the attitude subdomain of cognitive (i.e., usefulness of fitness test results) and the affective (i.e., enjoyment of implementing fitness tests, and enjoyment of using fitness test results) components were 4.25 (SD = 1.38), 4.90 (SD = 1.15), and 4.39 (SD = 1.17), respectively. The data from the study suggested that teachers marginally agreed that the test results were useful and that they somewhat enjoyed implementing the test. Class size and student grade levels taught were important profession-related variables to consider regarding teacher attitudes toward the Fitnessgram.

Background: The use of fitness testing for the identification of women at high-risk of metabolic syndrome (MS), and therefore of cardiovascular disease, is clinically relevant. Objective: The purpose of this study was to determine the ability of a set of physical fitness tests to establish the risk of MS in perimenopausal Moroccan women. Methods: The study comprised 151 women (45–65 years) from the North of Morocco. We used standardized field-based fitness tests to assess cardiorespiratory fitness, muscular strength, flexibility and balance. Fatness was assessed by impedanciometry and anthropometry. We also measured resting heart rate, blood pressure and plasma fasting glucose, total cholesterol, low density lipoprotein (LDL)-cholesterol, high density lipoprotein (HDL)-cholesterol and triglycerides. Results: Women with MS performed worse in most of the fitness tests studied. Among the fitness test studied, the six-minute walk test was the most associated to MS. Receiver operating characteristics curve analyses revealed that the six-minute walk test threshold that best discriminated between the presence and absence of MS was 480.5 m (area under curve (AUC): 0.719, 95% confidence interval (CI): 0.62–0.82; p<0.001). Logistic regression after adjustment for age and weight showed that a distance ≤480 m is associated with 2.9 times higher risk (95% CI: 1.56–7.65; p<0.05) for having MS. Conclusions: Including cardiorespiratory fitness as a MS risk factor may improve early identification of at-risk Moroccan women. Fitness testing provides useful information and is cheap, easy to perform, and not time-consuming, which makes its use in this specific clinical settings feasible.

Tactical personnel work in an occupation that involves tasks requiring a high level of cardiovascular fitness as well as muscular strength and endurance. The aim of this literature review was to identify and critique studies investigating the relationship between physical fitness, quantified by fitness assessment measures, and occupational task performance. Databases were searched for relevant articles which assessed a fitness measure and a measure of occupational performance. A total of 15 articles were included and were deemed to be of acceptable methodological quality (8.4/12 on the Critical Appraisal Skills Programme checklist). Included articles assessed a variety of fitness attributes and occupational tasks. Across tactical groups, there appear to be no standardized fitness tests that can determine occupational performance, with aerobic fitness, anaerobic fitness, strength, endurance, power, and agility all being associated with occupational task performance. A wide range of fitness assessments appears to be required to predict occupational performance within tactical personnel. Efforts should be made to base fitness assessments on occupational demands unique to both the environment and requirements of each individual tactical unit.
