Introduction: Undernutrition is a major health problem in developing countries which may have adverse effects on growth and development of an individual. The objectives of this study were to assess the prevalence and grades of malnutrition in children below 60 months of age, attending outpatient department at Gandaki Medical College teaching hospital.Materials and Methods: A prospective study was conducted among 556 children below 60 months of age from August to December 2013. Weight and length/height were measured using standard technique and were plotted on WHO centiles curves and severity determined based on WHO classification. The prevalence of underweight, stunting and wasting were determined by anthropometry.Results: Based on WHO classification, out of 556 children, 20.2% were underweight, 34% were stunted and 15.1% were wasted respectively. Of them, severe underweight were 4.7%, severe stunted were 14.1% and severe wasted were 7.2% respectively.Conclusion: A high prevalence of undernutrition exists in under- five children attending a tertiary hospital in a western region of Nepal.DOI: http://dx.doi.org/10.3126/jnps.v34i2.10566J Nepal Paediatr Soc 2014;34(2):119-124

Background: Malaria is a life-threatening parasitic disease caused by the plasmodium parasite and women and under-five children are more prone to its adverse consequences. The use of insecticide-treated nets (ITNs) is recommended to reduce malaria burden in endemic communities. The study aimed to determine knowledge of malaria and utilization of insecticide-treated nets (ITNs) amongst mothers of under-five children in rural communities of Nigeria’s Federal Capital Territory, and the predictors.Methods: A community-based cross-sectional design was used. Multi-stage sampling method was used to select 160 mothers of under-five children in two rural communities. A structured questionnaire was used for data collection. Data analysis was done using SPSS statistical software version 22.0 and level of statistical significance was determined by a p value of < 0.05.Results: Mean age of respondents was 29.0±5.4 years, and majority 144 (90%) of the women were married. Lower proportion of respondents had good knowledge of malaria 33 (20.6%). Ninety-four (58.8%) of respondents owned ITNs, but less than half 45 (47.9%) utilized the nets. Predictors of good knowledge of malaria were having attained tertiary education [Adjusted Odds Ratio (AOR); 2.7, 95% Confidence Interval (CI):1.1–8.1], p=0.042, and being self-employed, (AOR; 3.4, 95% CI: 1.1-13.1), p=0.043). Predictor of utilization of ITNs was being aged 30 years and above (AOR; 2.5, 95% CI: 1.1-6.1, p=0.031).Conclusion: Lower proportions of respondents had good knowledge of malaria and utilization of ITNs. Health education of mothers on malaria and benefits of ITNs use should be intensified in the study area.

ObjectivesThis study seeks to determine not only the reliability of parental touch in detecting fever as compared to rectal thermometry in under-five children, but also the sociodemographic factors that may predict its reliability.SettingThe study was carried out in the Emergency Paediatric Unit of a tertiary hospital in North Central Nigeria.Participants409 children aged less than 5 years with a history of fever in the 48 h prior to presentation and their mothers were recruited consecutively. All the children recruited completed the study. Children with clinical parameters suggestive of shock, and those who were too ill, were excluded from the study.Primary and secondary outcome measuresThe primary outcome was the proportion of mothers who could accurately predict if their child was febrile or not (defined by rectal temperature) using tactile assessment only. Secondary outcomes were the validity and accuracy of touch in detecting fever and factors related to its accuracy.ResultsAbout 85% of the children were febrile using rectal thermometry. The sensitivity, specificity, positive predictive and negative predictive values for touch as a screening tool were 63%, 54%, 88.3% and 21%, respectively. High maternal socioeconomic status and low maternal age influenced positively the accuracy of touch in correctly determining the presence or absence of fever.ConclusionsThis study has shown that tactile assessment of temperature is not reliable and that absence of fever in a previously febrile child should be confirmed by objective methods of temperature measurement.

Background: Anaemia is the most common nutritional problem in the world with significant impact on the neurological and cognitive development of young, growing children. India has a high prevalence of anaemia among its under-five children. The objectives of the study were to estimate the prevalence of anaemia and iron deficiency anaemia in children in the age group 6 months to 5 years.Methods: A hospital based descriptive study was done on 51 children attending the outpatient department of a tertiary hospital. They were evaluated by obtaining a detailed history, physical examination and haemoglobin and serum ferritin estimation.Results: The prevalence of anaemia in the present study was 41.2%. 13 (25.5%) had mild anaemia, 8 (15.7%) had moderate anaemia and none had severe anaemia. Iron deficiency anaemia was present in 17 out of 21 children (81%) with anaemia (p<0.00001). There were significant associations between haemoglobin levels and age (p=0.049) and nutritional status (moderate acute malnutrition) (p=0.034) of the children in the study.Conclusions: The prevalence of anaemia in children under 5 years in Kerala, which has performed well in other health quality indicators, remains high. A multimodal strategy to address this public health problem is required.

Abstract BackgroundMalaria remains a burden globally with the WHO African region accounting for 94% of the overall disease burden and deaths in 2019. Nigeria alone accounted for 27% of total malarial cases and 23% of deaths. Insecticide-treated nets (ITN) use and receiving early care for fever represent the most cost-effective means of malaria prevention and control. In this study, we assessed the utilization of ITNs in households with under-five children in Southwest Nigeria, the health-seeking behavior of their caregivers for fever, and associated factors.MethodsThis was a secondary data analysis of the 2018 Nigeria Demographic and Health Survey. Analysis was restricted to under-five children residing in Southwest Nigeria. Multivariate analysis using logistic regression was conducted to determine factors associated with utilization of ITNs and care-seeking for fever in under-five children. Complex samples command of SPSS (Version 23) was used to account for survey sampling design.ResultsOf 2,281 under-five children in households with at least one ITN, 67.6% (1542) slept under an ITN the night before the survey. Predictors of ITN use were the age of child (<12 months), wealth index (poorest), number of nets in a household (2 or more), and number of household members (1-3). Of the 4,153 under-five children, 8.7% (360) had fever, and advice or treatment was sought for 72.4% (261). Predictors of good health-seeking behavior were the age of a child <12 months (AOR=2.81, 95% CI = 1.08-7.31), mothers’ education, and state of residence. Mothers who had secondary education were less likely to report care-seeking than those with higher education (AOR = 0.43, 95% CI = 0.19-0.98). Mothers from Osun, Ekiti, and Lagos state were less likely to seek care than those from Ogun state.ConclusionWe found that the utilization of insecticide-treated nets and seeking of care for fever in under five years of age in Southwest Nigeria were below the National Malaria Strategic Plan goal of 80% and 100% respectively. The state governments of Osun, Ekiti, and Lagos should develop strategies to improve ITN use and early seeking of care for fever in under-five children.
