The Stages of Change for Dietary Fat and Fruit and Vegetable Intake of Patients at the Outset of a Cardiac Rehabilitation Program

Purpose. To determine the stages of change for dietary fat and fruit and vegetable intake of cardiac patients entering a rehabilitation program. Design. A cross-sectional study using a convenience sample of newly admitted cardiac rehabilitation patients. Setting. Department of Pulmonary and Cardiac Rehabilitation, Moses H. Cone Memorial Hospital, Greensboro, North Carolina. Subjects. Subjects (n = 226) were predominantly men, Caucasian, married, smokers with 9 to 12 years of education, and with primary diagnoses of coronary artery bypass graft. Measures. Stages of change and food frequency questionnaires were completed by subjects upon admission to cardiac rehabilitation. Results. Subjects in action and maintenance stages for dietary fat reduction comprised 78.7% of the population. Subjects' percentage of energy from fat decreased linearly from the precontemplation stage (38.8%) to the maintenance stage (30.9%). Eighty-one percent of subjects were in precontemplation/contemplation for increasing fruit and vegetable intake. Daily servings of fruits and vegetables ranged from 2.6 for precontemplation to 5.1 for maintenance subjects. Age, body mass index (BMI), education, and family history for coronary disease were unrelated to stage of change for the dependent variables. Conclusions. Patients were in different stages of change for two nutritional behaviors linked to the same disease. Results support the need to assess patients for food behaviors and apply different educational interventions for each food behavior.

Purpose. To track the 2-year movement of cardiac rehabilitation patients through the stages of change for dietary fat reduction and increasing fruit and vegetable intake. Design. A cross-sectional, longitudinal evaluation of a convenience sample of cardiac rehabilitation patients. Setting. Department of Pulmonary and Cardiac Rehabilitation, Moses H. Cone Memorial Hospital, Greensboro, North Carolina. Subjects. Subjects (n = 118) were predominately white, overweight, married men with a history of tobacco use. Intervention. Group nutrition education sessions for 12 weeks. Measures. Stage of change assessments for dietary fat reduction and increasing fruit and vegetable intake were performed at the start (baseline), conclusion (3 months), and 2 years after cardiac rehabilitation. Results. Baseline staging demonstrated patients' efforts to reduce dietary fat intake (preparation, 10.2%; action, 35.6%; maintenance, 47.5%). The assessment at 2 years staged 87.3% of the population in the maintenance stage. In contrast, the precontemplation (30.0%), contemplation (7.6%), and preparation (49.2%) stages dominated baseline staging for modifying fruit and vegetable intake. Patients in the precontemplation and contemplation stages increased to 58.5% at 2 years. Conclusions. Cardiac rehabilitation patients are in different stages for two food behaviors linked to the same illness. Results support the appropriateness of nutrition education that emphasizes instrumental information for affecting fat reduction behaviors. Modifying fruit and vegetable intake behaviors is more appropriately addressed with noninstrumental interventions that emphasize health benefits and barriers to change.

Abstract Background In 2015, the Partnership for a Healthier America launched the branded Fruits & Veggies (FNV) Campaign to apply a unique industry-inspired marketing approach to promote fruit and vegetable sales and intake to moms and teens in two US pilot markets: Fresno, California and Hampton Roads, Virginia. The aims of this cross-sectional study were to: 1) assess brand awareness and fruit- and vegetable-related outcomes among FNV Campaign target audiences in the California and Virginia market locations; and 2) examine whether reported awareness of the FNV Campaign was associated with differences in fruit- and vegetable-related cognitive and behavioral outcomes. Methods Data for this cross-sectional study were collected using an online survey administered to a non-probability convenience sample (n = 1604; February–July 2017) of youth aged 14–20 years (n = 744) and moms aged 21–36 years (n = 860) in the two pilot markets. Descriptive statistics were computed and outcomes compared between unaware and aware respondents, controlling for sociodemographic covariates. Multivariate analysis of covariance (MANCOVA) was conducted to assess whether fruit- and vegetable-related attitude, belief, and encouragement outcomes differed by FNV Campaign awareness; logistic regression was used to examine associations between FNV brand awareness and dichotomous variables (fruit- and vegetable-related behavioral intentions, trying new fruits and vegetables); and ANCOVA was used to assess associations with daily fruit and vegetable intake frequency. Results Approximately 20% (n = 315/1604) of respondents reported awareness of the FNV Campaign. Youth that reported awareness of the FNV Campaign (n = 167, 22.4%) had higher intentions to buy (p = 0.003) and eat (p = 0.009) fruits and vegetables than unaware respondents. Mothers that reported awareness of the FNV Campaign (n = 148, 17.2%) reported greater encouragement for friends and family to eat fruits and vegetables (p = 0.013) and were approximately 1.5 times more likely to report trying a new fruit or vegetable (p = 0.04) than mothers unaware of the Campaign. Daily fruit and vegetable intake frequency did not differ by Campaign awareness. Conclusions FNV Campaign awareness was associated with limited but positive short- and intermediate-term cognitive and behavioral outcomes among target audience respondents. These findings can inform future research to enhance understanding and improve the FNV Campaign as it is expanded to new markets nationwide.

Abstract Objectives To determine whether nutrition knowledge is associated with skin carotenoids, as measured by reflection spectroscopy, a valid and objective assessment of fruit and vegetable intake. Methods A convenience sample of undergraduate and graduate students were recruited through flyers, classroom announcements, and email distribution lists at a large public university in California. Nutrition knowledge was assessed with a validated questionnaire. Skin carotenoids were measured with reflection spectroscopy (RS) using a device known as the Veggie MeterTM. Participants completed the nutrition knowledge questionnaire and height, weight, and RS data were collected by trained researchers at a single time-point. Data were collected as part of a larger, on-going study. Pearson's correlation and ordinary least-squares (OLS) regression analysis were used to determine the relationship between nutrition knowledge and skin carotenoids. Results A total of 40 students participated in the study; 72% were female. Nutrition knowledge scores ranged from 13 to 53 with a mean of 37.95 (out of 58 possible), and the mean RS score was 334.0. Skin carotenoids were found to be significantly correlated with nutrition knowledge (r = 0.332; P = 0.036), with 8.7% of variance in RS score explained by nutrition knowledge. Body mass index (BMI) was found to be negatively associated with RS score at P < 0.05; when BMI was incorporated, the proportion of variance in RS score explained by the regression model was 26.9%. Gender was not found to significantly associated with RS score (P = 0.965). Conclusions These preliminary data suggest that nutrition knowledge may be a significant predictor of dietary fruit and vegetable intake when accounting for BMI in university students. Future studies will incorporate additional variables, such as food security status and diet quality to further elaborate on the relationship between nutrition knowledge and behavior. OBJECTIVE: Thhis study aimed to investigate the reason why people choose to consume less vegetable and fruit through examining how the stage of change and psycho-social parameters relate to vegetable and fruit intake. METHODS: We carried out a cross-sectional study in 4 regions of Japan in which 2308 individuals (1012 men and 1296 women) aged 18 years or older who completed the questionnaires were included in the study. RESULTS: The results showed 56% of the participants were in the precontemplation stage (not thinking about consuming recommended amount of fruit and vegetable) and their average amounts of vegetable and fruit intake were far below the level of current recommendations, and subjects in the precontemplation stage showed lower scores of attitude and self efficacy. Men were more likely to be in precontemplation (67.7%) and less likely to be in action/maintenance stage (7.9%) than women (46.9%, 12.1%, respectively) (p < 0.001). We also observed the scores of attitude (p = 0.06) and self-efficacy (p < 0.01) rose as the stage went up from the precontemplation to action/maintenance for increasing vegetable and fruit intake. Moreover, a linear trend was found toward higher vegetable (p < 0.05) and fruit (p = 0.121) intake from precontemplation to action/maintenance stage. CONCLUSION: The present evidence suggests more attention should be focused on strategy for perceptions of personal need for recommended amount of vegetable and fruit intake for those who are in the precontemplation stage. Moreover, effective programs on enhancement of self-efficacy and attitude toward vegetable and fruit consumption are needed for increasing the vegetable and fruit intake.
