Introduction: Kasa is a Vata-kapha pradhana disease, caused due to Vata vimargagaman and Pranavaha shrotodusti. Due to the various similarities in its clinical presentation, Kasa can be correlated with cough (bronchitis). According to National center for health statistics, 62 million cases of common cold and cough occurs each year. Methodology: A total of 44 patients diagnosed as Kasa were randomly divided as per the lottery system of randomization. The patients of Group A were given 5 gram of Sitopaladi Churna twice a day for 30 days. The patients of Group B were given 5 gram of Taalishadi Churna twice a day for 30 days. The patients of both the groups were supposed to follow the dietary and behavioural advices. The response of the drug was assessed at pre-treatment and post-treatment time. Results: There was equal significant change in all of the Roga bala, Agni bala and Deha bala parameters by both Sitopaladi and Taalishadi Churna. It was revealed that all other blood parameters except Hb, TLC and Neutrophil count used in the study were not significant in both the groups. Both drugs were found equally highly significant (p< 0.0001) in Sushka kasa and Swarabheda. Sitopaladi churna was more effective in relieving Hritparswashool (p=0.002) whereas Taalishadi churna is more effective in reducing Pitanisthivanam (p=0.007). In case of Nirghosh, Sitopaladi churna was more effective (p<0.0001). On the other hand, Taalishadi churna was more effective in relieving Peenasa (p<0.001). Conclusion: Both Sitopaladi Churna and Taalishadi Churna were found to be equally effective in the treatment of Kasa.

Introduction: Mutrasmari is considered as Astamahagada in Ayurveda in which there is formation of stone in the Mutravaha srota. It is an ancient disease with global distribution. The symptoms of Mutrasmari are excruciating pain in urethra, ureter, urinary bladder and over umbilical region, haematuria etc and later that may lead to emergence by the obstruction in urination. Hence on the basis of pathology and its clinical presentation the disease is correlated with Renal calculus or Urolithiasis in modern Urology. Methodology: 38 patients diagnosed as Mutrasmari were randomly divided in two groups as per the lottery system of randomization. The patients of Group A were given 1000mg Goksuradi guggulu twice a day for 45 days. The patients of Group B were given 1000 mg of Gokshuradi guggulu along with 45 ml of Varunadi kwatha twice a day for 45 days. The patients of both the groups were advised to follow the dietary regimen and lifestyle modification. The effect of the intervention was assessed before and after treatment. Result: The treatment outcomes were statically analyzed and found that both treatment groups were significant in relieving the symptoms like abdominal pain, burning micturition, dysuria and expulsion of renal culculi. 100% relief  noted on Surudhir mutrata (Hematuria) where only single patient complaint had been registered as Hematuria in Group B. In case of Mutradaha (Burning micturation) there was mild improvement in group A and moderate improvement in group B. Similarly there was complete remission in Mahati vedana (Dysuria and Pain in abdomen) case in both Group A and Group B after the treatment. Conclusion: Gokshuradi guggulu along with Varunadi Kwatha shows more significant result than Gokshuradi guggulu.

Purpose: To evaluate the effectiveness of implant treatment using computer 3D modeling and surgical guided. Materials and Methods: The study included 148 patients who underwent prosthodontics rehabilitation using dental implants in 2015-2020. 3D computer-aided modeling and surgical guide were used to plan the operation. To conduct a comparative analysis of the treatment results, two groups were formed: In group A (included 75 patients, fully guided surgery), in group B (included 73 patients). Results: In patients Group A intraoperative or immediate postoperative complications were noted (errors in the position, inclination), anatomical risk structures were invaded, after 3 years producing a survival rate of 96.2% In patients Group B, intraoperative complications were recorded; membrane perforation 4, errors in the position of the implants 16, the inclination of the implants 18, fenestration 12, after 3 years producing a survival rate of 97.6%. Mean marginal bone loss (MBL) patients in Group A were significantly higher than patients in group B (p < 0.05). In patients Group A the average surgical time from time of anesthesia to the placement of the healing abutment was 10.6 ± 2.9 min per implant, in patients Group B, the average surgical time was 16.4 ± 1.5 min per implant. Conclusion: The 3D modeling method and the controlled positioning of the implant allows surgical access with minimal trauma, reducing treatment time and complications.

Objective: To compare the effects of isometric (stability) and isotonic training on core muscle in patients with non-specific low back pain on pain, endurance, and functional disability.Methods: Forty participants of both genders aged between 20 and 35 years suffering from non-specific low back pain were taken and equally divided into two groups: Group A isometric exercise and Group B isotonic exercise, both the group received baseline treatment of transcutaneous electrical nerve stimulation and hot moist pack. Outcomes measure visual analog scale, endurance test, and modified Oswestry disability index were used the pre-treatment and at the end of 4 weeks.Results: Experiment of both the groups showed a non-significant improvement in pain, endurance, and functional disability. Except for extensor endurance test which shows significant different in Group A.Conclusion: Both isometric and isotonic exercises are equally effective in reducing pain, increase endurance, and improve functional disability in patients with non-specific low back pain.

Cranial cruciate ligament rupture (CCrLR) is a common condition found in the small animal routine, being correlated to traumas, obesity, genetic factors, and primary osteoarthritis (OA) in dogs. Affected animals show articular instability that, if not corrected surgically, may cause secondary OA and loss of limb function. The aim of this study was to compare short-term results of the intra-articular technique for knee stabilization after CCrLR using a surgical button associated with polyester yarn (Group A), the surgical button associated with nylon yarn (Group B), and surgical toggle associated with polyester yarn (Group C). Eighteen dogs presenting CCrLR, weight varying from 5 to 35 kg, and different sex and breed were divided into three groups of six individuals. OA radiographic grade, pre- and post-operative lameness, surgical time, and the macroscopic aspect of cartilage were assessed. The intra-articular technique was performed by passing a suture through two tunnels, drilled in the femoral condyle and tibial crest to stabilize the knee joint. Twelve animals presented a decreased lameness and normal limb function after 15 days. On the other hand, four dogs from Group B presented complications: two dogs had suture rupture after 30 days and other two showed muscular contracture with decreased range of motion, followed by loss of limb function. In Group A, one dog showed suture rupture after 15 days and other had suture infection after 30 days. In Group C, dogs recovered normal limb function without complications. Therefore, surgical toggle associated with polyester yarn was better than the other studied materials.

PURPOSE: To evaluate and to compare macro and microscopically the intense injuries of the gastric mucosa of rats which were caused by NSAIDS celecoxib and indomethacin and the gastric cytoprotection with omeprazole and misoprostol. METHODS: The sample is formed by one hundred and fifty Wistar rats with average weight 200 g, distributed in four groups, such as: Group A, subdivided in groups A1 and A2 - pre-treatment with omeprazole (20 mg/rat) during seven days and on the 8th day - use of NSAIDS, concerning A1 (20 rats) were given celecoxib (1mg/rat) and A2 (20 rats) were given indomethacin. The Group B, subdivided in group B1 and B2 - pre-treatment with misoprostol (20mg/rat) during seven days and on the 8th day use of NSAIDS, concerning B1 (20 rats) were given celecoxib (1 mg/ rat) and B2 (20 rats) were given indomethacin (12.5 mg/rat). The Group C: were not given cytoprotection during seven days, from the 7th to the 8th day - fast of food and water ad libitum, on the 8th day of NSAIDS use, concerning C1 (20 rats) were given celecoxib, C2 (20 rats) were given indomethacin (12.5 mg/ rat), C3 (20 rats) were given celecoxib (200mg/rato), and Group D - control group, concerning 10 rats were observed during seven days ingesting food and water ad libitum. On the 9th day, the stomachs were taken out and were macro and microscopically evaluated for the identification of the gastric injuries. RESULTS: On the macroscopic studies, the groups A2, B2 and C2 presented a remarkable high number of injuries for cm² /animal, respectively 18.55 injuries for cm² /animal, 16.25 injuries for cm² /animal and 13.55 injuries for cm²/animal. On the microscopic studies, the percentage of the injured mucosa, presented expressive difference among the groups A1, B1, C1 when compared to the groups A2, B2, C2 (p<0.0001). The average of the length/injury and the average of the depth of the injuries did not present expressive statistics differences among the groups A2, B2 and C2. The average of the edema presented expressive statistics difference among the groups A2 and D; B2 and C2 and between C2 and D (p < 0.05). CONCLUSIONS: The indomethacin on the applied concentration causes a great number of macroscopic and microscopic injuries to gastric mucosa of rats when compared to celecoxib which does not cause lesions. Omeprazole and misoprostol on the applied concentrations do not present macroscopic and microscopic effectiveness on the gastric cytoprotection when applying indomethacin. Considering the microscopic analysis of the average of the edema, the group of animals, which was given misoprostol as cytoprotection, presented a lower average compared to the group which was given omeprazole.

Introduction: In this study, a single-blind and randomized controlled trial (RCT) for assessing the effectiveness of high-power (up to 12 W) laser therapy (HPLT) on patients with patellofemoral pain syndrome (PFPS) was carried out. Methods: Forty-four patients were randomly assigned to two treatment groups by generating random numbers with MATLAB 2014b software, where odd and even numbers were attributed to sham laser group (group A) and actual laser group (group B), respectively. Group B patients underwent HPLT with total dose of 300 J/session for 5 consecutive sessions separated by a 2-day interval. On the other hand, sham laser was applied to group A patients. Both groups had the same exercise therapy programs during the study period (3 months). The exercise therapy program included isometric knee exercise for 3 sets per day and 10 times in each set, with duration of 10 seconds per time and straight leg raise for 15 seconds 10 times a day. The group codes of patients were not revealed to subjects and data analyzer until completion of the study. Kujala, the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) and visual analog scale (VAS) questionnaires were chosen as outcome measures. These questionnaires were completed at three points during the study; at the beginning of the study to obtain the pre-therapy conditions and one month and three months after the start of the study to evaluate post-therapy conditions. Results: Two main analyses were conducted: within-group and between-group analyses. Withingroup analyses indicated significant improvements in respect to all measurements where pretherapy and post-therapy comparisons were conducted in both groups (P < 0.05). On the other hand, between-group comparisons did not reveal any statistically significant functional difference between group A and group B regarding the evaluative criteria (P > 0.05) except for pain VAS (P < 0.05). Conclusion: This study indicated that short-term HPLT accompanied by appropriate exercise regimen significantly decreased pain in patients with PFPS. But it was not recommended as an efficient modality in functional improvement. Also, it was observed that, in the short-term period of study, HPLT was a safe modality.

Abstract The recommended daily dose of vitamin D is 2000 IU was found to be insufficient in many patients. The objective of the present study is to find whether the daily dose of vitamin D should be based on BMI. Two hundred and thirty patients with an established vitamin D deficiency (serum level of 25 Hydroxy vitamin D3 (25OHD3) of ≤20 ng/ml) and patients with BMI ≥30 kg/m2 were included in the study. Demographic data, comorbidities and BMI were recorded. Pre-treatment and post-treatment serum 25OHD3, calcium, phosphorus and parathyroid hormone (PTH) were tested at 0-, 3- and 6-month periods. Patients were treated with a standard dose of 50 000 IU of vitamin D weekly and 600/1200 mg of calcium a day. Once their level of 25OHD3 reached ≥30 ng/ml, patients were randomised into two groups. Group A received a standard recommended maintenance dose of 2000 IU daily and Group B patients received 125 IU/kg/m2 of vitamin D3. The data were entered in the database and analysed. The mean age of Group A was 50⋅74 ± 7⋅64 years compared to 52⋅32 ± 7⋅21 years in Group B. In both groups, pre-treatment vitamin D level was ≤15 ng/ml and increased to 34⋅6 ± 2⋅6 and 33⋅7 ± 2⋅4 ng/ml at the end of 3 months treatment with a dose 50 000 IU of vitamin D3 and calcium 600/1200 mg once a day for group A and group B, respectively. At 6 months, patients in Group A 25OHD3 level was 22⋅8 ± 3⋅80 and in Group B was 34⋅0 ± 1⋅85 ng/ml (P < 0⋅001). This preliminary study suggests that obese patients need higher dosage of vitamin D than the recommended dose. It is prudent that the dosage should be based on the BMI to maintain normal levels for a healthy musculoskeletal system.

Background: The failure of rotator cuffs to heal after repair is an unresolved surgical issue. There have been substantial efforts, including the use of biological supplements, to enhance tendon healing. Dermal fibroblasts are a good candidate for tendon tissue engineering because they are similar to the tenocytes used for collagen synthesis. In addition, they are easily accessible because autologous dermal fibroblasts can be obtained from individual skin without major skin defects and allogenic dermal fibroblasts (ADFs) have already been commercialized in the field of skin engineering. Purpose: To determine the effects of dermal fibroblasts on tendon-to-bone healing in a rabbit model of a chronic rotator cuff tear. Study Design: Controlled laboratory study. Methods: A total of 33 rabbits were randomly allocated into 3 groups (n = 11 each). Supraspinatus tendons were detached and left for 6 weeks to establish a chronic rotator tear model. Torn tendons were repaired in a transosseous manner with the injection of 5 × 106 ADFs with fibrin in group A, fibrin only in group B, and saline only in group C. At 12 weeks after repair, the mechanical test and histological evaluation were performed. Results: Seven rabbits died before the evaluation (1 in group A, 2 in group B, 4 in group C). In the final evaluation, the mean ± SD load to failure was 48.1 ± 13.3 N/kg for group A, 34.5 ± 8.9 N/kg for group B, and 31.1 ± 8.3 N/kg for group C, and group A showed significantly higher load-to-failure values than the other groups ( P = .011). The midsubstance tear rate, which presented stronger tendon-to-bone healing than insertional tear, was 50.0% in group A, 22.2% in group B, 28.6% in group C, but the differences were not statistically significant ( P = .413). In the histological evaluation, group A showed greater collagen fiber continuity and better orientation than the other groups. Conclusion: This controlled laboratory study verified, on the basis of biomechanics and histology, the potential for the use of ADFs in rotator cuff healing. The current results suggest a new biological supplement to increase the rate of rotator cuff healing. Clinical Relevance: The most important finding of this study was the potential for a new biological supplement to enhance rotator cuff healing—a continuing challenge.

With the object of proving whether seqeuntial or alternate forms of chemotherapy would be advantageous one over the other in treating advanced breast cancer and with the purpose of evaluating two different anthracyclines at equimolecular doses in the above-mentioned alternating regimens, 250 patients who had received no prior chemo- or hormonotherapy were entered in a prospective randomized trial. Group A was administered 4-epiadriamycin and cyclophosphamide for 8 courses, followed by 6 cycles of CMF, and medroxyprogesterone acetate (MPA) from the beginning of therapy until progression. In group B, adriamycin + cyclophosphamide were alternated with CMF every two courses until 14 cycles were completed. Group C received 4'-epiadriamycin + cyclophosphamide alternated with CMF for 14 courses. In groups B and C, MPA was administered as in group A. Two hundred and twenty-four patients were evaluated. CR+PR were observed in 55.8 % of group A, 43.4 % of group B, and 46.4 % of group C. Median duration of responses was 16 months (m) in group A, 13 m in group B and 20 m in group C., and median survival (CR + PR) was 16.5 m in group A, 16 m in group B and 24 m in group C. There were no statsitically significant differences among the three groups in terms of response rate, duration of response and survival; furthermore, toxicity was moderate in all groups. At equimolecular doses there were no differences between adriamycin and epirubicin in the alternating schedules.

Background: Osteoarthritis (OA) is the most common form of arthritis accounting for about 30% of general physician visits. Objectives: To detect the effects of ultrasound therapy on patients with osteoarthritis of the knee joint.Methodology: It was a prospective non randomized clinical study, conducted in the Department of Physical Medicine and Rehabilitation, Bangabandhu Sheikh Mujib Medical University, Shahbagh, Dhaka during the periods of 1st March, 2012 to 31st August, 2012. Fifty four patients between 35-75 years of age without consideration of gender with a history of not less than three months knee pain with radiographic confirmation of primary osteoarthritis were selected purposefully. Then they were divided randomly in group A & group B. Group A received g NSAID [Aceclofenac 100mg B.D.-10 days]+Omeprazol (20mg B.D.-10days) + Therapeutic exercise + ADL and Group B received gUST+NSAID [Aceclofenac 100mg B.D.-10 days]+ Omeprazol (20mg B.D.-10days) + Therapeutic exercise + ADL. In both groups the patients were observed for six weeks. Outcome measured by VAS, ROM, 50 feet walking time and WOMAC index.Results: Mean ± SD of age in group A and group B were 52.33 ± 9.62 years and 52.29 ± 9.67 years respectively. Among the 27 patients in group A 9 (33.3%) were male and 18 (66.7%) were female. Among the 27 patients in group B 10 (37.0%) were male and 18 (63.0%) were female. Mean ± SD of VAS during pre treatment in group A and group B were 6.22 ± 1.60 and 7.15 ± 1.56 respectively. Mean ± SD of ROM during pre treatment in group A and group B were 117.33 ± 13.05 and 112.37 ± 19.01 respectively. Mean ± SD of time taken to walk 50 feet during pre treatment in group A and group B were 18.22 ± 2.39 and 18.81 ± 2.13 minutes respectively. Mean ± SD of WOMAC index in group A and group B were 60.85 ± 15.86 and 67.33 ± 16.33 minutes respectively. After treatment in both groups VAS, time taken to walk 50 feet and WOMAC index gradually decreased and ROM gradually increased.Conclusion: Although the study conducted with small sample size in a single centre in Dhaka city which may not be representative for the whole country it was found that there was statistically significant difference found in VAS, ROM, and time taken to walk 50 feet and WOMAC index between the groups.KYAMC Journal Vol. 7, No.-2, Jan 2017, Page 750-757
