The quality assurance program has 3 components, namely standards, accreditation and continuous quality improvement using the management cycle in order to maintain and or improve quality. However, the weakness of the quality assurance system due to the application of standards has not become a culture for internal customers in carrying out services in accordance with service indicators set by the quality team at the time of accreditation. The purpose of the study was to analyze the performance of the Quality Team at the Binjai City Health Center in 2021. The research design used was qualitative with a descriptive approach. The research informants were 8 main informants and 2 additional informants. Data collection methods are primary, secondary and tertiary data. Data collection techniques were carried out by means of in-depth interviews and documentation. Data analysis techniques in this study are reduction, display and conclusion or verification. The results showed that education was not in accordance with the main duties and functions, lack of understanding of quality assurance, facility resources were optimal, the leadership style of the head of the puskesmas was not in accordance with the management function of the puskesmas, the team perceived that they were carrying out their duties in accordance with their main functions, the attitude of not wanting to make changes, limited human resources education, lack of continuous training, lack of teamwork, attention, two-way communication, and the absence of a post-accreditation mentoring team.

The purpose of this qualitative case study was to explore quality improvement strategies senior manufacturing production managers use to reduce Cost of Poor Quality (COPQ) to increase profit. The participants for this study were production managers within a manufacturing company located in the southeastern region of the United States who successfully developed and implemented strategies to lower COPQ to increase profitability. Six major themes emerged from the study: continuous improvement, quality assurance, employees as agents of quality improvement, communication between stakeholders, holding all firm members accountable for quality, and training. Manufacturing managers can use these strategies to lower COPQ and increase profits, which could result in enhancing other organizations’ financial performance. Findings from this study may enable manufacturing managers to improve organizational performance when continuous quality improvement processes are implemented throughout the manufacturing process and senior leaders champion lessons learned, support the training program approach, and implement a quality assurance program that empowers frontline employees as agents of quality throughout the manufacturing process.

277 Background: MRI/US guided biopsy (fusion biopsy) is increasingly utilized over systemic 12-core transrectal ultrasound biopsy (12-core biopsy) for men with MRI-visible prostate lesions. Methods: Patients with MRI visible prostate lesions who underwent fusion and 12-core biopsy from 2016-2020 in the Intermountain Healthcare (IHC) system were consecutively analyzed. This was in the setting of a continuous quality assurance initiative among the reading radiologists. Primary outcome was PCa detection defined by Gleason grade group (GGG) 1 or higher. Clinically significant cancer (CSC) was defined as GGG 2 or higher. Patients were stratified by date biopsy was performed, 2016-2017 and 2018-2020, and lesions were stratified by PI-RADS v2 category. For men with multiple lesions, the highest PI-RADS v2 category lesion was used. Results: A total of 142 men with 254 MRI-detectable lesions underwent both fusion and 12-core biopsies in the IHC system from 2016 to 2020. CSC was detected in 21.6% (55/254) of fusion biopsies. Comparing PI-RAD v2 categories 1-3 to PI-RADS v2 categories 4-5, the PPV for detecting CSC was 9% (15/162) compared to 44% (40/92) respectively. Fusion and 12-core biopsies were concordant for any PCa in 79% of men (112/142) and CSC in 83% (118/142). Fusion biopsy detected any PCa in 22/84 (26%) and CSC in 15/103 (15%) of men in whom 12-core biopsy was negative. 12-core biopsy detected any PCa in 8/70 (11%) and CSC in 9/97 (9%) of men in whom fusion was negative. In total, 15 patients (11%) had a CSC that would have been missed if fusion biopsy was omitted while 9 (6%) had a CSC that would have been missed without 12-core biopsy. Conclusions: Omitting fusion or 12-core biopsy for PI-RADS v2 lesions would have resulted in a missed CSC in 11% or 6% of patients from 2016-20, respectively. The combination of MRI/US-guided fusion biopsy and systematic 12-core biopsy increased detection rate of CSC. These results are in the setting of a continuous, multi-disciplinary quality assurance program and results are not necessarily applicable to other healthcare systems. [Table: see text]

This study assessed the effect of the American Hotel and Motel Association sponsored quality assurance programs on the following groups of success variables: operating profit, guest satisfaction, operational factors, and employee satisfaction. Comparisons were made between a group of hotels which the researcher identified as having adopted the AH&MA's quality assurance program and a similar group of hotels which did not. In addition, the leadership adaptability and effectiveness scores and basic leadership style of the general managers of both groups of hotels were assessed to see if a significant difference existed between the two groups of hotels.

We present an overview of key aspects of the Atmospheric Radiation Measurement (ARM) Program Climate Research Facility (ACRF) data quality assurance program. Processes described include instrument deployment and calibration; instrument and facility maintenance; data collection and processing infrastructure; data stream inspection and assessment; problem reporting, review and resolution; data archival, display and distribution; data stream reprocessing; engineering and operations management; and the roles of value-added data processing and targeted field campaigns in specifying data quality and characterizing field measurements. The paper also includes a discussion of recent directions in ACRF data quality assurance. A comprehensive, end-to-end data quality assurance program is essential for producing a high-quality data set from measurements made by automated weather and climate networks. The processes developed during the ARM Program offer a possible framework for use by other instrumentation- and geographically-diverse data collection networks and highlight the myriad aspects that go into producing research-quality data.

Cultural value is expected in accordance with the local wisdom of the city of Surakarta as a city that preserves the culture and cultural values as a crutch to improve the welfare of merchants this research using descriptive qualitative method with purposive sampling and unit analysis of traders antiques merchants motor parts, Heirloom traders, scales traders. Data collection using in-depth interviews. The result of the Culture which is the philosophy that developed in the merchant community and still used as one of the trading principles is "kintir ora keli" which means although following the current trade flows or developing trend but not to lose its trade identity. There is also "jeneng first new jenang" which means to be successful in trading must find a good name first in the eyes of buyers by providing the best service and quality assurance commodities, new customers will get a lot of fortune.

This study is intended to determine the leadership style of the village head in Balaroa, Sub-District West Palu, Palu. This type of research is descriptive. The types of data used are primary data and secondary data with data collection techniques namely observation, in-depth interviews, and documentation. While the theory used in this research is using the theory of Harsey and Blancherd (in Harbani Pasolong 2008) the indicators are: Instruction, Consultation, Participation, and Delegation. While the informants involved in this study were 5 people, four village apparatus and one community member. Based on the results of the research obtained by the author in the field, that the Leadership Style of the Lurah in Balaroa, Sub-District West Palu can be categorized as good, but the village apparatus has not been able to carry out their duties optimally in accordance with the leadership style of the Lurah, indicators that need to be a concern to be able to realize this style. The expected leadership for the apparatus is the Delegation aspect and the Control aspect, it is hoped that in the future the apparatus can carry out directions and tasks according to the leadership style that the Lurah has applied to his apparatus.

The validity of computer-entered and computer-generated toxicology data is currently of concern to the quality assurance professional. In order to insure this validity, a suitable quality assurance program must be devised. This program should include (1) maintenance of standard operational procedures for both system maintenance and operation, (2) documentation of program changes, (3) inspection of automated data collection, (4) audit of acquired data, (5) establishment of the data trail, including the use of intermediate printouts and inspection of edit procedures, (6) validation of security procedures, and (7) documentation of system failure.

Abstract The techniques of performing a tracheostomy has transformed over time. Percutaneous tracheostomy is gaining popularity over open tracheostomy given its advantages and as a result the number of bedside tracheostomies has increased necessitating the need for a Percutaneous Tracheostomy Program. The Percutaneous Tracheostomy Program at the Johns Hopkins Hospital is a comprehensive service that provides care to patients before, during, and after a tracheostomy with a multidisciplinary approach aimed at decreasing complications. Education is provided to patients, families, and health-care professionals who are involved in the management of a tracheostomy. Ongoing prospective data collection serves as a tool for Quality Assurance.
