Background: The existing treatment for Osteoarthrits (OA)  is symptomatic by NSAIDS are effective for reducing pain associated with OA, they do not reverse the disease. This makes way for the emergence of nutraceuticals like collagen peptide which are chondroprotective in nature. Objectives: To assess the prevalence of knee OA, efficacy of combination of collagen peptide, aceclofenac and paracetamol against aceclofenac and paracetamol alone in the treatment of osteoarthritis, the medication adherence and drug related problems in OA. Methods: A prospective observational study was carried out in the outpatients of Department of Orthopaedics, BMCH & RC and Ambarish’s Clinic, Chitradurga. A total of 154 patients suffering from OA were identified. 60 subjects of Grade I knee OA were selected for the study from 112 cases of knee OA. The subjects were divided in two groups; Group A comprised of 30 subjects who were treated with collagen peptide, aceclofenac and paracetamol (Acetaminophen) for a period of two months, with a follow up of one month and the therapy was assessed by Visual Analogue Scale(VAS). Whereas Group B of 30 subjects who were treated with aceclofenac and paracetamol for  two months with a follow up of one month and assessed by VAS. Results:  The prevalence of knee osteoarthritis was found to be 72.72%.Among them Grade I was 77.67%. From this 60 patients were selected for further study of which 90% were females and 10% males. At the end Group A showed better results than Group B. Acetaminophen and Aceclofenac related gastrointestinal disturbances were found in both the groups. Majority of the patients also had good medication adherences. Conclusion: The study identifies and supports the use of collagen peptide to provide symptomatic relief and can be used as adjunct therapy for OA management. Introduction. In the treatment of various orthopedic conditions, regenerativetherapies, including platelet rich plasma and autologous stem-cell therapy, have recently been advancing. Knee cartilage lesions are a debilitating disease resulting in fibrillation and subsequent degradation which can also involve the subchondral bone and lead to the development of osteoarthritis. Bone marrow mesenchymal stem cells are a heterogeneous mixture of cells involved in cartilage formation and regenerative repair, whereas other mesenchymal stem cells have the capacity to play a role as immunomodulatory and trophic factors. Nowadays, stem-cell therapy is widely used for the treatment of knee osteoarthritis and cartilage lesions. The purpose of this study was to evaluate preliminary clinical data of treatment of knee osteoarthritis with stem cell injection and treatment of osteochondral lesions with stem-cell scaffold. Material and Methods. Stem cells were obtained by concentrating the content taken with aspiration needles from the bone marrow my means of Arthrex Angel Bone Marrow Aspirate Concentrate centrifuge. Results. The study sample consisted of 39 patients who were included in knee osteoarthritis treatment. Surgical implantation was performed in 7 patients from the osteochondral group. In the first group, an average Visual Analogue Scale pain felt before intervention decreased statistically significantly three days after the intervention (from 7.27 to 2.12, p?0.05) and remained 1.2 until the check-up after 3 months. The same results were achieved in the average Western Ontario and McMaster Universities Arthritis Index score (prior to intervention = 51.5; after 1 month = 72 and after 3 months = 76). For the second group, an average preintervention Visual Analogue Scale pain decreased statistically significantly three days after intervention (from 8.1 to 2.7) and remained 1.2 until the check-up after months. The same results were achieved on the Knee injury and Osteoarthritis Outcome Score pain (p?0.05). Both procedures were proved as safe providing pain relief and function improvement of treated knee joints.

Objective: This descriptive cross-sectional study was carried out to investigate the relationship between fatigue, and sleep quality, pain and depression in patients with knee osteoarthritis. Methods: A cross-sectional, correlational design was used. The study sample comprised 151 individuals with knee osteoarthritis and 147 healthy individuals. To collect the study data, the Sociodemographic Characteristics Questionnaire (SCQ), Visual Analogue Scale to Evaluate Fatigue Severity (VAS-F), Pittsburgh Sleep Quality Index (PSQI), Visual Analogue Scale and Beck Depression Inventory (BDI) were used. To analyze the study data, appropriate tests were used. Results: It was found that the patients with knee osteoarthritis had higher fatigue scores, and higher PSQI and BDI total scores than did the healthy participants, and that the difference between them was significant (p<0.05). There was a positive correlation between fatigue score and PSQI, VAS and BDI scores (p<0.05). Conclusion: The participants with knee osteoarthritis suffered fatigue more than did the healthy participants. This fatigue suffered by them affected their sleep quality, pain and depression negatively. doi: https://doi.org/10.12669/pjms.35.4.383 How to cite this:Fertelli TK, Tuncay FO. Optimizing K-Means Initial Number of Cluster Based Heuristic Approach: Literature Review Analysis Perspective One popular clustering technique - the K-means widely use in educational scope to clustering and mapping document, data, and user performance in skill. K-means clustering is one of the classical and most widely used clustering algorithms shows its efficiency in many traditional applications its defect appears obviously when the data set to become much more complicated. Based on some research on K-means algorithm shows that Number of a cluster of K-means cannot easily be specified in much real-world application, several algorithms requiring the number of cluster as a parameter cannot be effectively employed. The aim of this paper describes the perspective K-means problems underlying research. Literature analysis of previous studies suggesting that selection of the number of clusters randomly cause problems such as suitable producing globular cluster, less efficient if as the number of cluster grow K-means clustering becomes untenable. From those literature reviews, the heuristic optimization will be approached to solve an initial number of cluster randomly.
