OBJECTIVES: To determine the frequency of acute and chronic hepatitis HBeAg positivity in patients ofdifferent ages and gender suffering from Hepatitis B Virus infection, and to rectify the key factorsresponsible for its transmission.METHODOLOGY: Cross sectional study conducted for duration of 3 months at tertiary care hospitals ofPeshawar on 220 patients suffering from hepatitis B infection that are screened for serum markers.nonprobability convenient sampling technique was used and structured questionnaire was used as datacollection tool. Patient’s record was checked for serum markers. Data collected was analyzed by SPSSversion 16.RESULTS: Out of total 220 cases with hepatitis B infection 145 were male while 75 were females.103(46.8%) were found to be positive with raised level of serum marker HBeAg, predominantly more inmale gender and young age patients with mean age of 22 years. The serum level of (HBeAg) was high inpatients having chronic hepatitis as compare to acute hepatitis. Patients with positive HBeAg weresignificantly associated with raised level ofALT.CONCLUSIONS: The frequency of HBe Ag+ve in patients suffering from hepatitis B infection is 46.8%with more predominant in young age patients associated with raised level ofALT.RECOMMENDATIONS: Health education of the community regarding preventive measures fromhepatitis B infection and early diagnosis through mass screening program is recommended.KEYWORDS: Serum marker HBeAg, acute hepatitis, chronic hepatitis, incubation period, ALT.

A liver biopsy is the gold standard for diagnosing and staging liver fibrosis in chronic hepatitis B patient but with many disadvantages so it is difficult to use as a routine examination. Research on index based on serum marker is helpful in predicting liver fibrosis. We conducted a study of the alpha-fetoprotein (AFP) and partial activated thromboplastin time (APTT) index or AA index in relation to the degree of liver fibrosis in chronic hepatitis B patients. This study was a retrospective cross-sectional study in dr.Wahidin Sudirohusodo Hospital Makassar by taking the data from medical records of chronic hepatitis B patients from January 2015 to December 2016. Samples were chronic hepatitis B patients who were tested for AFP, APTT and Fibroscan. The result showed the total of 79 chronic hepatitis B patients, 23 with severe fibrosis, 9 with moderate fibrosis, 26 with mild fibrosis, and 21 with no fibrosis. Spearman correlation test showed a significant correlation between the AA index and the degree of fibrosis (p <0.001) and showed a positive correlation between both of them with a very strong correlation (r = 0.830). Oneway Anova test showed a significant difference between the AA index and the  degree of fibrosis (p<0.001). The results of this study indicate that the AA index can be used as a predictor of fibrosis in chronic hepatitis B patients. It is suggested to do another study with larger sample based on the degree of fibrosis.

Background: Elucidating differences in HLA DRB1* genes distribution may be useful in understanding the molecular pathogenesis of viral hepatitis B.Objective: The aim of the study was to find out the HLA DRB1*15 gene susceptibility among acute and chronic Hepatitis B infected Bangladeshi patients.Methodology: This cross-sectional study was carried out in the Department of Virology at Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Bangladesh during the period of June 2012 to June 2013 for a period of one year. Evaluation HLA DRB15*gene distribution was performed among acute hepatitis B and chronic hepatitis B infected (HBV) Bangladeshi patients. HLA DRB15*gene distribution was detected by conventional PCR followed by agarose gel electrophoresis.Result: A total of 60 serologically pre-diagnosed 30 acute hepatitis B and 30 chronic hepatitis B infected (HBV) Bangladeshi patients were evaluated. The study revealed a significant increase of DRB1*15 allele among chronic hepatitis B infected patients compared to acute hepatitis B (46.7% vs 13.3%; RR=5.8, X2 test=7.2; P< 0.05). This is the first report to investigate HLA DRB1* gene associations among acute and chronic HBV infected Bangladeshi patients.Conclusion: In conclusion HLA DRB1*15 is more frequent in chronic hepatitis B infected Bangladeshi patients compared to acute hepatitis B.Bangladesh Journal of Infectious Diseases, June 2018;5(1):3-9
