OBJECTIVE: To study the incidence of renal colic attacks due to renal stone and its relationship withclimate during different months of the year.MATERIAL AND MONTHS: This prospective study was conducted on patients with renal colicsecondary to stones, who were admitted in the Institute of Kidney Diseases Hayatabad Peshawar betweenOctober 2010 & September 2013.RESULTS: This study included 980 patients, 765 (78.06%) were male and 215 (21.93%) were female.Themean age of the patients was ( 33.1± 9.8years). Majority of the patients 807(82.4%) were under 40 years ofage. Renal colic was more common in summer months like June 118 (12%), July 111 (11.3%) August 109(11.1%) and September 88 (9%) respectively. The lowest no of admission was in February 40 (4.1%),January43 (4.4%), and March 45 (4.6%). It shows that the mean admission of the year due to renal colicsecondary to stone was significantly different from mean admission of the warmer months 578 patients(59%) in comparison to winter month 402 patients (41%). It shows that there was 18% increased incidenceof renal colic visits in the warmer months of June, July and august compared with the colder months ofDecember,January and February.CONCLUSION: The renal colic visit was more in the summer and highest in the month of June, July,August and declining start from September onward and lowest in February. We found a strong positivecorrelation between ambient temperature and renal colic visits.

Objective To test the hypothesis that there is seasonal variation in the rates of gestational diabetes (GDM) diagnosed using a 2 hour oral glucose tolerance test. Design Monthly assessment of the percentage of women screened from 1st April 2016 to the 31st December 2020 who were diagnosed as having gestational diabetes Setting London Teaching Hospital Population 28,128 women receiving antenatal care between April 1st 2016 and 31 December 2020. Methods Retrospective study of prospectively collected data. Main Outcome Measures Proportion of women screened diagnosed as having gestational diabetes. Results The mean (SD) percentage of women diagnosed with GDM was 14.78 (2.24) in summer (June, July, August) compared with 11.23 (1.62) in winter (p < 0.001), 12.13 (1.94) in spring (p = 0.002), and 11.88 (2.67) in autumn (p = 0.003). There was a highly significant positive correlation of the percentage testing positive for GDM with the mean maximum monthly temperature (R2 = 0.248, p < 0.001). There was a statistically significant 33.8% increase in the proportion of GDM diagnoses from June 2020 onwards, possibly related to a reduction in exercise secondary to the Covid-19 pandemic. Conclusions There is a 23.3% higher rate of GDM diagnoses in the warmer summer months. There has been a 33.8% rise in GDM diagnoses associated with the Covid-19 pandemic.

Intisari Keberadaan pergeseran puncak curah hujan diurnal (DR) terhadap Madden-Julian Oscillation (MJO) aktif di Maritime Continent (MC) masih diperdebatkan sehingga studi ini bertujuan untuk menginvestigasi perubahan tersebut. Selain itu, intensitas rata-rata dan amplitudo DR juga dikaji dalam penelitian ini berbasis GSMaP Gauge-Calibrated V7. Komposit anomali intensitas rata-rata (Ra), amplitudo (Rax) DR MJO aktif dan perbandingan fase puncak DR MJO aktif terhadap klimatologinya (Pax-Pm) pada periode Desember-Januari-Februari (DJF), Maret-April-Mei (MAM), Juni-Juli-Agustus (JJA) dan September-Oktober-November (SON) digunakan dalam tulisan ini dengan uji-z 80%. MJO aktif berbasis rekonstruksi outgoing longwave radiation (OLR) dari kedua indeks realtime multivariate MJO (RMM). Hasil memperlihatkan bahwa MJO aktif memodulasi peningkatan intensitas rata-rata dan amplitudo DR di lautan dan mempengaruhi pergeseran puncak DR menjadi lebih cepat 1 jam dari klimatologi musimannya. Abstract The occurrence of peak phase shift of diurnal rainfall (DR) to active Madden-Jullian Oscillation (MJO) has been debatable, so this study is aimed to investigate the change. Moreover, the mean and amplitude intensity of DR were also analyzed in this study based on GSMaP Gauge-Calibrated V7. The composite of the mean (Ra) and amplitude (Rax) intensity anomaly of DR, and the comparison of DR peak phase during the active MJO to its climatology (Pax-Pm) in the period December-January-February (DJF), March-April-May (MAM), June-July-August (JJA), and September-October-November (SON) were used in the study with the z-test of 80%. The active MJO was based on reconstructed outgoing longwave radiation (OLR) of two real-time multivariate MJO (RMM) indexes. The results showed that active MJO modulated the increased mean and amplitude intensity of DR over the ocean and influenced the DR peak phase shift to be faster than its seasonal climatology by one hour.

This study investigates the individual and combined impacts of El Niño and the positive Indian Ocean Dipole (IOD) on the Southeast Asia (SEA) rainfall variability. Using composite and partial correlation techniques, it is shown that both inter-annual events have individually distinct impacts on the SEA rainfall anomaly distribution. The results showed that the impacts of the co-occurrence of El Niño and IOD events are significant compared to the individual effects of pure El Niño or pure IOD. During June-July-August and September-October-November, the individual impacts of the pure El Niño and IOD events are similar but less significant. Both events caused negative impacts over the southern part of SEA during June-July-August (JJA) and propagated northeastward/eastward during September-October-November (SON). Thus, there are significant negative impacts over the southern part of SEA during the co-occurrence of both events. The differential impacts on the anomalous rainfall patterns are due to the changes in the sea surface temperature (SST) surrounding the region. Additionally, the differences are also related to the anomalous regional atmospheric circulations that interact with the regional SST. The anomalous Walker circulation that connects the Indian Ocean and tropical Pacific Ocean also plays a significant role in determining the regional anomalous rainfall patterns.

Abstract Background The goal for using smaller caliber instruments in PNL was to reduce the access-related complications and to decrease morbidity. The objective of this study was to evaluate the safety and efficacy of Chinese minimally invasive percutaneous nephrolithotomy (MIPNL) in the treatment of renal stones ≤ 20 mm. Results Sixty-seven patients completed the study protocol. The mean age was 41.10 ± 13.99 years (range 18–68 years). There were 43 (64%) male and 24 (36%) females. The mean stone size was ranged from 78.5 to 439.6 mm2 (mean ± SD 172.48 ± 69.54 mm2). The overall SFR was (82%). Twelve (18%) needed post-MIPNL auxiliary procedure, in the form of second MIPNL in 3 (4.5%) cases, SWL in 7 (10%), and RIRS in 2 (3%) cases. The intraoperative complication was present in four patients (5%) include bleeding necessitate blood transfusion in one patient (1.5%) and renal collecting system perforation 3 (4.5%). The postoperative complication was urine leakage 5 (7.5) and fever in 6 (9%) of patients. Conclusion Chinese MIPNL is safe and effective method for treatment of renal stone ≤ 20 mm size with satisfactory SFR and low complication rate when SWL failed or contraindicated. It is considered a feasible treatment alternative to standard PNL, in the absence of flexible URS or miniature nephroscope.

Background: Labor is a naturally occurring physiological process associated with uterine contractions, effacement, dilatation of cervix and descent of presenting part. Drotaverine hydrochloride is a non-anticholinergic isoquinoline derivative which acts by elevating intracellular cyclic Adenosine Mono Phosphate (cAMP) and cyclic Guanosine Mono Phosphate (cGMP) promoting smooth muscle relaxation. Materials and Method: This was a hospital based prospective study on effectiveness of Drotaverine Hydrochloride on enhancing dilatation of cervix and acceleration of active phase of labor. The sample size was 100. Fifty cases of women in active phase of labor received injection drotaverine hydrochloride 40 mg (group i) and fifty cases of women did not receive any drug (group ii) among those at term with singleton pregnancy and vertex presentation. Variables like maternal age, interval between administration of drug and delivery, mode of delivery, apgar score at 5 minutes, NCU (Neonatal Care Unit) admission and neonatal outcomes were recorded. Data analysis was done with the help of SPSS program. Results: The mean interval between drug administration to delivery in primipara and multipara in group i was 3.05 hours and 2.31 hours while in group ii was 4.5 hours and 3.75 hours respectively. The mean interval between drug administration and delivery was shorter in both groups of multipara. In group i, 96% had normal delivery and 4 % had vacuum delivery and in group ii 90% and 10% had normal delivery and vacuum delivery respectively. None of the participants had caeserian section. There were no perinatal mortalities. Conclusion: The administration of drug Drotaverine Hydrochloride is effective in shortening duration of labor with favorable feto-maternal outcome.

The radiation dose to patients and personnel was estimated during 11 percutaneous renal stone extractions. For the patients the energy imparted, the mean absorbed dose to various organs, and the effective dose equivalent were estimated. For different personnel categories some organ doses and the effective dose equivalent were also estimated. Large differences in the radiation dose between patients were observed. The mean effective dose equivalent to the patient was 4.2 (range 0.6–8.3) mSv, and the energy imparted 285 (range 50–500) mJ. These figures are comparable to those reported for routine colon examination and urography. For the personnel there were also large differences between individuals and categories. The highest radiation dose was received by the radiologist. It was estimated that a radiologist who performs 150 percutaneous renal stone extractions per year will receive a yearly contribution to his/her effective dose equivalent of 2.4 mSv. Even when the contribution from other diagnostic and interventional radiologic procedures is added, the total effective dose equivalent hardly exceeds 5 mSv or 1/10 of the present dose limit for persons engaged in radiologic work. For the hands of the radiologist there is a risk of doses closer to the present limit for single organs or tissues of 500 mSv/year.

Background: Sevoflurane is a new volatile anesthetic agent with rapid induction and recovery. A randomized study was carried to access conditions for LMA insertion using Sevoflurane in 25 ASA I & II patients undergoing short duration surgeries.Subjects and Methods:This prospective study was conducted at Department of Anesthesiology and Critical Care, SVS Medical College and Hospital, Mahabubnagar, Telangana, India. After obtaining the institutional ethics committee and written informed consent from the patients, 25 subjects of either sex were included in this study. Age of the subjects was 18 to 60 years. Patients received injection Fentanyl 1 – 2mcg/kg prior to induction. All patients were pre-oxygenated for 3 min with 100% oxygen using a fresh gas flow of 81/min. All patients received inhalational induction with 8% Sevoflurane and O2 flow at 8 L/min with single vital capacity breathe technique. Loss of verbal contact was considered as the desired endpoint for induction, which was assessed by the response to calling out the patient’s name. Then the time of loss of eyelash reflex and jaw relaxation was assessed by anesthesiologist. After adequate jaw relaxation, LMA insertion was attempted.Results:The mean loss of verbal contact was 65.40±9.67second, while the mean for time for loss of eyelash reflex and jaw relaxation were found to be 81.20±9.39 seconds and 103.20 ±12.07 seconds respectively. The mean time for LMA insertion was 122.00±15.61 and the mean attempts for successful LMA insertion was 1.12±0.33. LMA insertion was easy in 23 cases as against difficult in 2 cases. In 2 cases transient cough and biting were recorded. LMA insertion was excellent and satisfactory in 88.0 and 12 percent. However, the mean heart rate at 5 minute after induction showed a significant fall at 5 minutes after induction. The mean values of SBP, DBP and MAP did not differ significantly at pre and induction. However, a significant decrease in SBP was noticed at 1, 2 and at 5 minutes.Conclusion:Sevoflurane is an smooth inhalation anesthesia with rapid onset with adequate jaw relaxation for insertion of LMA in Adults for short duration surgeries. Sevoflurane has got good hemodynamic profile with lesser complications owing to choice of inhalation agent for insertion of LMA.

Cataract surgery is no more a blind rehabilitation surgery, it absolutely gives normal vision. In the era of modern cataract surgery patients expectations are also high about visual outcome. This prospective study was carried out to investigate the magnitude and pattern of pre-existing corneal astigmatism in age related cataract patient at Faridpur Medical College Hospital, Faridpur and Agha Yusuf Adhunik Hospital, Kustia, from July 2009 to June 2012. We examined 850 eyes of 730 patients who underwent cataract surgery. The mean age at the time of surgery was 61.9±8.1 (40 to 70) years. Corneal astigmatism was measured by Auto Refracto Keratometer at least two times for each patient. Astigmatism was calculated from diopteric difference of vertical reading from horizontal reading. With the rule (WTR) astigmatism was considered when steep meridian at 90°± 20°. Against the rule (ATR) astigmatism was considered when steep meridian at 180°±20°. Astigmatism is in other direction is defined as oblique. On keratometry, when vertical reading (k1) was found greater than horizontal (k2) was considered WTR astigmatism and the reverse reading for ATR astigmatism. The percentage of corneal astigmatism was 1D or less was 69.6%, more than 1D and less than 1.5D, 27.6% and more than 1.5D and less then 2D 2.8%. Prevalence of ATR astigmatism was more than WTR astigmatism and prevalence of ATR astigmatism increases significantly with age. OBJECTIVE: Benign acute childhood myositis is characterized by acute musculoskeletal involvement leading to transient limitations on deambulation followed by a viral illness. Our study objective to evaluate clinical and laboratory features of patients in a pediatric emergency department. METHODOS: We conducted a prospective study in patients with symptoms and laboratory findings compatible with viral myositis in the period of August 2017 to August 2018. RESULTS: We assessed 20 patients in the period of twelve months. The mean age was 8,25 years. Of these, 83,3% had infectious symptoms in the week before the musculoskeletal involvement. By the time of the diagnosis, the symptoms were: calf pain, reluctance to walk, gait abnormality, diffuse myalgia and calf weakness. The most relevant laboratory finding was the elevation of CPK (mean 3359,556U/L) level, followed by AST (mean 131U/L) and ALT (mean 64,66U/L) elevation. The mean time for symptom relief was 3 days and in 7 days all exams were normal. CONCLUSION: Though the exact incidence of this condition remains undetermined, the lower extremity pain and the gait abnormality is of concern of both parents and health care providers. We emphasize the importance of knowing this condition to avoid unnecessary exams and the delay in the diagnosis of severe conditions.
