The purpose of the study is made where the importance of the data of a medical record in a hospital either before or afterwards must be input and the document is controlled for the history of a person who enters or leaves so that the importance of not being misdiagnosed or confused in a diagnosis. Every citizen has the right to adequate health and also an important medical record in order to know the disease suffered, because a medical record is very important for every patient to know various health efforts for the entire community through the implementation of quality and affordable health development for the community. Health development is aimed at increasing awareness, willingness and ability to live a healthy life for everyone in the context of realizing an optimal degree of health as an element of general welfare as referred to in the Preamble to the 1945 Constitution of the Republic of Indonesia. The authority and obligations of midwife practices in Indonesia are regulated in the Decree of the Minister of Health of the Republic of Indonesia No.900 / Menkes / SK / VII / 2002 concerning Registration and Practice of Midwives. Delivery assistance allowed by midwives are normal and emergency deliveries, and basic health services only deal with minor cases that are within their capabilities. Implementation of midwife practice services found many problems regarding the management of medical records of pregnant women that have not been optimal, including: incomplete medical records, difficulties in searching for medical record data, less synchronous data, recording and reporting of basic health services that are often too late. This can result in a decrease in the quality of health services for pregnant mother patients until delivery. An alternative solution to this problem is to develop an electronic medical record application for pregnant women that can present a complete and accurate history of the examination and development of the health of pregnant women so as to improve the health services of pregnant women and facilitate the reporting process to related parties. The method of developing an electronic medical record application for pregnant women uses the waterfall method with stages of development: system requirements analysis, system design, implementation and system testing. The programming language used by PHP and MySQL databases. This application program consists of 1) data input for pregnant women, 2) input for examining pregnant women, 3) input medical treatment, and 4) report on examination results. The test results show that this application can store and present information about the health history of pregnant women with a valid so that it can be used as an alternative service for medical records of pregnant women that is more optimal.

Introduction: The medical record documentation of patient treatment Provides the which in turn, must be maintained Clearly, concisely, comprehensively and accurately. Medical record and its filling criteria must be based on the regulation of the Minister of Health of The Republic of Indonesia No. 269/Menkes/Per/III / 2008 regarding to the medical record. The research was Aimed to unveil the completeness of both criteria and filling on medical records at the General Hospital’s Dental Polyclinic of Cianjur District. Methods: Survey-based descriptive method was applied within the research. Its Data was acquired through the examination on medical records and interviews. Random sampling was conducted to run the sampling technique. 89 pieces of outpatient’s medical records were embodied as samples. Results: Based on the research results, it is discovered that 6 out of 12 criteria (50%) are not listed within the medical record. Thus, the filling on medical records of 100% is found incomplete. Conclusion: Medical records Dental Clinic Regional General Hospital Cianjur according to standards Permenkes No. 269/2008 not inlude on complete criteria according to standards Permenkes No. 269/2008.

In the activities of Posyandu Kutilang, Sukowiyono Village, Kec. Padas, Kabupaten Ngawi, a health check was carried out by a health center health worker, but there was no form of a toddler's medical record that could be used to document the results of a health examination. The target of this community service activity is the Posyandu Kutilang posyandu participants. Guidance and consultation was carried out by Lecturers and Students of the Department of Medical Records and Health Information at the Indonusa Polytechnic Surakarta when accompanying cadres of Posyandu Kutilang, Sukowiyono Village, Kec. Padas, Ngawi Regency in documenting the health history of toddler participants. The assistance provided includes how to fill out medical record forms for toddlers, what items must be filled in, and how to manage forms. Health checks are only recorded in the agenda book for elderly examinations belonging to the Mobile Puskesmas, so that the participants of Posyandu Kutilang, Sukowiyono Village, Kec. Padas, Kabupaten Ngawi cannot monitor the results of monthly health checks. Results of Assistance in Making Form Design and Recording Medical Records for Toddlers at Posyandu Kutilang, in the hall of Posyandu Kutilang, Sukowiyono Village, Kec. Padas, Ngawi Regency was attended by 11 people. Activities started according to the agreed schedule. The training participants participated in the activity enthusiastically because they felt that this activity had a role in administrative order through the implementation of recording in the form / resume of toddler medical records.

The article is devoted to the burning problem that arose in 2014 in the Ukraine, in the regions of Lugansk and Donetsk, and that concerns the right of the people of Donbass to self-determination. This problem is not only of a local territorial nature, but it is also one of the most complex debatable problems of international law. Since the right to self-determination contradicts the principle of territorial integrity of the state, the consideration and solution of this issue is the most burning for the whole population living on the territory of the self-proclaimed people's republics of Lugansk and Donetsk. In the article, the authors analyse the concept of "self-determination of the people" and give a generalised characteristic of it, approving that it is the right of every nation to solve the issues of state structure, political status, economic, social and cultural development independently and at its own discretion. The author also examines the historical past of the people of Donbass, where, in terms of the Republic of Donetsk and Krivoy Rog and various documentary historical and legal materials, we come to the conclusion that the population of Donbass has the right to social, economic, cultural, spiritual and other development just as all the recognised countries of the world.

The Republic of Suriname (South America) is among the culturally, ethnically, and religiously most diverse countries in the world. Suriname’s population of about 600,000 consists of peoples from all continents including the Javanese who arrived in the country between 1890 and 1939 as indentured laborers to work on sugar cane plantations. After expiration of their five-year contract, some Javanese returned to Indonesia while others migrated to The Netherlands (the former colonial master of both Suriname and Indonesia), but many settled in Suriname. Today, the Javanese community of about 80,000 has been integrated well in Suriname but has preserved many of their traditions and rituals. This holds true for their language, religion, cultural expressions, and forms of entertainment. The Javanese have also maintained their traditional medical practices that are based on Jamu. Jamu has its origin in the Mataram Kingdom era in ancient Java, some 1300 years ago, and is mostly based on a variety of plant species. The many Jamu products are called jamus. The first part of this chapter presents a brief background of Suriname, addresses the history of the Surinamese Javanese as well as some of the religious and cultural expressions of this group, focuses on Jamu, and comprehensively deals with four medicinal plants that are commonly used by the Javanese. The second part of this chapter continues with an equally extensive narrative of six more such plants and concludes with a few remarks on the contribution of Javanese jamus to Suriname’s traditional medicinal pharmacopeia.

The right to confidentiality or privacy is something unique between a patient's relationship with a doctor because it is based on trust. This patient-doctor relationship is unique because the patient believes in the doctor's ability and the patient believes the doctor will keep the patient's secret. The influence of the covid 19 outbreak has made people nervous and afraid because the virus is very easy to spread and spreads globally. The community asks to open data related to the secrets of Covid 19 patients, both in the form of names, addresses, symptoms, diagnosis, and interactions that have been made. Patient data which is classified as a basic right to privacy is an important component in breaking the chain of the spread of Covid 19 because the government has implemented large-scale social restrictions in certain areas to maintain social interactions so that the community can avoid exposure to this virus. This needs to be studied in depth regarding the Forms of Disclosure of Medical Secrets in the PANDEMIC Covid 19 Situation in Indonesia.This type of research is normative juridical research or what is known as doctrinal research. Normative juridical research is research that looks at problems through the aims and objectives to be achieved using surgical tools in the form of statutory regulations and regulations in a formal hierarchy of regulations as well as theoretical studies of updated and modern literature.Covid 19 Patient Information is data stored in medical records that can be provided based on patient consent or the law. But in Article 57 paragraph (2) of Health Law, the right to confidentiality is excluded, one of which is for the benefit of society. The doctor's obligation to keep information related to the patient's medical condition is an obligation as described in the Republic of Indonesia Minister of Health Regulation Number 269 / MENKES / PER / III / 2008 concerning Medical Records but in the event of an outbreak or pandemic covid 19, the disclosure of secrets is carried out with the covid procedure, namely by evaluating the interaction of patients who have been in SWAB Positive and carry out self-isolation for COVID 19 patients without symptoms and medical isolation for patients with symptoms. Keyword: Information, Covid 19, Hukum, Privacy

Latar Belakang: Pengembangan kesehatan berkelanjutan adalah proses untuk memberikan hak atas kehidupan yang sehat yang harus diperoleh oleh masyarakat yang berguna untuk memperoleh kesejahteraan bagi masyarakat dalam menjalankan kehidupan yang sehat. Konsep pendekatan dalam upaya menangani kesehatan populasi mengalami banyak perubahan sejalan dengan pemahaman dan pengetahuan kita tentang bagaimana masyarakat hidup dan menghormati bahwa kesehatan adalah "Sumber Daya Manusia" yang bernilai sangat besar. Dan tujuan dari penelitian ini yaitu untuk melihat bagaimana faktor yang mempangaruhi dalam melakukan pembangunan kesehatan berkelanjutan.Metode: Metode penelitian yang digunakan adalah penelitian deskriptif yaitu melihat hasil penelitian sebelumnya dan selanjutnya dikembangkan kembali. Teknik pengumpulan data studi literatur ini dari beberapa buku, surat kabar, jurnal, catatan, undang-undang dan media informasi lainnya yang relevan dengan masalah yang diteliti dan observasi, termasuk observasi langsung baik pada subjek yang diteliti maupun wawancara.Hasil: Pemerintah belum mampu menangani masalah yang akan dihadapi terkait kesehatan karena dalam penanganan pemerintah yang masih kurang dikarena banyak kekurangan seperti alat kesehatan dan juga peraturan/regulasi terkait pembangunan kesehatan berkelanjutan yang belum dapat dikatakan efisien pada pertumbuhan kesehatan atau pembangunan kesehatan yang merupakan tujuan pembangunan berkelanjutan atauSubtanable Development Goal’s(SDG’s).Kesimpulan: Peran dari pemerintah dari tahun ke tahun belum dapat bisa melihat adanya kemajuan dalam mengatasi masalah yang harus diantisipasi sebelum masalah terjadi baik dalam kematian ibu, kekurangan gizi untuk anak-anak dan wanita hamil, dan lain-lain. Yang dimana diharapkan dapat memberikan solusi terhadap permasalahan dari kesehatan. ABSTRACT Title: Analysis of Sustainable Health Development in Indonesia (Sustainable Development Goal's).Background: Sustainable health development is a process to provide the right to a healthy life that must be obtained by the community that is useful for obtaining welfare for the community in running a healthy life. The concept of approach in the effort to handle the health of the population underwent many changes in line with our understanding and knowledge of how a society lives and respects that health is a "Human Capital" of enormous value. Method: The research method used is a descriptive study that is looking at the results of previous studies and further developed again. This literature study data collection technique from several books, newspapers, journals, notes, laws and other information media that are relevant to the problem under study and observation, including direct observation both on the subject under study and interview. Results: The government has not been able to handle the problems that will be faced related to health because in managing the government which is still lacking due to many deficiencies such as medical devices and also regulations/regulations related to sustainable health development that cannot be said to be efficient in health growth or health development which is a goal of sustainable development or Sustainable Development Goal's (SDG's).Conclusion: The role of government from year to year has not been able to see any progress in overcoming problems that must be anticipated before problems occur both in maternal deaths, malnutrition for children and pregnant women, and others. Which is expected to provide solutions to problems from health.

Background: Coding is one of the competencies of the  health  information  recorder  which has  a  very  important  role  in  supporting  the  improvement  of  the  quality  of health services in accordance with the republic of Indonesia decree  No. 377/Menkes/SK/III/2007 regarding the professional standards of medical record  and health information, medical recorders must be able to establish codes for diagnosis of disease and medical treatment appropriately. The accuracy of coding is related to financing claims, especially for hospitals that work with health service providers such as health insurance. The purpose of this study is to analyze the accuracy of coding based on international classification of diseases the 10th revision (ICD-10).Methods: Research using descriptive methods with a qualitative approach. The data collection technique used is the observation method that is direct observation of the medical record file. 56 medical records were randomly selected and recoded blindly (as gold standard). Processing statistical data using pivot tables and for coding analysis using ICD-10.Results: Accurate diagnosis code based on the ICD-10 is 14 (25%) and an inaccurate 42 (75%) of 56 diagnoses in the medical record file.  The most inaccurate code found is the fourth character with 22 codes.Conclusions: The inaccuracy of coding at hospital X in Padang was caused among others by the doctor's writing that was not clearly read, errors in the selection in sub categories and in the selection of the character code. In addition, people who work in the medical records section are generally not from a medical record background.
