Interobserver Agreement in the Study of 2D and 3D Sonographic Criteria for Adenomyosis

Objective To study the degree of interobserver agreement for already known two-dimensional (2D) and three-dimensional (3D) sonography criteria of adenomyosis. Methods Fifty cases (25 adenomyosis cases, 25 non-adenomyosis cases) were studied by four expert observers and a less-experienced researcher. Each sonographic adenomyosis criterion was separately studied. Interobserver agreement was analysed by the kappa index with 95% confidence interval (CI). Results The degree of agreement about diagnosing adenomyosis was high or very high in all pair-wise comparisons (kappa values of 0.58–0–92). The agreement on identifying intra-myometrial cysts (kappa values of 0.46–0.79) and evaluating the junctional zone (JZ) by 3D-transvaginal ultrasound (3D-TVS) (kappa index of 0.46–0.88) was good. The agreements reached when evaluating the other criteria varied, but good agreement was generally reached. Conclusions Both the 2D-TVS and 3D-TVS criteria employed in diagnosing adenomyosis were reproducible parameters in adenomyosis diagnostics. The results were reproducible, even by a less experienced observer.

A Monte Carlo procedure has been used to study the ordering of both two and three dimensional (2d and 3d) Potts Hamiltonians, further to the work of Anderson et al. For the 3d lattice, the short time growth rate is found to be much slower than previously reported, though the simulated microstructure is in agreement with the earlier studies. We propose a new stochastic model that gives good agreement with the simulations.

In this work, a numerical study devoted to the two-dimensional and three-dimensional flow of a viscous, incompressible fluid inside a lid-driven cavity is undertaking. All transport equations are solved using the finite volume formulation on a staggered grid system and multi-grid acceleration. Quantitative aspects of two and three-dimensional flows in a lid-driven cavity for Reynolds number Re = 1000 show good agreement with benchmark results. An analysis of the flow evolution demonstrates that, with increments in Re beyond a certain critical value Rec, the steady flow becomes unstable and bifurcates into unsteady flow. It is observed that the transition from steadiness to unsteadiness follows the classical Hopf bifurcation. The time-dependent velocity distribution is studied in detail and the critical Reynolds number is localized for both 2D and 3D cases. Benchmark solutions for 2D and 3D lid-driven cavity flows are performed for Re = 1500 and 6000.

Aim. This study aimed to evaluate the reliability of 3D-TMT, previously used only for dynamic testing, in a static cephalometric evaluation. Material and Method. A group of 40 patients (20 males and 20 females; mean age 14.2±1.2 years; 12–18 years old) was included in the study. The measurements obtained by the 3D-TMT cephalometric analysis with a conventional frontal cephalometric analysis were compared for each subject. Nine passive markers reflectors were positioned on the face skin for the detection of the profile of the patient. Through the acquisition of these points, corresponding plans for three-dimensional posterior-anterior cephalometric analysis were found. Results. The cephalometric results carried out with 3D-TMT and with traditional posterior-anterior cephalometric analysis showed the 3D-TMT system values are slightly higher than the values measured on radiographs but statistically significant; nevertheless their correlation is very high. Conclusion. The recorded values obtained using the 3D-TMT analysis were correlated to cephalometric analysis, with small but statistically significant differences. The Dahlberg errors resulted to be always lower than the mean difference between the 2D and 3D measurements. A clinician should use, during the clinical monitoring of a patient, always the same method, to avoid comparing different millimeter magnitudes.

We discuss the nonparaxial focusing of laser light into a three-dimensional (3D) spiral distribution. For calculating the tangential and normal components of the electromagnetic field on a preset curved surface we propose an asymptotic method, using which we derive equations for calculating stationary points and asymptotic relations for the electromagnetic field components in the form of one-dimensional (1D) integrals over a radial component. The results obtained through the asymptotic approach and the direct calculation of the Kirchhoff integral are identical. For a particular case of focusing into a ring, an analytical relation for stationary points is derived. Based on the electromagnetic theory, we design and numerically model the performance of diffractive optical elements (DOEs) to generate field distributions shaped as two-dimensional (2D) and 3D light spirals with the variable angular momentum. We reveal that under certain conditions, there is an effect of splitting the longitudinal electromagnetic field component. Experimental results obtained with the use of a spatial light modulator are in good agreement with the modeling results.

We investigate the effective elastic properties of disordered heterogeneous materials whose Young's modulus varies spatially as a lognormal random field. For one-, two-, and three-dimensional (1D, 2D, and 3D) rectangular blocks, we decompose the spatial fluctuations of the Young's log-modulus F=lnE into first- and higher-order terms and find the joint distribution of the effective elastic tensor by multiplicatively combining the term-specific effects. The analytical results are in good agreement with Monte Carlo simulations. Through parametric analysis of the analytical solutions, we gain insight into the effective elastic properties of this class of heterogeneous materials. The results have applications to structural/mechanical reliability assessment and design.

Abstract Objective Currently, there are up to three different classifications for diagnosing septate uterus. The interobserver agreement among them has been poorly assessed. Objective To assess the interobserver agreement of nonexpert sonographers for classifying septate uterus using the European Society of Human Reproduction and Embryology/European Society for Gynaecological Endoscopy (ESHRE/ESGE), American Society for Reproductive Medicine (ASRM), and Congenital Uterine Malformations by Experts (CUME) classifications. Methods A total of 50 three-dimensional (3D) volumes of a nonconsecutive series of women with suspected uterine malformation were used. Two nonexpert examiners evaluated a single 3D volume of the uterus of each woman, blinded to each other. The following measurements were performed: indentation depth, indentation angle, uterine fundal wall thickness, external fundal indentation, and indentation-to-wall-thickness (I:WT) ratio. Each observer had to assign a diagnosis in each case, according to the three classification systems (ESHRE/ESGE, ASRM, and CUME). The interobserver agreement regarding the ESHRE/ESGE, ASRM, and CUME classifications was assessed using the Cohen weighted kappa index (k). Agreement regarding the three classifications (ASRM versus ESHRE/ESGE, ASRM versus CUME, ESHRE/ESGE versus CUME) was also assessed. Results The interobserver agreement between the 2 nonexpert examiners was good for the ESHRE/ESGE (k = 0.74; 95% confidence interval [CI]: 0.55–0.92) and very good for the ASRM and CUME classification systems (k = 0.95; 95%CI: 0.86–1.00; and k = 0.91; 95%CI: 0.79–1.00, respectively). Agreement between the ESHRE/ESGE and ASRM classifications was moderate for both examiners. Agreement between the ESHRE/ESGE and CUME classifications was moderate for examiner 1 and good for examiner 2. Agreement between the ASRM and CUME classifications was good for both examiners. Conclusion The three classifications have good (ESHRE/ESGE) or very good (ASRM and CUME) interobserver agreement. Agreement between the ASRM and CUME classifications was higher than that for the ESHRE/ESGE and ASRM and ESHRE/ESGE and CUME classifications.

Abstract Introduction Various forms of female infertility were associated with congenital uterine anomalies and acquired uterine disease. In fact, the myometrium can only be indirectly imaged by X-ray hysterosalpingography but is directly seen on Ultrasonography. More recently, further advances in ultrasonography have led to three-dimensional multiplanar or volume imaging (three-dimensional ultrasonography [3DUS]) in which sonographic images are obtained from a volume of ultrasonographic data (rather than from a slice of data) the application of 3DUS to gynecologic imaging has suggested a role for transvaginal 3DUS in the assessment of uterine anomalies and endometrial lesions. Aim To evaluate the diagnostic accuracy and sensitivity of Hysteroscopy vs TV3D in the evaluation of the uterine abnormalities in infertile women before ICSI. Methodology This is a study including 60 patients were selected from the outpatient gynecological clinic, private center at Sohag government (Ibn Sina Center) in collaboration with (Ain Shams University Hospital in IVF unit) from12/2017 -8/2018. All cases files are computerized and saved in Ibn Sina Center. Results The overall result of accuracy of different modalities was 98.3% for 3D sonography and the accuracy of different modalities in detection of endometrial polyp was 93.3% for 3D sonography and 100% for hysteroscopy. The accuracy of different modalities in detection of septate uterus was 100% for 3D sonography and hysteroscopy. The accuracy of different modalities in detection of intrauterine adhesions was 100% for 100% for 3D and hysteroscopy. Conclusion Hysteroscopy is the gold standard diagnostic & therapeutic tool for uterine anomalies (bicornuate, septate, arcuate, polyp), However 3D ultrasonography is diagnostic tool only but superior to hysteroscopy in certain lesions e.g. (subserous and intramural fibroid). Recommendation we recommend 3D to become soon as the diagnostic procedure of choice in assessment of suspected uterine lesions. Furthermore, we recommend that 3D US, if available, to be performed routinely for all cases of uterine cavity anomalies and prior to corrective uterine surgery.

IntroductionThere have been several t.e.m. studies (1,2,3,4) of the dislocation arrangements in the matrix and around the particles in dispersion strengthened single crystals deformed in single slip. Good agreement has been obtained in general between the observed structures and the various theories for the flow stress and work hardening of this class of alloy. There has been though some difficulty in obtaining an accurate picture of these arrangements in the case when the obstacles are large (of the order of several 1000's Å). This is due to both the physical loss of dislocations from the thin foil in its preparation and to rearrangement of the structure on unloading and standing at room temperature under the influence of the very high localised stresses in the vicinity of the particles (2,3).This contribution presents part of a study of the Cu-Cr-SiO2 system where age hardening from the Cu-Cr and dispersion strengthening from Cu-Sio2 is combined.
