Background: COVID-19 is a wide spreading disease starts from Wuhan a city of China in east. Earlier symptoms include fever, cough and fatigue later on patients may present with loss of smell and taste, sore throat, nasal congestion and muscle or joint pain. Its long term effects may include respiratory distress and neurological symptoms. Aim: To determine the frequency of different symptoms in patients with COVID-19 presenting at outpatients department of Nishtar Hospital Multan. Methodology: This cross sectional study was conducted at department of medicine, Nishtar Medical University, Multan, from September 2020 to September 2021. This study was carried out on 200 patients presenting with symptoms of COVID-19 and diagnosis was confirmed after admission. Main variables of study were symptoms of COVID-19 like cough, fever, running nose, breathlessness, headache and palpitations. SPSS version 22 was used for data analysis. Results: Majority of the patients were (60.0%) between age group 46-60 years. The most common symptoms were fever and cough, (79.0%) and (65.0%), respectively. The symptoms recorded between both male and female were almost equal, (standardized residuals<1.96 and p>0.05). But, fever and running nose were most common among the females, (p=0.028) and (p=0.050), respectively. Conclusion: COVID-19 presents with variety of symptoms and fever and cough were most common among these symptoms. COVID-19 positive patients can be present asymptomatically. All persons who come into contact with COVID19 patients should be go through the PCR lab test. Keywords: Novel COVID 19, Acute Respiratory Distress Syndrome, Contagious, Pandemic

Abstract Background There is little evidence about the utilisation of healthcare services and disease recognition in the older population, which was urged to self-isolate during the COVID-19 lockdown. Objectives We aimed to describe the utilisation of physician consultations, specialist referrals, hospital admissions and the recognition of incident diseases in Germany for this age group during the COVID-19 lockdown. Design Cross-sectional observational study. Setting 1,095 general practitioners (GPs) and 960 specialist practices in Germany. Subjects 2.45 million older patients aged 65 or older. Methods The number of documented physician consultations, specialist referrals, hospital admissions and incident diagnoses during the imposed lockdown in 2020 was descriptively analysed and compared to 2019. Results Physician consultations decrease slightly in February (−2%), increase before the imposed lockdown in March (+9%) and decline in April (−18%) and May (−14%) 2020 compared to the same periods in 2019. Volumes of hospital admissions decrease earlier and more intensely than physician consultations (−39 versus −6%, respectively). Overall, 15, 16 and 18% fewer incident diagnoses were documented by GPs, neurologists and diabetologists, respectively, in 2020. Diabetes, dementia, depression, cancer and stroke were diagnosed less frequently during the lockdown (−17 to −26%), meaning that the decrease in the recognition of diseases was greater than the decrease in physician consultations. Conclusion The data suggest that organisational changes were adopted quickly by practice management but also raise concerns about the maintenance of routine care. Prospective studies should evaluate the long-term effects of lockdowns on patient-related outcomes.

Discomfort due to riding a motorcycle is an issue that need to be addressed as it has long-term effects of musculoskeletal disorders on motorcyclists especially among occupational motorcyclist. Thus, this study was conducted to analyse the rating of muscle discomfort and correlation with the risk factors among traffic police riders. A cross-sectional study was carried out among 137 male traffic police riders (high-powered motorcycle) with the age between 20 to 39 years old. The 100-mm visual analogue scale questionnaire included ratings of perceived discomfort scales for 20 specific body regions was used in the study. The results indicate that the lower back (left and right) were the highest mean of discomfort which were 56.6 mm and 55.9 mm respectively. This followed with right (48.5±36.2 mm) and left (48.4±30.3 mm) upper back, and right hand (47.0±33.0 mm). The mean of overall discomfort ratings for all regions were more than 20 mm.  Besides, there is a strong positive significant correlation between duration of ridings (hours) and overall discomfort ratings (p<0.01, r=0.785) and moderate positive correlation between year of traffic police motorcycle riding experience and overall discomfort ratings (p<0.01, r=0.410). As a conclusion, cumulative riding hours, riding experience and no support of the back area of the body in motorcycle seat, are the most concern in this study as this are the contributing factors to the muscle discomfort among traffic police riders while riding high-powered motorcycle. Thus, this study suggested an additional feature is needed in current motorcycle design in order to enhance comforts of traffic police riders. Also, it will improve the condition of traffic police riders’ discomfort and indirectly also improve their work and health performance as well as productivity.

Objective To identify the long-term effects of palate repair on craniofacial growth in patients with unilateral cleft lip and palate (UCLP). Design Retrospective cross-sectional study. Setting Sri Lankan Cleft Lip and Palate Project. Subjects Forty-eight adults with nonsyndromic unilateral cleft lip and palate, 29 men and 19 women, had lip repair only (LRO group). Fifty-eight adults with nonsyndromic unilateral cleft lip and palate, 35 men and 23 women, had lip and palate repairs by the age of 9 (LPR group). Main Outcome Measures Clinical notes were used to record surgical treatment histories. Cephalometry was used to determine craniofacial morphology. Results In the lip and palate repair group, the depth of the bony pharynx (Ba-PMP), the maxillary length at the alveolar level (PMP-A), the effective length of the maxilla (Ar-IZ, Ar-ANS, Ar-A), the maxillary protrusion (S-N-ANS, SNA), the anteroposterior jaw relation (ANS-N-Pog, ANB), and the overjet were smaller than in the lip repair only group. There were no significant differences in the maxillary length at the basal level (PMP-IZ, PMP-ANS) and the anterior and posterior maxillary heights (N-ANS and R-PMP, respectively) in the two groups. Conclusion Palate repair inhibits the forward displacement of the basal maxilla and anteroposterior development of the maxillary dentoalveolus in patients with unilateral cleft lip and palate. Palate repair has no detrimental effects on the downward displacement of the basal maxilla or on palatal remodeling in patients with unilateral cleft lip and palate.

Background: Research in sport concussion has increased greatly over the previous decade due to increased scientific interest as well as the media and political spotlight that has been cast on this injury. However, a dearth of literature is available regarding the long-term (>1 year after concussion) effects of adolescent concussion on cognitive and motor performance of high school athletes. Purpose: To evaluate the potential for long-term effects of concussion sustained during high school on cognitive and motor performance across the lifespan. Study Design: Cross-sectional study; Level of evidence, 3. Methods: Adults with (n = 30) and without (n = 53) a concussion history were recruited in 3 age groups: younger (18-30 years; n = 43), middle-aged (40-50 years; n = 18), and older (≥60 years; n = 22). Each participant completed a computerized neurocognitive assessment and continuous tracking and discrete temporal auditory tasks with the hand and foot. Root mean squared error and timing variability were derived from the tracking and temporal auditory tasks, respectively. Data were analyzed by regression analyses for each recorded variable. Results: The analysis revealed significant age effects on neurocognitive task, continuous tracking task, and discrete auditory timing task performance ( P values < .05). No concussion history or interaction (concussion history by age) effects were found for performance on any task ( P values > .05). Conclusion: While longitudinal investigations are still needed, this cross-sectional study failed to identify any observable effect of adolescent concussion history on cognition or motor performance with age.

Objective Quality of survival, prognosis and long-term outcome are often severely impaired in childhood-onset craniopharyngioma (CP) patients. Identification of risk factors for sequelae such as growth hormone (GH) deficiency is important for appropriate treatment and rehabilitation. Design In a cross-sectional study, 79 CP patients recruited in HIT-Endo before 2000 were analyzed according to GH substitution: (a) CP never GH treated (noGH); (b) CP GH treated only during childhood (pedGH); (c) CP under GH, initiated at adulthood (adultGH); (d) CP under GH during childhood and continued during adulthood (contGH). Methods Progression-free (PFS) and overall survival (OS), height, BMI, psychosocial and neuropsychological status (EORTC QLQ-C30, MFI-20). Results OS and PFS rates were similar in all subgroups. ContGH and pedGH CP presented with increases in height (P = 0.002; P = 0.0001) during long-term follow-up when compared with baseline. In all subgroups except for pedGH, increases in BMI were observed when compared with BMI at diagnosis. For emotional functionality and physical fatigue, adultGH CP showed worse (P = 0.037; P = 0.034) response (mean: 61.4%; 12.5%) when compared with pedGH CP (mean: 83.5%; 7.7%). Observed differences were not related to irradiation and hypothalamic involvement. In terms of psychosocial status, no differences were observed between subgroups. Conclusions We conclude that GH substitution was safe with regard to risk of tumor progression/relapse in CP. Growth was improved by GH, whereas the development of obesity was not influenced by GH substitution. However, early initiation of GH substitution after CP diagnosis might have beneficial effects on weight development and neuropsychological outcome.

The long-term effects of secondhand smoke (SHS) on dental caries among Japanese young adults remain unclear. The purpose of this cross-sectional study was to evaluate whether household exposure to SHS is associated with dental caries in permanent dentition among Japanese young adults. The study sample included 1905 first-year university students (age range: 18–19 years) who answered a questionnaire and participated in oral examinations. The degree of household exposure to SHS was categorized into four levels according to the SHS duration: no experience (−), past, current SHS < 10 years, and current SHS ≥ 10 years. Dental caries are expressed as the total number of decayed, missing, and filled teeth (DMFT) score. The relationships between SHS and dental caries were determined by logistic regression analysis. DMFT scores (median (25th percentile, 75th percentile)) were significantly higher in the current SHS ≥ 10 years (median: 1.0 (0.0, 3.0)) than in the SHS—(median: 0.0 (0.0, 2.0)); p = 0.001). DMFT ≥ 1 was significantly associated with SHS ≥ 10 years (adjusted odds ratio: 1.50, 95% confidence interval: 1.20–1.87, p < 0.001). Long-term exposure to SHS (≥10 years) was associated with dental caries in permanent dentition among Japanese young adults.

Abstract Importance A small number of COVID-19 patients has been reported to suffer from acute keratoconjunctivitis. In very rare cases, acute inflammatory retinal vein occlusion, papillophlebitis or retinopathy have been observed. Objective To determine possible long-term effects on the eye, especially on the retina, in patients who had suffered from COVID-19 at least 3 months after recovery. Design Prospective cross-sectional study. Setting Hospital of the Ludwig Maximilians University, Munich. Participants Patients who had been tested positive for SARS-CoV-2 or for anti-SARS-CoV-2 IgG serum antibodies in the Hospital of the Ludwig Maximilians University, Munich between May and September. Methods Patients who had tested positive were either hospitalized or discharged into home quarantine via the emergency room. Three months after recovery, they were invited to participate voluntarily for this study during their follow-up in our clinic. A complete ophthalmological exam including functional and imaging end points (including optical coherence tomography (OCT), OCT angiography) was performed. Main outcomes and measures Visual acuity, slit lamp, bio microscopy and fundoscopy, multimodal imaging findings. Results In total, 21 patients were examined. The mean age (SD) of the patients was 48.7 (18.3) years. Of these, 14 (66.6%) were hospitalized and 7 (33.3) were discharged home. Two hospitalized patients (9.5%) received invasive ventilation. During the infection, 14 of the 21 patients (66.6%) were in regular care whereas 2 patients (9.5%) received intensive care ventilation for 8.5 (SD) (0.7) days on average in the COVID ICU. Ophthalmological examination of the previously hospitalized group took place 111.4 (23.2) days after recovery and discharge from the hospital, while non-hospitalized patients were examined after mean 123.4 (44.7) days. All patients showed normal findings for anterior and posterior segment of both eyes. OCT and OCT-A showed no evidence of retinal damage, or vascular or microvascular events. Conclusion and relevance This study with a small prospective cohort of 21 patients indicates that there might be no evidence of ocular complications at 3 months after recovery from COVID-19, without previous eye involvement. Further studies with more participants with and without acute ocular symptoms are necessary for final evidence.
