83 patients (men) with stable chronic coronary disease (mean age 62 ± 9 years, mean body weight 86± 11 kg, mean height 177 ± 6 cm, mean ejection fraction of left ventricle 47± 8%) were examined before and aft er three months lasting aerobic training combined with stregth components . Before the training period symptom-limited spiroergometry was provided. Spiroergometry was applied for the evaluation of anaerobic threshold to decide on the aerobic training intensity. Load intensity for the strength training phase was determined by the method of 1-RM (30-60%). Th e training (exercise unit) was performed three times a week. Th e impact on quality of life was measured using SAQ (Th e Seattle Angina Questionnaire). Th e all 5 SAQ dimensions were signifi cantly better aft er the training. Physical limitations (79,7 ± 19,0 vs. 84,9 ± 16,4*) stability of symptoms (78,2 ± 19,1 vs. 85,6 ± 16,1*) frequency of symptoms (83,5 ± 15,6 vs. 88,2 ± 14,5**), treatment satisfaction (87,8 ± 14,6 vs. 92,9 ± 11,5*), and disease perception (67,2 ± 19,7 vs. 73,8 ± 19,5**). Peak exercise oxygen uptake (VO2peak ) increased from 18,6 ± 3,1 to 19,8 ± 4,0** ml. kg-1.min-1 (*P < 0.05, ** P < 0.01, Wilcoxon).

Background: The Seattle Angina Questionnaire (SAQ) has been widely used in China since it was introduced in 1996. However, the validity and reliability of the translated SAQ has never been assessed in published papers. Even worse, when we use the translated SAQ in clinical studies, we found some patients have difficulty upon understanding some items due to the translation and cultural inadaptation. In this study, we are going to find out if the SAQ is suitable for Chinese patients. Methods: Step one: to assess the psychometric properties of SAQ, 1787 angina patients’ data of our previous cross-section study was used. Exploratory Factor Analysis (EFA) was used to evaluate the construct validity, and internal consistency (Cronbach’s alpha coefficient) was used to evaluate the reliability, and t-test was used to discriminate patients with different Canadian Cardiovascular Society Classification (CCSC) levels. Step two: to find out which item is unsuitable, semi-structured individual patient interview was used. Patients were asked to complete a copy of SAQ first, and then were asked did they encounter difficulties on understanding the items and why. Results: Step one: Cronbach’s alpha coefficient was 0.885, but 0.605 for the quality of life domain. EFA showed 4 factors and couldn’t match the original five domains. More than 50% of patients give the same answer “mostly satisfied” to item 6-8 about treatment satisfaction, and these items can’t detect the differences among patients with different CCSC levels. Item 5 “bothersome of taking pills” is highly correlated with Quality of life domain other than the treatment satisfaction domain it is intended to measure. The correlation between item 6 and 8 is 0.816, which shows a redundancy. The reliability and validity of the translated SAQ is not satisfied. Step two: 92 out of 112 invited patients participated in the interview, 39 (42.39%) couldn’t understand item 10, and 7 (7.61%) of item 6. As to item 10, “feeling of the current angina if long-lasting”, patients were confused of the answer “level of satisfaction”, which result a low internal consistency of Qol. As to item 6, “everything is done for treatment”, 75 (81.52%) patients thought it’s the same meaning with item 8 “satisfaction with current treatment.” Maybe influenced by Confucian, patients chose “satisfaction” even their angina getting worse. Among the 1787 patients, 131 (7.33%) patients took nitros when angina attacked; however, 234(13.09%) patients took Chinese herbal medicine. So they couldn’t answer item 4 “frequency of nitros taken for angina” correctly. Conclusions: The currently used SAQ is unsuitable for Chinese patients. It doesn’t completely capture the characteristics and conditions of Chinese patients. Further study is needed to adapt both, the translation and culture of the SAQ.

Abstract Background Stable angina pectoris (SAP) is a common cardiovascular disease, which brings health burden to society. Acupuncture therapy is effective in improving SAP as adjunctive therapy, nevertheless, there were controversies on the effect of acupuncture on disease-affected meridian (DAM), sham acupoints and nonaffected meridian (NAM). This study evaluated the effect of acupuncture on DAM as adjunctive therapy, and the difference in acupuncture on DAM, sham acupoints, and nonaffected meridian NAM. Methods Thirteen online databases were searched from inception to December 2, 2019. Risk of bias and quality of evidence for outcomes were respectively assessed by the Cochrane risk of bias assessment tool and the GRADE approach. RevMan 5.3 was adopted to conduct meta-analysis. Results Thirteen RCTs including 1026 participants were included in this study. Acupuncture therapy could be effective in the improvement of angina severity, ECG results, 6-MWT, SAQ results (physical limitation, angina stability, angina frequency, treatment satisfaction, disease perception), anxiety, HRV (LF, LF/HF), ET, CRP, and reduction of nitroglycerin intake. Acupuncture on DAM could be more beneficial than acupuncture on sham acupoints in aspects of angina severity, 6-MWT, SAQ results (angina stability, angina frequency, treatment satisfaction, disease perception). Compared to acupuncture in NAM, acupuncture on DAM could be more effective in SAQ results (angina stability, angina frequency, treatment satisfaction). Moreover, acupuncture on DAM did not increase the risk of dropout and adverse effect. Conclusions Acupuncture on DAM could effectively improving SAP, and it could be more effective compared with acupuncture on sham acupoints or NAM. Prevalensi Penyakit Jantung Koroner (PJK) terus mengalami peningkatan setiap tahunnya dan menjadi masalah kesehatan utama di masyarakat saat ini. PJK berdampak terhadap berbagai aspek kehidupan penderitanya baik fisik, psikososial maupun spiritual yang berpengaruh terhadap kualitas hidup pasien. Isu kualitas hidup dan faktor-faktor yang berhubungan didalamnya belum tergambar jelas di Indonesia. Tujuan dari penelitian ini adalah mengidentifikasi faktor yang memengaruhi kualitas hidup pada pasien PJK yang sedang menjalani rawat jalan. Faktor-faktor yang diteliti dalam penelitian ini meliputi jenis kelamin, tingkat penghasilan, revaskularisasi jantung, rehabilitasi jantung, kecemasan, depresi dan kesejahteraan spiritual. Kecemasan diukur dengan Zung Self-rating Anxiety Scale, depresi diukur dengan Beck Depression Inventory II, kesejahteraan spiritual diukur dengan kuesioner Spirituality Index of Well-Beingdan kualitas hidup diukur menggunakan Seattle Angina Questionnaire. Penelitian ini menggunakan rancangan kuantitatif deskriptif dan analitik multivariatedengan regresi logistic. Diteliti pada 100 responden yang diambil secara randomdalam kurun waktu 1 bulan di Poli Jantung. Hasil penelitian menunjukkan faktor yang memengaruhi kualitas hidup pada pasien PJK adalah cemas (p) 0,002; Odd Ratio(OR) 4,736 (95% confidence interval(CI), 1,749 – 12,827); depresi (p) 0,003; OR 5,450 ( 95% CI, 1,794 – 16,562); dan revaskularisasi (p) 0,033; OR 3,232 (95% CI, 1,096 – 9,528). Depresi menjadi faktor yang paling berpengaruh terhadap kualitas hidup pasien PJK. Faktor yang memengaruhi kualitas hidup pada pasien PJK meliputi depresi, cemas dan revaskularisasi. Dari ketiga variabel tersebut depresi merupakan variabel yang paling signifikan berpengaruh, sehingga manajemen untuk mencegah depresi perlu mendapatkan perhatian lebih baik lagi dalam discharge planningataupun rehabilitasi jantung.Kata kunci: Cemas, depresi, faktor yang memengaruhi, kualitas hidup, spiritual.Factors Influenced the Quality of Life among Patients Diagnosed with Coronary Heart Disease AbstractCoronary Heart Disease (CHD) has affected multidimensional aspects of human live nowadays. Yet, quality of life and factors associated with quality of life among people who live with heart disease has not been explored in Indonesia. This study aimed to identify factors influenced the quality of life among people with CHD received outpatient services. Those factors are gender, income, revascularization, cardiac rehabilitation, anxiety, depression and spiritual well-being. Zung Self-rating Anxiety Scale was used to measure anxiety where depression level measured using Beck Depression Inventory II. Spirituality index was used to measure spiritual well-being. The quality of life level was measured using the Seattle Angina Questionnaire. This study used quantitative descriptive with multivariate analysis using logistic regression. 100 respondents were randomly selected from the Cardiac Outpatient Unit. Findings indicated factors influenced the quality of life of CHD patients using a significance of ƿ-value < 0.005 were: anxiety (ƿ=0,002, OR = 4,736, 95% CI, 1,749 – 12,827); depression (ƿ=0,003; OR=5,450, 95% CI, 1,794 – 16,562); and revascularizations (ƿ=0,033; OR=3,232, 95% CI, 1,096 – 9,528). Depression was considered as the most significant factor; therefore, managing depression is a priority in the discharge planning or cardiac rehabilitation programme. More than 20 years of clinical and research experience with affected people in the British Isles has provided insight into particular challenges for therapists, educators, or parents wishing to facilitate learning and to support the development of skills in people with Rett syndrome. This paper considers the challenges in two groups: those due to constraints imposed by the disabilities associated with the disorder and those stemming from the opportunities, often masked by the disorder, allowing the development of skills that depend on less-affected areas of the brain. Because the disorder interferes with the synaptic links between neurones, the functions of the brain that are most dependent on complex neural networks are the most profoundly affected. These functions include speech, memory, learning, generation of ideas, and the planning of fine movements, especially those of the hands. In contrast, spontaneous emotional and hormonal responses appear relatively intact. Whereas failure to appreciate the physical limitations of the disease leads to frustration for therapist and client alike, a clear understanding of the better-preserved areas of competence offers avenues for real progress in learning, the building of satisfying relationships, and achievement of a quality of life.

Objetivo: Avaliar os aspectos referentes as limitações físicas, psicossociais e qualidade de vida das pessoas atingidas pela hanseníase. Método: Estudo quantitativo, transversal. Foram aplicadas as escalas Screening Activity Limitation and Safety Awareness (SALSA), Participação Social e World Health Organization Quality of Life (WHOQOL-bref) com 31 sujeitos. Resultado: Evidenciou-se que a maioria dos participantes apresentava algum grau de restrição física e social. Correlacionando com o grau de incapacidade, destaca-se que os pacientes que apresentaram restrições, também possuíam grau II de incapacidade. Quanto à qualidade de vida, o domínio psicológico e o de relações sociais apresentaram as médias mais altas, enquanto o físico e o ambiental tiveram as mais baixas. Conclusão: Afirma-se a necessidade de priorização e intensificação das ações de prevenção de incapacidades da hanseníase.Descritores: Hanseníase, autocuidado, qualidade de vida.

The aim of the present clinical trial is to evaluate the effectiveness of neuromuscular versus classical strength-resistance training as part of a cardiac rehabilitation programme in patients following acute coronary syndrome. The study is designed as a double-blinded, randomised, and controlled clinical trial. Thirty participants suffering from acute coronary syndrome who meet our inclusion criteria will be recruited by a private tertiary hospital. The intervention group will follow 20 sessions of a cardiac rehabilitation programme divided into two parts: aerobic training and neuromuscular strength-resistance training. The control group will complete the same aerobic training as well as a classical strength-resistance training workout programme. The primary outcome of the study will be the mean difference in change from baseline in the Incremental Shuttle Walking Test. The secondary outcomes will be the cardiorespiratory fitness of the patients (assessed by means of the Chester Step Test), lower-limb performance (assessed with the 30-Second Chair Stand Test and Single-Leg Squat Test), lower-limb strength (hip flexor handheld dynamometry), sexual dysfunction assessment (Sex Health Inventory for Men) and quality of life (EQ-5D-5L). This work will provide evidence for the effectiveness of a neuromuscular versus a classic strength-training programme in terms of cardiorespiratory fitness, lower-limb performance capacities and quality of life, in cardiac patients. The data obtained could lead to more effective and functional workouts which, in turn, may enhance the speed at which these patients can return to their everyday activities of life and improve the efficiency of their movement patterns and heart responses. Furthermore, patients may find neuromuscular workout routines more motivating and engaging, thus encouraging them to adopt healthier lifestyle patterns.
