Background. Screening strategies for gestational diabetes mellitus (GDM) earlier than 24-28 weeks of gestation should be considered to prevent adverse pregnancy outcomes. Nonetheless, there is uncertainty about which women would benefit most from early screening and which screening strategies should be offered to women with GDM. The Italian National Healthcare Service (NHS) recommendations on selective screening for GDM at 16-18 weeks of gestation are effective in preventing fetal macrosomia in high-risk (HR) women, but the appropriateness of timing and effectiveness of these recommendations in medium- (MR) and low-risk (LR) women are still controversial. Patients and Methods. We retrospectively enrolled 769 consecutive singleton pregnant women who underwent both anomaly scan at 19-21 weeks of gestation and screening for GDM at 16-18 and/or 24-28 weeks of gestation, in agreement with the NHS recommendations and risk stratification criteria. Comparison of maternal characteristics, fetal biometric parameters at anomaly scan (head circumference (HC), biparietal diameter (BPD), abdominal circumference (AC), femur length (FL), estimated fetal weight (EFW)), and neonatal birth weight (BW) percentile among risk groups was examined. Results. 219 (28.5%) women were diagnosed with GDM, while 550 (71.5%) were normal glucose-tolerant women. Out of 164 HR women, only 62 (37.8%) underwent the recommended early screening for GDM at 16-18 weeks of gestation. AC and EFW percentiles, as well as neonates’ BW percentiles, were significantly higher in HR women diagnosed with GDM at 24-28 weeks of gestation with respect to normal glucose-tolerant women, as well as MR and LR women who tested positive for GDM. Comparative analysis between MR and LR women with GDM and women with normal glucose tolerance revealed significant differences in both AC and EFW percentiles (P<0.05), while there was no significant difference in neonatal BW percentiles. Conclusion. In MR and LR women with GDM, a mild acceleration of fetal growth can be detected at the time of anomaly scan. However, in these at-risk categories, the NHS recommendations for screening and treatment of GDM at 24-28 weeks of gestation are still effective in normalizing BW and preventing fetal macrosomia, thus supporting a risk factor-based selective screening program for GDM.

UNSTRUCTURED Chronic obstructive pulmonary disease (COPD) predominantly affects older adults, and claimed 3 million lives in 2016, making it the third leading cause of death worldwide. Over 35 million Americans aged 40 or older have lung function consistent with diagnosable COPD. COPD and cardiovascular disease (CVD) have a bidirectional relationship, in that one is a risk factor for developing the other. National and international consortiums recommend early screening of adults at risk of COPD, such as those with CVD. Recommended screening strategies include screening tools to assess symptoms, medical history, and handheld spirometry. Handheld spirometry has high diagnostic accuracy and if impaired lung function is indicated, these patients are referred for pulmonary function testing (PFT), the diagnostic gold standard for COPD. However, there is no clinical consensus for pulmonary screening in people with CVD. Current knowledge relating to the prevalence and incidence of CVD in people with COPD and the mechanisms that underlie their coexistence is key in combating the global burden of COPD.

Introduction Acrochordons or skin tags are common benign cutaneous tumors that occur especially over the neck and major flexures. A possible association between diabetes mellitus and dyslipidemia is observed in numerous past studies with varying results. We aim to find out the association of diabetes mellitus with acrochordons Methods: One hundred patients were enrolled in our study. Among them, 50 (27 males and 23 females) with skin tags were selected as cases and 50 with other dermatologic diseases after matching age and gender were taken as controls. Blood glucose levels including both fasting and postprandial glucose levels were determined for both cases and controls and compared. Results: There was a higher frequency of Diabetes Mellitus and impaired glucose tolerance in patients with skin tags in comparison to controls (p<0.001). Moreover, there were higher odds of acquiring skin tags in patient with abnormal blood glucose levels. Conclusions: There is an increased risk of developing DM in patients with skin tags. It is highly recommended that suspicion for Diabetes Mellitus is to be done in patients with skin tags for early screening and diagnosis of Diabetes.

Frailty is a vulnerable situation among older adults which can lead to unfavorable health outcomes such as falls, mortality, functional decline and institutionalization. The increasing number of elderly people and low rate of mortality has necessitated the need for high-quality medical services for this aging population, and this has led to a high cost of geriatric health care. There exist a huge number of screening tools to detect frailty and it is important for researchers and general practitioners (GPs) to select the appropriate and precise tool that would effectively lead to quality results. Frail individuals can be managed effectively when there is an early screening and intervention. Healthcare providers need tools that are simple and validated in order for screening and interventions to become effective. Self-reported frailty screening tools are very simple to use, not expensive and test results can be interpreted by non-health professionals. This work reviewed some of the commonly used frailty screening tools, and proposed a practical approach that would assist GPs in assessing frailty in older adults.

Background: Type 2 diabetes mellitus is a chronic, debilitating disease characterized by insulin resistance, impaired insulin secretion and hyperglycemia. It represents more than 90% of total prevalence of diabetes in the world and is responsible for 9% of the global mortality corresponding to four million deaths per year. The objective of the study was to assess the prevalence of diabetes mellitus type-2 in a rural population of age 30 years and above.Methods: A community based cross sectional study carried out in population 30 years and above in rural area of Khammam district in Telangana during the study period from 1st January to 31st December 2015. A total of 910 persons aged 30 years or more were included in study.Results: Near about 74 (8.1%) were diagnosed as type -2 diabetes mellitus. The prevalence of DM was 16.22% in 30-40 years age group, 24.32% in 41-50 years age group, 43.34% in 51-60 years age group and 16.2% in 61-70 years age group which shows that DM increases with age and the association between age and prevalence of type 2 DM was found to be statistically significant.Conclusions: There is a need to increase awareness of type–2 diabetes mellitus in the general population. Health education should be given in terms of risk factors of diabetes. They should be made aware about early screening methods.
