This review discusses early diagnostics and early intervention in developmental disorders in the light of brain development. The best instruments for early detection of cerebral palsy (CP) with or without intellectual disability are neonatal magnetic resonance imaging, general movements assessment at 2–4 months and from 2–4 months onwards, the Hammersmith Infant Neurological Examination and Standardized Infant NeuroDevelopmental Assessment. Early detection of autism spectrum disorders (ASD) is difficult; its first signs emerge at the end of the first year. Prediction with the Modified Checklist for Autism in Toddlers and Infant Toddler Checklist is possible to some extent and improves during the second year, especially in children at familial risk of ASD. Thus, prediction improves substantially when transient brain structures have been replaced by permanent circuitries. At around 3 months the cortical subplate has dissolved in primary motor and sensory cortices; around 12 months the cortical subplate in prefrontal and parieto-temporal cortices and cerebellar external granular layer have disappeared. This review stresses that families are pivotal in early intervention. It summarizes evidence on the effectiveness of early intervention in medically fragile neonates, infants at low to moderate risk, infants with or at high risk of CP and with or at high risk of ASD.

Studies have shown that the severity and outcome of acute pneumonia largely depends on the prevalence of the focus of inflammation in the lung tissue, which is determined radiologically. We analyzed and determined a statistically significant effect of the degree of lung tissue damage on the severity and prognosis of pneumonia in young children, taking into account the conditions of infection. In pneumonia, χ² = 47.13 (p <0.001), indicates that the greater the degree of damage, the greater the likelihood of a severe course and unfavorable outcome of pneumonia. The objective: to study features of the somatic and genesial anamnesis at pregnant women with an undifferentiated dysplasia of connecting tissue for possibility of early diagnostics of the subsequent obstetric and perinatal complications. Patients and methods. 100 patients with clinical-laboratory signs an undifferentiated dysplasia of connecting tissue are surveyed. On the basis of the received results of the patient were divided into two groups: the main – 50 women (expression і6 points) regarded as patients with the expressed undifferentiated dysplasia of connecting tissue and group of comparison – 50 women (expression <6 points), regarded as patients without the connecting tissue expressed by an undifferentiated dysplasia. Results. Results of the conducted researches testify to an important role of the established features of the somatic and genesial anamnesis at pregnant women with clinical-laboratory and functional signs of an undifferentiated dysplasia of a connecting tissue. Conclusion. The received results need to be used for carrying out pregravidarny preparation and forecasting of possible obstetric and perinatal complications at pregnancy. Presented a clinical observation of hemolytic disease of the newborn as a result of fetus immunization with low-frequency erythrocyte antigen (Lutheran). The special aspect of the case is the presence of hyperleukocytosis with evident regenerative shift of neutrophils to the left to singular blastic variants, thrombocytopenia, high reticulocytosis (more than 1000 ‰) and a great number of nucleated red blood cells (normoblasts) in the newborn’s hemogram. In myelogram a sharp expansion of erythroid sprout (90 %) and signs of dizerythropoiesis were determinded. Typical clinical signs and isoserological incompatibility of mother and fetus in erythrocyte antigen provided basis for early diagnostics and adequate treatment of hemolytic disease of the newborn.

THE AIM:to assess excretion value of podocytes injury biomarkers in urine and to clarify their significance for early diabetic nephropathy (DN) diagnostics in diabetes mellitus (DM) patients with different severity of albuminuria (AU)/proteinuria(PU).PATIENTS AND METHODS.74 DM pts were studied, including 30 with type1 DM (T1DM) and 44 pts with type2 DM (T2DM). They were divided into three groups: 41 pts with AU <30 mg/gCr (A1), 13 pts with AU 30-300 mg/gCr (A2), 20 pts with PU (A3). CKD S1 was revealed in 41pts, CKD S2 – in 25 pts, CKD S3 – in 8 pts. Arterial hypertension was observed in 52 pts of 74(70%), mainly in T2DM. 10 healthy subjects were studied as control. Urinary levels of nephrin and podocin (an important slit diaphragm proteins) were measured by ELISA.RESULTS.Nephrinuria (NU) >5,84ng/ml/g, which not detecting in controls, was revealed in 63% of A1 pts, in 77% – in A2, in 80% – in A3. Podocinuria (PdU)>1,73ng/ml/g was revealed in 78% of A1 pts, in 54% of A2 and in 83% – A3. NU in pts with PU was significantly higher than in AU<30 mg/g. PDU in groups with different AU/PU was equally high and has no differ between DM types. Direct correlation was obtained between NU and AU (R=0,947 p<0,05). NU and PdU in T1DM correlated directly with serum creatinine (R=0,489 p<0,05 and R=0,468 p<0,05) and indirectly with GFR (R=-0,461 p<0,05 and R=-0,36 р<0,05). In DM duration less than 5 years NU directly correlated with НbА1с level, in T2DM – indirectly with systolic blood pressure.CONCLUSON. Nephrin and podocin levels can be useful for early diagnostics and monitoring of DN.
