The Corona virus disease 2019 (COVID-19) health crisis has not spared any country in the world, from the most advanced to the poorest. COVID-19 pandemic has exposed much vulnerability in systems and societies. In all resource-constrained countries, the response has been very complicated. The pandemic had to be dealt with very quickly with limited health and economic resources to avoid the hecatomb already predicted for these countries. In Côte d'Ivoire in particular, health and government authorities worked with state or private structures with infrastructure and human resource capacities to develop a rapid and effective response plan. Several measures have been adopted to reduce the spread of COVID-19. As a result of all these strategies, the number of cases of infected persons has been gradually decreasing. This new situation has favored the revision of certain restrictive measures, in this case the closure of air borders, which have of course been eased under certain conditions. Travelers are required to have a paper attesting to their negativity in COVID-19 test. Thus, Pasteur Institute of Côte d'Ivoire (IPCI), the reference laboratory for carrying out COVID-19 tests, was responsible for supplying a certificate of analysis to travellers. The purpose of this article is to show the internal process developed and different strategies implemented by IPCI to establish and deliver this document in order to meet the demand in the context of a pandemic.

Medical testing laboratories are an essential link in the efficient management of infectious diseases by the identification of the pathogens involved. However, the arrangements for their operation may appear more difficult in times of health crises and raise multiple issues that may compromise the usual level of quality assurance of biological analyses and the response to needs. The smooth running and control of laboratory activities in a health crisis situation requires the implementation of a management system that allows the federation of all available energies. We report here on the experience of the Pasteur Institute of Côte d'Ivoire, in charge of nasopharyngeal samples management during the COVID 19 pandemic, describing the assessment of pre-analytical activities in the first quarter of the crisis (March to May 2020). We then present the implemented strategy and the results obtained from June to September 2020. This article proposes a framework for sharing experiences to contribute to a better preparation of the pre-analytical phase of laboratory samples during health crises.

Background: The ATLAS program promotes and implements HIVST in Côte d'Ivoire, Mali, and Senegal. Priority groups include members of key populations—female sex workers (FSW), men having sex with men (MSM), and people who use drugs (PWUD)—and their partners and relatives. HIVST distribution activities, which began in mid-2019, were impacted in early 2020 by the COVID-19 pandemic.Methods: This article, focusing only on outreach activities among key populations, analyzes quantitative, and qualitative program data collected during implementation to examine temporal trends in HIVST distribution and their evolution in the context of the COVID-19 health crisis. Specifically, we investigated the impact on, the adaptation of and the disruption of field activities.Results: In all three countries, the pre-COVID-19 period was marked by a gradual increase in HIVST distribution. The period corresponding to the initial emergency response (March-May 2020) witnessed an important disruption of activities: a total suspension in Senegal, a significant decline in Côte d'Ivoire, and a less pronounced decrease in Mali. Secondary distribution was also negatively impacted. Peer educators showed resilience and adapted by relocating from public to private areas, reducing group sizes, moving night activities to the daytime, increasing the use of social networks, integrating hygiene measures, and promoting assisted HIVST as an alternative to conventional rapid testing. From June 2020 onward, with the routine management of the COVID-19 pandemic, a catch-up phenomenon was observed with the resumption of activities in Senegal, the opening of new distribution sites, a rebound in the number of distributed HIVST kits, a resurgence in larger group activities, and a rebound in the average number of distributed HIVST kits per primary contact.Conclusions: Although imperfect, the program data provide useful information to describe changes in the implementation of HIVST outreach activities over time. The impact of the COVID-19 pandemic on HIVST distribution among key populations was visible in the monthly activity reports. Focus groups and individual interviews allowed us to document the adaptations made by peer educators, with variations across countries and populations. These adaptations demonstrate the resilience and learning capacities of peer educators and key populations.

Aims: The aim of this study was to characterize fluoroquinolone resistance genes in enterobacteriaceae that produce extended-spectrum β-lactamases, isolated in Abidjan. Place and Duration of Study: Pasteur Institute of Côte d'Ivoire and research unit on emerging tropical infectious diseases of Aix-Marseille University from January 2017 at July 2017. Methodology: The study included 90 enterobacteriaceae producing extended-spectrum β-lactamases isolated from biological products from various hospital services in Abidjan. These strains have been pre-identified and stored at the Center for Biological Resources (CeReB) of Pasteur Institute of Côte d'Ivoire. The identification of the strains was confirmed using the mass spectrometry MALDI-TOF (MS) and the antibiotic sensitivity test was performed using Müeller Hinton's agar diffusion method. The fluoroquinolone resistant genes were detected by conventional PCR and then, sequenced. Results: The strains studied were Escherichia coli (44), Klebsiella pneumoniae (31) and Enterobacter cloacae (15). High resistance rates to ceftriaxone (96.7%), cefotaxime (95.6%), aztreonam (95.6%) and cefoxitin (72.2%) were observed in all strains producing broad spectrum β-lactamases. The resistance rate to fluororquinolones represented by ciprofloxacin was 86.7%. The fluoroquinolone resistance genes detected were qnr A (3.3%) and qnr B (42.2%). Sequencing identified the qnr A1 (3.3%), qnr B1 (31.1%), qnr B6 (2.2%) and qnr B7 (1.1%) genes. Conclusion: This study made it possible to identify fluoroquinolone resistance genes in enterobacteriaceae producing β-lactamases which have an extended spectrum in Abidjan.

Aquatic pollution in Côte d'Ivoire is severe in several places. The Ivorian government therefore requested and later obtained technical assistance from the Danish government for the training of personnel and the equipment of an environmental central laboratory and of a pollution combating unit. The structure CIAPOL (Centre Ivorian Anti-Pollution) in charge of setting up the two components of the project, was created by the government of Côte d'Ivoire in May 1991. A coastal and a continental monitoring network have been implemented. Nutrients, heavy metals, pesticides, hydrocarbons and microbiological methods have been adapted for the survey programs of water, sediment and biota. Results presented in this paper show spatiotemporal variations of the ammonia concentrations in the lagoon waters. Very high concentrations in the Bietry bay of hydrocarbons in sediment and zinc in oysters trace petroleum refinery pollution. The extremely high microbiological contamination of the lagoon water around Abidjan should preclude its use for any purpose. The staff of the centre has been trained in the use of the oil spill recovery equipment. Alert procedures and communications have been introduced. Today the CIAPOL is involved in national conventions and international projects. It is designated to become the Reference Laboratory in the region and will be in charge of training and intercalibration sessions within these projects.

Climate change which is considered as a threat to biodiversity, causes paradoxical situations everywhere it manifests itself. These include, among others, drought and desertification; floods and storms; sea level rise, reduction of the vegetation cover; water supply sources and, animal and plant species disappearance. In Korhogo, in the North of Côte d’Ivoire, the climate variability and environmental change phenomena are perceived and described by the population. A qualitative approach with historical and comparative analysis tools are used in this study to focus on the perceptions of the populations through indicators of both climate variability and environmental changes. It also analyses the major challenges the population will face, which are mainly related to alteration in farming practices and seasons, loss of production, and extension of the lean period, the risk of food insecurity and conflicts related to the exploitation and the distribution of water resources; and mostly, to the lack of information- educationcommunication on the phenomenon. On the whole, although the study brings out an awareness about climate variability and environmental changes, it especially show the need for emergent multidimensional actions for effective response strategies to the changes observed in the area.
