INTRODUCTION: Chronic-degenerative diseases need specialized health care, especially in palliative care, since from the beginning of the disease the elderly is in this classification. In this context, the study aimed to know the meaning of palliative care to the elderly person from the nurse METHODS: This is a descriptive, cross-sectional, qualitative study carried out in a private tertiary hospital in the Northeast of Brazil. The survey was conducted from March to June 2016, with 19 nurses. In the data collection, a semi-structured interview, with four questions were used: what does palliative care mean to you? What does care for the elderly in hospice care mean to you? What strategies do you adopt to promote palliative care for the elderly? Tell me about your experience in providing palliative care to the elderly. The technique of data analysis and organization was the Discourse of the Collective Subject. RESULTS: Four categories of palliative care were produced: meaning palliative care; Experience in providing palliative care to the elderly; Meaning of caring for the elderly in palliative care; Strategies to promote palliative care for the elderly. CONCLUSION: The meaning of palliative care was synonymous with comfort and quality of life. It is important to emphasize the patient-centered care and to carry out further studies on the subject to make nursing professionals aware of the importance of promoting adequate behavior in palliative care.

ABSTRACTThe elderly in palliative care are confronted with difficult decisions relating to treatments. The philosophy of palliative care, namely, including the patient and his/her family right away, leads the doctor to consult with the two parties involved when choosing a treatment. As no theoretical model allows us to understand how the decision-making process hinges on the trio (a capable elderly person, a family caregiver, and the doctor) in a context of palliative care, we propose one which was developed from three strategies of document analysis: theoretical synthesis, theoretical analysis, and theoretical derivation. According to our model, the decision-making process depends on individual factors influencing the decision of the participant, expectations and attitudes as to the role, the level of confidence amongst the parties involved, the manner in which they communicate with each other, their mutual understanding of the clinical and ethical issues, and, finally, their ability to cooperate.

Abstract The number of elderly persons institutionalized in residential facilities that provide various types of care is increasing all the time. Care is provided by caregivers who, often unknowingly, are delivering palliative care, and in doing so, are accompanying the elderly persons to their death. The aim of the present study was to investigate how terminal illness and death are experienced by caregivers. A qualitative explorative-descriptive study was carried out, featuring a phenomenological approach, in accordance with Giorgi. The sample was comprised of nine individuals who worked in residential facilities, with data collected through a semi-structured interview. Following data analysis, eight categories were defined: relationship with the elderly person; positive experiences, positive feelings, negative experiences, negative feelings, strategies, consequences and training. These categories were then divided into subcategories. It can be concluded that the caregivers experienced great difficulty when dealing with suffering at the end of life. This did not prevent them from enjoying their work, and a number of positive aspects of their relationships with the elderly were mentioned. The need to provide more support for these professionals, whether through further training or the level of psychological support provided, is also fundamental in this area of palliative care, as only in this way will individuals feel more able and confident about performing their tasks, brining greater benefit for the elderly persons.

In outpatient care the home care, including hospices, is recognized as a model for providing quality, cost-effective and charitable care. The focus is mainly on the care that helps everyday lifeof the patient as well as the relatives, rather than on treatment, and in most cases it takes place in the patients' home. In Europe, in recent years there has been a real "boom" in home care due to demographic processes linked to increased needs for elderly care and chronically ill under the conditions of limited financial resources. In outpatient medical care in our country by means of a national framework contract there are regulated visits to the patient's home by a doctor, as well as visits by medical staff employed by him - nurse, midwife, medical assistant / paramedic / for manipulation, counseling and monitoring. At the same time there is no regulated legal activity in the Republic of Bulgaria, which is essentially the subject of home care.Since 1994 „Caritas“ has carried out the "Home Care" service, which provides a complex - health and social care for over 360 sick adults in a place where the elderly person feels the most comfortable - in their own home. „Caritas Home Care“ is provided by mobile teams of nurses and social assistants who visit the elderly at home and provide them with the necessary care according to their health and social needs. With the establishment of the first „Home Care Center“ in Lozenets region, Sofia, with the support of the PHARE ACCESS program in 2003, the Bulgarian Red Cross introduces in Bulgaria an integrated model for provision of health care and social services in the home of adults, chronically ill and people with permanent disabilities. To date, there are a number of problems in home care related to the realization of home care for patients in need in out-of-hospital settings: lack of legal regulation for home care, lack of qualified staff in outpatient care; lack of organization and structures for care; unsettled funding and the inability of the part of the population that is most in need of care to pay for it, there is no regulation to control the activity. Although home care began over 20 years ago, our country is yet to make its way to the European program called „Home care in Europe“.

Chronic pain is among problems of old people and causes changes in their life pattern and processes. Teaching palliative care can help old people suffering from chronic pain to live an active life. The aim of this research was to determine effects of educating of palliative care on life pattern of elderly women with chronic pain. The present study was a Quasi-experimental design with pre-test and post test was conducted on 30 elderly women suffering from chronic pain in 2018 in Iran. The Questionnaire for evaluating the Pattern of Life with Pain in the elderly was filled before the intervention, group educating of palliative care was carried out using an educational package, and the questionnaire was completed again immediately and one and three months after. The data was analyzed using mean, standard deviations, Fisher’s F test, and Greenhouse-Geisser and Bonferroni post-hoc test by employing SPSS- 16. Mean changes before teaching palliative care significantly differed from those of immediately and one and three months after the educational program (p = 0.0), (p = 0.004). There were significant differences between the stages of immediately and one month after the educational program and that of three months after it (p = 0.001), (p = 0.002). Concerning the personal life patterns, there were statistically significant differences between the stage immediately after the educational program and those before the intervention and three months after it (p = 0.005), (p = 0.000). Regarding the social life pattern, only the stage of one month after the educational program significantly differed from that of three months (p = 0.005). Mean growth in life pattern of the old women suffering from chronic pain in the stages after the intervention indicated the importance of and the necessity for palliative care during old age. Moreover, the success of this education three months after the educational program as compared to immediately and one month after it indicates that allocation of sufficient time plays a very important role in transferring information and in teaching methods of palliative care to old people.

Over the last two decades, experimental economics has been gaining relevance in the research of a wide range of issues related to agriculture. In turn, the agricultural activity provides an excellent field of study within which to validate the use of instruments employed by experimental economics. The aim of this study is to analyze the dynamics of the research on the application of experimental economics in agriculture on a global level. Thus, a literature review has been carried out for the period between the years 2000 and 2020 based on a bibliometric study. The main results show that there has been a growing use of experimental economics methods in the research on agriculture, particularly over the last five years. This evolution is evident in the different indicators analyzed and is reflected in the greater scientific production and number of actors involved. The most relevant topics within the research on experimental economics in agriculture focus on the farmer, the markets, the consumer, environmental policy, and public goods. These results can be useful for policy makers and researchers interested in this line of research.
