<p>This study breaks new ground in articulating how sexual identity impacts on the therapeutic relationship between the client and the lesbian nurse in psychiatric mental health nursing. There is little consideration given in the literature or in research as to how sexuality of the nurse impacts on nursing practice. Most attitudes held by the public and nursing staff are based on the assumption that everyone is heterosexual, including nurses. Fifteen lesbian psychiatric mental health nurses from throughout New Zealand volunteered for two interviews and shared their experiences of becoming and being a lesbian psychiatric mental health nurse. The stories they told give new insights into how these nurses negotiate and position their lesbian identity in the therapeutic relationship. To work therapeutically with people in mental distress the nurse uses personal information about themselves to gain rapport with the client through appropriate selfdisclosure. Being real, honest and authentic are also key concepts in this relationship so the negotiation of reveal/conceal of the nurse’s identity is central to ongoing therapeutic engagement. One of the most significant things arising from the research is that participants areable to maintain their honesty and authenticity in the therapeutic relationship whether they self-disclose their lesbian identity or not. This is because the experiences in their personal lives have influenced how the participants ‘know themselves’ and therefore guide how they ‘use self’ in their therapeutic nursing. The concept of a ‘licensed narrative’ has also been developed during this research reflecting the negotiated understandings between the researcher and the participants. Further, the use of NVivo a qualitative software package helps to track and make transparent the research processes. These two aspects make a unique contribution to the field of narrative inquiry.</p>

Title: Effectiveness of selected nursing interventions on fatigue and quality of life among Chronic Renal Failure Patients undergoing Hemodialysis in a selected hospital at Madurai. Background: Non communicable diseases are increasing day by day. Health is wealth is an old saying, which is realised by people who has CKD today. As their wealth is reduced by treating the disease throughout their life time, the restrictions in food and fluid impose lot of stress, affects their Quality of life. Nearly 10% of the populations were affected by CKD and millions of them die each year because they could not afford the treatment. Hemodialysis has been proved to be the effective treatment modality. It results in a marked change in the QOL, due to number of modifications and restrictions, which affect patient’s psychological and physiological wellbeing. Dialytic nurses can play a crucial role in enhancing quality of life and reducing fatigue level through exercises and teaching. Aim: The aim of the study was to evaluate the effectiveness of selected nursing interventions on fatigue and quality of life (QOL) among chronic renal failure patients undergoing hemodialysis in a selected hospital at, Madurai. Methodology: Quasi experimental non-equivalent pretest and post-test control group design was used for the study. The study was conducted in patients undergoing hemodialysis in selected hospital at Madurai. A total of 100 patients were selected using convenient sampling technique. Tools used were Piper Fatigue Scale (PFS) and SF 36 World Health Organisation BREF QOL (SF36 WHO BREF QOL) interview scale. The therapeutic nursing intervention which includes planned teaching about care of patient with haemodialysis, demonstration of dialytic exercises, dietary management, adherence to medication, follow up, teaching on sexual relationship were administered over 4 days in a week for 4 consecutive weeks. Data was analysed using descriptive and inferential statistics. Results: The therapeutic nursing interventions were effective in improving quality of life and reducing fatigue level. Pre-test with 15th day post-test fatigue scores comparison [t =23.05, P(< 0.001)] , Comparison of pre-test with 30th day post-test fatigue scores [t =41.19, P(< 0.001)], Comparison between 15th and 30th day post-test fatigue scores [t= 25.58, P(<0.001)], Comparison of pre-test with 15th day post-test level of quality of life score [t =16.32, P(<0.001)], Comparison of pre-test with 30th day post-test level of quality of life score [t=23.76; P(<0.001)], Comparison between 15th and 30th day post-test quality of life score [t = -15.19, P(<0.001)]. The problems reported by the people were, physiological problems such as anaemia, muscle cramping, difficulty sleeping, itching, fatigue, neurological disturbances, cognitive impairment, Sexual dysfunction and psychological problems such as depression, feeling of loneliness, sadness, problems with adjustment, lack of love and affection, suicidal behaviour, delirium, anxiety and panic symptoms. The post tests in experimental group, the correlation between fatigue and quality of life in post-test-1 (15th day) was r = - 0.372 and it was significant at p = 0.008. Similarly, the correlation between fatigue and quality of life in post-test - 2 (30th day) was r = -0.370 and it was significant at p=0.008. Conclusion: The problems of the people vary from person to person, study findings concluded that, selected nursing interventions are effective intervention to improved Quality of life and reduced fatigue level. There is a real need for nurses to provide physical, mental and social support to treat the health problems and to heighten their spirit.

Objectives The objective of the study was to synthesize existing quantitative evidence on palliative care intervention among patients with various types of illness in Thailand. Methods A synthesis of quasi and experimental studies published between 2010-2020 was conducted. The sample included theses and research articles that focused on nursing therapeutic therapeutic nursing. programs and which utilized a quasi or experimental study design. The main research instrument was a coding sheet developed by the researchers to record characteristics of the studies that met the inclusion criteria. Data were analyzed by frequency and percentage Results The study found seven studies that were included in literature reviews. Of the seven studies, one was classified as level 2.B and other six were classified as level 2.C. The outcomes of these existing nursing therapeutic programs specific focuses of the therapeutic nursing studies were quality of life (2 studies, 28.5%), symptom management in patients with advanced cancer (1 study, 14.3%), measurement of palliative care behavior (1 study, 14.3%), measurement of self-adaptation (1 study, 14.3%), measurement of quality of nursing care (1 study, 14.3%), and perceived symptom distress and quality of care (1 study, 14.3%). There were four types of interventions for caring patients and families during periods of palliative care: structured information programs, concrete nursing care programs, teaching and counseling programs, and family participation programs. Conclusion The findings of this study could be utilized as empirical evidence in developing clinical practice guidelines for patients in need of palliative care related to the patients’ health problems, symptoms, and needs.

Objective: to demonstrate the influence of the preoperative nursing consultation in the practice of self-care performed by patients with respiratory ostomy at the A.C. Camargo Cancer Center and to describe what are the self-care practices performed and their limiting factors. Method: qualitative, exploratory study, carried out with 7 patients aged 18 years or older, who underwent preoperative nursing consultation, who underwent surgery with the manufacture of a respiratory ostomy and were able to establish verbal communication. Data was collected through semi-structured interviews and a digital recorder and submitted to content analysis. Results: male participants predominated, aged 56 years or over and completed high school. Two analytical categories were identified: “Repercussions of the actions promoted by the preoperative nursing consultation”; and “Obstacles in the search for autonomy in the care of the tracheostomy cannula”. Conclusion: nursing consultation is an important instrument to guide, form bonds and clarify doubts about care with ostomy and respiratory devices, and nursing actions can limit and even prevent the patients from developing their self-care, making it necessary that the therapeutic nursing project is planned to aggregate the patient in this process.

Therapeutic communication is the one of the standards of nursing care that must be implemented by all nurses. The good implementation of therapeutic communication can increase the satisfaction of nursing care perceived by patients. Knowing the relationship between therapeutic communication with patient satisfaction inCempaka room PMI Hospita, Bogor.Typeof thisresearch is using random sampling technique with the descriptive analytical method and the cross sectionaldesign. To collect the data, the researcherusing questionnaire instrument on two variablesas follow therapeutic communication of nursing and patient satisfaction questionnaire instrument. Theresearch site of this research is held at PMIHospital Bogor on October, 2018 with patients in the Cempaka room PMI HospitalBogorwho have experienced 85 respondances were brought into the hospital. Analysis techniques using univariate and bivariate analyzers with kendaal’s tau-c statistics test.The resultOf the 96 respondents found that did therapeutic nursing communication applied amounted to 38 respondances (65,5%). And patiens say satisfied with the nursing services amounted to 40 respondances (69,0%).On the statistics test is gotten that P Value 0,001. Ha is accepted and Ho is rejected that P Value <0,05.

ABSTRACT Objective: To understand the transitions experienced, and the conditions and expected response patterns to changes in sexuality of the spouse-caregiver of the elderly, during progression of the dementia process. Method: A qualitative research study, conducted at the neurogeriatric clinic between May of 2014 and May of 2015. An intensive, individual interview was administered to 12 elderly caregivers. Thematic content analysis was applied, using the theoretical model of Transition Theory. Results: Seven categories emerged, involving relationship and conjugal sexuality; disease repercussions; care and professional approach; attitudes, beliefs and social imagery of sexuality and care; family relationship and redefining of sexuality. Final considerations: Family development and marital life, the aspects of formation and development of sexuality, the specifics that involved living and caring for the other were understood, with successive events and changes influenced by old age, dementia, beliefs and social imagery.

Introduction: Prevalence of violence is highly occur in mental disorders clients at psychiatric hospitals. The impact is injure to others. This research aims to examine the effectiveness of music therapy and RECBT to sign and symptom and ability to control violent behaviour. Methods: Quasi-experimental research design with a sample of 64 respondents. Result: The study found a decrease symptoms of violent behaviour, ability to control violent behavior include relaxation, change negative thingking, irational belief, and negative behavior have increased significantly than the clients that did not receiving therapy. Discussion: Music therapy and RECBT is recommended as a therapeutic nursing at the client’s violent behaviour.Key Word: violent, sign and simptom, ability, music therapy, RECBT
