Teacher uses language as a medium of communication to teach, guide and instruct students in the classroom. This study aims to investigate the English teacher's directives and the students' responses to these directives. This study participants were an English teacher of a junior high school in Indonesia who teaches an eighth-grade class and her students who were chosen purposively. This qualitative case study was collected through observation, and interviews and were then analyzed by following Miles' interactive cyclical model. The results revealed that the teacher mostly produced directive speech acts types of commands and requests rather than suggestions. Commands have been the most commonly used type of directives since the teacher plays a control in the classroom because it is more understandable and familiar for the students. Hence, the students' responses varied, and depended on their response strategies and each personal reason.

  This research aims to identify the ways of conveying the types of preference performed by the characters in Calibre movie. This research used a descriptive qualitative method. 70 utterances transcribed and then identified as many as 31 data. The utterances from the characters in Calibre movie were specified as data. They were taken from the transcript of the movie and used as an interpretation of the context. The main instrument was the researcher assisted with the movie and its transcript. In analyzing the data, the theory of Yule’s conversation analysis was used. This theory clarified the forms of the preferred and dispreferred acts including the types and ways of performing. The results showed that there are nine ways to convince the types of preference: (1) delaying/hesitating, (2) prefacing, (3) expressing doubt, (4) using token yes, (5) mentioning obligation, (6) making a non-personal reason, (7) giving an account, (8) direct agreement and direct declination, and (9) arguing.

Abstract Diversifying has been a challenge for police agencies, despite their increased efforts to attract qualified candidates. Even when recruiters manage an adequately diverse applicant pool, the dropout rate of candidates throughout the notably long hiring process threatens the representativeness of the applicant pool. Thus, police agencies are eager to examine why applicants—especially women and minority applicants—withdraw from the hiring process to address any common reasons and retain desirable candidates. To explore reasons for withdrawal, 143 women and minority police applicants who withdrew from the hiring process reported their reason for withdrawal during a telephone interview conducted in the spring and summer of 2016. The primary reasons for applicant attrition, after changing their mind or another personal reason, was finding other employment and the length of the hiring process. Recommendations for policy include increasing communication to applicants, shortening the hiring process and hiring on a continuous basis. Implications are discussed.

Abstract Background: To explore the clinical efficacy and long-term prognosis of laparoscopic technique in patients with gallbladder cancer.Methods: Retrospective analysis and summary of the clinical data of 50 patients with gallbladder carcinoma treated by laparoscope from January 2010 to November 2018 in the Third Affiliated Hospital of Soochow University.Results: The intraoperative or postoperative pathology of 43 cases were confirmed as the gallbladder carcinoma. Among them, 11 cases were confirmed as Tis stage, 12 cases as T1b stage, 20 cases as T2 stage and 7 cases as T3 stage. Follow up data of one patient was lost due to personal reason. During the follow-up period, no one at Tis stage died, two cases at T1b stage died, six cases at T2 stage died and six cases at T3 stage died. Conclusion: Laparoscope is a secure and feasible way in the treatment of gallbladder carcinoma at an early stage, but a large number of clinical data is still required to demonstrate its long-term curative effect.

Category: Sports Introduction/Purpose: The management of Achilles tendon ruptures in recreational athletes can be challenging. This study assessed the average time to return to sport after open repair for Achilles rupture in recreational athletes. Methods: Twenty one recreational athletes with prodromal tendinous problems sustained an acute tear of the AT and surgical repair with Krachow method from June 2013 to April 2017. Their average age was 39.7 years, and the average follow-up time was 53.2 months. Each patient was evaluated for postoperative ATRS (Achilles Tendon Total Rupture Score), complication and time to return to sports. Results: The mean postoperative ATRS was 85.1. The mean length of time to return to sports was 8 months. Eighteen patients of 21 athletes returned to full sport participation. Eleven athletes returned to the original sports. Seven athletes changed their sports due to fear of re-rupture (four athletes), discomfort during running or jumping (one athlete), general weakness (one athlete), dissuade of family (one athlete). Three athletes didn’t return to sport participation due to thickening of Achilles tendon (two athletes) and personal reason (one athlete). Conclusion: The return to play was 85 % at 8 months postoperatively. The results provide reference data for sports physician in evaluation surgical results and informing athletes about expectations after surgery in terms of timing of return to sports in recreational athletes.

<p>The objective of this research was to detect and clarify which reasons caused the school dropout in Mathematics and Physics Degree Course at the Federal Institute of Paraíba (IFPB), Campina Grande Campus (CG).In order to do so, a theoretical-empirical, descriptive, ex-post-facto, bibliographic and documentary research was developed, focusing on the qualitative dimension of the phenomenon seeking to understand the school dropout from the perspective of the ones who dropped out school. Data were collected through online questionnaires sent to a total of 218 students who had dropped out the courses previously mentioned between 2013 and 2018. Through the stud,y it was possible to see that the difficulty of working and studying at the same time was the personal reason that most contributed to students’ exit of the course. Among the internal factors ( academic and/or institutional nature), the one that most influenced the decision to drop out involves issues related to the teacher and pedagogical practices, especially the ones regarding didactics and the assessment used in class. Finally, the item dissatisfaction with the labor market of the chosen profession was pointed out as the most decisive external factor for the abandonment of the course.</p>

Background. Relapsed/refractory(R/R) AML has poor prognosis and lacks of standard treatment, while clinical trial has been recommended for these patients. It is necessary to find innovation and effective chemotherapy regimens for R/R AML. Cladribine-based regimens have been proved to be effective in R/R AML. As low-dose cytarabine (Ara-C) could reduce marrow myeloblasts and ATRA may sensitize leukemic cells to Ara-C, we performed the phase 1/2, single-arm, multicenter study of cladribine in combination with Ara-C,ATRA,G-CSF and mitoxantrone (CLAAG-M Regimen) in R/R AML (ChiCTR-OIC-16008110). Methods. R/R AML patients aged 18-70 years were eligible to enroll in this study. The objective was to assess the responses and safety of this therapy. All patients received continuous infusion cladribine 5mg/m2 on days 1-5 or 1-3 and mitoxantrone 6mg/m2 on days 1,3,5 or 1,3. ATRA was taken orally at 45mg/m2 on day 4-6 and 15mg/m2 on days 7-20. Subcutaneous injection of cytarabine 15mg/m2 every 12 hours on days 1-10 and G-CSF 300ug/d on day 0 until disease remission or absolute neutrophils count ≥ 0.5×109/L were administered. All patents received one or two courses of induction treatments. Consolidation chemotherapy consisted of cladribine, low-dose Ara-C and G-CSF with or without mitoxatrone or ATRA according to the choice of study site. Results. Between April 2014 and July 2019, a total of 52 patients from five centers were screened,data of 7 patients who did not meet the enrollment criteria and 3 patients did not follow the established treatment were excluded. Therefore, 42 patients were enrolled in this study with 22 females and 20 males (Table 1). Median follow-up duration is 202.0 days(range 12.0-1560.0) from enrollment. The median age was 46 yr (range 25-68 ) . 27 out of 42 patients were refractory AML, with 15 patients in relapsed disease.One patient did not evaluate curative effect owing to personal reason,thus among 41 estimated patients, the overall response rate (ORR= CR+PR) was 70.7%, with complete remission rate (CRR = CR + CRi) of 68.3%, while early death rate and non-remission rate were 7.3% and 22.0% respectively (Table 2). For patients achieved CR, 10 of them (24.4%) received hematopoietic stem cell transplantation (HSCT) and more patients (36.6%) were only treated with consolidation therapy. The median overall survival (OS) of all patients was 306.0 days (95%CI 150.0-462.5) and the rate of 1-year and 2-year OS were 38% and 32% (Figure A). The median disease-free survival(DFS)was 183.0 days (95%CI 16.7-349.2) with half-year survival rate of 37% and 1-year survival rate of 32% (Figure B). Hematological toxicity was the most prominent toxicity of the CLAAG-M regimens since the median duration of neutrophils<0.5×109/L and platelets<20×109/L were 22 days (range8-42) and 17 days (range5-112),respectively. WHO>2 extra-hematological toxicities were infection (21/51) and bleeding (2/51). Three patients (7.3%) died early,with one from infection and two from organ hemorrhage. In multivariate analysis, transplantation would prolong the OS (P=0.013,95%CI of HR 0.010-0.578) . Conclusion.CLAAG-M regimen is effective and tolerated in patients with R/R AML and could be as a bridging therapy regimen followed by HSCT. To confirm the clinical benefit, a randomized study will be further carried out. Disclosures No relevant conflicts of interest to declare.

Context Concussions elicit changes in brain function that may extend well beyond clinical symptom recovery. Whether these changes produce meaningful deficits outside the laboratory environment is unclear. The results of player performance postconcussion within professional sports have been mixed. Objective To determine whether National Hockey League (NHL) players with concussions performed worse after returning to sport than players with lower body injuries or uninjured players. Design Cohort study. Setting Publicly available Web sites that compiled injury and player statistics of NHL players. Patients or Other Participants Male NHL players who missed games due to a concussion (n = 22), lower body injury (n = 21), or noninjury (ie, personal reason or season break; n = 13) during the 2013–2014 and 2014–2015 regular seasons. Data on concussed athletes were used to identify similar players with lower body injury and noninjury based on (1) position, (2) time loss, (3) time on the ice, and (4) team. Main Outcome Measure(s) The primary performance metric was a modified plus-minus statistic calculated by weighting the players' plus-minus metric by their team's simple rating system to account for varying team performances. Linear mixed models assessed the relationship between injury type (concussion, lower body, or noninjury) and performance (plus-minus score). Results We observed a quadratic effect for a time2 × group interaction (\upchi _2^2 = 8.85, P = .01). This interaction revealed that the concussion and lower body injury groups had similar patterns of an initial decrease (ie, 2 weeks after return to play), followed by an increase in performance compared with the uninjured group in weeks 5 and 6. Meanwhile, the uninjured group had an initial increase in performance. We observed no group × linear time interaction (P = .47) or overall group effect (P = .57). Conclusions The NHL players in the concussion and lower body injury groups displayed similar performance impairments. Both injured cohorts experienced an initial decrease in performance at weeks 1 to 2 after return to play, followed by improved performance at weeks 5 to 6 after return to play, suggesting that the performance implications of concussion may be short lived.
